Felipe Coury 673827448d feat(core): generate default session titles ...

Generate a short title after the first accepted user prompt and store it
as the session `thread_name`. The title uses a small auxiliary model by
default, runs in the background, and shares the same persistence path as
`/rename` so a manual rename still wins.

Add `[session_titles]` config for disabling the feature or selecting a
custom title model, and include the generated schema update.
