## Summary
- Let `close_agent` clean up an agent that is still registered in
`AgentRegistry` even when its underlying thread is already missing.
- Preserve the explicit-close boundary: for known stale thread-spawn
agents, mark the persisted spawn edge `Closed`, then treat
`ThreadNotFound` / `InternalAgentDied` as a successful close so the
registry slot can be released.
- Add a regression for MultiAgentV2 task-name targets where
`close_agent("worker")` succeeds after the worker thread has already
disappeared.
## Motivation
A worker can disappear from `ThreadManager` while its metadata still
exists in the root `AgentRegistry`. Before this change, the close tool
failed while trying to subscribe to the missing thread status, so it
never reached the cleanup path that releases the registered agent slot.
With `agents.max_threads = 1`, an explicit close of that stale task-name
agent could fail and leave the session unable to spawn a replacement.
## Scope
This PR intentionally does not add automatic stale-agent reaping to
`spawn_agent`, `resume_agent`, or `list_agents`. A thread being missing
from `ThreadManager` is not the same as an explicit close: persisted
open spawn edges are still the durable source of truth for resume and
task-name ownership until `close_agent` is called.

## Why
When Codex calls responsesapi, we currently send `session_id`,
`thread_id`, and `turn_id` among other things as
`client_metadata["x-codex-turn-metadata"]`. This PR adds
`forked_from_thread_id` which helps explain the "lineage" of a forked
thread.
## What's changed
- Track the immediate history source copied into a forked thread through
thread/session creation, including subagent and review turn metadata
paths.
- Include `forked_from_thread_id` in Codex turn metadata while
preventing turn-scoped Responses API client metadata from overwriting
Codex-owned lineage fields.

## Summary
Adds experimental `additionalContext` support to `turn/start` and
`turn/steer` so clients can provide ephemeral external context, such as
browser or automation state, without turning that plumbing into a
visible user prompt or triggering user-prompt lifecycle behavior.
## API Shape

## Context Injection
When provided, accepted entries are inserted into model context as
hidden contextual message items, not as visible thread user-message
items.

Values are not escaped. Each value is truncated to 1k approximate tokens
before wrapping.
For `turn/start`, accepted additional context is inserted before normal
user input. For `turn/steer`, additional context is merged only when the
steer includes non-empty user input; context-only steers still reject as
empty input.
## Dedupe Strategy
`AdditionalContextStore` lives on session state and stores the latest
complete additional-context map.
Each `turn/start` or non-empty `turn/steer` treats its
`additionalContext` as the current complete set of values. Entries are
injected only when the key is new or the exact entry for that key
changed, including `value` or `kind`. After merging, the store is
replaced with the provided map, so omitted keys are removed from the
retained set and can be injected again later if reintroduced.
Omitting `additionalContext`, passing `null`, or passing an empty object
resets the store to empty and injects nothing.
## What Changed
- Threads experimental v2 `additionalContext` through app-server into
core turn start and steer handling.
- Adds separate contextual fragment types for untrusted user-role
context and application developer-role context.
- Uses pending response input items so additional context can be
combined with normal user input without treating it as prompt text.

## Why
`/feedback` asks `ThreadManager` for the selected agent subtree before
it uploads logs. The previous live subtree path reconstructed
parent-child links by iterating every loaded thread and awaiting each
thread config snapshot, so unrelated loaded-thread state could stall
feedback subtree enumeration.
The loaded-thread set already belongs to
[`ThreadManagerState`](50e6644c94/codex-rs/core/src/thread_manager.rs).
Reading thread-spawn parents from the captured `CodexThread` session
sources at that boundary keeps unload and resume behavior manager-owned
while avoiding per-session config inspection.

## Why
[#23883](https://github.com/openai/codex/pull/23883) moved user-facing
`--profile` selection onto profile v2, and
[#23886](https://github.com/openai/codex/pull/23886) removed the old CLI
`config_profile` override path. Core still had a second legacy path:
`profile = "..."` could select `[profiles.*]` values while runtime
config was built. Keeping that resolver alive preserves the old
precedence model and profile-carrying surfaces even though profile
selection now points at `$CODEX_HOME/<name>.config.toml`.
## What
- Reject legacy top-level `profile = "..."` config while loading runtime
config, with an error that points callers at `--profile <name>` and
`<name>.config.toml` in the [core load
path](3d923366ec/codex-rs/core/src/config/mod.rs (L2524-L2531)).
- Remove the remaining profile-v1 merge points from runtime config
resolution, including features, permissions, model/provider selection,

## Why
Custom agent roles are ordinary config layers, so a role file can
already express `service_tier` just like other config values. The
spawned-agent tier path needs to preserve that effective role config and
follow the same precedence pattern as model/reasoning.
## What changed
- Apply an explicit spawn-time `service_tier` onto the child config
before role application, so a role config layer can override it just
like role-defined model/reasoning settings do.
- Validate the final effective child tier after the final child model is
known, while still falling back to the parent tier when no child tier
survives.
- Add focused integration coverage for both v1 and v2 proving role TOML
loads a service tier, spawned children keep that role-configured tier,
and a role tier wins over a conflicting spawn-time tier.

## Why
Full-history agent forks should continue from the same prompt prefix as
the parent. Dropping the stored `TurnContext` baseline forced the child
to rebuild startup context on its first turn, which can duplicate
developer instructions and also loses the cache continuity that a
full-history fork is supposed to preserve.
Truncated forks are different: once we keep only the last N turns, the
original prompt prefix is no longer intact, so the child must establish
a fresh context baseline.
## What changed
- Preserve `RolloutItem::TurnContext` when forking with
`SpawnAgentForkMode::FullHistory`, and keep dropping it for truncated
forks:

## Summary
The first three PRs in this stack are a cleanup pass before the actual
thread settings API work.
Today, core has several overlapping "user input" ops: `UserInput`,
`UserInputWithTurnContext`, and `UserTurn`. They differ mostly in how
much next-turn state they carry, which makes the later queued thread
settings update harder to reason about and review.
This PR starts that cleanup by adding the shared
`ThreadSettingsOverrides` payload and allowing `Op::UserInput` to carry
it. Existing variants remain in place here, so this layer is mostly a
behavior-preserving API shape change plus mechanical constructor
updates.
## End State After PR3
By the end of PR3, `Op::UserInput` is the only "user input" core op. It
can carry optional thread settings overrides for callers that need to
update stored defaults with a turn, while callers without updates use
empty settings. `Op::UserInputWithTurnContext` and `Op::UserTurn` are
deleted.

## Why
Pending model input was split across `Session`, `TurnState`, and the
agent mailbox. That made it easy for new paths to manage queued user
input or mailbox delivery outside the intended ownership boundary.
This PR consolidates the model-facing input lifecycle behind the session
input queue so turn-local pending input, next-turn queued items, and
mailbox delivery coordination are owned in one place.
## What Changed
- Added `session/input_queue.rs` to own pending input queues and mailbox
delivery coordination.
- Removed the standalone `agent/mailbox.rs` channel wrapper and store
mailbox items directly in the input queue.

Remote compaction v2 was still using `context_compaction` as both the
request trigger and the compacted output shape. The Responses API now
has the landed contract for this flow: Codex sends a dedicated `{
"type": "compaction_trigger" }` input item, and the backend returns the
standard `compaction` output item with encrypted content.
This aligns the v2 path with that wire contract while preserving the
existing local compacted-history post-processing behavior.

- Add `ResponseItem::CompactionTrigger` and regenerate the app-server
protocol schema fixtures.
- Send `compaction_trigger` from `remote_compaction_v2` instead of a

This is the base PR in the split stack for the permissions migration. It
isolates stack-safety work that had been mixed into the larger
permissions PR, so reviewers can evaluate the async-future changes
separately from the permissions model changes in #22267.
The main risk this addresses is large or recursive multi-agent futures
overflowing smaller runner stacks. A follow-up review also called out
that `shutdown_live_agent` must remain quiescent: callers should not
remove a live agent from tracking or release its spawn slot until the
worker loop has actually terminated.
## What Changed
- Boxes the large async futures in the multi-agent spawn, resume, and
close tool handlers.
- Boxes the `AgentControl` spawn and recursive close/shutdown paths that
can otherwise build very deep futures.
- Keeps `shutdown_live_agent` waiting for thread termination before

Remove the remote thread-store backend and checked-in protobuf
artifacts. We've moved these into another crate that link against this
one.
Also remove the config settings for thread store backend selection,
since we'll instead pass an instantiated thread store into the core-api
crate's main entrypoint.

## Summary

## Why
Before this change, analytics `thread_source` was populated by a
best-effort mapping from `session_source`. `session_source` describes
the runtime/client surface, not the actual thread-level origin, so that
projection was not accurate enough to distinguish cases such as `user`,
`subagent`, `memory_consolidation`, and future thread origins reliably.
Making `thread_source` explicit keeps one thread-level analytics field
while letting callers provide the real classification directly instead
of recovering it indirectly from `session_source`.
## Impact
For new analytics events, `thread_source` now reflects the explicit
thread-level classification supplied by the caller rather than an
inferred value derived from `session_source`. Existing protocol fields
remain optional; callers that omit `threadSource` now produce `null`
instead of a best-effort inferred value.

## Why
We want the agent graph store to be passed down the stack as a real
dependency, the same way we already treat the thread store.
This will let us inject the agent graph store as a real dependency and
support implementations other than the local SQLite-backed one. Right
now most code instantiates a state DB and an agent graph store
just-in-time. Ideally, we would not depend on the state DB directly but
only read through the higher-level interfaces.
This change makes the dependency boundaries explicit and moves state DB
initialization to process bootstrap instead of hiding it inside local
store implementations.
## What changed
- `ThreadManager` now requires a `StateDbHandle` and an
`AgentGraphStore` at construction time instead of treating them as
optional internals.
- The local store constructors no longer lazily initialize SQLite.
Callers now initialize the state DB once per process and use that shared
handle to build:

SQLite state was still being opened from consumer paths, including lazy
`OnceCell`-backed thread-store call sites. That let one process
construct multiple state DB connections for the same Codex home, which
makes SQLite lock contention and `database is locked` failures much
easier to hit.
State DB lifetime should be chosen by main-like entrypoints and tests,
then passed through explicitly. Consumers should use the supplied
`Option<StateDbHandle>` or `StateDbHandle` and keep their existing
filesystem fallback or error behavior when no handle is available.
The startup path also needs to keep the rollout crate in charge of
SQLite state initialization. Opening `codex_state::StateRuntime`
directly bypasses rollout metadata backfill, so entrypoints should
initialize through `codex_rollout::state_db` and receive a handle only
after required rollout backfills have completed.
## What Changed
- Initialize the state DB in main-like entrypoints for CLI, TUI,
app-server, exec, MCP server, and the thread-manager sample.
- Pass `Option<StateDbHandle>` through `ThreadManager`,

- Switch app-server startup from direct
`codex_state::StateRuntime::init` to the rollout state initializer so
app-server cannot skip rollout backfill.
- Collapse split rollout lookup/list APIs so callers use the normal
methods with an optional state handle instead of `_with_state_db`
variants.
- Restore `getConversationSummary(ThreadId)` to delegate through
`ThreadStore::read_thread` instead of a LocalThreadStore-specific
rollout path special case.
- Keep DB-backed rollout path lookup keyed on the DB row and file
existence, without imposing the filesystem filename convention on
existing DB rows.
- Verify readable DB-backed rollout paths against `session_meta.id`
before returning them, so a stale SQLite row that points at another
thread's JSONL falls back to filesystem search and read-repairs the DB
row.
- Keep `debug prompt-input` filesystem-only so a one-off debug command
does not initialize or backfill SQLite state just to print prompt input.
- Keep goal-session test Codex homes alive only in the goal-specific
helper, rather than leaking tempdirs from the shared session test
helper.
- Update tests and call sites to pass explicit state handles where DB
behavior is expected and explicit `None` where filesystem-only behavior
is intended.

This adds the `remote_compaction_v2` client path so remote compaction
can run through the normal Responses stream and install a
`context_compaction` item that trigger a compaction.

- Build one app-server process ThreadStore from startup config and share
it with ThreadManager and CodexMessageProcessor.
- Remove per-thread/fork store reconstruction so effective thread config
cannot switch the persistence backend.
- Add params to ThreadStore create/resume for specifying thread
metadata, since otherwise the metadata from store creation would be used
(incorrectly).
Summary:
- Add codex-thread-manager-sample, a one-shot binary that starts a
ThreadManager thread, submits a prompt, and prints the final assistant
output.

## Why
`agents.max_depth` is a legacy multi-agent v1 guard. Multi-agent v2 uses
task-path routing and its own session/thread limits, so v2 should not
reject nested `spawn_agent` calls just because the thread-spawn depth
has reached the v1 maximum.
Keeping the v1 depth guard active in v2 prevents deeper task trees even
though the v2 path still needs the depth value only for lineage and
task-path metadata.
## What Changed
- Removed the depth-limit rejection from the multi-agent v2
`spawn_agent` handler while still computing child depth for lineage/path
metadata.
- Made the depth-based disabling of legacy `SpawnCsv`/`Collab` tools
apply only when `Feature::MultiAgentV2` is disabled.
- Added `multi_agent_v2_spawn_agent_ignores_configured_max_depth` to
cover a v2 child spawning another agent when `agent_max_depth = 1`,
while the existing v1 depth-limit tests continue to enforce the legacy

## Why
MultiAgentV2 sessions need startup guidance that matches the role of the
thread that is actually being created. Root agents and subagents have
different responsibilities, and forked subagents can inherit parent
rollout history. If the parent hint is carried into the child context,
the child can see stale or conflicting developer guidance before its own
session-specific context is added.
## What changed
- Added `features.multi_agent_v2.root_agent_usage_hint_text` and
`features.multi_agent_v2.subagent_usage_hint_text` config fields,
including schema/config parsing support.
- Injected the matching root or subagent hint into the initial context
as its own developer message when `multi_agent_v2` is enabled.
- Filtered configured MultiAgentV2 usage-hint developer messages out of
forked parent history so a child thread receives fresh guidance for its
own session source/config.
- Added targeted coverage for config parsing, initial-context rendering,
feature-config deserialization, and forked-history filtering.
## Context examples

and generated SDK/schema artifacts.
- Keep legacy config loading compatible, but make undo a no-op that
reports the feature is unavailable.
- Clean up core history, compaction, telemetry, rollout, and tests to
stop carrying ghost snapshot items.
## Testing
- Unit tests passed for `codex-protocol`, `codex-core` targeted undo and
compaction flows, `codex-rollout`, and `codex-app-server-protocol`.
- Regenerated config and app-server schemas plus Python SDK artifacts
and verified they match the checked-in outputs.
## Why
Config loading had become split across crates: `codex-config` owned the
config types and merge logic, while `codex-core` still owned the loader
that assembled the layer stack. This change consolidates that
responsibility in `codex-config`, so the crate that defines config
behavior also owns how configs are discovered and loaded.
To make that move possible without reintroducing the old dependency
cycle, the shell-environment policy types and helpers that
`codex-exec-server` needs now live in `codex-protocol` instead of
flowing through `codex-config`.
This also makes the migrated loader tests more deterministic on machines
that already have managed or system Codex config installed by letting
tests override the system config and requirements paths instead of
reading the host's `/etc/codex`.
## What Changed
- moved the config loader implementation from `codex-core` into
`codex-config::loader` and deleted the old `core::config_loader` module
instead of leaving a compatibility shim
- moved shell-environment policy types and helpers into
`codex-protocol`, then updated `codex-exec-server` and other downstream
crates to import them from their new home
- updated downstream callers to use loader/config APIs from
`codex-config`
- added test-only loader overrides for system config and requirements
paths so loader-focused tests do not depend on host-managed config state

Adds the app-server v2 goal API on top of the persisted goal state from
PR 1.
## Why
Clients need a stable app-server surface for reading and controlling
materialized thread goals before the model tools and TUI can use them.
Goal changes also need to be observable by app-server clients, including
clients that resume an existing thread.
## What changed
- Added v2 `thread/goal/get`, `thread/goal/set`, and `thread/goal/clear`
RPCs for materialized threads.
- Added `thread/goal/updated` and `thread/goal/cleared` notifications so
clients can keep local goal state in sync.
- Added resume/snapshot wiring so reconnecting clients see the current
goal state for a thread.
- Added app-server handlers that reconcile persisted rollout state

## Why
While debugging the Windows stack overflows we saw in
[#13429](https://github.com/openai/codex/pull/13429) and then again in
[#18893](https://github.com/openai/codex/pull/18893), I hit another
overflow in
`tools::handlers::multi_agents::tests::tool_handlers_cascade_close_and_resume_and_keep_explicitly_closed_subtrees_closed`.
That test drives the legacy multi-agent spawn / close / resume path. The
behavior was fine, but several thin async wrappers were still inlining
much larger `AgentControl` futures into their callers, which was enough
to overflow the default Windows stack.
## What
- Box the thin `AgentControl` wrappers around `spawn_agent_internal`,
`resume_single_agent_from_rollout`, and `shutdown_agent_tree`.
- Box the corresponding legacy `multi_agents` handler calls in `spawn`,
`resume_agent`, and `close_agent`.
- Keep behavior unchanged while reducing future size on this call path
so the Windows test no longer overflows its stack.
## Testing

## Why
Guardian analytics includes time-to-first-token, but the Guardian
reviewer runs as a normal Codex session and `TurnCompleteEvent` did not
expose TTFT. The timing needs to flow through the standard
turn-completion protocol so Guardian review analytics can consume the
same value as the rest of the session machinery.
## What changed
Adds optional `time_to_first_token_ms` to `TurnCompleteEvent` and
populates it from `TurnTiming`. The value is carried through app-server
thread history, rollout reconstruction, TUI/app-server adapters, and
Guardian review session handling.
Guardian review analytics now captures TTFT from the reviewer
turn-complete event when available. Existing tests and fixtures are
updated to set the new optional field to `None` where TTFT is not
relevant.

## Summary
This PR fully reverts the previously merged Agent Identity runtime
integration from the old stack:

It removes the Codex-side task lifecycle wiring, rollout/session
persistence, feature flag plumbing, lazy `auth.json` mutation,
background task auth paths, and request callsite changes introduced by
that stack.
This leaves the repo in a clean pre-AgentIdentity integration state so
the follow-up PRs can reintroduce the pieces in smaller reviewable
layers.
