ioanbudea-OAI 2889d8e1bf Normalize turn context permissions to a required profile (#59963 )

## What changed

Make `TurnContextItem.permission_profile` required and remove its legacy `sandbox_policy` and `file_system_sandbox_policy` fields. Normalize older histories into a permission profile during deserialization, preserving the restrictive fallback for malformed filesystem rules.

Keep legacy compatibility fields in rollout serialization so older readers remain usable. Update turn recording, metadata extraction, and permission checks to consume the profile directly.

## Testing

Add coverage for legacy normalization, profile-only serialization, missing permissions, and malformed filesystem rules. Extend rollout round-trip fixtures to cover deny rules and filesystem roots from another platform.

codex-protocol

This crate defines the "types" for the protocol used by Codex CLI, which includes both "internal types" for communication between codex-core and codex-tui, as well as "external types" used with codex app-server.

This crate should have minimal dependencies.

Ideally, we should avoid "material business logic" in this crate, as we can always introduce Ext-style traits to add functionality to types in other crates.
