## Why

Explicit cyber access program selections were dropped for requests using `CodexAuth::Headers`, preventing the server from evaluating the requested program.

## What changed

Forward explicit program selections for header-authenticated requests, leaving entitlement decisions to the server.

## Testing

Add integration tests covering explicit selections and omission over HTTP and a reused WebSocket connection, preservation of actor and account headers, and propagation of backend HTTP 403 denials without dropping the selected program.
