## What changed

- Add environment-provided MCP restrictions for configured and plugin-provided servers.

- Apply the resolved policy to runtime startup, model tool exposure, telemetry, OAuth flows, and skill dependency installation.

a397079287 Preserve MCP resource origins across compaction (#39192)
## Why

Compaction can remove the tool-call events that associate an MCP app widget with
the tool, account, and URI needed to authorize later resource reads.

## What changed

- Store a bounded MCP resource-origin checkpoint with each compacted rollout.
- Restore the checkpoint when resuming a session so existing widgets remain
  readable after compaction and restart.
- Discard saved origins when rollback rewrites history across the compaction
  boundary, and reject oversized or invalid checkpoints during restoration.

## Testing

- Extend the app-server MCP resource-origin test to compact a paginated thread,
  continue the conversation, and read the original widgets before and after a
  restart.
- Verify rollback clears provenance that no longer matches retained history.

App widgets need to read resources using the app and account context of the
tool call that produced them, including after a thread is restored.

## What changed

- Add an optional `originCallId` to `mcpServer/resource/read` requests and
  return it on successful scoped reads.
- Track bounded provenance for successful app tool calls in thread history and
  use the current tool binding, account link, and app policy when reading the
  associated widget resource.
- Reject missing, mismatched, failed, or ambiguous origins, while keeping
  app-only tools available for widget reads without exposing them to the model.

## Testing

Add app-server coverage for scoped widget reads across both history modes,
ephemeral and persistent threads, server restarts, app-only visibility, and
invalid origin cases.

2013e04354 Preserve filesystem permission path conventions (#39084)
## Why

Filesystem permission paths can use a convention that differs from the host
running Codex. Converting them immediately to native absolute paths can change
the meaning of ambiguous paths such as `/C:/secret` or Windows UNC paths.

## What changed

- Store literal filesystem permission paths as `PathUri` values through the
  runtime policy and execution protocol.
- Keep legacy string-based serialization at explicit protocol boundaries and
  reject conversions that cannot be represented losslessly.
- Encode native paths as opaque URIs when a normal file URI would imply the
  wrong path convention.

## Testing

Added coverage for cross-platform and ambiguous path round trips, UNC path
variants, permission-profile serialization, and deny-policy enforcement.

fcdae21073 Support metadata staging for reserved thread IDs (#38819)
## Why

Callers need to associate host-owned state with a thread before Core starts it.

## What changed

- Add `ThreadManager::reserve_thread_id` and allow new threads to start with the reserved ID. Reject reserved IDs when resuming an existing thread.
- Let thread stores stage metadata for a reserved ID. The local store merges staged values into the first successful metadata update and then removes them.
- Clear staged metadata when an unmaterialized thread shuts down or is discarded, and require the state database for staging.

## Testing

- Cover reserved ID generation and resume validation.
- Cover metadata persistence, merging, rollout-compatible updates, cleanup, and invalid staging requests.

Thread settings can change while a turn is still active, including between sampling requests. Those updates should apply to the next turn instead of changing the model configuration partway through the current turn.

## What changed

- Snapshot the model, reasoning settings, service tier, approval settings, and model-attributed telemetry in `StepContext`.
- Build prompts, model requests, world state, tracing, and startup prewarms from that step-scoped snapshot.
- Share immutable model metadata between turn and step contexts.

## Testing

Added an integration test that pauses an active turn, updates its thread settings, and verifies that all requests in the active turn retain the original settings while the next turn uses the updates.

2ca575026c Support pending environment attachment configuration (#38684)
## Why

An environment connection can be available before its owner has supplied the
configuration for a particular thread attachment. Threads need to start without
blocking while ensuring turns do not use that attachment prematurely.

## What changed

- Accept `Pending` environment configuration and resolve each attachment only
  after both its shared executor connection and owner configuration are ready.
- Add a `Failed` configuration state and `environment_failed` callback so an
  owner can fail one thread's attachment without affecting other threads.
- Keep pending and failed attachments out of capability-root inspection and turn
  environments, and allow failed attachments to recover through a ready update.
- Apply owner configuration before waking a waiting turn so its permission
  profile, login-shell policy, capability roots, and tools are immediately
  consistent.

## Testing

Add an integration test covering non-blocking thread startup, independent ready
and failed callbacks, waiting-turn resumption, installed capability and tool
configuration, and recovery from failure.

Environment attachments can either inherit configuration from their thread or
provide their own. Later thread setting updates must refresh inherited
configuration without overwriting attachment-owned permissions and capability
roots.

## What changed

- Resolve each attachment's configuration when it is selected and retain
  whether it came from the thread or the attachment owner.
- Apply subsequent thread configuration updates only to thread-owned
  attachments, while preserving ownership across snapshots and child threads.
- Keep the resolved configuration with the environment selection so runtime
  consumers use a single canonical value.

## Testing

- Extend remote-environment coverage to verify that thread-owned permissions
  follow thread updates while owner-provided read-only permissions remain in
  effect.
- Cover configuration inheritance, attachment replacement, and owner-configured
  capability roots.

Concurrent Guardian samples can finish out of order, allowing an older sample to
replace the thread's newer in-memory security risk score.

## What changed

  with records that have no timestamp.
- Add `ExtensionData::insert_if` so checking and replacing a typed attachment happen
  under the same lock.
- Update Guardian V2 to replace the thread's risk score only when the completed sample
  is newer than the stored one.

## Testing

- Verify concurrent conditional inserts retain the newest value.
- Verify Guardian-generated risk scores include a sampling timestamp.

4d9f3021c8 Include node_repl images in Guardian review evidence (#38454)
## What changed

- Preserve valid image content from completed `node_repl` Code Mode responses alongside text evidence for Guardian reviews.
- Enable multimodal evidence for models that require automatic `node_repl` review, or when both `guardian_enhanced_node_repl_transcripts` and `guardian_node_repl_transcript_images` are enabled.

  into the new context window.

- Cover enabled and disabled retention for remote compaction v2, manual
  token-budget compaction, and mid-turn automatic compaction.

## Why

The model provider does not reliably identify which curated plugin catalog is
available. ChatGPT authentication can be used with a custom provider, while an
unauthenticated session should use the API-compatible catalog regardless of its
provider.

## What changed

- Select the ChatGPT curated catalog only for authentication modes that use the
  Codex backend; use the API curated catalog for API-key and unauthenticated
  sessions.
- Initialize standalone plugin managers with the current authentication mode
  across CLI, app-server, MCP, and external-agent migration paths.
- Preserve authentication mode while detecting and importing migrated plugins.

## Testing

- Cover catalog and skill routing across ChatGPT, API-key, unauthenticated,
  Bedrock, and custom-provider configurations.
- Verify authenticated plugin migration uses the ChatGPT curated marketplace.

Transient executor disconnects could leave capability discovery and skill catalogs
stuck on a cached failure for the rest of a thread, even after the executor
reconnected.

## What changed

- Replay capability discovery after executor recovery and retry transient failures
  on later requests while continuing to cache permanent failures.
- Avoid caching skill catalogs produced from failed discovery so a later step can
  load the recovered catalog.
- Mark the MCP runtime dirty when recovered manifests change the projected MCP
  servers, and allow discovery to be cancelled with the turn.

## Testing

- Cover same-request recovery after a disconnect and recovery on a later request.
- Cover retry classification through connection-attempt errors and skill catalog
  caching after discovery recovers.

- Capture accepted, successful `node_repl` results from Code Mode and include them as bounded, untrusted evidence in Guardian review prompts.
- Enable the enhanced transcript through `guardian_enhanced_node_repl_transcripts` or when the selected model requires automatic Node REPL review.
- Keep this evidence out of the parent model history, exclude encrypted and failed results, and avoid resending admitted evidence when a Guardian session is reused.
- Clear retained evidence and invalidate review sessions when a thread is rolled back.

Hooks supplied as managed requirements must not be silently skipped when their
handlers cannot be loaded.

## What changed

- Fail session and app-server thread startup when an enabled managed requirement
  contains an invalid matcher, an empty command, or an unsupported handler type.
- Keep load failures for ordinary managed configuration hooks as warnings, and do
  not enforce managed hook requirements while the hooks feature is disabled.

## Testing

Added hook-engine, core session, and app-server coverage for valid and invalid
managed hook requirements.

- Add `TurnInputRequest` and typed submission results for atomically starting a turn, steering the active turn, or declining input with a specific reason.
- Expose `start_or_steer_turn`, `start_turn_if_idle`, and `steer_turn` on `CodexThread`, and migrate Core consumers to these APIs.
- Make app-server `turn/start` steer an active regular turn and return that turn's ID. Reject incompatible output schemas and non-steerable turns without applying settings or enqueueing input.

- Cover concurrent start-or-steer submissions, accepted and rejected settings updates, output-schema compatibility, idle-start rejection, and app-server steering.

4b07886d59 Represent persisted world state as JSON objects (#38274)
## Why

World-state snapshots and merge patches are keyed collections of sections. Allowing
their persisted `state` field to contain any JSON value required replay code to
handle shapes that cannot represent world state.

## What changed

- Type `WorldStateItem.state` and snapshot patches as JSON object maps.
- Reconstruct full snapshots directly from those maps and apply patches without
  fallible arbitrary-value conversions.
- Regenerate the app-server protocol schema for the object-only representation.
