## What changed

Record `codex.tools.incremental_updates` through session telemetry with an `action` tag for `added`, `removed`, and `schema_changed`. Definition changes include description edits.

Exclude initial catalogs, unchanged tools, and namespace headers from the counts. Count each removed namespace member even when the removal notice lists only the namespace.

## Testing

Extend the namespace transition snapshot test with in-memory metric assertions covering additions, removals, definition changes, and excluded initial or unchanged catalogs.
