## Why
Callers need to await acceptance or rejection of thread settings without consuming the thread event stream. Acceptance must precede event delivery because the caller may hold a lock needed by the event consumer.
## What changed
- Add `CodexThread::update_thread_settings` and re-export `ThreadSettingsOverrides` from `core-api`.
- Add an optional reply channel to `Op::ThreadSettings`. Return validation failures as `InvalidRequest` through that channel, retaining error events for callers without a reply channel.
- Reply to successful updates before emitting `ThreadSettingsApplied`, while holding the settings persistence lock through notification.
## Testing
Extend settings constraint coverage to direct replies, including rejection without an error event and successful notification. Verify that acceptance and continuation invalidation precede blocked event delivery, while checkpointing waits for notification.

## Why
Fixed schema budgets can strip parameter descriptions from large MCP tools or render their Code Mode input types as `unknown`. Allow larger budgets to preserve that guidance.
## What changed
- Add `mcp_servers.<name>.tool_input_schema_max_bytes` to configure the UTF-8 byte threshold for best-effort input schema compaction, retaining the 5,000-byte default.
- Add `features.code_mode.tool_input_schema_max_bytes` for rendered input types. Use the largest of the 16,000-byte default, the Code Mode setting, and the ordinary MCP server's explicit setting.
- Apply rendering budgets to prompt declarations and the runtime tool catalog, and avoid reusing cached declarations after the Code Mode budget changes.
## Testing
Add coverage for UTF-8 compaction thresholds, budget precedence, configuration round trips, and preservation of parameter guidance in both the Code Mode prompt and `ALL_TOOLS`.

## Why
Required MCP servers can block startup while establishing a live connection even when a usable tool catalog is already cached.
## What changed
- Add per-server `startup_readiness = "catalog"` to expose cached tools while the connection starts, retaining `"connection"` as the default.
- Allow an eligible cached catalog to satisfy required-server startup validation and model binding capture. Explicit server or plugin requirements still wait for a live connection, and tool execution still requires the current connection.
- Check tool filters and model visibility when accepting cached catalogs, and preserve cached catalog revisions for binding reuse.
- Serialize the new setting, expose it in the configuration schema, and trace startup readiness at validation and model binding gates.

Remove the basic-session restriction from the early return in
`maybe_run_previous_model_inline_compact`. When the model is unchanged and
no compaction compatibility hash change is detected, all session sources now
skip rebuilding the previous model's turn context and checking its limits.

## Why
Saved model selections can outlive their catalog entries. Removing GPT-5.4 needs to preserve a migration path without applying it to other providers that may still support the same model slug.
## What changed
- Remove `gpt-5.4` from the bundled catalog and `openai.gpt-5.4` from the Bedrock catalogs, including GovCloud.
- Add fallback migration prompts to GPT-6 Sol for saved GPT-5.4 selections on OpenAI and Amazon Bedrock. Scope fallback migrations, including the existing GPT-5.4 Mini migration, to their owning providers while honoring catalog metadata.
- Update tests that depend on bundled GPT-5.4 metadata to use GPT-5.5 or explicit model overrides.
## Testing
Extend migration tests to cover catalog metadata precedence, acknowledged targets, missing or hidden replacements, and preserving the provider while saving the replacement model and its default reasoning effort. Update Bedrock catalog assertions and picker snapshots.

## What changed
- Export `ProjectTrustPath` and make `ProjectTrustLookup::from_paths` accept an explicit working directory and optional repository root instead of an arbitrary sequence of path pairs.
- Defer repository-root canonicalization until neither working-directory key matches, avoiding unnecessary filesystem access while preserving lookup precedence.
## Testing
Extend the active-project regression test to cover repository-root fallback and ensure a working-directory entry without a trust level takes precedence over a trusted root. Adapt the existing POSIX and Windows lookup tests to the new API.

## Why
Expanded recursive deny lists can exceed the Windows command-line limit, preventing sandbox wrappers and setup helpers from launching.
## What changed
- Use chunked environment transport for wrapper and non-elevating setup payloads above 24,000 UTF-16 code units, preserving the existing argument protocol for smaller requests.
- Share the transport with the MXC launcher, supporting payloads up to 16 MiB with Unicode-safe chunking and validation.
- Keep transport variables out of the workload environment and remove inherited chunks before launching setup helpers.
## Testing
Add coverage for transport boundaries, Unicode round trips, malformed payload rejection, environment cleanup, and large payloads crossing Windows process creation. Extend the elevated sandbox integration test with 1,000 nested denied files to verify read restrictions and `AGENTS.md` discovery with a payload exceeding the command-line limit.

## Why
Agent messages arriving during a response can interrupt sampling at reasoning or commentary boundaries, cutting off remaining tool calls.
## What changed
Add the disabled-by-default `defer_mailbox_preemption` feature flag. When enabled, sampling continues past these boundaries and queued agent messages reach the next model request alongside completed tool results.
## Testing
Add streaming scenarios and snapshots for reasoning and commentary boundaries with deferral enabled and disabled. Verify that deferral preserves the planned tool call and that both queued messages arrive in order in the next request.

## What changed
Add `tui.prompt_suggestions`, defaulting to `false`, along with suggestion request state, a generation prompt, and strict parsing of single-line suggestions up to 240 characters. Suggestion generation is not connected yet.
Keep suggestions outside the draft and render them dimly in an empty, focused composer. `Tab` accepts a suggestion for editing, a separate `Enter` submits, and `Escape` cancels pending suggestions. Respect active key bindings, popups, attachments, paste bursts, and interactive transcript footers; ignore stale results and cancel suggestions when a task starts.
## Testing
Add parser tests, composer interaction tests, and snapshots covering acceptance, dismissal, stale results, input precedence, wrapping, and transcript search layout.

## What changed
- Make `LocalThreadStore` default to paginated history while retaining explicit legacy mode.
- Explicitly use paginated history for Guardian review sessions and allow in-memory stores to create paginated threads.
- Rename full-history resume, fork, and subagent helpers to identify them as legacy APIs, and document the paginated alternatives.
- Reject `thread/revert` for ephemeral threads and prevent the TUI from replaying or attaching to ephemeral agent threads.
## Testing
Add coverage for preserving the default and explicitly selected history modes and conversation context across restart, and for retaining context after rejected ephemeral reverts. Extend Guardian checkpoint coverage across both history modes and verify that saved reviews use paginated history.

## Why
The `attribution_error` status does not explain why MCP attribution is incomplete. Preserve a diagnostic reason so callers can distinguish history and recorder errors from request serialization failures.
## What changed
- Add optional `error_reason` diagnostics to `McpAttribution`, retaining the first recorder error through checkpoints, compaction, resume, and fork.
- Identify missing or invalid checkpoints, conflicting sources, invalid sources, and poisoned recorder state. Use `restored_error_unknown` for older error checkpoints without a reason.
- Distinguish `payload_too_large` from `serialization_failed` in request metadata fallbacks.
- Tolerate missing, unknown, or malformed diagnostic reasons when reading history, without letting diagnostics determine attribution status.
## Testing
Add coverage for reason preservation, recorder poisoning, backward-compatible checkpoint decoding, resume and cold-fork restoration, and request metadata serialization, including oversized payloads.

## Why
Treating an omitted `allow_local_binding` setting as `false` prevents environment policies from inheriting local-binding permissions.
## What changed
Keep `allow_local_binding` optional when composing controller and environment policies. Preserve explicit denials from either policy and resolve executor defaults after composition. Without a controller, default to denying local binding unless the environment explicitly allows it.
## Testing
Add coverage for omitted, allowed, and denied settings, managed constraints, executor defaults, and remote launch rejection when local-binding restrictions cannot be enforced. Extend remote network approval tests with loopback requests to verify controller and environment policy composition.

## Why
MCP OAuth configuration supported a pre-registered client ID but could not supply a client secret for token exchange or refresh.
## What changed
- Add `oauth.client_secret` and `codex mcp add --oauth-client-secret`, requiring a nonempty secret and client ID. Persist the secret in server configuration and accept `clientSecret` in plugin configuration.
- Pass configured credentials through CLI, app-server, and plugin login flows and token refresh. Require a new login when a configured confidential client's ID differs from stored credentials.
- Redact secrets in debug output and keep them out of authorization URLs and persisted OAuth token records.
- Invalidate cached OAuth connections when the configured client ID or secret changes.
## Testing
Add coverage for configuration validation and round trips, CLI argument redaction and subsequent login, app-server login, connection invalidation, and refresh with `client_secret_basic` and `client_secret_post` in both refresh modes. Verify secret exclusion from token records and reject mismatched client IDs before contacting the provider.

## Why
SQLite write failures can leave persisted logs incomplete, while log snapshots taken before collection miss errors raised during that collection.
## What changed
- Report SQLite flush failures directly to the in-memory log buffer without re-entering tracing, using an error category and batch size instead of raw error details.
- Track whether the log writer has lost a batch and make app-server diagnostic uploads use the memory buffer when SQLite logs are incomplete.
- Refresh log bytes after flushing and querying SQLite so the same submission includes collection failures, while preserving captured metadata and thread identity.
## Testing
Add regression tests for failure reporting without recursion, subsequent write recovery, metadata preservation, and healthy and failing SQLite collection. Add an upload integration test that captures attachments locally and checks failure diagnostics, memory-buffer fallback, and omission of raw SQLite error details.

## Why
Local `apply_patch` permission checks can treat a macOS system alias and its canonical spelling as different locations, sending writes covered by temporary-directory grants for unnecessary approval.
## What changed
- Normalize trusted top-level aliases in local policy roots and patch targets, reusing a prepared matcher for permission requests and patch safety checks.
- Share alias normalization with Seatbelt and the macOS executor sandbox while preserving mutable symlinks and missing descendants.
- Preserve read-only and deny precedence, protected metadata restrictions, and remote URI matching. Propagate normalization failures from fallible permission checks.
## Testing
Add macOS regressions for multi-file patches with missing parent directories through both direct `apply_patch` calls and shell interception, asserting automatic approval and file contents. Add unit coverage for alias restriction precedence, mutable symlink preservation, missing descendants, and remote permission isolation.

## What changed
Record `codex.thread.tools.namespaces_total` and `codex.thread.tools.fragment_bytes` histograms through session telemetry. Tag samples by `stage` (`before` or `after` the 4 KiB cap) and `kind` (`snapshot` or `delta`). Measure bytes after description normalization and XML escaping, including the `<tools>` wrapper, with explicit fragment-size buckets.
Delta counts include only changed entries, and unchanged state emits no samples.
## Testing
Extend rendering tests to verify snapshot and delta metrics, omission counts, byte sizes with Unicode and XML escaping, and no samples for unchanged state.

## Why
The PID reservation test could race with the backend's read on Windows by deleting the PID file from its cleanup task. The guardian heartbeat snapshot and environment normalization also needed to match the current permission-context wording.
## What changed
- Release the reservation lock and let `stop()` remove the stale PID file, then assert that the file is gone.
- Update the guardian heartbeat snapshot and normalization pattern to use `The active permission profile for environment`.

## What changed
Add `otel.log_guardian_assessments`, disabled by default and requiring an OTLP HTTP or gRPC log exporter. Emit `codex.guardian_assessment` when a synchronous Guardian review finishes, including review identifiers, status, risk level, user authorization, and rationale. Include `allow` or `deny` outcomes only for completed model assessments, not review errors, timeouts, or cancellations.
Cap exported rationales at 64 KiB on a UTF-8 character boundary and record their original byte length and truncation status. Use the log-only route to exclude assessment payloads from trace export, local state logs, and bug-report log buffers. Cached V2 approvals are not included.
## Testing
Add coverage for configuration gating, enabled and disabled denial logging, terminal statuses and outcomes, UTF-8 rationale truncation, export routing, and bug-report log buffer exclusion.

## What changed
- Resolve remote sandbox modes, writable roots, and denied read paths/globs using the executor's filesystem context, displaying paths in its native format. Preserve local filesystem resolution for local environments.
- Resolve remote read denials individually so missing executor facts omit only the unresolved entries.
- Apply a shared 32 KiB budget to displayed permission paths/globs, omitting whole entries and warning that all restrictions still apply, including omitted read denials.
## Testing
Add coverage for remote Windows writable roots and read denials, missing executor path facts, and whole-entry omission with multibyte paths under the shared budget.

## Why
Image and audio estimate cache misses computed values while holding the cache mutex, blocking access to unrelated keys during initialization.
## What changed
Add `BlockingLruCache::get_or_init` using `Arc<OnceLock<V>>` and migrate the image and audio estimate caches to it. Initialization and same-key waits run in a Tokio blocking region after releasing the cache mutex. Callers share initialization while an entry remains cached; eviction can allow another initialization, so factories must be deterministic.
## Testing
Add a concurrency test covering access to other keys during initialization, eviction of an in-flight entry, and successful completion of both the evicted initializer and its replacement.

## Why
Unchanged scheduled heartbeat instructions should not become fresh authorization evidence that supersedes a later human override or floods Guardian review context.
## What changed
- Capture heartbeat provenance per input, including when steering an active turn, and persist it in history and retained context.
- Coalesce unchanged instructions for each automation at their original acceptance order. Preserve changed instruction versions and ordinary human messages.
- Represent repeated heartbeats as references to earlier instructions in Guardian transcripts, while tracking the latest input turn separately for trusted skill selection.
- Limit coalescing to recognized, bounded heartbeat envelopes with host-provided provenance.
## Testing
Add regression coverage for human overrides across repeated and steered heartbeats, instruction changes, checkpoint restoration, transcript references, envelope parsing, and latest-turn trusted skills in delegated reviews.

## Why
Cold Rosetta translation and cache invalidation from hard-linked executables can interfere with timed CLI tests. Sampling the clock twice also makes local retry telemetry report a delay shorter than the selected backoff.
## What changed
- Warm CLI fixtures with `--version` on macOS x86_64 before timed checks, and use copies or Unix symlinks to avoid invalidating the shared executable's Rosetta cache.
- Use one clock sample to calculate the response retry deadline and recorded delay.
- Include the retry telemetry event in delay-range assertion failures for the headerless rate-limit stream test.

## Why
Asynchronous Guardian scoring omitted the target environment's denied-read restrictions, and cached score authorization did not include that permission context. A score from one environment could therefore be reused for an action in another environment with different restrictions.
## What changed
- Share permission resolution between synchronous review and asynchronous scoring, including resolving `write_stdin` targets from the owning terminal session.
- Include environment identity and denied-read paths/globs in async scoring context and cached score authorization.
- Fall back to synchronous review when permission evidence cannot be resolved or exceeds the async limit of 3,000 bytes, rather than omitting restrictions.
## Testing
Add regression coverage for rejecting cached scores with different environments or denied paths, passing the target environment's restrictions to the classifier, and rejecting oversized async permission evidence.

## What changed
Add a `TestCodexBuilder::with_thread_manager` hook for configuring the thread manager before test threads start, and register an integration suite using a test `AgentControl` implementation.
The tests verify:
- Controller factory calls for new roots, forks, and cold resumes, with reuse for warm resumes and shared session identity for internal sessions.
- Child session identity, persisted parent lineage, message-board authorship, and routing of child reloads through the host controller.
- Preservation of the controller's service tier when child threads start or root starts are rejected.
- Dispatch of `spawn_agent`, `send_message`, and `list_agents` to the host controller, including propagation of its errors to tool results.

## Why
New user input during a Guardian review invalidated an in-flight approval and aborted the pending action, even when the input was only a status inquiry. Review the action again against the latest authorization evidence so it can proceed or be denied based on the updated instructions.
## What changed
- Treat stale authorization as a retryable review error within the existing attempt budget and shared deadline.
- Refresh authorization evidence for each attempt while keeping the pending action fixed, and record evidence from the completed attempt.
- Preserve cancellation and history-reset aborts, and deny approval if stale authorization exhausts the retry budget.
## Testing
Add and update coverage for parent status updates, authorization revocation, repeated updates, cancellation, history resets, and retry deadlines. Verify that refreshed reviews see the latest input and that the pending action executes exactly once only after approval.

## Why
The list-voices barrier can consume the sleep completion event that `any_new_input_interrupts_sleep` needs to observe.
## What changed
Enqueue agent mail directly in the test so the subsequent wait can observe sleep completion. Keep the `TestCodex` fixture alive for the duration of the test by cloning its `codex` handle instead of moving it out.

## Why
Explicit user goal edits can restrict permitted actions, such as “Inspect only; do not publish.” Guardian needs these edits as authorization evidence even though they are recorded as contextual messages.
## What changed
- Add `UserGoalUpdate` and a recording API for objective, status, and clear updates without scheduling a model response.
- Use host annotations to retain goal updates in Guardian review history and root authorization evidence, and invalidate cached authorization when updates are recorded.
- Omit oversized objectives whole and mark their evidence incomplete so truncation cannot turn a restriction into a grant.
- Keep item injection from updating shared model and multi-agent metadata.
## Testing
Extend authorization, retained-context, and review snapshot tests to cover goal-edit cache invalidation, root goal visibility from child threads, and restrictive goals reaching Guardian alongside managed policies.

## What changed
- Add `ThreadManager::with_agent_control_factory` to select a host-owned controller asynchronously using the final thread ID for new and cold-resumed threads. Keep local control as the default, reuse live controllers on warm resume, and preserve parent controllers for internal sessions.
- Fail startup on factory errors or when a provided controller's identity does not match the session identity.
- Defer root service-tier updates to host controllers until thread registration succeeds, preventing failed starts from changing settings on a shared controller.

## Why
Passing extra descriptors to a PTY previously selected behavior intended for inherited escalation sockets: no EOF on stdin close, different signal exit statuses, and no default `SHELL`. Launch attachments need ordinary PTY behavior and must survive execution even when marked `FD_CLOEXEC`.
## What changed
- Add `ChildFds::Attached` alongside `ChildFds::Inherited` to distinguish launch attachments from legacy inherited descriptors.
- Preserve ordinary EOF, default `SHELL`, and signal exit status behavior for PTY attachments.
- Allow explicitly supplied close-on-exec descriptors through pipe and PTY launches without changing the parent's descriptor flags, using child-side flag updates and macOS spawn inheritance actions.
- Update existing callers to use `ChildFds::Inherited`.
## Testing
Add Unix coverage for attachment accessibility, unchanged parent descriptor flags, default `SHELL`, and signal exit statuses. Extend the PTY stdin-draining test to cover attachments and EOF delivery.

## What changed
Use the model catalog's `send_user_message_async.parameters` override for the `request_user_input_async` tool schema. Fall back to the bundled schema when the override is absent or invalid, logging a warning for invalid overrides while preserving the catalog description.
## Testing
Extend integration coverage for catalog schemas, empty and invalid overrides, and schema updates during mid-turn model changes. Update the asynchronous question-and-answer scenario to verify that requests carry the catalog schema while work continues and the user's answer arrives.

## Why
The 32 KiB per-result cap and 128 KiB aggregate budget discard tool metadata even when the outgoing request has room to retain it.
## What changed
- Remove the per-result capture cap and raise the aggregate executed-tool metadata budget to 2 MiB.
- Apply a 15 MiB soft budget to serialized HTTP requests and WebSocket messages, using the actual WebSocket delta. Trim optional result metadata on the wire copy while preserving ordinary content and logical history.
- Prioritize `openai/resource_access` metadata during trimming, and include `overage_bytes` in omission markers.
- Record bytes removed by metadata budgets in `codex.tool_metadata.shed_bytes`, tagged by stage.
## Testing
Add coverage for HTTP and WebSocket message boundaries, resource preservation, residual marker removal, unchanged history, and shedding metrics. Update direct-tool and code-mode tests to verify metadata survives beyond the previous limits.

## Why
Repeated prewarm calls still prepared instructions, tools, and a prompt even when an existing WebSocket connection was already warm.
## What changed
Check for a previous request and a connection that is not known to be closed before startup prewarm preparation. Reuse this readiness check in `prewarm_websocket`, and record `ready` or `needs_prewarm` in the `codex.startup_prewarm.websocket_check` counter before preparation can fail.
## Testing
Extend the WebSocket resume test to verify that repeated prewarm calls skip instruction loading on a healthy connection, while a closed connection triggers preparation again.

## Why
History notes and image generation requests used default HTTP clients that did not carry the configured application network policy.
## What changed
Pass `Config::http_client_factory()` into both extension backends and create transports with `ClientRouteClass::Api`, so their requests honor application network restrictions and configured proxy behavior.
## Testing
Add regression coverage verifying that denied image requests never reach the server and that history notes context requests stop after the network policy becomes restrictive. Retain coverage for image file access restrictions.

## Why
Tool telemetry needs to distinguish product SKUs without turning arbitrary configuration values into metric labels.
## What changed
- Read `apps_mcp_product_sku` from the invoking turn when dispatching tools, replacing any stale SKU in the telemetry metadata.
- Add `product_sku` to tool call count and duration metrics and tool result log and trace events.
- Preserve `codex`, map other nonempty values to `other`, and omit the attribute for empty or missing values.
## Testing
Add coverage for SKU separation on a shared metrics client, normalization, and successful and failing dispatches with stale telemetry metadata. Extend export routing tests to check SKU attributes in logs and matching traces.

## Why
Exceeding the aggregate metadata budget previously replaced all tool result metadata with omission markers, discarding small results unnecessarily. Oversized results also lost resource-access evidence even when that evidence alone would fit.
## What changed
- Raise the executed-tool-call metadata budget per request from 32 KiB to 128 KiB while retaining the 32 KiB per-result limit.
- Reduce the largest result metadata first and stop once the request fits, preserving smaller results.
- Retain `openai/resource_access` when full metadata exceeds capture, request, or direct-call retained-history limits. Keep resource lists intact, and discard optional result metadata before sacrificing call records, source evidence, or completion markers.
## Testing
Add regression coverage for selective reduction, resource-access retention, serialized request size, and direct-call and Code Mode behavior. Update compaction and truncation tests for the larger request budget.

## What changed
- Move `Provider` and `RetryConfig` into `codex-client`, retaining re-exports from `codex-api`, and switch `codex-model-provider-info` to depend on `codex-client`.
- Move feature-state metric emission into `codex-core`, preserving its behavior while removing the `codex-features` dependency on `codex-otel`.
- Explicitly enable TLS features for `tonic` in `codex-config` and `reqwest` in `codex-http-client`.
- Enable `png`'s `zlib-rs` feature directly in `codex-utils-image` to keep encoded PNG bytes stable when WebSocket dependencies are absent.

## Why
Appending output to the completion transcript and pending polling buffer under separate locks allows cancellation to interrupt a chunk update between the two buffers.
## What changed
Store both buffers in `OutputBuffers` behind a shared mutex and append each chunk to both while holding that lock. Polling drains only the pending buffer, while completion events read the retained transcript. Apply this to both local and exec-server output handling.
## Testing
Extend the repeated-drain test to verify that the completion transcript preserves the expected bounded output after polling drains the pending buffer.

## What changed
Return completed startup prewarm sessions to the client cache and schedule resume prewarm through the normal session setup path. Keep in-flight warmups shared, remove the separate nonblocking prewarmed checks, and simplify the prewarm handle to own a single task.
## Testing
Update the resume prewarm integration test to assert `User-Agent` and `x-codex-window-id` handshake headers while retaining coverage for connection reuse, response continuation, and reconnection after closure. Remove the assertion that healthy resumes skip instruction preparation.

## What changed
- Cast `POSIX_SPAWN_SETSID` to the inferred flag type in the `ProcessMode::NewSession` branch.
- Use `LoaderOverrides::without_managed_config_for_tests()` for both cases in `project_layers_disabled_when_untrusted_or_unknown` so host-managed configuration does not affect the test.

## Why
A previously used WebSocket can close while a thread is idle. Hosts need a way to prepare the connection before the next turn without repeating warmup on a healthy connection.
## What changed
- Expose `CodexThread::prewarm()` using the existing startup warmup and next-turn handoff.
- Reuse healthy connections and pending warmups, retry failed warmups, and repair closed connections. Treat a finished WebSocket reader task as a closed connection.
- Skip scheduling during an active turn or shutdown, and stop warmup admission before shutdown takes the pending task.
- Expose `active_turn_environment_selections()` so hosts can inspect the environments captured by the active turn, including those still starting or failed, when authorizing steering.
## Testing
Add regression coverage for repeated idle prewarm calls, healthy connection reuse with preserved response continuation, reconnection with `generate: false`, and warmup rejection after shutdown. Update delayed MCP discovery coverage to verify prewarm repairs an eagerly closed socket and the first turn reuses it.

Use `tokio::select!` to listen for shutdown alongside `ws_stream.next()`
in the test server's request loop, allowing the server task to exit when
shutdown is requested while waiting for the next client message.

## What changed
Call `ensure_rollout_materialized` after recording pending inter-agent communication, using the supplied persistence context. This also triggers the first execution checkpoint when an agent message starts the first turn.
## Testing
Extend the first-input persistence test to cover inter-agent communication, checking that the checkpoint is requested and sampling proceeds while durable metadata is blocked. Verify that prepared context precedes the agent message and that its author and recipient reach the model.

## What changed
Remove the session-level capture mode and make thread-owned retained context the default throughout history management. Simplify input ordering, compaction identity preservation, fork handling, and approval freshness checks by removing legacy capture branches.
Keep checkpoint compatibility review separate: unknown or incompatible checkpoints can still use legacy review while retaining user evidence. Set the already-removed `guardianv2.thread_context` feature's default to false and remove redundant opt-out test variants.
## Testing
Update checkpoint tests to exercise legacy review through restored checkpoint data. Adapt existing compaction, resume, rollback, and fork tests to use unconditional context capture.

## What changed
Always use `GuardianContextMode::ThreadOwned` for session capture and subagent history forks, regardless of `guardianv2.thread_context`. Continue selecting reviewer policy from saved checkpoints, preserving legacy review for incompatible checkpoints and removing the feature gate from reviewer compaction compatibility checks.
## Testing
Update coverage for unconditional retained context across compaction, resume, and subagent forks. Exercise legacy review with saved checkpoints, and verify that local and managed opt-outs do not disable sender context capture or checkpoint migration.

## What changed
Use retained root context for subagent authorization reviews, removing the separate legacy history path. Remove the runtime `request_user_input` answer buffer and its response counter; authorization versions now track the user-message revision and retained-context completeness.
## Testing
Update authorization and input-handler tests to exercise retained context, and add coverage for the `user_messaging` connector in subagent reviews.

## What changed
- Deprecate and ignore `features.guardianv2.thread_context`, including profile overrides, with a migration notice. Ignore managed requirements for the removed flag with a warning.
- Preserve surviving legacy root instructions across compaction and resume when retained instructions are incomplete. Include unmatched instructions in worker authorization context with an explicit unknown-ordering notice, without replacing newer retained facts or expanding the reviewer's model window.
- Use surviving source calls to roll back verified answers from older checkpoints that lack acceptance-order metadata.
## Testing
Add regression coverage for legacy instruction recovery, compaction and resume, source matching, and answer rollback around same-turn user input. Update configuration and Guardian review tests to verify that the deprecated opt-out still uses thread-owned context and rejects incompatible compaction evidence.
