## What changed

Remove snapshot instruction deduplication, separate retained-assistant framing, and the associated omission-metadata special cases. Use `retain_new_instructions` in transcript rendering tests and replace the test-only Decisions snapshot adapter with the conversation adapter.

## Testing

Extend retained-source delivery and incomplete-metadata coverage to both synchronous and asynchronous context profiles. Keep Decisions HTTP, eviction, and cancellation tests exercising the conversation adapter.
