App-server clients can report whether Amazon Bedrock is using
AWS-managed credentials or a Codex-managed API key, but they do not have
a matching API for creating the managed login. This PR defines that
experimental wire contract independently from its implementation.
Managed Bedrock API keys are already a primary `CodexAuth` mode. The API
therefore describes a normal Codex login that replaces the current
stored auth rather than introducing provider-scoped credential storage.

Future extension items can have typed app-server APIs without adding a
new `TurnItem` variant, `ExtensionTurnItem` variant, or core emitter
match arm.
## What changed
- Added `codex-extension-items` with the closed `ExtensionItem` enum and
shared `image_generation::ImageGenerationItem` schema.
- Added generic `TurnItem::Extension(ExtensionItem)` and
`ExtensionTurnItem::Extension { item, legacy_events }` paths.
- Updated standalone image generation to emit a typed extension item and
provide its existing legacy `ImageGenerationBegin` /
`ImageGenerationEnd` events explicitly.
- Kept canonical lifecycle ordering: core emits `ItemStarted` /
`ItemCompleted` before extension-provided legacy events.
## Follow-up
Standalone web search still uses its typed special-case path. Migrating
it later would let `ExtensionTurnItem` collapse into a single
extension-item struct.
## What
Adds an optional hosted login-success redirect path for app-server login
requests.
- Keeps the existing localhost success page as the default.
- Lets app-server callers opt into a hosted success page with an
optional protocol field.
- Persists credentials before redirecting to the hosted success page.
- Keeps org setup and existing CLI/device-code login flows on the local
success page.
- Accepts an optional typed `appBrand` value and forwards it to the
hosted page as `app_brand` so web can select the correct asset.
- Generates the app-server protocol schema updates for the new optional
fields.
## Why
This supports the hosted Codex login success page rollout without
changing existing login behavior by default. The Codex Apps frontend can
gate the opt-in with Statsig after the hosted web page.
## Rollout safety
- Old callers omit the new field and continue using localhost.
- New callers talking to old app-server builds remain safe because the
Codex Apps side treats the field as optional and defaults the flag off.
- Missing brand values default to Codex.
- The hosted redirect always uses the app-login source so the hosted
page can reopen Codex; the existing streamlined-login visual flag
remains separate.

The focused login and protocol run passed all 380 tests.
I also started the broader `just test -p codex-app-server`; it compiled
successfully, then many tests failed on this machine because spawned
test servers tried to use the ambient `/Users/rafaelj/.codex/sqlite`
state DB, which is read-only in this sandbox. I stopped that run after
confirming the failures shared that environment issue.
## Summary
- Adds `writes` to `AppToolApproval` and exposes it through config and
app-server schemas, including
`[apps._default].default_tools_approval_mode`.
- In `writes`, tools with `readOnlyHint = true` skip approval; all other
tools prompt, including non-destructive writes and tools without
annotations.
- Prevents session or persistent approval choices in this mode so later
writes still prompt.
## Why
`auto` only prompts for risk-hinted actions, while `prompt` also
interrupts reads. Apps need a middle mode that gates writes without
prompting for declared read-only actions.

## Why
A realtime session can end after more transcript has accumulated than
was included in its last handoff. That tail already lives in core's
active transcript state, but the stop path aborted the realtime
input/fanout tasks before routing it, so the final bit of the
conversation could disappear before `thread/realtime/closed`.
This behavior is still being evaluated, so clients must opt in per
realtime session. Omitted or false leaves shutdown behavior unchanged.
## What changed
- Add optional `flushTranscriptTailOnSessionEnd` to
`thread/realtime/start`, defaulting to false in app-server.
- Expose an idempotent `take_transcript_tail()` from the existing active
transcript state using `last_handoff_entry_count`.
- When enabled, let shutdown cancel the input owner cleanly and publish
at most one final existing `<realtime_delegation>` with the remaining
text in `<transcript_delta>`.
- Have the existing fanout drain already-parsed events before routing
that final delegation, so a queued handoff wins first and is not
duplicated in the tail.
- Flush realtime shutdown before ordinary session task abort during core
cleanup.

## Why
Supported clients currently receive only a reset-credit count from
`account/rateLimits/read`. The redemption UI and other app-server
clients need each available credit's expiry and ID so they can explain
what will expire and consume the credit a user selected. This
information belongs on the existing rate-limit read surface rather than
a second app-server list RPC that clients would need to coordinate.
## What changed
- extend `rateLimitResetCredits` on `account/rateLimits/read` with
nullable `credits` detail rows
- fetch usage and reset-credit details concurrently; if the detail
request fails, times out, or cannot be parsed, preserve the usage
response and return `credits: null`
- expose each credit's ID, reset type, status, grant time, expiry time,
title, and description
- add an optional nullable `creditId` to
`account/rateLimitResetCredit/consume`; omitting it preserves the
existing automatic-selection behavior
- forward a selected credit ID to the Codex backend and update the
app-server documentation and generated schemas
The TUI consumer is stacked in #30488.

Part of #29618.
## Why
Code-mode tool results could return to the model while an MCP
elicitation was still waiting for user input. This differed from
parallel tool calling and could let the model continue before the user
resolved the request.
We need one session-level view of outstanding elicitations so tool
runtimes can consistently hold results until every pending elicitation
is resolved.
## What changed
- Added a counted, session-owned ElicitationService with RAII
registrations.
- Registered both core-originated and server-originated MCP elicitations
with the service.
- Migrated out-of-band elicitation tracking and unified exec timeout
pausing to the shared service.
- Made code-mode functions.exec and functions.wait capture their runtime
result normally, then hold it before returning while an elicitation is
outstanding.
- Kept terminate: true immediate; only its result is held.
- Preserved model-visible wall time across the elicitation hold.
- Kept the behavior session-scoped, with concurrent elicitations holding
the pause until all registrations are released.
## Why
App-server clients that configure named execution environments need to
discover an environment's shell and working directory before selecting
it for a thread or turn. Because the environment can run on a different
operating system than app-server, its working directory is represented
as a canonical `file:` URI rather than a host-local path string. The
probe also needs a bounded response time: an exec-server that completes
initialization but never answers `environment/info` must not hold the
environment serialization queue indefinitely.
## What changed
- Add an experimental `environment/info` app-server RPC for named
environments.
- Route the probe through the managed environment connection and return
target-native shell metadata plus the default working directory as a
`PathUri`.
- Return connection and protocol failures as JSON-RPC errors.
- Bound the exec-server probe response to 30 seconds and remove
timed-out calls from the pending-request table so later environment
mutations can proceed.
- Cover successful responses, omitted working directories, unknown
environments, connection failures, and pending-call cleanup.

## Description
This PR adds canonical core `TurnItem` shapes for command execution,
dynamic tool calls, collab agent tool calls, and sub-agent activity, to
be stored in the rollout file soon.
It also teaches app-server protocol / `ThreadHistoryBuilder` how to
render those items, and adds the small legacy fanout helpers needed for
existing event-based consumers. No core producer or rollout persistence
behavior changes here, that will be done in a followup.
## Making ThreadHistoryBuilder stateless
This is the first PR in a stack to make `ThreadHistoryBuilder` stateless
enough that we can materialize app-server `ThreadItem`s from only a
given slice of `RolloutItem` history, without ever needing to replay the
whole thread from the beginning.
The persisted legacy `RolloutItem::EventMsg` records are mostly shaped
like live UI events, not like materialized `ThreadItem`s. They work if
we replay the full rollout in order, but they often do not contain
enough stable identity or complete item state to project an arbitrary
suffix on its own.
A few examples:
- `UserMessageEvent` and `AgentMessageEvent` have content, but
historically do not carry the persisted app-server item ID that should
become the SQLite primary key.
- `AgentReasoningEvent` and `AgentReasoningRawContentEvent` are
fragments. `ThreadHistoryBuilder` currently merges them into the last
reasoning item, which means a slice starting in the middle of reasoning
cannot know whether to append to an earlier item or create a new one.
- `WebSearchEndEvent`, `McpToolCallEndEvent`, collab end events, and
similar legacy events can often render a final-looking item, but they
usually rely on prior replay state to know which turn owns the item.
- Begin/end legacy events are partial views of one logical item. The
builder correlates them by `call_id` and mutates prior state to
synthesize the final `ThreadItem`.
That is the problem this direction fixes. A persisted canonical
lifecycle record looks much closer to the read model we actually want
later:

Once rollout has explicit `turn_id`, stable `item.id`, and a canonical
completed item snapshot, the future SQLite projector can reduce only the
new rollout suffix and upsert the affected `thread_items` rows. It no
longer needs to synthesize `item-N`, infer item ownership from the
active turn, or replay earlier events just to reconstruct the current
item snapshot.
## What changed
- Added core `TurnItem` variants and item structs for command execution,
dynamic tool calls, collab agent tool calls, and sub-agent activity.
- Added conversions from those canonical items back into the legacy
event shapes where current consumers still need them.
- Added app-server v2 `ThreadItem` conversion for the new core item
variants.
- Taught `ThreadHistoryBuilder` and rollout persistence metrics to
recognize the new item variants.
## Follow-up
The next PR https://github.com/openai/codex/pull/30283 switches the live
core producers for these item families onto canonical `ItemStarted` /
`ItemCompleted` events.
## Why
Marketplace source deserialization treated `{"source":"npm", ...}` as
unsupported. The loader logged and skipped the entry, so npm-backed
plugins never appeared in `plugin list --available` and `plugin add`
returned "plugin not found".
Codex plugins are installed from a plugin root, not from an npm
dependency tree. For npm-backed marketplace entries, Codex should fetch
the published package contents without running package scripts or
installing unrelated dependencies.
## What changed
- Add `npm` marketplace plugin sources with `package`, optional semver
`version` or version range, and optional HTTPS `registry`.
- Reject unsafe npm source fields before materialization, including
invalid package names, non-semver version selectors, plaintext or
credential-bearing registry URLs, and registry query/fragment data.
- Materialize npm plugins with `npm pack --ignore-scripts`, then unpack
the resulting tarball through the existing hardened plugin bundle
extractor.
- Enforce npm archive and extracted-size limits, require the standard
npm `package/` archive root, and verify the extracted `package.json`
name matches the requested package before installing.
- Keep plugin listings, install-source descriptions, CLI JSON/human
output, app-server v2 `PluginSource`, TUI source summaries, regenerated
schema fixtures, and app-server documentation in sync.
## Impact
Marketplaces can distribute Codex plugins from public or configured
private HTTPS npm registries using the same install flow as existing
materialized plugin sources. `npm` must be available on `PATH` when an
npm-backed plugin is installed.

- The full focused command finished with `1739 passed`, `11 failed`, and
`6 timed out`; the failures were unrelated local app-server environment
failures from `sandbox-exec: sandbox_apply: Operation not permitted`
plus one missing `test_stdio_server` helper binary.
- Installed an npm-published Codex plugin package through a throwaway
local marketplace and throwaway `CODEX_HOME` to exercise the real npm
materialization path end to end.
## Description
This adds stable optional `turnId` support to `thread/fork`. When
supplied, the fork copies persisted history through that terminal turn,
inclusive, and drops later turns from the new thread.
Omitting or passing `null` preserves the existing full-history fork
behavior, including the interruption marker when the stored source
history ends mid-turn.
## Why
We're deprecating `thread/rollback` and this will help certain UX use
cases work around it by using `thread/fork` + `turn_id` instead.
