ioanbudea-OAI 1f281290e3 Centralize Guardian root context preparation (#57312)
## What changed

Move root message preparation and handoff context selection into `codex-guardian-context`. Keep source collection and handoff boundary detection in core, while shared helpers handle text limits, whole-record verified answer admission, delivered assistant message lookup, and retained assistant fallback.
