## What changed

Record `codex.thread.tools.namespaces_total` and `codex.thread.tools.fragment_bytes` histograms through session telemetry. Tag samples by `stage` (`before` or `after` the 4 KiB cap) and `kind` (`snapshot` or `delta`). Measure bytes after description normalization and XML escaping, including the `<tools>` wrapper, with explicit fragment-size buckets.

Delta counts include only changed entries, and unchanged state emits no samples.

## Testing

Extend rendering tests to verify snapshot and delta metrics, omission counts, byte sizes with Unicode and XML escaping, and no samples for unchanged state.
