rhan-oai 68c6e5df2b Support model catalog templates for Guardian policy prompts (#33177 )

## What changed

- Add an optional `policy_template` field to auto-review model messages.
- Build Guardian instructions from the catalog template when present, while
  preserving the existing precedence for managed and catalog policy content.
- Fall back independently to the bundled template and bundled policy, and
  preserve explicit empty catalog values.

## Testing

- Cover missing and empty template deserialization, policy precedence, bundled
  fallbacks, and Guardian prewarm instructions.

codex-protocol

This crate defines the "types" for the protocol used by Codex CLI, which includes both "internal types" for communication between codex-core and codex-tui, as well as "external types" used with codex app-server.

This crate should have minimal dependencies.

Ideally, we should avoid "material business logic" in this crate, as we can always introduce Ext-style traits to add functionality to types in other crates.
