88eb3a2b8a Enable git attribution across Codex entry points (#34819)
## What changed

- Install the git attribution extension in the app server, MCP server, and `codex debug prompt-input` so authenticated workspace policy controls the commit and pull request attribution instructions sent to the model.
- Resolve attribution settings from the process-level ChatGPT base URL, independent of per-thread or per-tool configuration overrides.
- Treat git attribution as contextual developer content when mapping model events.

## Testing

- Cover policy fetch retries, workspace switches, rollbacks, cold thread resumes with legacy instructions, MCP tool calls, and prompt-debug output.

4ebd976312 Support configurable realtime BEM channel prefixes (#34816)
## What changed

- Route realtime V3 `bemTags` handoffs using `[ANALYSIS]`, `[COMMENTARY]`, and `[FINAL]` prefixes by default.
- Add `codexResponseHandoffChannelPrefixes` to realtime start requests so clients can replace the accepted prefixes for individual channels. Each channel may accept multiple prefixes, and omitted channels retain their defaults.
- Apply the configured prefixes to both complete and streamed handoff messages, without treating empty prefixes as matches.

## Testing

- Cover request deserialization, default and custom routing, partial streamed prefixes, empty-prefix handling, and end-to-end V3 handoffs.

80f3c3141e Include the final agent message in turn completion summaries (#34777)
## Why

Streamed message deltas can be dropped when the transport is saturated, leaving
clients with a truncated final response.

## What changed

- Include the last non-empty final agent message in successful `turn/completed`
  notifications and mark the items as a summary.
- Treat completed message content as authoritative in the TUI, repairing an
  incomplete stream while avoiding duplicate messages when `item/completed` was
  already received.
- Continue backfilling full turn items for persisted `codex exec` threads when a
  completion contains only the summary view.

## Testing

Added coverage for completion summaries, dropped stream deltas, duplicate item
handling, and directive-only responses.

## What changed

- Decode `JSONRPCRequest` into `ClientRequest` without serializing the entire request back into an intermediate JSON value.
- Define every client method's wire name explicitly and use it directly for request, response, and tracing method lookups.
- Serialize outgoing transport messages directly to JSON strings instead of first building a `serde_json::Value`.

## Testing

- Add regression coverage that compares direct request conversion with Serde decoding, including omitted and null parameters, legacy methods, and unknown methods.
- Verify that direct outgoing-message serialization preserves the notification wire shape.

cefcffd692 Preserve approvals reviewer when forking threads (#34664)
## Why

Forking a persisted thread could lose its approvals reviewer and fall back to
configuration defaults.

## What changed

- Restore the latest persisted approvals reviewer from the full source history
  when creating a fork, while continuing to honor explicit overrides.
- Keep boundary-based history truncation independent from reviewer restoration,
  so a fork at an earlier turn retains the source thread's latest setting.

## Testing

- Add an app-server test that sets auto-review, forks at an earlier turn, and
  verifies the fork reports auto-review.

d4fcb2873b Honor configured proxy routes for auth refreshes (#34655)
## Why

ChatGPT token refresh requests need to follow the same configured routing policy as other authentication traffic, including environments that use the system proxy.

## What changed

- Require an `AuthRouteConfig` throughout login, logout, token refresh, personal access token, and agent identity flows.
- Build auth HTTP clients directly from that configuration instead of falling back to a default HTTP client when routing configuration is absent.

## Testing

- Add an integration test that refreshes a token through a cached system-proxy route and verifies the proxy receives the expected request.

f899a79c03 Propagate resolved proxy policy through auth routing (#34649)
## Why

Auth routing represented the default proxy policy as an absent configuration,
leaving consumers to reconstruct the fallback HTTP client independently. Carry
the application's resolved policy explicitly so auth and related requests use
the same client configuration.

## What changed

- Build `AuthRouteConfig` from the resolved `HttpClientFactory` for both
  `ReqwestDefault` and `RespectSystemProxy` policies.
- Pass that route configuration through login, auth, cloud task, and cloud
  configuration flows, and reuse its factory when loading cloud configuration.
- Log cloud-task configuration load failures before falling back to the
  transport-default proxy policy.

## Testing

Extended configuration tests to verify that effective and bootstrap auth routes
select the expected proxy policy when system-proxy support is enabled or
disabled by feature requirements.

dfd2d8133c Detach non-interactive subprocesses from stdin (#34612)
## What changed

Redirect stdin to null when launching:

- `codex doctor --json` for uploaded diagnostic reports
- Git commands used by `codex doctor`
- ripgrep searches over rollout files
- the Windows sandbox setup refresh helper

This keeps these non-interactive subprocesses from reading Codex's inherited
input stream.

87f71e35b8 Skip missing paths in filesystem sandbox entries (#34598)
## Why

Default read-only protections for project metadata should apply when paths such
as `.git`, `.agents`, and `.codex` exist, without causing sandbox setup to
materialize missing paths as ACL targets.

## What changed

- Add an optional `missing_path_behavior` to filesystem sandbox entries and
  mark default project-metadata protections with `skip`.
- Preserve the behavior through permission transforms and exec/MCP protocol
  serialization while keeping existing path wire variants stable.
- Ignore skip-missing entries when projecting configuration or Windows sandbox
  overrides, while retaining explicit metadata carveouts.

## Testing

- Cover protocol round trips for path and special-path entries.
- Verify default metadata protections and Windows explicit carveout handling.

## What changed

- Apply managed values for `sqlite_home`, `log_dir`, `model_catalog_json`,
  `check_for_update_on_startup`, `allow_login_shell`, the report-submission
  opt-in, and `windows.sandbox_private_desktop` to the runtime configuration,
  with source-aware warnings when they override configured values.
- Expose these values through `configRequirements/read`, return them from
  `config/read`, include them in session config locks, and show their sources in
  the TUI config debug view.
- Reject app-server writes that overlap an exact managed requirement with
  `configRequirementReadonly` while leaving unrelated sibling keys writable.

## Testing

- Cover runtime overrides, requirement API mapping, config reads and writes,
  session lock serialization, cross-platform path URIs, and debug output.

7442f5f932 Add keyed shell environment policy filters (#34590)
## What changed

- Add `shell_environment_policy.filters`, mapping environment-variable patterns to `include` or `exclude`, while continuing to accept the legacy `exclude` and `include_only` arrays.
- Merge filter keys case-insensitively across config layers so higher-precedence entries can override individual patterns. Switching representations replaces the other representation instead of combining them.
- Reject mixed, duplicate, or malformed filter definitions in enabled layers, config writes, and reloads, while retaining the previous session configuration when a reload is invalid.
- Make config-manager reads and writes representation-aware and preserve existing formatting when updating individual policy values.

## Testing

Add coverage for parsing, schema constraints, layered merging, validation and diagnostics, config writes, override metadata, and invalid session reloads.

0b175e6439 Protect fork history references during rollout cleanup (#34566)
## Why

Paginated forks can retain a `history_base` pointer to another rollout. Deleting or compressing either side of that relationship can leave the fork without the rollout history it references.

## What changed

- Add a rollout reference index that scans active, archived, and compressed rollout metadata.
- Reject deletion when a rollout is still referenced by a fork outside the requested deletion set.
- Delete spawned thread subtrees as one store operation so internal references do not block deleting the complete subtree, while external references preflight the request before any files are removed.
- Skip referenced rollouts, fork-pointer rollouts, and rollouts with unreadable metadata during background compression.

## Testing

- Cover reference indexing across active, archived, compressed, duplicate, and self-referencing rollouts.
- Verify deletion rejects external references without partially deleting a subtree and permits batch deletion of internally referenced threads.
- Verify compression preserves fork pointer chains and continues past unreadable metadata.

d937bfac84 Honor system proxy settings for remote plugins (#34509)
## Why

Remote plugin requests did not use Codex's effective outbound proxy policy, so
they could bypass configured system proxy and PAC routing.

## What changed

- Route remote plugin catalog, mutation, sharing, upload, and bundle download
  requests through the configured route-aware HTTP client.
- Select routes using the complete request URL, including encoded query
  parameters and backend-provided signed upload or download URLs.
- Preserve standard Codex headers and suppress diagnostics for URLs or headers
  that may contain credentials.

## Testing

Add coverage for route selection of catalog queries, workspace plugin uploads,
and backend-provided bundle download URLs.

94bb6a09a6 Respect system proxies during plugin startup sync (#34506)
## Why

The curated plugin startup sync can fall back from Git to HTTP, but those
requests did not use Codex's effective outbound proxy policy.

## What changed

- Thread the configured HTTP client factory into curated plugin sync.
- Route GitHub API, zipball, backup metadata, and returned download URLs
  through the route-aware client when system proxy support is enabled.
- Preserve the existing default HTTP client behavior, including its custom CA
  fallback, when system proxy support is disabled.
- Apply the standard Codex headers to startup-sync HTTP requests.

## Testing

Add coverage that records route selection for repository, archive, metadata,
and backend-supplied download URLs, and verifies standard headers are sent.

4f1992732c Preserve custom arg0 for sandboxed exec-server processes (#34497)
## Why

Sandbox wrappers replaced the process launch command and did not carry an
`ExecParams.arg0` override through to the inner process.

## What changed

- Route sandboxed Unix launches with a custom `arg0` through a helper mode that
  re-execs the requested program with the override.
- Expose the helper executable to the filesystem sandbox and dispatch its mode
  from Codex and exec-server test binaries.

## Testing

Add coverage for the prepared sandbox command and an end-to-end remote process
that verifies both the custom `arg0` and filesystem restrictions.

Backend-backed features need to use the same outbound routing configuration as the rest of Codex.

1836ae0612 Preserve thread settings for goal-first and forked threads (#34469)
## Why

Goal-first and forked threads could create rollouts without recording their
effective thread settings. Resuming those threads after a restart could then
use current configuration instead of the settings selected when the thread was
created.

## What changed

- Persist a `thread_settings_applied` item when a goal first materializes a
  rollout, including a follow-up snapshot if settings changed concurrently.
- Append each fork's effective settings after its copied history so they take
  precedence over inherited settings during resume.
- Centralize conversion from `ThreadConfigSnapshot` to
  `ThreadSettingsSnapshot`.

## Testing

Added coverage that restarts the app server and verifies goal-first and forked
threads retain their respective `approvals_reviewer` settings.

## Why

Sandboxed process launch through the exec server was unsupported on Windows.

## What changed

- Add a shared native process launcher that selects the Windows sandbox session backend when required and otherwise launches PTY or pipe-based processes.
- Pass Windows permission, workspace-root, proxy, filesystem-override, and desktop settings through exec-server process preparation.
- Preserve piped stdin and inherited file descriptors across the shared launch path.

## Testing

- Verify that a remote Windows sandbox process accepts stdin while blocking a write under a read-only policy.
- Enable workspace-root sandbox tests on native Windows while continuing to skip environments that cannot emulate Windows sandbox semantics.

e4836f998d Add configurable hook context spill limits (#34393)
## What changed

- Add `additionalContextLimit` to command hook configuration for events that can emit `additionalContext`.
- Apply the limit independently to each hook's context before it is sent to the model. Unset values retain the 2,500-token default, while `0` disables spilling for that hook.
- Expose configured limits through app-server hook and requirements responses, and show them in the TUI hooks browser.

## Testing

- Cover JSON and TOML parsing, discovery and hashing, per-hook spilling behavior, app-server responses, and TUI rendering.

1e20272fa5 Avoid cloning thread history for token usage replay (#34361)
## What changed

- Resolve the turn ID for restored token usage before sending resume and fork responses, avoiding a clone of the full thread history.
- Add `ThreadHistoryBuilder::active_turn_id` so attribution can inspect the active turn without materializing its items.
- Stop history reconstruction at the latest persisted `TokenCount` and verify that a later tail turn does not receive the replayed usage.

86102db5a1 Reject unsupported history modes when loading rollouts (#34344)
## Why

The first session metadata record defines a rollout's history mode. If that
record uses an unsupported mode, skipping it as a parse error can cause later
metadata copied from fork history to be treated as canonical.

## What changed

- Validate session metadata history modes before deserializing rollout lines,
  while continuing to tolerate unknown modes in copied fork history.
- Simplify thread status registration to pass only the thread ID instead of a
  fully materialized `Thread`.

5a208c1fc3 Persist names for paginated threads (#34229)
## Why

Paginated threads need explicit user-facing names to remain distinct from
derived titles and previews without requiring rollout metadata writes.

## What changed

- Add a nullable `name` column to persisted thread metadata and use it as the
  canonical name for paginated threads.
- Route paginated name updates through the state database while retaining the
  name index as best-effort compatibility data and preserving legacy thread
  behavior.
- Return persisted names when reading, listing, searching, and resuming
  paginated threads, and include them in state-database search matching.

## Testing

- Cover paginated name updates, reads, list/search results, and metadata-only
  resume behavior, including stale or unavailable compatibility data.

Clients that use full-history resume or request full items in turn pages should
work consistently across legacy and paginated threads.

## What changed

- Materialize complete turns and items from the paginated projection for
  `thread/resume` and `thread/turns/list` when requested.
- Support `initialTurnsPage` during paginated resume, including merging a live
  active turn while preserving the requested page size and cursors.
- Keep metadata-only resume behavior and backwards cursors available through
  `excludeTurns: true`.

## Testing

- Extend app-server coverage for full paginated history, full item views, and
  initial resume pages on running threads.

312caf176a Seed realtime V3 sessions with initial text items (#34067)
## What changed

- Add an optional `initialItems` field to `thread/realtime/start` for seeding
  realtime V3 session history with `user`, `developer`, and `assistant` text.
- Encode the items in Frameless Bidi session bootstrap payloads for websocket
  and WebRTC transports while preserving existing payloads when the list is
  omitted or empty.
- Reject non-empty initial items for V1 and V2, more than 128 items, any item
  over 8,192 estimated tokens, or a combined total over 8,192 estimated tokens.

- Add the experimental `thread/searchOccurrences` app-server method for case-insensitive literal search across visible user messages and final assistant messages without replaying the thread.
- Return paginated snippets with UTF-16 match ranges and `thread/turns/list` cursors for navigating to each matching turn.
- Index persisted item types to efficiently select searchable messages from paginated thread history.

## Testing

- Add an app-server integration test covering pagination, steering messages, final-answer selection, Markdown normalization, and UTF-16 offsets.

025db22058 Route realtime V3 handoffs by response channel (#33903)
## What changed

- Add `codexResponseHandoffMode` to `thread/realtime/start` for V3 sessions, with `thinking` as the default and `commentary` and `bemTags` routing modes.
- Map BEM `analysis` and `commentary` output to the commentary channel and `final` output to the speakable channel, while preserving the original BEM envelope. Unrecognized output falls back to speakable.
- Mark explicit speech appends as speakable and stop adding the legacy `"Agent Final Message"` prefix to V3 handoffs. V1 and V2 continue to ignore the new setting.

## Testing

- Cover channel selection for every routing mode, streamed BEM header parsing, fallback behavior, explicit speech, and V1 compatibility.

7bd44085e1 Add SessionEnd hooks for thread teardown (#33895)
## What changed

- Add the `SessionEnd` hook event to hook configuration, protocol schemas, analytics, and the hooks browser.
- Run matching hooks for root threads during shutdown, including app-server archive, delete, idle unload, and graceful shutdown. Flush the transcript first and provide the session ID, transcript path, working directory, and `reason: "other"`.
- Keep teardown bounded: hook output is advisory, the default timeout is one second, configured timeouts are capped at three seconds, and async hooks run synchronously with a warning.

## Testing

- Cover transcript availability, matcher selection, timeout normalization, ignored control output, subagent exclusion, archive/delete ordering, and graceful shutdown of multiple loaded threads.

a238b753af Add an API for reading installed app runtime state (#33843)

- Add `app/installed` to return each connector observed in the committed runtime tool snapshot, including its runtime name and effective `enabled` and `callable` state.
- Support evaluating thread-specific configuration with `threadId` and refreshing the hosted connector tool snapshot with `forceRefresh`.
- Preserve observed connector identities as disabled when global or workspace policy disables Apps, and retain the previous snapshot when a refresh fails.

3f0669dbd6 Make parent-owned sub-agent threads read-only in the TUI (#33841)
## Why

Multi-agent V2 spawned threads are controlled by their parent and reject direct
app-server input. The TUI should expose that ownership instead of presenting an
input flow that the server will reject.

## What changed

- Add the experimental `Thread.canAcceptDirectInput` capability for loaded
  threads and populate it from the same policy used to validate `turn/start`
  and `turn/steer`. Stored, unloaded threads report `null`.
- Carry the capability through TUI thread discovery, selection, startup, and
  resume flows, with a compatibility fallback for older app servers.
- Present parent-owned threads as view-only, preserve drafts and queued input,
  block agent-directed commands and settings shortcuts, and keep local
  navigation and inspection commands available.

## Testing

- Cover V1 writable and V2 view-only spawned threads through selection and
  resume.
- Cover composer submission paths, queued and initial input preservation,
  allowed navigation commands, settings shortcuts, and the view-only UI.
