Add direct Guardian review of recorded exec calls (#58028)

## What changed

Add `CodexThread::guardian_review` to assess the final injected `exec_command` call against recorded history without executing the command. Reuse command preparation and approval policy, returning the call ID and an optional assessment when approval is required.

Reject active threads, unsupported calls, commands intercepted by `apply_patch`, and histories containing `request_permissions`. Honor cancellation and the configured review timeout.

## Testing

Add a scenario test and request snapshot verifying that direct review matches live Guardian input, leaves commands unexecuted, skips assessment for explicitly allowed commands, and rejects trailing tool output and recorded permission grants.
