Eric Traut bf832f4678 Allow reasoning shortcuts to reach Max (#48116) ...

## Why

Reasoning shortcuts stopped before Max even when the active model supported it, requiring users to open the advanced-reasoning picker.

## What changed

Allow `Alt+.` and `Shift+Up` to raise reasoning to Max in default and Plan modes. Keep Ultra behind the explicit picker and update the guidance to mention only Ultra.

## Testing

Add coverage for both shortcuts reaching Max in both modes, with footer snapshots. Update the Ultra boundary test to verify that raising from Max shows picker guidance without changing reasoning effort.
