jif 55b1c642f5 Split MCP direct-mode exposure coverage into separate test cases (#59691) ...

Give each independent MCP server and session setup its own test identity
and timeout budget by replacing nested loops with `test_case::test_matrix`.
Preserve all 64 configuration combinations and the existing assertions for
direct tool exposure, tool search availability, and advertised tool calls.
