andrewgu-oai c62e9df39f Preserve reasoning token accounting across resume and fork (#57839)
## Why

Resuming or forking a thread loses the session flag indicating that reported token usage already includes retained reasoning. Counting that reasoning again can trigger automatic compaction too early.

## What changed

Persist `last_usage_includes_reasoning` with `TokenUsageInfo` and use it when estimating context usage. Record the flag with completed sampling and compaction usage, and mark local estimates as including reasoning. Older records without the field retain the existing accounting behavior.

## Testing

Add coverage for saved inclusive, exclusive, and legacy token counts. Extend the encrypted-reasoning compaction test to verify the compaction threshold for live, forked, and cold-resumed threads.

alicec-oai d359945843 Declare the image generation result type in code mode (#57836)
## What changed

Add an output schema for `image_gen.imagegen` so its code-mode declaration returns `Promise<{ image_url: string; output_hint?: string; }>`. Direct API tool serialization remains unchanged.

## Testing

Add coverage for the generated return type and unchanged direct API serialization, assert the full declaration in the app-server integration test, and verify that code-mode results omit oversized output hints.

swastik-oai 3da7de59b6 Add isolated Guardian assessments for prepared actions (#57834)
## What changed

Add `ReviewInput` to bundle an approval request with its captured parent history and review context. Its `assess_isolated` method runs a fresh reviewer without publishing a live approval, honors deadlines and cancellation, and returns a `GuardianAssessment` or review error.

Share review parameter construction with the live review path and extract assessment parsing and error conversion into `GuardianReviewSessionOutcome::into_assessment`.

## Testing

Add a mock-backed test that verifies the isolated assessment is returned and the reviewer receives the captured history without instructions added after capture.

ioanbudea-OAI c6fce1cace Centralize synchronous Guardian prompt preparation (#57790)
## What changed

- Separate evidence collection from prompt preparation and pass reviewer history, model limits, and image preparation policy through shared `guardian-context` APIs.
- Detect captured images before composition and measure reviewer history tokens only when images and a valid context window require it.
- Share instruction and native-message delivery filtering across budget checks, including restoration of retained evidence after history changes.

jif 7c11b8074d Include terminal launch commands in Guardian stdin reviews (#57758)
## Why

Guardian's `write_stdin` approval requests lacked the command used to launch the receiving terminal, leaving reviews without that context.

## What changed

- Carry `original_command` from the terminal's launch command into the approval request and Guardian action JSON. This describes the original launch, not the terminal's current state.
- Exclude `original_command` from the 8,000-byte stdin approval size check so launch context does not consume the budget for input, permission details, and the approval reason. Launch context remains subject to Guardian's normal request budget.

## Testing

Extend serialization coverage to check `original_command` and the strict stdin review integration test to verify that a launch command longer than 8,000 bytes reaches Guardian intact.

jif fc916a22b5 Expose Guardian assessments in response streams (#57757)
## What changed

Decode `response.metadata` Guardian assessments into typed `GuardianAssessment` events over SSE and WebSocket, carrying `response_id`, `item_id`, and `requires_review`. Ignore assessments with missing, empty, or incorrectly typed required fields while preserving independent delivery of other metadata.

Expose the assessment type to request-scoped extension interceptors. Keep assessments out of conversation history; they do not authorize tool execution.

## Testing

Add SSE coverage for assessment ordering, malformed metadata, and mixed metadata delivery. Extend model request tests to cover SSE and WebSocket interceptor delivery and exclusion from subsequent model input.

adams-oai 28cade4f5a Cache empty remote skill discovery with filesystem watches (#57598)
## Why

Repeated repository skill discovery performs remote metadata requests even when no `.agents/skills` directories exist. Reuse verified empty results to avoid those round trips while watching for changes that require a new scan.

## What changed

- Add the Linux executor capability `fs_watch_missing`, the `fs/watchMissing` request, and `fs/missingChanged` notifications.
- Register watches in the background and reuse empty results only after the executor acknowledges coverage. Key reuse by filesystem, working directory, and project root markers.
- Invalidate observations on relevant filesystem or mount changes, watch supersession, or connection loss. Clear cached observations on forced reloads and cache resets.
- Retain live discovery while registration is pending or watching is unsupported, unavailable, or cannot verify the observation.

## Testing

Add tests for empty-result reuse and invalidation, skill directory creation before watch acknowledgement, unsupported executors, idle WebSocket notifications, reconnection, case aliases, and bind mounts and unmounts. Include an ignored manual latency comparison for repeated empty remote scans.

rhan-oai 37f199dab1 Add host-reserved V2 child creation APIs (#57586)
## What changed

- Add `CodexThread::create_child` to create an idle child from a live tool invocation using a host-reserved thread ID and controller.
- Add `prepare_child` and `ThreadManager::create_prepared_child` to capture inherited settings and fresh or forked history, then start the child on another manager even after the parent shuts down. Reject detached preparation when live bindings cannot be transferred.
- Add opt-in `HostToolInvocationLookup` to track live calls and reject duplicate active call IDs within a turn. Revoke invocation access when dispatch ends and clean up provisional children when startup fails or is cancelled.

## Testing

Add tests for invocation lifetime and call ID reservation, inherited policy and live bindings, detached startup with fresh and full history, and cancellation cleanup.

ioanbudea-OAI 6574bcdba8 Defer synchronous Guardian heartbeat compression to preparation (#57519)
## What changed

Keep original scheduled messages and heartbeat provenance during synchronous transcript collection. Add `prepare_sync_context` to compress repeated heartbeat instructions across the complete collected transcript before selecting the requested suffix, preserving reference numbering in full and delta prompts.

Route synchronous reviewer prompts through this preparation step and reject composition of unprepared scheduled messages. Move transcript profiles and REPL evidence rendering into the preparation module. Asynchronous heartbeat compression remains in collection.

## Testing

Extend heartbeat transcript coverage to compare prepared full and delta prompts with the existing rendering and verify that direct composition rejects unprepared scheduled messages.

- Add the managed `unsafe_enforce_sqlite_journal_mode` requirement to migrate existing NFS databases before opening writable pools and require `DELETE` on each connection.
- Share migration code between startup and `codex migrate-nfs-db`, preserving WAL backups and resuming interrupted replacements, including lazily opened databases.
- Hold the migration lock through pool initialization and keep migration failures out of automatic corruption recovery.
- Preserve local logs behavior and skip legacy NFS logs cleanup when enforcement is enabled.

## Testing

Add coverage for requirement precedence, database settings, data and backup preservation across restarts, interrupted replacement recovery, refusal of active WAL databases, migration lock waiting, and local logs remaining in WAL mode.

lsingh-oai 3d1001e865 Route FedRAMP API keys to government endpoints by default (#57178)
## Why

API keys with FedRAMP issuance markers previously used commercial endpoint defaults without a FedRAMP request header.

## What changed

- Recognize `sk-proj-fedramp-` and `sk-svcacct-fedramp-` prefixes as routing hints and send `X-OpenAI-Fedramp: true`, preserving the complete key for authentication.
- Select `https://gov.api.openai.com/v1` for default OpenAI API routing and `https://gov.chatgpt.com/backend-api/` for default ChatGPT backend requests, including model discovery and analytics.
- Preserve explicit endpoint overrides, including an explicitly configured commercial default URL.
- Resolve provider routing and authentication from the same credential snapshot, and apply key-dependent routing to realtime WebSocket startup.

## Testing

Add regression coverage for prefix recognition, unchanged key storage and authorization, provider credential precedence, endpoint overrides, credential rotation, model discovery, analytics routing, and realtime request preparation.

felixxia-oai 7848e3393c Remove Guardian snapshot-specific context helpers (#56559)
## What changed

Remove snapshot instruction deduplication, separate retained-assistant framing, and the associated omission-metadata special cases. Use `retain_new_instructions` in transcript rendering tests and replace the test-only Decisions snapshot adapter with the conversation adapter.

## Testing

Extend retained-source delivery and incomplete-metadata coverage to both synchronous and asynchronous context profiles. Keep Decisions HTTP, eviction, and cancellation tests exercising the conversation adapter.

felixxia-oai 2d86506823 Exercise retained sampling in Guardian transport tests (#56557)
## Why

The transport tests used a separate, test-only snapshot sampler instead of the production retained sampler, leaving them exercising different response ownership and cancellation paths.

## What changed

Route the transport test helper through `sample_retained` and remove snapshot execution, its active-request supersession bookkeeping, and test-only compaction compatibility fields. Simplify request-token metrics to reflect the remaining admitted-request path.

## Testing

Update tests to verify early score delivery while the retained response remains pending, WebSocket cancellation and connection reuse, and HTTP cancellation before headers or during response draining. Assert that HTTP cancellation releases classification permits.

felixxia-oai 4b1cf9a4c9 Use conversations for all Guardian v2 async classification (#56556)
## What changed

- Always use the retained conversation backend for asynchronous classification; keep snapshot sampling test-only.
- Accept legacy `async_classifier_mode` values in configuration and model metadata but ignore them. Emit a startup warning when `[features.guardianv2].async_classifier_mode` is configured.
- Ignore legacy classifier mode differences when checking model-switch compatibility, and fail closed if the conversation backend is missing.

## Testing

Update coverage for legacy configuration warnings, model-switch compatibility, retained approval-review context, and conversation-based scoring.

Anthony Tafoya ea819e1f4d Record skill and plugin catalog sizes on publication (#56530)
## What changed

- Add `codex.skills.catalog.entries` and `codex.plugins.catalog.entries` histograms with `cloud` or `executor` stages and `complete`, `warning`, or `partial` status. Partial results take precedence over warnings.
- Exclude cached reads and repeated observations of unchanged failed skill discovery from metric samples.
- Mark executor skill catalogs partial when discovery fails, and clear unpublished executor plugin catalogs instead of publishing an empty result.

## Testing

Extend skill integration tests to check catalog counts and statuses, fresh discovery snapshots, partial discovery, and repeated reads without duplicate samples. Update plugin state tests to distinguish unpublished catalogs from empty discovery results.

bkotsopoulos b82d542aef Add turn, tool, and skill attribution to traces (#56524)
## What changed

- Emit `codex.turn_profile` events with finalized sampling request and retry counts when turns complete or abort, and preserve the caller's tracing span for detached extension interrupts.
- Add conversation and turn identifiers to turn and tool dispatch spans, identify tool sources, and include dispatch-to-result-ready duration on tool completion events.
- Emit `codex.skill_read.source_selected` before cache or provider access, recording the selected cloud or executor source, skill and plugin identity, resource kind, and continuation status.

## Testing

Add regression coverage for trace correlation, tool completion timing, turn profile counters on completion and interruption, detached interrupt span propagation, and skill source attribution including cached cloud reads.

Charlie Marsh 60adfa8513 Treat model-not-found provider errors as terminal failures (#56445)
## Why

Provider errors with code `model_not_found` should end the turn immediately and preserve the provider's message instead of triggering retries.

## What changed

- Recognize `model_not_found` in HTTP 400, 403, and 404 responses and in streamed `error` and `response.failed` events.
- Preserve the provider message and any available HTTP status, and expose `modelNotFound` through the app-server protocol and generated schemas and SDK types.

## Testing

Add coverage for HTTP classification, statusless stream errors, and terminal failures without retries across HTTP, SSE, and WebSocket responses. Verify that `thread/read` preserves the error reported by the live notification.

Vivian Fang d6817454d7 Make the Guardian review timeout configurable (#56371)
## What changed

Add `auto_review.timeout_ms` to configure the overall Guardian review budget, including setup and retries. Keep the default at 90,000 milliseconds; setting it to `0` disables the review deadline while preserving cancellation, retry attempt limits, and independent transport timeouts.

Carry the configured deadline through review execution and report the configured budget in review telemetry, using zero for unlimited reviews.

## Testing

Add coverage for shared setup budgets, configured timeouts, cancellation with unlimited reviews, and retry attempt limits without a deadline.

andrewgu-oai 4610b573e5 Run environment hooks on their owning executors (#56343)
## What changed

- Include ready environments' configured hooks in lifecycle events and execute command hooks using each owner's shell, working directory, shell environment policy, and managed network policy.
- Resolve tool environments before `PreToolUse` and use those targets for tool hooks, execution, and permission review. Route MCP hooks by server ownership and file-copy hooks to both endpoints.
- Preserve environment, MCP server, and terminal session selectors across hook input rewrites, rejecting attempts to switch targets.

## Testing

Add regression coverage for permission-request selector preservation, MCP resource hook ownership, and file-copy hooks that rewrite paths while preserving source and target environments.

rhan-oai e103bbc06f Apply model catalog overrides to history, notes, and context tools (#56313)
## What changed

- Read optional tool descriptions and parameter schemas for `history`, `notes`, `new_context`, and `get_context_remaining` from the acting model's catalog messages.
- Support catalog namespace descriptions for history and notes, preserving explicit empty and whitespace descriptions.
- Share catalog override handling across native tools and MCP resource helpers. Keep bundled schemas when overrides are invalid, preserve encrypted parameters, and leave tool identity and transport fields unchanged.

## Testing

Add coverage for sparse catalog serialization, schema fallback and encryption preservation, per-step tool definitions, and context tool updates when switching models. Extend outgoing-request tests to verify history and notes overrides while retaining history image attachments.
