## What changed

Add an output schema for `image_gen.imagegen` so its code-mode declaration returns `Promise<{ image_url: string; output_hint?: string; }>`. Direct API tool serialization remains unchanged.

## Testing

Add coverage for the generated return type and unchanged direct API serialization, assert the full declaration in the app-server integration test, and verify that code-mode results omit oversized output hints.
