jif 30bdfec59d Persist Guardian review failures for reports across restarts (#51651)
## Why

Failed Guardian review evidence was process-local, so restarting Codex lost the context needed for later diagnostic reports.

## What changed

- Persist failed-review records in SQLite before reviewer cleanup, retaining at most 64 records globally, eight per thread, and 8 MiB of evidence. Delete persisted evidence when its reviewed thread is deleted.
- Recover and deduplicate persisted and in-memory records for the reported task tree when uploading `auto-review-failures.jsonl`.
- Bound persistence attempts to 250 ms and retain an in-memory fallback when storage is unavailable. Keep capture gated by user opt-in and non-ephemeral sessions, and export evidence only when logs are included.

## Testing

Add coverage for persistence across reopen and process restart, retention limits, cascading deletion, busy-storage fallback, and report uploads with and without logs, including SQLite log failures.

Dmitriy Kovalenko 9b0a676d5d Bundle rollout attachments into a gzip tar archive (#50446)
## What changed

- Combine file-backed rollouts and buffered rollout prefixes into `rollouts.tar.gz`, keeping diagnostics as separate attachments.
- Bound the archive and its upload envelope by the existing size limit, skip invalid or duplicate archive filenames, and fall back to individual rollout attachments if archiving fails.
- Add `rollout_archive_filename` to the report thread index so rollout filenames identify archive entries or fallback attachments.

## Testing

Add an upload test that extracts the archive and verifies the filenames and contents of both a compressed file-backed rollout and a buffered rollout. Update the thread index test to check the archive filename.

jif 2e5ee418ad Improve Guardian report diagnostics (#42033)
## Why

Reports from a large thread tree can exceed the rollout attachment limit, making
the threads involved in a failed Guardian review harder to diagnose.

## What changed

- Prioritize descendants with retained failed reviews when selecting bounded
report rollouts, while always preserving the reported thread.
- Include each selected thread's available Guardian trunk rollout alongside its
  conversation rollout.
- Add a thread index attachment with selected filenames, bounded omission details,
retained failure threads, and the process-wide discarded-record count.
- Link failed-review records to their reviewed turn and target item when available.

## Testing

- Cover failed-review prioritization, bounded omission metadata, discarded-record
accounting, and turn/item linkage in Guardian report records.
