## What changed

Mark `code_mode_only_strict_3p_tools` as removed and stop overriding MCP/app and dynamic tool exposure settings. Code Mode Only now honors `omit_tools_from`, namespace exclusions, direct-only namespaces, and dynamic tool deferral settings without strict-mode exceptions.

## Testing

Update coverage for exposure settings, hidden and policy-denied tools, app access restrictions, and deferred MCP catalog refreshes that preserve the cached prefix while `exec` uses the current tools.
