Tamir Duberstein b06b7d2f77 Upgrade Windows bindings to windows-sys 0.61.2 (#50058)
## What changed

- Centralize `windows-sys` at version `0.61.2` in the workspace and migrate dependent crates to it.
- Adapt Windows API calls to the updated handle, boolean, and type definitions. Replace custom handle owners with `OwnedHandle` and use `BorrowedHandle` where ownership stays with the caller.
- Add explicit wrappers for non-owning Windows handles passed between threads.

## Testing

Enable the MCP refresh cleanup test on Windows. Retain process handles to verify that a superseded MCP server stays alive during an in-flight call, exits after cancellation, and leaves the replacement running until shutdown.

Nick Steele 15fd656ddb Discover and validate MCP authorization servers before ID-JAG exchange (#49478)
## What changed

Add `exchange_ema_auth_token` to discover and validate the MCP resource's authorization server before resolving enterprise identity credentials and performing the ID-JAG token exchange. Keep the lower-level exchange primitive private.

- Probe `server/discover` with the enterprise-managed authorization extension, support recognized legacy discovery rejections, and follow protected-resource metadata challenges or well-known fallbacks.
- Match configured resource and issuer constraints, require advertised public-client JWT bearer support, and validate token endpoint metadata without requiring an authorization endpoint.
- Bound discovery with a shared timeout, redirect and authorization-server limits, reject cross-origin metadata redirects, and restrict private authorization-server destinations while allowing local loopback discovery.

## Testing

Add tests covering resource and issuer mismatches before identity access, metadata capability validation, query preservation, modern challenges, JSON and SSE legacy fallback behavior, cross-origin redirect rejection, and private or malformed issuer destinations.

open-matt b8d5e3f12e Allow exec-server to proxy permitted private IPs upstream (#48568)
## Why

Private IP destinations always bypassed inherited upstream proxies, preventing their use for private networks reachable through an upstream VPN proxy.

## What changed

- Add `codex exec-server --proxy-private-ips-via-upstream`, also configurable with `CODEX_EXEC_SERVER_PROXY_PRIVATE_IPS_VIA_UPSTREAM=true`. The setting defaults to disabled.
- Allow permitted RFC 1918, carrier-grade NAT, and IPv6 unique-local destinations to use an applicable upstream proxy. Loopback and link-local destinations retain direct routing, and destination access policy still applies.
- Keep connections direct when no valid upstream proxy applies or `allow_upstream_proxy=false`. Errors after selecting an upstream proxy do not trigger a direct retry.
- Rename `ExecServerRuntimePaths` to `ExecServerRuntimeOptions` and carry the routing setting from executor startup into the managed network proxy.

## Testing

Add routing coverage for private address ranges, special-use addresses, and public targets with the option enabled and disabled. Verify that HTTP and CONNECT requests still enforce destination allowlists and denylists, and update the CLI help snapshot.

Sean Huang 8f5387d104 Honor execution environment proxy requirements (#48198)
## Why

An execution environment's enabled proxy configuration was reduced to traffic restrictions, leaving restricted commands offline without a controller proxy or an approved network grant.

## What changed

Carry `NetworkProxyConfig.enabled` into `EnvironmentNetworkPolicy.requires_proxy` and honor it when activating managed networking. This lets environment owners require filtered proxy access while direct network access remains restricted. Policies without an explicit proxy requirement retain the existing activation behavior.

## Testing

Extend remote command coverage to check allowed and denied proxy requests without a controller proxy, and verify that direct network access remains blocked.

Sean Huang 2457571e4c Preserve local-binding inheritance in environment network policies (#47898)
## Why

Treating an omitted `allow_local_binding` setting as `false` prevents environment policies from inheriting local-binding permissions.

## What changed

Keep `allow_local_binding` optional when composing controller and environment policies. Preserve explicit denials from either policy and resolve executor defaults after composition. Without a controller, default to denying local binding unless the environment explicitly allows it.

## Testing

Add coverage for omitted, allowed, and denied settings, managed constraints, executor defaults, and remote launch rejection when local-binding restrictions cannot be enforced. Extend remote network approval tests with loopback requests to verify controller and environment policy composition.

viyatb-oai e4dba902ab Support caller-provided MITM CAs in the network proxy (#47132)
## What changed

- Add optional `network.mitm_ca` configuration with `certificate_file` and
`private_key_file` for trusted proxy configuration. Require MITM to be enabled
and retain the generated CA when no external CA is configured.
- Validate absolute paths, regular files, file sizes, certificate/key matching,
and Unix private-key permissions. Reject symlinks on Unix, private keys embedded
in certificate files, and external private keys on non-Unix platforms.
- Store derived external-CA trust bundles in the proxy artifact directory,
include only certificates, and skip inherited generated bundles to avoid
  retaining stale roots.
- Preserve MITM for external CAs when disabling credential brokerage. Reject
external CA configuration for remote execution, even when the proxy is disabled,
and reject unknown fields in `NetworkProxyConfig`.
- Default the upstream request version adapter to HTTP/1.1 and remove the request
URI from upstream request failure context.

## Testing

Add regression coverage for external CA configuration, mismatched keys, embedded
private keys, non-Unix rejection, trust-bundle placement and contents, stale-root
exclusion, credential-broker toggling, and remote configuration rejection.

viyatb-oai 94e3c20906 Add a standalone network proxy binary with JSON configuration (#46573)
## Why

Allow the network policy proxy to run without a full Codex permissions profile.

## What changed

- Add `codex-network-proxy --config <PATH>` to load a standalone JSON file containing a `network` object and run the proxy independently of Codex.
- Require `network.enabled = true`, limit configuration size to 1 MiB, and reject unknown fields, including nested MITM hook fields, and trailing JSON values.
- Enable HTTPS MITM automatically for limited mode or configured `mitm_hooks`, while supporting explicit `mitm` configuration.
- Document the standalone configuration and invocation.

## Testing

Add tests for MITM defaults, dynamic policy keys, unknown fields, and trailing JSON values. Add a regression test that verifies `wait()` promptly returns an error when either the HTTP or SOCKS listener fails.

iceweasel-oai a633ebc124 Always use private desktops for legacy Windows sandboxes (#46554)
## What changed

- Remove the private-desktop opt-out from elevated and unelevated Windows sandbox launches.
- Remove `windows.sandbox_private_desktop` and its managed requirement and API fields. Warn users to remove the obsolete setting.
- Require a private desktop name when launching through the Windows sandbox wrapper and command runner.

## Testing

Add coverage for the obsolete-setting migration warning and update wrapper tests to verify a live private desktop is passed and a missing desktop name is rejected.

iceweasel-oai 74af2496c9 Default local binding to true for MXC managed networking (#46523)
## Why

MXC's native host-loopback access is bidirectional, so it cannot enforce `allow_local_binding = false`. Treating an omitted setting as `false` prevents managed networking from working with the default configuration.

## What changed

- Preserve an omitted `allow_local_binding` until the executor's sandbox policy is known. Default to `true` for Windows MXC and `false` elsewhere, including remote execution.
- Reject an effective `false` for MXC managed networking after applying policy restrictions, without enabling disabled networking.
- Use the executor's resolved value for remote network approval decisions while preserving explicit controller restrictions.
- Document that local binding permits local servers and direct host-loopback connections and skips additional private-network destination checks; proxy domain rules still apply.

## Testing

Add coverage for per-executor defaults, explicit values, MXC rejection of `false`, and remote network review cleanup with the resolved policy.

Sean Huang 3724dc8361 Share platform identity across path, network, and sandbox configuration (#46334)
## What changed

- Add `Platform` to `codex-utils-path-uri` with metadata parsing, native platform detection, and path convention mapping. Preserve missing or unrecognized metadata as `Unknown`.
- Replace `NetworkProxyExecutorOs` with the shared type and keep executor-specific socket path validation in the network proxy.
- Extract `effective_sandbox_mode` with explicit platform and Windows sandbox level inputs, preserving the native Windows fallback from `workspace-write` to `read-only` when the sandbox is disabled.

## Testing

Add unit tests for platform metadata, path conventions, native platform detection, and sandbox mode selection across platforms and Windows sandbox levels.

Sean Huang ea218f5cd8 Validate network socket policies using the executor OS (#46302)
## Why

A controller and its executor can run different operating systems. Validating socket paths against the controller's OS can reject absolute paths that are valid on the executor, such as Windows paths on a Linux controller.

## What changed

- Thread `NetworkProxyExecutorOs` through network policy validation, proxy construction, and policy updates.
- Require allowed socket paths to be NUL-free and absolute for the executor OS, while preserving deny entries unchanged.
- Accept either Unix or Windows absolute syntax when executor metadata omits the OS, then validate against the executor's own OS at launch.
- Keep native path normalization and socket support checks at execution time.

## Testing

Add coverage for cross-platform absolute path syntax, invalid allow entries, preserved deny entries, and remote policy round trips that retain executor semantics through domain edits and proxy construction.

viyatb-oai 47c27cbffa Document and test ? wildcards in network proxy domain patterns (#46027)
## What changed

Document the existing `?` wildcard behavior: it matches exactly one character,
including a dot, in allow and deny patterns. For example, `api?.example.com`
matches `api1.example.com`, but not `api.example.com` or `api12.example.com`.

## Testing

Add table-driven tests for both allowlist and denylist compilation, covering
single and repeated `?` wildcards, combinations with `*`, `*.`, and `**.`,
dot matching, and host and pattern normalization.

open-matt 43354d0f61 Preserve attachment Unix socket grants when controller policy is omitted (#46004)
## Why

An omitted controller socket policy was treated as an explicit denial, preventing execution environments from supplying their own Unix socket grants.

## What changed

- Preserve omission of `dangerously_allow_all_unix_sockets` separately from `false`, and retain explicitly empty `unix_sockets` maps.
- Defer to attachment socket permissions when the controller supplies neither setting. Continue enforcing explicit restrictions, socket denials, and managed requirements.
- Resolve omitted values to `false` for ordinary execution and remote configuration, preserving the default for commands without attachment grants.
- Add debug logging for effective environment and remote execution network policies.

## Testing

Add regression coverage for omitted, explicit, finite, empty, and managed socket policies through remote launch configuration, including live policy replacement and serialization round trips. Adjust remote environment tests to tolerate child-completion ordering and box large cold-resume test futures to reduce Windows stack usage.

Jonathan Kula 53401a2808 Use native DNS resolution for the network proxy on macOS (#45982)
## Why

The default resolver misses macOS supplemental resolvers and VPN split DNS, preventing Codex from reaching hosts that depend on those DNS settings.

## What changed

Use `tokio::net::lookup_host` for IPv4 and IPv6 lookups in the macOS TCP connector, including connections to upstream proxies. Preserve existing checks on resolved destination addresses and reject unsupported TXT lookups without falling back to a different resolver.

## Testing

Add macOS tests for IPv4 and IPv6 localhost resolution and connections, local-network rejection, and resolving the upstream proxy instead of the destination.

iceweasel-oai a5c15ab5c0 Wire Windows sandbox selection into managed proxy routing (#45757)
## What changed

- Track the Windows sandbox implementation separately from its legacy setup level.
- Select dedicated proxy listeners for `WindowsMxc` on Windows across sessions, app-server command execution, and the sandbox CLI.
- Rebuild the session proxy when routing changes, retaining its network policy decider.
- Add a sandbox CLI execution path through the MXC sandbox transform when `WindowsMxc` is selected.

## Testing

Extend configuration assertions for the selected sandbox type and the session refresh test to verify dedicated routing and policy decider retention on Windows.

viyatb-oai 99914f4950 Honor explicit Unix socket grants in the Linux managed sandbox (#45534)
## Why

Linux proxy-routed sandboxing denied standalone Unix sockets even when the effective network policy enabled `dangerously_allow_all_unix_sockets`.

## What changed

- Carry Unix socket permissions in `ManagedNetworkSandboxContext` and pass the prepared context through Linux sandbox launches with `--managed-network`.
- Allow `AF_UNIX` socket creation in proxy-routed mode when `dangerously_allow_all_unix_sockets` is enabled, while preserving network namespace isolation and restrictions on other socket families.
- Keep standalone Unix sockets denied by default and for path-only grants. Default missing fields in older serialized contexts to restrictive values.

## Testing

Add coverage for policy preparation and transport, legacy deserialization, and malformed policy rejection. Add a Linux integration test covering default denial, path-only denial, and explicit allow-all access, while checking that direct TCP access and `AF_NETLINK`/`AF_VSOCK` sockets remain blocked.

iceweasel-oai 99b3ab2131 Allow dedicated listeners for managed network proxies (#45463)
## Why

Sandboxes that enforce endpoint access directly need dedicated loopback proxy ports instead of shared SID-attributed ingress.

## What changed

Expose `ManagedProxyRouting` through `NetworkProxyBuilder::managed_proxy_routing`. Selecting `DedicatedListeners` reserves per-proxy loopback listeners on Windows as well as other platforms. Keep `SharedIngress` as the default and include the routing mode in proxy equality.

## Testing

Add regression coverage for distinct loopback endpoints, sandbox port metadata, and HTTP and SOCKS allow/deny policy enforcement. On Windows, verify that dedicated routing requires no restricting SID and omits the shared-ingress proxy-port environment variable.

Winston Howes ed4ca07ba6 Handle credential provider source remapping across config layers (#44241)
## What changed

- Let higher-priority providers in `features.network_proxy.credentials` displace lower-priority providers with overlapping `env` sources, including in profiles. Preserve inherited settings when providers swap sources, and compare environment names case-insensitively on Windows.
- Preserve provider definitions across ordered batch remaps and persist displaced-provider removals without overwriting unrelated sibling updates.
- Validate remapped providers using the credential broker's compilation rules before persisting writes, while allowing incomplete drafts and explicit deletions.
- Omit displaced providers from config origins and report writes overridden by another provider's source ownership.
- Build trusted credential-broker configuration before merging managed layers for project discovery, preserving remapped providers' environment protections.

## Testing

Add unit and config RPC regression tests for source precedence, source swaps, ordered edits, concurrent sibling updates, atomic rejection of invalid remaps, persistence, and override reporting. Add a macOS managed-config regression test for remapped provider bindings during project discovery.

Winston Howes 634ebc1865 Support credential brokering in plaintext HTTP tunnels (#44089)
## Why

Configured HTTP credential destinations need dummy credentials translated inside `CONNECT` and SOCKS5 tunnels, where credential interception previously detected only TLS.

## What changed

- Detect plaintext HTTP for configured credential destinations and proxy requests with URL-scoped credential substitution.
- Keep requests bound to the authorized tunnel destination, rejecting mismatched authorities and nested `CONNECT` requests.
- Support HTTP/2 clients and preserve HTTP upgrades, including `h2c`, and opaque traffic in full mode.
- Enforce limited-mode method restrictions on plaintext HTTP and reject upgrades and opaque traffic.

## Testing

Add end-to-end tests for both tunnel transports covering credential URL scope, request bodies, HTTP/2, destination mismatches, upgrades, lossless opaque forwarding, and limited-mode enforcement.

Winston Howes 38cbebaf3f Support configured credential providers across shell snapshots (#44072)
## Why

Credential brokerage must retain trusted destination hints even when shell environment policy hides them from child processes. Snapshot replay also needs to preserve credential aliases when tokens are short or captured dummy values change.

## What changed

- Add `features.network_proxy.credentials` and preserve provider settings when toggling the proxy. Prevent project configuration from overriding providers or their credential and destination environment variables.
- Use provider metadata throughout snapshot capture and replay, and pass hidden destination context to the broker without exposing it in the child environment.
- Rewrite aliases containing short credentials or previously captured dummies, respecting environment filters and explicit overrides.
- Preserve case-distinct `shell_environment_policy.set` keys so Windows credential ambiguity checks survive configuration merging and permission profile changes.
- Restore independent MITM settings when credential brokerage is disabled.

## Testing

Add regression coverage for custom providers, hidden destination hints, Bash and Zsh alias replay, configuration edits and merging, Windows ambiguity handling, and credential protection during approved execution.

Winston Howes f45115a137 Preserve credential broker destinations across environment filtering (#44068)
## Why

Filtering destination variables out of a child environment can undo credential registrations. Rotating a provider token and destination must also leave older credential aliases bound to their original destinations.

## What changed

- Add `CredentialBrokerContext` to retain local destination hints for built-in and configured providers without adding them to child environments or serialized configuration. Explicit environment values, including empty values, override these fallbacks.
- Preserve registered destinations when hints are absent, reconcile fallback changes, and keep credential aliases scoped to their source identities and environments during rotation and inheritance.
- Expose provider context and source matching for trusted captured text, including configured provider context keys.
- Disable brokerage on Windows when provider environment overrides contain conflicting case-insensitive keys.

## Testing

Winston Howes ec512d2347 Harden credential handling in shell snapshots and replay (#44040)
## Why

Shell quoting can hide credentials from raw-text checks, and startup files can restore real credentials after the broker replaces them with dummy values.

## What changed

- Decode shell literals without evaluating them and reject snapshots containing credentials in executable source, including aliases, functions, and heredocs.
- Preserve credential policy overrides, explicit unsets, and aliases whose source variables were removed. Support credential aliases in Zsh tied arrays while rejecting credentials that span array elements.
- Guard snapshot replay against credential restoration through shell startup files and preserve unrelated `ENV` settings.
- Apply Windows environment-key casing rules to credential overrides, suppress unredacted sandbox diagnostics during snapshot capture, and clear inherited environment variables before launching escalated commands.

Winston Howes a548463b78 Handle copied credentials in the broker and shell snapshots (#44038)
## Why

Shell startup can copy a credential into another variable and unset its original source. These aliases need credential brokering without losing source restrictions or mistaking ordinary shell content for credentials.

## What changed

- Discover supported GitHub and OpenAI credentials embedded in environment values, even without their canonical variables. Preserve source ownership so unbound enterprise tokens cannot acquire a default host binding.
- Redact unregistered supported credentials during text virtualization, distinguish adjacent credentials, and avoid matching unrelated provider prefixes and common hashed paths.
- Expose helpers for checking allowed credential sources and restoring known dummy credentials in trusted text for fail-open execution.
- Render allowed credential aliases as snapshot exports and replace known credential values in shell state, preserving exported functions and credential-shaped function names.

## Testing

Add regression coverage for copied aliases, enterprise host binding, source filtering, adjacent tokens, path false positives, and dummy restoration. Add a Bash snapshot test that preserves exported functions while replacing credentials in heredoc content.

Winston Howes 808b3411fd Cache protected shell snapshots and harden capture cleanup (#43954)
## Why

Credential-brokered commands rebuild shell snapshots on every invocation. Snapshot capture failures can also include credential-bearing startup output, and descendants holding output pipes need cleanup even after the shell exits.

## What changed

- Cache successful protected snapshots per environment, keyed by working directory, shell, login mode, and sandbox configuration. Rebuild missing or stale snapshots while keeping concurrent captures independently cancellable.
- Invalidate snapshots when shell settings or credential broker configuration change, and retry capture once if broker configuration changes during startup.
- Restore credentials and associated provider context while respecting environment policy filters and explicit overrides.
- Keep full-buffer capture subject to timeout and cancellation through output draining, clean up descendants on capture expiration or drain failure, and omit startup output from snapshot errors.

## Testing

Winston Howes 2e220af1f6 Protect shell snapshots when credential brokerage is enabled (#43909)
## Why

Shell startup files can introduce credentials or overwrite brokered dummy values. Snapshot capture and replay need to respect the command's sandbox and environment policy without persisting real credentials.

## What changed

- Capture and validate brokered snapshots per command using its requested shell, working directory, login mode, sandbox, and environment policy.
- Discover shell-initialized credentials and prepare snapshot exports for brokered replay. Preserve shell functions and aliases while preventing startup files from overwriting dummy credentials during replay.
- Rebuild snapshots as credential broker state changes and retain each command's snapshot for the lifetime of its process.
- Reject brokered POSIX shell execution when a protected snapshot cannot be created, and reject brokerage with `shell_zsh_fork`. Restore real credentials and remove brokerage markers for unsupported PowerShell and Cmd execution; escalated commands bypass protected snapshots.

## Testing

Add regression coverage for credential discovery and redaction, environment filtering, readonly exports, shell function and heredoc replay, command working directories and login modes, sandbox enforcement, and missing or disabled snapshots.

jelson-oai 5a65fd87d8 Close active network proxy connections on teardown (#43884)
## Why

Stopping proxy listeners could leave accepted connections and half-closed tunnels alive after their owning thread was unloaded.

## What changed

Tie HTTP and SOCKS5 connection work, including HTTP CONNECT upgrades, to the lifetime of the proxy listeners. Cancel connections across the main and environment proxies when the handle is dropped, shutdown is requested, or `wait()` is canceled. Explicit shutdown waits for connection cleanup.

## Testing

Add live TCP regression tests for HTTP keep-alive connections and open or half-closed HTTP CONNECT and SOCKS5 tunnels across shutdown, handle drop, and canceled waits. Verify that half-closed tunnels still carry return traffic while the proxy is running, and that repeated thread creation and unloading closes managed proxy tunnels.

jif 8e85265c39 Handle pending network reviews after process completion (#42746)
## Why

A remote process can finish while a network policy review is still pending. Normal process cleanup should withdraw that review without turning the completed command into a review failure or losing its output.

## What changed

- Record whether a network policy request was withdrawn because the process finished, was cancelled, lost its executor connection, or timed out.
- Treat normal process completion as cleanup while retaining fail-closed behavior for other cancellation causes.
- Preserve explicit network denials before policy persistence so cleanup cannot replace the reported call outcome.

## Testing

Add an integration test that completes a remote process during a pending network review and verifies that the command reports its successful exit and output without approving the withdrawn request.

viyatb-oai f53c91be2c Add attributed exec process lifecycle telemetry (#42373)
## Why

Exec-server process and network-policy events need stable attribution to the
launching tool call and executor without recording process payloads.

## What changed

- Add optional `ExecMetadata` to `ExecParams` and propagate the thread and tool
  call IDs from unified exec.
- Emit bounded OpenTelemetry events for process start, spawn failure, sandbox
denial, and exit. Correlate them with the launch trace and registry-issued
executor identity while excluding arguments, paths, environment values,
  output, and error text.
- Preserve launch attribution across long-running processes and reconnects, and
attach the same metadata to network-policy audit events.
- Keep the protocol backward compatible when metadata is omitted, and prevent
invalid trace headers from inheriting an unrelated active span.

## Testing

Add coverage for metadata serialization and propagation, lifecycle event
fields, trace relationships, reconnect behavior, spawn failures, sandbox
denials, and network-policy attribution.

chess 7e1ee6df6c Reuse private desktops across Windows sandbox commands (#40672)
## Why

Private desktops need to remain available across command-runner exits and idle
gaps.

## What changed

- Keep parent-owned private desktops alive and reuse them only when the sandbox
  account and effective permissions match.
- Pass the selected desktop name to elevated command runners, which validate and
open the existing desktop instead of creating their own.
- Apply the same policy-aware reuse to the legacy Windows sandbox path.

## Testing

Add Windows tests for desktop-name validation, lifetime and concurrent reuse,
and separation when effective permissions differ.

Winston Howes 0280d73c1e Broker credential aliases in child environments (#40484)
## What changed

- Discover supported credentials inherited from the parent even when their canonical provider variable is filtered from the child environment.
- Replace matching values and credentials embedded in longer child environment values with broker dummies, then restore only aliases that the broker virtualized.
- Preserve provider and host bindings when selecting credentials for outbound requests, including GitHub Enterprise and custom OpenAI hosts.
- Expose helpers for identifying provider and binding variables, plus an API for virtualizing credential-bearing text before it is persisted.

## Testing

- Extend credential broker tests for filtered parent variables, exact and embedded aliases, text virtualization, user overrides, and host-bound GitHub and OpenAI credentials.

Winston Howes c3d5e4418f Add credential brokering to network proxy feature config (#40466)
## What changed

- Add `features.network_proxy.credential_broker` and preserve structured network proxy settings when toggling the feature through config APIs.
- Protect broker enablement and provider context variables from project config, while carrying trusted context into filtered shell environments.
- Propagate brokered credentials through shell snapshots and restore real credentials for unsandboxed escalated commands.
- Normalize broker context environment keys case-insensitively on Windows and avoid conflicting remote managed-network enforcement when no proxy launch configuration is available.

## Testing

- Cover config layering, editing, project sanitization, Windows environment handling, shell snapshots, escalated execution, and end-to-end GitHub credential brokering in the zsh-fork runtime.

sayan-oai f580dd886f Enforce environment network policies for remote execution (#39980)
## Why

`EnvironmentConfig.network_policy` could describe attachment-owned traffic
restrictions, but core rejected every configured policy because execution did
not enforce it.

## What changed

- Resolve each remote environment's network policy for the selected command
and apply it to the execution-scoped proxy.
- Compose owner rules with controller constraints and saved network decisions
while preserving inherited domain and Unix-socket denials.
- Keep strict allowlists non-expandable, allow reviewable policies to use
network approvals, and reject sandbox escalation that would bypass an owner
  policy.
- Reject policies for local execution, disabled managed enforcement, or a
  disabled controller proxy.

## Testing

Added coverage for policy composition, scoped remote proxy behavior, approval
and denial flows, offline execution, and unsupported environment authority.

Dylan Hurd ba37d0c45b Report network disconnects during approval (#39284)
## Why

When a local proxy request disconnects before network approval completes, the
owning tool call needs a model-visible explanation instead of remaining tied to
the abandoned request.

## What changed

- Track disconnect timing while plain HTTP and CONNECT requests await policy
  decisions.
- Cancel the owning execution and report how long the request waited when it
  disconnects before approval completes.
- Preserve an explicit approval outcome when disconnect cleanup runs afterward.

## Testing

Added unit coverage for disconnect tracking and outcome precedence, plus
end-to-end coverage for plain HTTP and CONNECT requests.

sayan-oai 4a7b51c560 Add network policy metadata to environment configuration (#39055)
## What changed

- Add `EnvironmentNetworkPolicy` as a serializable view of portable domain,
  Unix-socket, upstream-proxy, and local-binding restrictions without exposing
  controller-owned proxy runtime settings.
- Add an optional `network_policy` field to `EnvironmentConfig` and re-export
the policy and related permission types through the core APIs.
- Reject owner-provided network policies during environment preview and
readiness until runtime enforcement is implemented, while preserving the
existing controller policy when the field is absent.

## Testing

- Cover rejection through both environment preview and readiness, including
preservation of the existing environment selection.

viyatb-oai 15fde8c1f2 Forward executor network policy decisions for auditing (#38670)
## What changed

- Add a best-effort `network/policyDecision` notification for final domain and non-domain policy decisions made by executor-local proxies.
- Validate notifications against the active process on the controller and emit audit events with controller-trusted session and execution metadata.
- Reserve outbound RPC capacity so audit notifications cannot block control messages, and expose valid `chatgpt-account-id` header values for audit attribution.

## Testing

- Cover notification serialization, proxy decision capture, executor-to-controller delivery, trusted metadata handling, and reserved RPC capacity.

chess 631bbb33cc Use bounded fallback ports for Windows managed proxies (#38265)
## What changed

- Try the explicitly configured Windows HTTP or SOCKS5 proxy port first, then scan the protocol's preferred port range when that port is unavailable.
- Reserve HTTP and SOCKS5 listeners independently so a SOCKS5 collision does not replace an already available HTTP listener.
- Retain ephemeral loopback fallback when all preferred ports are unavailable, and document the HTTP (`3128-3159`) and SOCKS5 (`8081-8112`) ranges.

## Testing

- Cover bounded HTTP and SOCKS5 fallback, independent listener reservation, and ephemeral fallback after preferred-port exhaustion.

Winston Howes eea28321ad Harden network proxy credential brokerage (#38049)
## What changed

- Bind OpenAI credentials to `api.openai.com` and trusted HTTPS hosts derived from local configuration or `OPENAI_BASE_URL`.
- Support GitHub credentials for `uploads.github.com` and translate Bearer, token, and Basic authorization forms while preserving their schemes.
- Apply credential broker configuration changes without restarting the proxy, including clearing affected credential mappings when trusted OpenAI hosts change.
- Restore brokered credentials in environments and command arguments that intentionally leave managed networking, while preserving user overrides and handling Windows environment keys case-insensitively.
- Keep local credential brokerage out of remote proxy launch configuration and avoid launching a remote proxy when brokerage alone enabled the local proxy.

## Testing

Added coverage for trusted host binding, authorization translation, live reconfiguration, credential restoration, managed-disabled behavior, remote launch configuration, and Windows environment key matching.

viyatb-oai 9742cc8ed5 Ignore Unix socket proxy settings on Windows (#37889)
## Why

Unix socket proxy permissions are macOS-only, but configuring them could still
clamp Windows proxy listeners to loopback and emit warnings about unsupported
settings.

## What changed

- Exclude Unix socket permissions from Windows runtime settings and bind-address
  clamping.
- Suppress the macOS-only Unix socket warning on Windows.
- Preserve the original configuration for state updates and remote launch
  configuration.

## Testing

Add coverage for proxy startup, live configuration replacement, remote launch
configuration, and warning behavior with Unix socket permissions configured.

kevinlin-openai 7a0e974e08 Harden network proxy MITM authorization (#37211)
## Why

MITM hooks authorize requests before the upstream server parses them. Paths that
can be decoded or normalized to a different resource must not match an allowed
path, and hosts that require MITM inspection must not bypass it through the
plain HTTP proxy path.

## What changed

- Reject ambiguous hook paths, including traversal segments, backslashes,
malformed percent encodings, and encoded separators or percent signs.
- Block plain HTTP proxy requests for hosts whose policy always requires MITM,
  recording the decision as `mitm_required`.

## Testing

- Cover safe and ambiguous path forms, encoded traversal through repository
allowlists, and absolute-form HTTPS requests sent to the HTTP proxy.

## Why

Concurrent tests without a tracing subscriber can cache the network policy

- Bound concurrent callbacks and fail closed when requests are invalid, exceed capacity, time out, or outlive their process session.
- Replace the callback opt-in flag with a launch-level `policyDecisionTimeoutMs` value and include transport overhead in the executor timeout.
- Preserve callback admission limits across connection recovery and clean up policy state when process startup is abandoned or a session ends.

viyatb-oai d838ea0f64 Add exec-server network policy callback types (#34620)
## What changed

- Define the `network/policyRequest` RPC payloads for associating a network request with a process and returning an `allow`, `deny`, or `ask` decision.
- Cover HTTP, HTTPS CONNECT, and SOCKS5 TCP/UDP requests, with shared size limits for callback fields.
- Add the backward-compatible `requestPolicyDecisions` remote proxy option so executors can opt in to sending policy decisions to clients.
- Re-export the new protocol types from `codex-exec-server`.

## Testing
