pakrym-oai 172b2218a5 core: remove redundant TurnContext and Prompt fields (#28638)
## Why

`TurnContext` had accumulated dead fields and cached projections of
values already owned by its per-turn `Config` or `ModelInfo`. Keeping
both copies made ownership unclear and allowed artificial split-brain
states, such as a compatibility hash differing from the model metadata
it came from.

`Prompt` similarly carried a write-only personality after personality
selection had already been materialized into its base instructions.

This makes the canonical owner explicit: configuration-backed values
come from `config`, model-derived values come from `model_info`, and
prompts contain only data consumed by request construction.

## What changed

- Remove the unused `ghost_snapshot`, `codex_self_exe`, and
`thread_source` fields.
- Remove duplicate `comp_hash`, `truncation_policy`, `features`,
`shell_environment_policy`, `codex_linux_sandbox_exe`, `compact_prompt`,
and `tool_mode` fields.
- Read those values directly from `TurnContext::config` or
`TurnContext::model_info` at their consumers.
- Remove the write-only `Prompt::personality` field and its constructor
assignments.
- Preserve review-turn inheritance of the parent turn's shell policy,
Linux sandbox executable, and compact prompt through the review config.

## Testing

- `cargo check -p codex-core --tests`

pakrym-oai 6f77491e95 thread-store: fix response fixture compilation (#28642)
## Why

A `codex-thread-store` test fixture still constructs
`ResponseItem::FunctionCallOutput` without its required `metadata`
field, preventing the crate's test targets from compiling on `main`.

## What changed

- Set the fixture's response-item metadata to `None`.

pakrym-oai 7baf7e467e [codex] Route MCP file uploads through environment filesystem (#27923)
## Why

Codex Apps tools can mark arguments with `openai/fileParams`, but the
execution path resolved and opened those files directly on the host.
That bypassed the selected turn environment and prevented annotated file
arguments from working with remote environments.

## What changed

- resolve annotated file arguments against the primary turn environment
- read file metadata and contents through that environment's sandboxed
`ExecutorFileSystem`
- reject files over the 512 MiB limit from metadata before reading or
transferring them
- retain the buffered upload-size check as defense in depth
- make the OpenAI upload API accept a filename and buffered contents
instead of owning local filesystem access
- describe the model-visible argument as a path in the primary
environment

This builds on #27927, which added `size` to internal filesystem
metadata.

pakrym-oai a4711b88dd [codex] exec-server: stream files in chunks (#28354)
## Why

`fs/readFile` buffers the entire file in one response, which makes large
remote reads expensive and prevents callers from applying backpressure.
We need an opt-in streaming path with bounded block sizes while
preserving the existing single-call API for small and sandboxed reads.

## What changed

- Add `ExecServerClient::stream`, returning a named `FileReadStream`
that implements `futures::Stream` and yields immutable 1 MiB byte
blocks.
- Add internal `fs/open`, `fs/readBlock`, and `fs/close` RPCs.
`fs/readBlock` accepts an explicit offset and length.
- Keep unsandboxed files open between block reads, cap open handles per
connection, and clean them up on EOF, error, stream drop, explicit
close, or connection shutdown.
- Reject platform-sandboxed streaming opens instead of turning the
one-shot sandbox helper into a persistent server. Existing `fs/readFile`
behavior is unchanged.

## Testing

- `just test -p codex-exec-server`
- Integration coverage for 1 MiB chunking, exact block-boundary EOF,
sandbox rejection, and continued reads from the opened file after path
replacement.
- Handle-manager coverage for non-sequential offsets, variable block
lengths, the 128-handle limit, and capacity release after close.

pakrym-oai 1e015884c5 [codex] Use local environment for user shell commands (#28163)
## Why

User shell commands still read the legacy turn cwd and session shell
even though execution context is now owned by selected turn
environments. App-server also defines `thread/shellCommand` as a
local-host escape hatch, so it must use an available local environment
even when a remote environment is primary.

## What changed

- Add `ResolvedTurnEnvironments::local()` to find the selected local
environment.
- Resolve the user shell command cwd and shell from that local
`TurnEnvironment`.
- Emit the standard `shell is unavailable in this session` error when no
selected local environment or resolved local shell is available.
- Add an integration test covering `/shell` without a local environment.

pakrym-oai e752f7b4ae [codex] Use expect in integration tests (#28441)
The workspace denies `clippy::expect_used` in production. Although
`clippy.toml` allows `expect` in tests, Bazel Clippy compiles
integration-test helper code in a way that does not receive that
exemption, which encouraged verbose `unwrap_or_else(... panic!(...))`
and equivalent `match`/`let else` forms.

This allows `clippy::expect_used` once at each integration-test crate
root (including aggregated suites and test-support libraries), then
replaces manual panic-based Result and Option unwraps with
`expect`/`expect_err`. Standalone `tests/*.rs` files remain their own
crate roots. Intentional assertion and unexpected-variant panics remain
unchanged, and the production `expect_used = "deny"` lint remains in
place.

The cleanup is mechanical and net-negative in line count.

pakrym-oai 08901fc8e1 [codex] Add interruptible sleep tool (#28429)
## Why

Models sometimes need to pause briefly while waiting for external work,
but using a shell command for that delay ties the wait to a process and
does not naturally resume when new turn input arrives.

## What changed

- add a built-in `sleep` tool behind the under-development `sleep_tool`
feature
- accept a bounded `duration_ms` argument, matching the millisecond
convention used by unified exec
- end the sleep early when either steered user input or mailbox input
arrives
- include elapsed wall-clock time in completed and interrupted outputs
- emit a dedicated core `SleepItem` through `item/started` and
`item/completed`
- expose the sleep item as app-server v2 `ThreadItem::Sleep` and retain
it in reconstructed thread history

pakrym-oai 022f1221e8 [codex] Bind shell snapshots to retained thread environments (#28421)
## Why

Shell snapshots are currently session-scoped even though shell and cwd
are properties of a selected turn environment. That makes snapshot
refresh depend on separate session-cwd plumbing, prevents retained
environments from retaining their snapshot work, and can make snapshot
construction use a different shell than command execution.

This follows #27955 by making the retained thread-environment service
own environment snapshot lifecycles. Session configuration remains the
requested selection state, while `ThreadEnvironments` remains the source
of successfully resolved environments.

## What changed

- Configure the shell-snapshot builder before initial environment
resolution.
- Start each local environment snapshot task when its `TurnEnvironment`
is built and retain that shared task while environment ID and cwd still
match.
- Inherit retained environment snapshots into spawned child threads.
- Carry the selected `TurnEnvironment` through shell runtimes so
snapshot construction and command execution use the same
environment-specific shell and cwd.
- Load project instructions and warm plugins/skills after initial
environment resolution.
- Continue decoding invalid UTF-8 instruction files lossily without
emitting a startup warning.
- Keep requested selections in `SessionConfiguration`; failed or
duplicate resolutions only affect the resolved environment snapshot.

## Validation

- `cargo check -p codex-core --tests`
- `just test -p codex-home instructions` (6 passed)
- Focused environment, instruction, shell-snapshot, and user-shell tests
(84 passed)
- Focused shell-snapshot, user-shell, and unified-exec tests (126

pakrym-oai 9728992fab [codex] retain resolved environments across turns (#27955)
## Why

Selected execution environments are thread-scoped resources, but startup
and turn construction repeatedly resolved their IDs and working
directories. That discarded existing environment handles and shell
metadata even when a selection had not changed.

Session configuration updates also need to affect future turns without
changing the resolved environment set already captured by a running
turn.

## What changed

- Create a `ThreadEnvironments` service inside `Codex` from the spawned
`EnvironmentManager` and raw environment selections, then store it on
`SessionServices`.
- Split service construction from `update_selections`, allowing session
configuration updates to mutate the resolved set in place.
- Retain an existing `TurnEnvironment` when its environment ID and
working directory match; resolve only added or changed selections and
remove selections that are no longer present.
- Normalize duplicate IDs by keeping the first selection and skip
individual selections that fail to resolve instead of rejecting the
entire update.
- Give each `TurnContext` a cloned `TurnEnvironmentSnapshot`, so later
session configuration updates affect future turns without rewriting an
active turn.
- Reuse the service-owned environment manager and resolved snapshot for
startup work, MCP initialization, and child-thread spawning instead of
flowing resolved environments through spawn arguments.

pakrym-oai bd4fe31c5a [codex] preserve explicit environment cwd (#27995)
## Why

`TurnEnvironmentSelections::new` rewrote the primary environment's
explicit `cwd` to the legacy fallback cwd. For a remote-first selection,
this could replace the remote working directory with a local fallback
path and made the legacy cwd overlay authoritative over
environment-owned state.

## What changed

- Preserve every explicit environment cwd when constructing turn
environment selections.
- Keep `cwd`-only app-server updates compatible by rebuilding the
default environment selections at the requested cwd.
- Cover both explicit primary cwd preservation and cwd-only updates
reaching the model-visible execution environment.

pakrym-oai 5e58b6d77f [codex] remove stale PathExt import (#28344)
## Why

`main` fails dev-profile Cargo and Bazel Clippy builds because
`core/src/tools/runtimes/mod_tests.rs` imports `PathExt` after its last
use was removed. With warnings denied, that stale import prevents
`codex-core` test targets from compiling across platforms.

## What changed

Remove the unused `PathExt` import. Remaining `.abs()` calls in the
module operate on `PathBuf` and continue to use `PathBufExt`.

## Validation

- `just fmt`
- Focused `codex-core` test compile attempted; blocked locally by disk
exhaustion before compilation completed. The CI failure itself is the
unused-import diagnostic this change removes.

pakrym-oai 42ad752f36 [codex] simplify memory read metrics (#28164)
## Why

Memory read telemetry currently reconstructs the executable shell
command after a tool call finishes. That duplicates shell, login-policy,
and cwd resolution owned by the tool handlers, and can diverge from the
environment-specific command that unified exec actually ran.

## What changed

- Expose the existing restricted shell-script parser directly for raw
script text.
- Parse `shell_command` and `exec_command` input into plain command argv
before classifying memory reads.
- Preserve all-or-nothing safe-command validation for multi-command
scripts.
- Remove cwd resolution, shell selection, and the unnecessary async

pakrym-oai 1aa3e432cc [codex] simplify shell snapshot ownership (#27756)
## Why

Shell snapshot lifecycle state was split between `Shell` and
`SessionServices`: `Shell` carried the receiver while session code
exposed and forwarded the raw sender. That coupled shell identity to
mutable snapshot state and made refresh, inheritance, and file lifetime
harder to reason about.

## What changed

- make each `Arc<ShellSnapshot>` represent one cwd-specific snapshot
generation
- store the active generation in `SessionServices` with `ArcSwapOption`
- have construction start the background build and expose only a
cwd-validated snapshot path
- use `ShellSnapshotFile` ownership to delete snapshot files
automatically
- pass snapshot paths explicitly to shell runtimes instead of storing
snapshot state on `Shell`
- preserve inherited and in-flight generations by pinning their `Arc`
while they are in use

pakrym-oai 7ebd10783b [codex] Let generic test turns inherit their environment (#27972)
## Why

The paired thread-environment migration changed several generic test
turn helpers from supplying a fallback cwd to explicitly selecting the
local environment. That changes their meaning under
`build_with_remote_env()`: remote-only fixtures cannot resolve the
forced local selection, so the tests fail before exercising apply-patch,
RMCP, unified-exec, or view-image behavior.

Generic helpers should inherit the environment selected by their
fixture. Tests that intentionally exercise local routing continue to
select the local environment explicitly.

## What changed

- Remove forced `local_selections(...)` overrides from the generic
apply-patch, RMCP, unified-exec, and view-image turn helpers.
- Remove the imports made unused by those deletions.

## Testing

- Not run locally; this is a test-fixture-only change and the `full-ci`
branch will exercise the affected remote shards.

pakrym-oai 2ee854eaa5 [codex] Add crate API surface review rule (#27939)
## Why

Review guidance should explicitly discourage widening crate APIs for
testing convenience. Keeping those boundaries narrow reduces accidental
coupling and prevents one-off test utilities from becoming durable
public surface area.

## What

- Add a crate API surface rule to `AGENTS.md`.
- Ask reviewers to keep crate APIs small and avoid proliferating
test-only helpers.

## Test plan

- Not run (documentation-only change).

pakrym-oai 76d8f20241 [codex] Add size to internal filesystem metadata (#27927)
## Why

`ExecutorFileSystem::get_metadata` reports file kind and timestamps but
not size. Internal callers that need to enforce a size limit therefore
have to read the complete file first, which is especially wasteful for
remote filesystems.

This adds the missing internal metadata so consumers can reject
oversized files before transferring or buffering them. The field is
named `size`, matching VS Code's `FileStat.size` filesystem convention.

## What changed

- add `size: u64` to internal `FileMetadata`
- populate it from the underlying filesystem metadata
- carry it through sandbox-helper and remote exec-server responses
- cover files, directories, symlink targets, and sandboxed reads across
local and remote filesystem implementations

The new field is intentionally not exposed through the app-server API.

pakrym-oai e26f734f91 [codex] unify apply patch parsing (#27913)
## Why

`apply_patch` maintained separate batch and streaming parsers for the
same patch grammar. That duplicated the parsing rules and allowed final
execution to disagree with the live streamed preview.

## What changed

- Make `StreamingPatchParser` the single owner of hunk and environment
ID parsing.
- Keep heredoc and outer patch-boundary normalization in the existing
`parse_patch` wrapper, preserving its public API.
- Reject non-whitespace content after `*** End Patch` and preserve
separator handling after `*** End of File`.
- Reject duplicate environment ID preambles explicitly.
- Remove the duplicate batch hunk parser and its implementation-specific
tests.

The change removes 201 net lines while retaining focused coverage for
the unified parser's boundary behavior.

## Validation

- `just test -p codex-apply-patch`
- Compared a 24-hour corpus of 2,788,059 observed `apply_patch` payloads
against the previous batch parser. All 2,779,502 accepted payloads
produced identical hunks, canonical patch text, and environment IDs; the
remaining 8,557 payloads were rejected by both parsers, with zero
acceptance or payload mismatches.

pakrym-oai be338ee9a2 [codex] resolve environment shell metadata eagerly (#27709)
## Why

Turn construction passed resolved environments through several layers
while leaving the environment shell unresolved. As a result,
model-visible environment context could fall back to the session shell
instead of reporting the selected remote environment's shell.

Resolve environment metadata at the turn-context boundary so each turn
carries the shell that belongs to its selected environment. Keep request
validation in app-server, where invalid selections can be returned as
straightforward JSON-RPC errors without coupling core turn construction
to that policy.

## What changed

- resolve environment selections eagerly in
`new_turn_context_from_configuration`
- store the full resolved `Shell` on each `TurnEnvironment`
- simplify the now-redundant resolved-environment constructor plumbing
- keep duplicate and unknown-environment validation as a small
app-server preflight
- add a remote-environment integration test that runs a full
`test_codex` turn and verifies the model-visible environment message
reports `bash`

pakrym-oai 7516eb5c70 Include thread id in token budget context (#27663)
## Why

The token budget full-context fragment identifies the current context
window, but not the thread that owns that window. Including the thread
id makes the initial context-window metadata self-contained, and
`get_context_remaining` also needs to be usable from Code Mode without
forcing callers to parse the model-facing fragment string.

## What changed

- Include the session thread id in the initial `<token_budget>` context
fragment.
- Expose `get_context_remaining` as a Code Mode nested tool while
keeping `new_context` direct-model-only.
- Keep direct model-facing `get_context_remaining` output as the
existing `<token_budget>` text fragment.

pakrym-oai dac5f07403 [codex] Add context remaining tool (#27518)
## Why

The token budget feature can inject remaining-context notices into
model-visible context, but the model does not have a direct way to ask
for that same remaining-token fragment on demand.

This PR adds a small model tool for the token budget feature so the
model can request the current remaining context window message without
duplicating the fragment format.

- Renders the tool output through `TokenBudgetRemainingContext`,
matching the existing budget message shape.

pakrym-oai 87ab01834a [codex] Add new context window tool (#27488)
## Why

The token budget feature tells the model how much room remains in the
current context window. When the model decides the current window is no
longer useful, it needs a way to ask Codex to start over with a fresh
context window without spending tokens on a compaction summary.

This PR adds that model-requestable escape hatch on top of #27438.

## What changed

- Added a direct-model-only `new_context` tool behind
`Feature::TokenBudget`.
- Stores the tool request on `AutoCompactWindow` and consumes it after
sampling so the next follow-up request in the same turn starts in the
new window.
- Starts the new window as a no-summary compaction checkpoint that
contains only fresh initial context, not preserved conversation history.
- Keeps the new window aligned with token-budget startup context,
including the `Current context window Z` message.
- Added integration coverage and a snapshot showing the same-turn
`new_context` flow into a fresh full-context follow-up request.

- Added `<token_budget>` developer fragments for full context-window
metadata and current-window remaining tokens.
- Inserted the threshold message during normal turn handling by
comparing token usage before and after sampling, avoiding persistent

pakrym-oai 30ddb3325e [codex] Store compact window id in rollout (#27264)
## Why

Compaction window identity is part of session history, not model-client
transport state. Persisting it with the compacted rollout item lets
resumed threads continue from the reconstructed window without keeping
mutable window state on `ModelClient`.

## What changed

- Added `window_id` to `CompactedItem` and stamp it when
`replace_compacted_history` installs compacted history.
- Moved auto-compact window id ownership into `AutoCompactWindow` /
`SessionState`; `ModelClient` now receives the request window id from
callers instead of storing it.
- Returned `window_id` from rollout reconstruction for resume.
Reconstruction uses the newest surviving compacted item's stored
`window_id` when present, and falls back to the legacy compacted-item
count when it is absent.
- Kept fork startup at the fresh default window id and updated direct
model-client tests to pass explicit test window ids.

pakrym-oai f2969f36e8 [codex] Handle Ctrl-C for non-TTY unified exec (#26734)
## Why

A long-running unified exec process started with `tty: false` could not
be interrupted via `write_stdin`: ordinary non-TTY stdin writes are
rejected once stdin is closed, but an exact U+0003 payload should still
map to a process interrupt. The interrupt should flow through the same
process lifecycle path as a real signal so Codex preserves
process-reported output and exit metadata instead of fabricating a
Ctrl-C exit code or tearing down the session early.

## What Changed

- Add `process/signal` to exec-server with `ProcessSignal::Interrupt`
and an empty response.
- Add a non-consuming `ProcessHandle::signal` path for spawned
processes; on Unix it sends SIGINT to the process group and leaves
terminate/hard-kill unchanged.
- Route non-TTY U+0003 `write_stdin` through `process.signal(...)`
instead of `terminate`, then let the normal post-write collection path
drain output and observe exit.
- Add exec-server coverage where a shell `trap INT` handler prints the
signal and exits with its own code.
- Add unified exec coverage where a `tty: false` process traps SIGINT,

pakrym-oai 08cb633c06 [codex] Remove remote compaction failure log (#27106)
## Why

`log_remote_compact_failure` was the only consumer of the
compact-request logging payload and most of the token-usage breakdown
fields. Once that failure log is removed, keeping the surrounding
carrier types leaves dead plumbing in the compaction path and context
manager.

## What changed

- Remove `log_remote_compact_failure`, `CompactRequestLogData`, and the
v2 wrapper that only fed that log.
- Let both remote compaction implementations return the original
compaction error directly.

only the remaining value needed by compaction analytics.

pakrym-oai f3c1283411 Pair thread environment settings (#26687)
## Why

Thread cwd and environment selections are a single logical setting in
core: updating one without the other can silently desynchronize the
next-turn execution context. This change makes that relationship
explicit in the internal thread settings flow while preserving the
existing app-server public API shape.

## What changed

- Moved the cwd/environment pair through internal
`ThreadSettingsOverrides.environment_settings` instead of a top-level
internal `cwd` field.
- Kept `thread/settings/update` public params unchanged, with app-server
translating top-level `cwd` into the paired internal settings shape.
- Moved `Op::UserInput` environment overrides into thread settings so
user turns and settings updates use the same core path.
- Updated core, app-server, MCP, memories, sample, and test callsites to
construct the paired settings shape.

pakrym-oai 470c20bf98 protocol: remove submission-side serde from Op (#26674)
## Why

Submission-side `Op` payloads are now an internal handoff inside the
Rust codebase, so keeping a stable serde contract there adds complexity
without a real wire consumer.

## What changed

- remove serde/schema annotations from `Submission`, `Op`, and
submission-only payload types like thread settings overrides, additional
context, realtime conversation params, `TurnEnvironmentSelection`, and
`RequestUserInputResponse`
- delete the `Op` serialization tests and the now-unused double-option
prompt serde helper
- keep event/API-facing serialization where it is still required, and
serialize the `request_user_input` tool output from its wire payload
instead of the core response struct
- update `protocol_v1.md` to call out that events remain the serialized
transport surface while submission payloads are implementation details

pakrym-oai 76c0a5379c Make runtime workspace roots absolute in app-server API (#26552)
Stacked on #26532.

## Why

#26532 moves cwd normalization to the app-server/core boundary.
`runtimeWorkspaceRoots` still accepted raw paths in v2 requests and in
`ConfigOverrides`, which left core responsible for interpreting those
roots later. This makes runtime workspace roots follow the same
absolute-path boundary as cwd.

## What

- Change v2 `runtimeWorkspaceRoots` request fields for `thread/start`,
`thread/resume`, `thread/fork`, and `turn/start` to `AbsolutePathBuf`.
- Deduplicate already-absolute runtime roots in app-server handlers and
pass them through `ConfigOverrides.workspace_roots` as
`AbsolutePathBuf`.
- Update TUI and exec client request builders to pass absolute runtime
roots directly.
- Update app-server docs, schema fixtures, and focused tests for
absolute runtime roots.

pakrym-oai 86a1ddd028 Make turn diff tracker multi-env aware (#26433)
## Why

Turn diffs were tracked as one flat set of absolute paths. In
multi-environment turns, local and remote environments can report the
same path while representing different filesystems, so a single path key
can collapse distinct changes or attribute them to the wrong
environment.

The environment name is **NOT** included in the generated unified diff.
This can come later.

## Why

Some Linux environments expose `bash` at `/usr/bin/bash` instead of

standard locations once PATH/user-shell probing fails.

Stacked on #26480.

## What changed

pakrym-oai 40c8f1a007 Require absolute cwd in thread settings (#26532)
## Why

Thread settings cwd overrides are expected to be resolved before they
enter core. Keeping this boundary as a plain `PathBuf` made it easy for
core/session code to keep fallback normalization and relative-path
resolution logic in places that should only receive an already-resolved
cwd.

This is intentionally the absolute-cwd-only slice: it does not change
environment selection stickiness or cwd-to-default-environment fallback
behavior.

## What changed

- Changes `ThreadSettingsOverrides.cwd`,
`CodexThreadSettingsOverrides.cwd`, and `SessionSettingsUpdate.cwd` to
use `AbsolutePathBuf`.
- Removes core-side cwd normalization/resolution from session settings
updates.
- Updates affected core/app-server test helpers and callsites to pass
existing absolute cwd values or use `abs()` helpers.

pakrym-oai 6a6a5f925e [codex] Add environment shell info (#26480)
## Why

Shell detection needs to be available through the `Environment`
abstraction so callers can ask the selected local or remote environment
for shell metadata without adding a separate HTTP endpoint or parallel
info-source path. This keeps shell metadata shaped like the existing
environment-owned filesystem capability and lets remote environments
answer through exec-server JSON-RPC.

## What changed

- Added `environment/info` to the exec-server protocol/client/server and
exposed `Environment::info()`.
- Added local and remote environment info providers on `Environment`,
following the existing capability-provider pattern used for filesystem
access.
- Moved the shared shell detection logic into `codex-shell-command` and
kept core shell APIs as wrappers around that implementation.
- Returned shell metadata as `EnvironmentInfo { shell: ShellInfo }`
using the existing shell detection path.
- Added a remote environment test that calls `Environment::info()`
through an exec-server-backed environment.

## Why

The Responses websocket client no longer needs to send a follow-up
`response.processed` request after a turn response has already been
recorded. Keeping that extra acknowledgement path adds feature-gated
control flow and a second websocket request shape that no longer carries
useful behavior.

## What Changed

- Removed the `response.processed` websocket request type and sender.
- Removed the `responses_websocket_response_processed` feature flag and
schema entry.
- Removed turn and remote-compaction plumbing that only tracked response
IDs to send the acknowledgement.
- Removed tests that existed solely to cover the deleted feature path.

## Why

The Windows ARM64 Cargo clippy job on `main` is failing because
workspace lints deny `clippy::expect_used`, and the

## What changed

## Why

Remote image submissions currently wrap native `input_image` spans in

## What Changed

- Serialize `UserInput::Image` directly as an `input_image` content

pakrym-oai 768848ab6f Add experimental turn additional context (#24154)
## Summary

Adds experimental `additionalContext` support to `turn/start` and
`turn/steer` so clients can provide ephemeral external context, such as
browser or automation state, without turning that plumbing into a
visible user prompt or triggering user-prompt lifecycle behavior.

## API Shape

The parameter shape is:

```ts
additionalContext?: Record<string, {
  value: string
  kind: "untrusted" | "application"
}> | null
```

Example:

```json
{
  "additionalContext": {
    "browser_info": {
      "value": "Active tab is CI failures.",
      "kind": "untrusted"
    },
    "automation_info": {
      "value": "CI rerun is in progress.",
      "kind": "application"
    }
  }
}
```

The keys are opaque and caller-defined.

## Context Injection

When provided, accepted entries are inserted into model context as
hidden contextual message items, not as visible thread user-message
items.

`kind: "untrusted"` entries are inserted with role `user`:

```text
<external_${key}>${value}</external_${key}>
```

`kind: "application"` entries are inserted with role `developer`:

```text
<${key}>${value}</${key}>
```

Values are not escaped. Each value is truncated to 1k approximate tokens
before wrapping.

For `turn/start`, accepted additional context is inserted before normal
user input. For `turn/steer`, additional context is merged only when the
steer includes non-empty user input; context-only steers still reject as
empty input.

## Dedupe Strategy

`AdditionalContextStore` lives on session state and stores the latest
complete additional-context map.

Each `turn/start` or non-empty `turn/steer` treats its
`additionalContext` as the current complete set of values. Entries are
injected only when the key is new or the exact entry for that key
changed, including `value` or `kind`. After merging, the store is
replaced with the provided map, so omitted keys are removed from the
retained set and can be injected again later if reintroduced.

Omitting `additionalContext`, passing `null`, or passing an empty object
resets the store to empty and injects nothing.

## What Changed

- Threads experimental v2 `additionalContext` through app-server into
core turn start and steer handling.
- Adds separate contextual fragment types for untrusted user-role
context and application developer-role context.
- Uses pending response input items so additional context can be
combined with normal user input without treating it as prompt text.
- Adds integration coverage for start/steer flow, role routing,
dedupe/reset behavior, deletion/re-add behavior, hook-blocked input
behavior, empty context-only steer rejection, external-fragment marker
matching, and truncation.

pakrym-oai ff7513cd83 Move MCP tool naming mode into manager (#21576)
## Why

The `non_prefixed_mcp_tool_names` feature should be applied where MCP
tools become model-visible, not by remapping names later in core.
Keeping the decision in `McpConnectionManager` construction makes
`ToolInfo` the single shaped view that spec building, deferred tool
search, routing, and unavailable-tool placeholders can consume directly.

This also preserves the existing external behavior while the feature is
off, and keeps the feature-on behavior for code mode and hooks explicit
at the manager boundary.

## What Changed

- Add `McpToolNameMode` to `codex-mcp` and flow it through `McpConfig`
into `McpConnectionManager::new`.
- Normalize MCP `ToolInfo` names in the manager using either
legacy-prefixed namespaces or non-prefixed namespaces; the legacy path
adds `mcp__` without restoring the old trailing namespace suffix.
- Remove the core-side MCP name remapping path so specs, tool search,
session resolution, and unavailable-tool placeholder construction use
the manager-provided `ToolName` values directly.
- Keep code mode flattening on the `__` namespace separator.
- Preserve hook compatibility by giving non-prefixed MCP hook names
legacy `mcp__...` matcher aliases.
- Add/adjust integration and unit coverage for non-prefixed code-mode
behavior, hook matching with the feature on and off, and manager-level
legacy prefixing.

pakrym-oai b637fd26aa [codex] Make active turn task singular (#24105)
## Why

`ActiveTurn` already runs at most one task: starting a task requires
that no task is present, and replacement aborts existing work first.
Representing that state as an `IndexMap` leaves a multi-task shape for a
single-task invariant and makes each lifecycle lookup operate like a
collection lookup.

The slot remains optional because goal continuation uses an empty active
turn as a reservation while deciding whether to start continuation work.

## What changed

- Replace `ActiveTurn.tasks` with `task: Option<RunningTask>`.
- Update task abort/completion, session lookup and steering, input-queue
matching, goal reservation, and network-approval lookup to operate on
the singular slot.
- Mutate the singular task slot directly instead of retaining
collection-era add/remove/take helpers.

pakrym-oai 6ad3a83509 [codex] Remove external client session reset plumbing (#24157)
## Why

The turn loop no longer needs to decide when a `ModelClientSession`
should reset its websocket state after compaction. That reset behavior
belongs inside the model client, where the websocket cache and retry
state are owned. The repo guidance now calls this out explicitly so
future changes let the incremental request logic decide whether the
previous request can be reused.

## What Changed

- Removed the `reset_client_session` return value from pre-sampling and
auto-compact helpers in `core/src/session/turn.rs`.
- Changed compaction helpers to return `CodexResult<()>` so callers only
handle success or failure.
- Made `ModelClientSession::reset_websocket_session` private to
`core/src/client.rs`, leaving it callable only from model-client
internals.
- Added `AGENTS.md` guidance not to call `reset_client_session`
unnecessarily.

## Why

The idea here is to erase the difference between initial and followup
inputs to a turn. Followup inputs are already represented as TurnInput.

Eventual goal is not to have explicit on task input at all and pull
everything from input Q.

## What Changed

- Changes `SessionTask::run` and the erased `AnySessionTask::run` path
to accept `Vec<TurnInput>`.
- Wraps user-submitted spawn input as `TurnInput::UserInput` at the
session task start boundary.
- Updates `run_turn` to record initial `TurnInput` using the same hook
and recording path used for pending input.
- Keeps review-specific conversion local to `ReviewTask`, where the

## Why

`code_mode_only_guides_all_tools_search_and_calls_deferred_app_tools`
was failing because code-mode prompt generation used the same nested
tool spec list for both the model-visible `exec` guide and the runtime
`ALL_TOOLS` surface. That allowed deferred MCP/app tools, such as
`calendar_timezone_option_99`, to leak into the `exec` description even
though they should only be discoverable through `ALL_TOOLS` at runtime.

## What changed

Split code-mode nested tool planning into two sets in
`core/src/tools/spec_plan.rs`:

- runtime nested tool specs still include deferred tools, so
`tools[...]` and `ALL_TOOLS` can call them
- `exec` prompt docs only render non-deferred tools, so deferred app

pakrym-oai f0663fd4fd [codex] Preserve steer input as user input (#23405)
## Why

Steered input was queued as a `ResponseInputItem`, then parsed back into
a user message before recording. That path loses information that only
exists on `UserInput`, such as UI text elements.

This change keeps turn-local pending input typed as either original
`UserInput` or existing response items, so steered user input reaches
user-message recording without being reconstructed from a response item.

## What changed

- Add `TurnInput` for active-turn pending input.
- Queue `Session::steer_input` as `TurnInput::UserInput`.
- Run pending-input hook inspection only for `TurnInput::UserInput`.

pakrym-oai 9289b7cea8 [codex] Move hook request plumbing into hook runtime (#23388)
## Why

`run_turn` was still hand-building hook payloads and lifecycle events
for a couple of hook paths. Most hook call sites already delegate
request construction and event emission to `hook_runtime`, which keeps
turn orchestration focused on model-flow decisions rather than hook
plumbing.

This also keeps the legacy `after_agent` message extraction next to the
legacy hook dispatch instead of leaving response-item walking in
`run_turn`.

## What changed

- Added `run_stop_hooks` in `hook_runtime` to build `StopRequest`, emit
preview start events, run the hook, and emit completion events.
- Added `run_legacy_after_agent_hook` in `hook_runtime` to build and
dispatch the legacy `AfterAgent` hook payload, including extracting
input messages from response items.
- Updated `run_turn` to call the hook runtime helpers and keep only the
resulting continuation/block/stop decisions inline.
- Removed the repeated pending session-start hook check from the run
loop.

pakrym-oai ef24ef127f [codex] Allow empty turn/start requests (#23409)
## Why

`turn/start` already accepts an input array on the wire, including an
empty array, but core treated empty input as a no-op before the turn
could reach the model. App-server clients need to be able to start a
real turn even when there is no new user message, for example to let the
model proceed from existing thread context.

## What changed

- Removed the `run_turn` early return that skipped empty-input turns
when there was no pending input.
- Kept empty active-turn steering rejected by moving the `steer_input`
empty-input check until after core has determined whether there is an
active regular turn.
- Empty regular turns now refresh `previous_turn_settings` like other
regular turns, so follow-up context injection state advances
consistently.
- Added an app-server v2 integration test proving `turn/start` with
`input: []` emits started/completed notifications, sends one Responses

pakrym-oai 9e9a62dc28 [codex] Extract turn skill and plugin injections (#23396)
## Why

`run_turn` had accumulated the turn-scoped skill, plugin, app, MCP,
connector-selection, and analytics setup inline. That made the
orchestration path harder to scan even though the actual turn item
injection still needs to stay in `run_turn` so ordering is explicit.

## What changed

This extracts that setup into `build_skills_and_plugins`, which returns
the combined injection `ResponseItem`s and the explicitly enabled
connector IDs. `run_turn` now keeps the required orchestration pieces:
context update recording, user input handling, connector selection
merge, and the explicit per-item `record_conversation_items` calls for
injection items.

The refactor keeps the change LOC-neutral in `core/src/session/turn.rs`
and preserves the existing response-item based injection path.

pakrym-oai 17d552fb4d [codex] Remove external websocket session resets (#23384)
## Why

Compaction now installs replacement history inside the session, but the
turn and compaction callers were still reaching into
`ModelClientSession` to reset websocket transport state after that
install. That made a transport-level reset part of the compaction API
even though websocket incremental request selection already checks
whether the next request is a strict extension of the previous one and
falls back to a full `response.create` when it is not.

## What changed

- Removed the compaction-side calls to `reset_websocket_session` from
`compact.rs` and `session/turn.rs`.
- Simplified pre-sampling and mid-turn compaction helpers so they return

pakrym-oai afa0101ae2 [codex] Move pending input into input queue (#22728)
## Why

Pending model input was split across `Session`, `TurnState`, and the
agent mailbox. That made it easy for new paths to manage queued user
input or mailbox delivery outside the intended ownership boundary.

This PR consolidates the model-facing input lifecycle behind the session
input queue so turn-local pending input, next-turn queued items, and
mailbox delivery coordination are owned in one place.

## What Changed

- Added `session/input_queue.rs` to own pending input queues and mailbox
delivery coordination.
- Removed the standalone `agent/mailbox.rs` channel wrapper and store
mailbox items directly in the input queue.
- Moved pending-input mutations off `TurnState`; `TurnState` now exposes
the queue-owned storage directly for now.
- Routed abort cleanup, mailbox delivery phase changes, next-turn queued
items, and active-turn pending input through `InputQueue`.
- Boxed stack-heavy agent resume/fork startup futures that the refactor
pushed over the default test stack.
- Updated session, task, goal, stream-event, and multi-agent call sites
and tests to use the new queue ownership.

pakrym-oai f2368b7de6 [codex] Trim unused TurnContextItem fields (#22709)
## Why

`TurnContextItem` is the durable baseline used to reconstruct context
diffs across resume/fork. Most of the old persisted-only fields on it
are no longer read, so keeping them in rollout snapshots adds schema
surface and state that can drift without affecting reconstruction.

`summary` is the exception: older Codex versions require it to
deserialize `turn_context` records, so keep writing a default
compatibility value until that schema surface can be removed safely.

## What changed

- Removed the unused persisted fields from `TurnContextItem`: trace ids,
user/developer instructions, output schema, and truncation policy.
- Kept `summary` with a compatibility comment and made
`TurnContext::to_turn_context_item` write `ReasoningSummary::Auto`
instead of live turn state.

pakrym-oai 82061660ae [codex] Remove legacy shell output formatting paths (#22706)
## Why

The client and tool pipeline still carried compatibility code for legacy
structured shell output. Current shell and apply_patch responses are
already plain text for model consumption, so keeping a
JSON-serialization path plus shell-item rewrite logic makes the request
formatter and tests preserve a format we do not need anymore.

## What Changed

- Removed the client-side shell output rewrite from
`core/src/client_common.rs`.
- Removed the structured exec-output formatter and the shell `freeform`
switch so tool emitters use one model-facing formatter.
- Collapsed apply_patch/shell serialization tests around the remaining
plain-text output expectations and removed duplicate one-variant

## Why

The Responses API test support already has structured SSE event

pakrym-oai 3ac1d15598 Use selected environment cwd for filesystem helpers (#22542)
## Why

`TurnContext::cwd` is deprecated in favor of resolving paths from the
selected turn environment cwd. A few filesystem-oriented paths were
still constructing sandbox context from the legacy cwd and then mutating
it afterward, or resolving local file paths through the deprecated
helper.

## What changed

explicitly.
- Pass the selected turn environment cwd directly from `apply_patch` and

## Why

Deprecating both APIs makes any new direct dependency visible while
preserving the existing migration path for current callers.

## What Changed

deprecated with guidance to use the selected turn environment cwd
instead.
- Added exact `#[allow(deprecated)]` suppressions at each existing

pakrym-oai 104fc14956 Encapsulate tool search entries in handlers (#22261)
## Why

This builds on the handler-owned spec refactor by moving deferred
tool-search metadata to the same handlers that already own tool specs.
The registry builder no longer needs a separate prebuilt
`tool_search_entries` path; it can collect searchable entries from
deferred handlers directly.

## What changed

- Added `search_info()` to tool handlers and implemented it for MCP and
dynamic handlers.
- Reused handler `spec()` output when constructing tool-search entries,
adapting it into the deferred `LoadableToolSpec` shape expected by
`tool_search`.
- Simplified `build_tool_registry_builder(...)` so `tool_search`
registration is based on deferred handlers with search info.

Code mode already builds the merged nested `ToolSpec`s that feed the

## Why

`CODEX_RS_SSE_FIXTURE` let integration-style CLI, exec, and TUI tests
bypass the normal Responses transport by reading SSE from local files.
That kept test-only behavior wired through production client code. The
affected tests can stay hermetic by using the existing
`core_test_support::responses` mock server and passing `openai_base_url`

pakrym-oai f11ad1eacb [codex] Add search term coverage for tool_search (#22398)
## Why

`tool_search` already had solid end-to-end coverage for discovery and
follow-up execution, but it did not prove that distinct pieces of
indexed search text actually work in integration. In particular, we were
not exercising whether unique tool names, descriptions, namespaces,
underscore-expanded dynamic names, and schema-property terms were
sufficient to surface the expected deferred tools.

This change adds focused integration coverage for those term sources so
regressions in search text construction are caught by a real `TestCodex`
flow instead of only by lower-level unit tests.

## What changed

- added a small helper in `core/tests/suite/search_tool.rs` to assert
that a `tool_search_output` contains an expected namespace child tool
- added an MCP integration test that issues several `tool_search_call`s
and verifies distinct query terms match the expected app tools:
  - exact tool name: `calendar_timezone_option_99`
  - tool description phrase: `uploaded document`
  - top-level schema property: `starts_at`
- added a dynamic-tool integration test that verifies distinct query
terms match the expected deferred dynamic tool:
  - exact name: `quasar_ping_beacon`
  - underscore-expanded name: `quasar ping beacon`
  - description phrase: `saffron metronome`
  - namespace: `orbit_ops`
  - schema property: `chrono_spec`

pakrym-oai 0173f71143 Refactor namespaced tool spec registration (#22256)
## Summary

This refactor makes tool handlers the owner of the specs they can
publish, so registry construction can register handlers once and
separately publish only the specs that should be model-visible.

The main motivation is deferred tools: MCP and dynamic tools still need
handlers registered up front, but deferred tools should be discoverable
through `tool_search` rather than emitted in the initial tool spec list.

## What changed

- `McpHandler` and `DynamicToolHandler` can return their own `ToolSpec`.
- `build_tool_registry_builder` now collects handlers, registers them
through the no-spec path, and publishes only non-deferred handler specs.
- Deferred MCP and dynamic tool names are combined into one
`all_deferred_tools` set that drives spec filtering, code-mode
deferred-tool signaling, and `tool_search` registration.
- `tool_search` registration now requires both deferred tools and
`namespace_tools`.
- Namespace specs are merged in `spec_plan`, preserving top-level spec
order, sorting tools within each namespace, and backfilling empty
namespace descriptions.
- Hosted web search and image-generation specs are included in the
collected spec vector before namespace merge/publication, and tool-name
tests that should not care about hosted relative order now compare sets.

pakrym-oai 960d42ddae code-mode: carry nested tool kind through runtime (#22377)
## Why

Code mode only used nested spec lookup at execution time to rediscover
whether a nested tool should be invoked as a function tool or a freeform
tool.

That information is already present in the enabled tool metadata that
code mode builds to expose `tools.*` and `ALL_TOOLS`, so re-looking it
up from the router was redundant and kept execution coupled to a
separate spec lookup path.

## What Changed

- thread `CodeModeToolKind` through the code-mode runtime `ToolCall`
event and `CodeModeNestedToolCall`
- emit the nested tool kind directly from the V8 callback using the
already-enabled tool metadata
- build nested tool payloads from the propagated kind instead of calling
`find_spec`
- remove the now-unused `find_spec` plumbing from the router and
parallel runtime helpers

pakrym-oai 862b2122ee tools: remove is_mutating dispatch gating (#22382)
## Why

Tool dispatch had two serialization mechanisms:

- `supports_parallel_tool_calls` decides whether a tool participates in
the shared parallel-execution lock.
- `is_mutating` separately gated some calls inside dispatch.

That second hook no longer carried its weight. The remaining
parallel-support flag is already the per-tool concurrency policy, so
keeping a second mutating gate made dispatch harder to follow and left
behind extra session plumbing that only existed for that path.

## What changed

- Removed `is_mutating` from tool handlers and deleted the
`tool_call_gate` path that existed only to support it.
- Simplified dispatch and routing to rely on the existing per-tool
`supports_parallel_tool_calls` boolean.
- Dropped the now-unused handler overrides and related session/test
scaffolding.
- Kept the router/parallel tests focused on the surviving per-tool
behavior.

pakrym-oai c9e46ed639 [codex] Make handlers own parallel tool support (#22254)
## Why

`ToolRouter::tool_supports_parallel()` was still consulting configured
specs when a handler lookup missed, even though parallel schedulability
is really a property of the executable handler. Keeping that metadata on
`ConfiguredToolSpec` duplicated state between the model-visible spec
layer and the runtime handler layer.

This change makes handlers the sole source of truth for parallel tool
support and removes the extra spec wrapper that only existed to carry
duplicated metadata.

## What changed

- removed `ConfiguredToolSpec` and store plain `ToolSpec` values in the
registry/router builder path
- changed `ToolRouter::tool_supports_parallel()` to consult only the
handler registry and fall back to `false`
- simplified spec collection and test helpers to operate directly on

pakrym-oai 79c65f816c [codex] Filter legacy warning messages during compaction (#22243)
## Why

Older sessions can contain model-warning records persisted as `user`
messages, including the unified exec process-limit warning, the
`apply_patch`-via-`exec_command` warning, and the model-mismatch
high-risk cyber fallback warning. Those warnings are no longer produced
as conversation history items, but when old sessions compact they should
still be recognized as injected context rather than preserved as real
user turns.

## What changed

- Removed `record_model_warning` and the production paths that emitted
these warning messages into conversation history.
- Added `LegacyUnifiedExecProcessLimitWarning`,
`LegacyApplyPatchExecCommandWarning`, and `LegacyModelMismatchWarning`
contextual fragments that are used only for matching old persisted
messages.
- Registered the legacy fragments with contextual user message detection
so compaction filters them through the existing fragment path.

pakrym-oai ed5944ba1d Simplify MCP tool handler plumbing (#21595)
## Why
The MCP tool path had accumulated a few core-owned special cases: a
dedicated payload variant, resolver plumbing, a legacy `AfterToolUse`
translation path, and a side channel for parallel-call metadata. That
made `ToolRegistry` and the spec builder know more about MCP than they
needed to.

This change moves MCP-specific execution details back onto `ToolInfo`
and `McpHandler` so `codex-core` can treat MCP calls like normal
function calls while still preserving MCP-specific dispatch and
telemetry behavior where it belongs.

## What changed
- removed `resolve_mcp_tool_info`, `ToolPayload::Mcp`, `ToolKind`, and
the remaining registry-side MCP resolver path
- stored MCP routing metadata directly on `McpHandler` and `ToolInfo`,
including `supports_parallel_tool_calls`
- deleted the legacy `AfterToolUse` consumer in `core`, which removes
the need for handler-specific `after_tool_use_payload` implementations
- switched tool-result telemetry to handler-provided tags and kept
MCP-specific dispatch payload construction inside the handler
- simplified tool spec planning/building by passing `ToolInfo` directly
and dropping the direct/deferred MCP wrapper structs and the
parallel-server side table

pakrym-oai c579da41b1 Move file watcher out of core (#21290)
## Why

The app-server watcher relocation leaves the generic filesystem watcher
as the last watcher-specific implementation still living inside
`codex-core`. Moving that code to a small crate keeps `codex-core`
focused on thread execution and lets app-server depend on the watcher
without reaching back into core for filesystem watching primitives.

This PR is stacked on #21287.

harder to reason about.

## What changed

- Removed the `ApplyPatchToolType::Function` variant, JSON `apply_patch`
spec, and handler support for function payloads.
- Kept `apply_patch_tool_type = freeform` as the supported model
metadata path, including Bedrock catalog metadata.
- Migrated `apply_patch` tests and SSE fixtures to custom/freeform tool

pakrym-oai 61142b6169 Remove ToolName display helper (#21465)
## Why

`ToolName::display()` made it too easy to flatten tool identity and
accidentally compare rendered strings. Tool identity should stay
structural until a legacy string boundary actually requires the
flattened spelling.

## What

- Removes `ToolName::display()` and relies on the existing `Display`
impl for messages and errors.
- Adds structural ordering for `ToolName` and uses it for
sorting/deduping deferred tools.
- Carries `ToolName` through tool/sandbox plumbing, flattening only at
legacy boundaries such as hook payloads, telemetry tags, and Responses
tool names.
- Updates MCP normalization tests to assert `ToolName` structure instead
of rendered strings.

pakrym-oai dfa1e864a2 Send response.processed after remote compaction v2 (#21642)
## Why

Remote compaction v2 consumes a normal Responses stream, but that
compaction-specific stream consumer dropped the `response.completed` id.
As a result, the `responses_websocket_response_processed` lifecycle
notification was emitted for normal turn sampling but not after a v2
remote compaction response was fully processed.

## What changed

- Return the completed response id alongside the v2 `context_compaction`
output item.
- After v2 compacted history is installed, send `response.processed`
through the same websocket session when the feature is enabled.
- Add websocket regression coverage for a remote compaction v2 request

pakrym-oai 566f2cb612 [codex] Move tool specs onto handlers (#21461)
## Why

This is the next stacked step after deleting the tool-handler kind
indirection. Specs should come from the registered handlers themselves
so registry construction has a single source of truth for handler
behavior and exposed tool definitions.

## What changed

- Added `ToolHandler::spec()` plus handler-provided parallel/code-mode
metadata, and made `ToolRegistryBuilder::register_handler` automatically
collect specs from registered handlers.
- Moved builtin tool spec construction into the corresponding handlers
and their adjacent `_spec` modules, including shell, unified exec, apply
patch, view image, request plugin install, tool search, MCP resource,
goals, planning, permissions, agent jobs, and multi-agent tools.
- Reworked configurable handlers to receive their tool-building options
through constructors, with non-optional handler options where the
handler is always spec-backed. Shell fallback handlers keep an explicit
no-spec mode because they are also registered as hidden dispatch
aliases.
- Kept `CodeModeExecuteHandler` on the explicit configured wrapper so
the code-mode exec spec can still be built from the nested registry.

pakrym-oai 857e731478 [codex] Remove string-keyed MCP tool maps (#21454)
## Summary

This PR removes the synthetic `HashMap<String, ToolInfo>` keys from MCP
tool discovery. `McpConnectionManager::list_all_tools()` now returns
normalized `Vec<ToolInfo>`, and downstream code derives identity from
`ToolInfo::canonical_tool_name()`.

The motivation is to keep model-visible tool identity on
`ToolName`/`ToolInfo` instead of parallel string map keys, so future
namespace changes do not have to preserve otherwise-unused lookup keys.

## Changes

- Rename the MCP normalization path from `qualify_tools` to
`normalize_tools_for_model` and return tool values directly.
- Flow MCP tool lists through connectors, plugin injection, router/spec
building, code mode, and tool search as vectors/slices.
- Keep direct/deferred subtraction local to `mcp_tool_exposure`, using
`ToolName` values.
- Update tests to compare `ToolName` instances where MCP identity
matters.

## Why

Feedback uploads already carry auth-derived context like

## Why

Reverts #20689 to restore the previous optional state DB plumbing. The
conflict resolution keeps the newer installation ID and session/thread
identity changes that landed after #20689, while removing the mandatory
state DB and agent graph store dependency from ThreadManager
construction.

## What changed

- Restored `Option<StateDbHandle>` through app-server, MCP server,
prompt debug, and test entry points.
- Removed the `codex-core` dependency on `codex-agent-graph-store` and
reverted descendant lookup back to the existing state DB path when

## Why

The spec split in the parent PR still left an intermediate registry plan
that recorded `ToolHandlerKind` values and translated them into concrete

pakrym-oai 9417cf9696 [codex] Move tool specs into core handlers (#21416)
## Why

This is the first mechanical slice of moving tool spec ownership toward
the handlers. `codex-tools` should keep shared primitives and conversion
helpers, while builtin tool specs and registration planning live in
`codex-core` with the handlers that own those tools.

Keeping this PR to relocation and import updates isolates the copy/move
review from the later logic change that wires specs through registered
handlers.

## What changed

- Moved builtin tool spec constructors from `codex-rs/tools/src` into
`codex-rs/core/src/tools/handlers/*_spec.rs` or nearby core tool
modules.
- Moved the registry planning code into
`codex-rs/core/src/tools/spec_plan.rs` and its associated types/tests
into core.
- Kept shared primitives in `codex-tools`, including `ToolSpec`,
schema/types, discovery/config primitives, dynamic/MCP conversion
helpers, and code-mode collection helpers.
- Updated handlers that referenced moved argument types or tool-name
constants to use the core spec modules.
- Moved spec tests next to the moved spec modules.

pakrym-oai d5eea229cc Move skills watcher to app-server (#21287)
## Why

Skills update notifications are app-server API behavior, but the watcher
lived in `codex-core` and surfaced through
`EventMsg::SkillsUpdateAvailable`. Moving the watcher out keeps core
focused on thread execution and lets app-server own both cache
invalidation and the `skills/changed` notification.

## What changed

- Added an app-server-owned skills watcher that watches local skill
roots, clears the shared skills cache, and emits `skills/changed`
directly.
- Registers skill watches from the common app-server thread listener
attach path, including direct starts, resumes, and app-server-observed
child or forked threads.
- Stores the `WatchRegistration` on `ThreadState`, so listener
replacement, thread teardown, idle unload, and app-server shutdown
deregister by dropping the RAII guard.
- Removed `EventMsg::SkillsUpdateAvailable`, the core watcher, and the
old core live-reload test.
- Extended the app-server skills change test to verify a cached skills
list is refreshed after a filesystem change without forcing reload.

## Why

Several tool handler modules still bundled multiple `ToolHandler`
implementations in one file. That made the handler directory harder to
navigate and made otherwise local handler edits land in large shared
modules.

## What

- Split grouped tool handlers into one handler file each for agent jobs,
goals, MCP resources, shell tools, and unified exec.
- Kept shared parsing, payload, and runtime helpers in the existing
parent modules, with re-exports preserving the existing handler import

pakrym-oai 2004173cd7 Move message history out of core (#21278)
## Why

Message history was implemented inside `codex-core` and surfaced through
core protocol ops and `SessionConfiguredEvent` fields even though the
current consumer is TUI-local prompt recall. That made core own UI
history persistence and exposed `history_log_id` / `history_entry_count`
through surfaces that app-server and other clients do not need.

This change moves message history persistence out of core and keeps the
recall plumbing local to the TUI.

## What changed

- Added a new `codex-message-history` crate for appending, looking up,
trimming, and reading metadata from `history.jsonl`.
- Removed core protocol history ops/events: `AddToHistory`,
`GetHistoryEntryRequest`, and `GetHistoryEntryResponse`.
- Removed `history_log_id` and `history_entry_count` from

## Why

Thread names are app-server metadata now, backed by the thread store and
sqlite state database. Keeping a core `SetThreadName` op plus a rollout
`thread_name_updated` event made rename persistence live in the wrong
layer and required historical replay support for an event that new
app-server flows should not write.

## What changed

- Removed `Op::SetThreadName` and `EventMsg::ThreadNameUpdated` from the
core protocol and deleted the core handler path that appended rename
events to rollouts.
- Updated app-server `thread/name/set` so both loaded and unloaded
threads write through thread-store metadata and app-server emits
`thread/name/updated` notifications.

pakrym-oai f593323ef1 [codex] Split tool handlers by tool name (#20687)
## Why

Tool registration used to bind a tool name to a handler externally,
which left ownership split between the registry plan and the handler
implementation. Some built-in handlers also multiplexed multiple in-core
tools by switching on the invoked tool name internally.

This moves the registry identity onto the handler itself and makes
built-in multi-tool areas use separate concrete handlers, so each
registered handler instance owns exactly one tool name and one dispatch
path.

## What Changed

- Added `ToolHandler::tool_name()` and changed
`ToolRegistryBuilder::register_handler` to derive the registry key from
the handler.
- Split built-in multiplexed handlers into concrete per-tool handlers
for unified exec, shell/local shell/container exec, MCP resources, goal
tools, and agent job tools.
- Kept name-carrying handler instances only where the runtime target is
inherently external or dynamic, such as MCP tools, dynamic tools, and

pakrym-oai b6d4c4ea6b [codex] Use shared app-server JSON-RPC error helpers (#21221)
## Why

App-server had repeated hand-built JSON-RPC error objects for standard
error shapes. Using the shared helpers keeps the common
`invalid_request`, `invalid_params`, and `internal_error` construction
in one place and reduces the chance of new call sites drifting from the
common error payload shape.

## What changed

- Replaced manual standard JSON-RPC error object creation with
`internal_error(...)`, `invalid_request(...)`, and `invalid_params(...)`
across app-server request processors and runtime paths.
- Removed local duplicate helper definitions from search and review
request handling.
- Preserved existing structured `data` payloads by creating the shared
helper error first and then attaching the existing metadata.
- Left custom non-standard errors and raw error-code assertions intact.

pakrym-oai 33b19bcfde [codex] Split app-server request processors (#20940)
## Why

The app-server request path had grown around a large
`CodexMessageProcessor` plus separate API wrapper/helper modules. That
made the dependency graph hard to see and forced unrelated request
families to share broad processor state.

This PR makes the split mechanical and command-prefix oriented so
request families own only the dependencies they use.

## What changed

- Replaced `CodexMessageProcessor` with command-prefix request
processors under `app-server/src/request_processors/`.
- Removed the old config, device-key, external-agent-config, and fs API
wrapper files by moving their API handling into processors.
- Split apps, plugins, marketplace, catalog, account, MCP, command exec,
fs, git, feedback, thread, turn, thread goals, and Windows sandbox
handling into dedicated processors.
- Kept shared lifecycle, summary conversion, token usage replay, and
shared error mapping only where multiple processors use them; single-use
helpers were inlined into their owning processor.
- Removed the fallback processor path and moved processor tests to
`_tests` files.

pakrym-oai c8c30d9d75 [codex] Emit MCP tool calls as turn items (#20677)
## Why

`McpToolCall` was still an app-server item synthesized from deprecated
legacy begin/end events. Recent item migrations moved this ownership
into core `TurnItem`s, so MCP tool calls now follow the same canonical
lifecycle and leave legacy events as compatibility fanout.

Keeping the core item close to the v2 `ThreadItem::McpToolCall` shape
also avoids spreading MCP result semantics across app-server conversion
code. Core now owns whether a completed call is `completed` or `failed`,
and whether the payload is a tool result or an error.

## What changed

- Added core `TurnItem::McpToolCall` with flattened `server`, `tool`,
`arguments`, `status`, `result`, and `error` fields.
- Updated MCP tool call emitters, including MCP resource tools, to emit
`ItemStarted`/`ItemCompleted` around directly constructed core MCP
items.
- Updated app-server v2 conversion to project the core MCP item into
`ThreadItem::McpToolCall` without deriving status or splitting `Result`
locally.

duplicate item notifications, while keeping thread history replay on the
legacy event path.

## Why

App-server request handling had response sending spread across many

## What changed

- Kept explicit-response methods explicit where they need custom

- Removed forward-only handler wrappers and immediate `async { ...

## Why

We saw Responses websocket sessions recover only after a long quiet

the write completed. If the socket write/pump stalls, the client can sit
in `ws_stream.send(...)` without reaching the existing receive timeout.

pakrym-oai aed74e5ee4 [codex] Emit image view as core item (#20512)
## Why

Image-view results should be represented as a core-produced turn item
instead of being reconstructed by app-server. At the same time, existing
rollout/history paths still understand the legacy `ViewImageToolCall`
event, so this keeps that event as compatibility output generated from
the new item lifecycle.

## What changed

- Added `TurnItem::ImageView` to `codex-protocol`.
- Emitted image-view item start/completion directly from the core
`view_image` handler.
- Kept `ViewImageToolCall` as a legacy event and generate it from
completed `TurnItem::ImageView` items.
- Kept `thread_history.rs` on the legacy `ViewImageToolCall` replay
path, with `ImageView` item lifecycle events ignored there.
- Updated app-server protocol conversion, rollout persistence, and
affected exhaustive event matches for the new item plus legacy fan-out
shape.

## Verification

- `cargo test -p codex-protocol -p codex-app-server-protocol -p
codex-rollout -p codex-rollout-trace -p codex-mcp-server -p
codex-app-server --lib`
- `cargo test -p codex-core --test all
view_image_tool_attaches_local_image`
- `just fix -p codex-protocol -p codex-core -p codex-app-server-protocol
-p codex-app-server -p codex-rollout -p codex-rollout-trace -p
codex-mcp-server`
- `git diff --check`

pakrym-oai 9b8d585075 [codex] Add Codex environment config (#20630)
## Why

This adds a checked-in Codex environment configuration so the repo
exposes a ready-to-run Codex action from the app environment metadata.

## What changed

- Added `.codex/environments/environment.toml` with a generated `Run`
action.
- The action runs the `codex` binary from `codex-rs/Cargo.toml` with
`mcp_oauth_credentials_store=file`.

## Verification

- Not run; configuration-only change.
