jif aa905bb962 Store security risk scores as a snapshot (#38567 )

## What changed

- Replace the single `category` and `score` fields in `SecurityRiskScore` with a
  deterministic map of category names to scores.
- Validate the full Guardian V2 classifier response before storing it as one
  thread extension value and, for non-ephemeral threads, one rollout item.
- Cover serialization and rollout loading with snapshots containing multiple
  score categories.

codex-protocol

This crate defines the "types" for the protocol used by Codex CLI, which includes both "internal types" for communication between codex-core and codex-tui, as well as "external types" used with codex app-server.

This crate should have minimal dependencies.

Ideally, we should avoid "material business logic" in this crate, as we can always introduce Ext-style traits to add functionality to types in other crates.
