jif 75667b341a Move Guardian approval request types into the reviewer crate (#57478) ...

## What changed

Export `GuardianApprovalRequest`, `GuardianNetworkAccessTrigger`, and
`GuardianMcpAnnotations` from `codex-guardian-reviewer` and re-export them in
core. Move environment lookup and scope classification with the request type,
preserving existing classification and credential-safe debug output.
