jif 0ae02915bd Add request-scoped Guardian approval decisions (#42807 )

## What changed

- Extend `ApprovalReviewContributor` with a `decide` hook that can allow an
  action, return a synchronous review result, or hand the request to the user.
- Pass contributors the request's approval state and a callback bound to the
  existing synchronous reviewer. Review results satisfy the review gate while
  the host continues to enforce permissions.
- Add structured reasons for requesting a fresh Guardian review and select the
  first contributor decision in registration order.

codex-protocol

This crate defines the "types" for the protocol used by Codex CLI, which includes both "internal types" for communication between codex-core and codex-tui, as well as "external types" used with codex app-server.

This crate should have minimal dependencies.

Ideally, we should avoid "material business logic" in this crate, as we can always introduce Ext-style traits to add functionality to types in other crates.
