jif 808d3c2702 Keep session defaults static during config batch writes (#34940)
## What changed

- Treat batch writes containing only `model`, `model_reasoning_effort`,
  `plan_mode_reasoning_effort`, `service_tier`, or `personality` as defaults for
future sessions, even when `reloadUserConfig` is requested.
- Keep runtime config reloads and skill/plugin cache invalidation for other
config changes, and document which settings remain session-static.
- Preserve cached skill listings after session-default writes until a forced
reload, with test coverage for each supported default.

felixxia-oai 48ebbf5334 Use the API plugin marketplace for Amazon Bedrock (#34931)
## Why

Amazon Bedrock configurations can operate without Codex auth, so auth mode alone
cannot determine which curated plugin marketplace to expose.

## What changed

- Pass the resolved model provider into plugin configuration.
- Select `openai-api-curated` when the resolved provider is `amazon-bedrock`,
while retaining API-key auth as a selection signal.
- Cover provider overrides and app-server plugin listing without an `auth.json`
  file.

jif e497325a6a Centralize thread MCP state in McpRuntime (#34930)
## What changed

- Make `McpRuntime` own the published MCP configuration, connections, elicitation routing, and selected capability roots for a thread.
- Capture immutable MCP bindings for model steps and tool calls so in-flight work keeps a consistent connection set and approval authority while refreshed state is published atomically.
- Mark MCP state dirty when relevant configuration, plugins, environments, authentication, or elicitation capabilities change, then rebuild it before the next sampling request or out-of-band MCP operation.
- Separate config reloads from server invalidation: `ReloadMcpConfig` applies resolved MCP inputs, while `RefreshMcpServers` requests reinitialization from the thread's latest state.

## Testing

- Cover refreshed state visibility for existing turns, stable step bindings, current approval authority, cancelled refresh retries, resource-client reconciliation, and Apps recovery between sampling requests.

Cam moten 4e0cee8030 Wait for local plugin cache refreshes in plugin/list (#34877)
## What changed

- Make `plugin/list` requests with `forceRefetch: true` wait for configured
local plugin caches to finish reconciling before building the response.
- Track marketplace plugin sources when deduplicating refreshes so source
changes can trigger a reinstall even when the plugin version is unchanged.
- Notify effective-plugin consumers when the awaited refresh changes the cache.

## Testing

Added coverage showing that a forced local plugin listing returns an upgraded
plugin from the same marketplace path only after the new version is installed
and the superseded cache entry is removed.

stevenlee-oai b72079a2cf Use batch metadata for plugin app summaries (#34851)
## What changed

- Load app metadata for plugin read and install responses through the authenticated batch API, splitting requests into batches of 100 and retaining cached metadata if a batch fails.
- Preserve every declared app in plugin responses, using its ID as the fallback name and retaining categories from the plugin declaration when metadata is unavailable.
- Add `isEnabled`, `disabledReason`, and `isReadOnly` to `AppToolSummary`, with defaults that keep legacy responses compatible.

## Testing

- Cover plugin reads with more than 100 apps, partial batch failures, install-time authentication results, tool-state propagation, and legacy tool summaries.

brisebois-oai 83ff1c2f80 Cache remote plugin catalogs by scope (#34849)
## What changed

- Cache global, user, and workspace remote plugin catalogs on disk with a three-hour TTL and scope- and account-specific keys.
- Serve cached catalogs from `plugin/list`, refresh stale entries in the background, and warm existing caches at app-server startup without blocking list requests.
- Make `forceRefetch: true` bypass cached catalog data and replace an entry only after a successful fetch.
- Invalidate user and workspace catalog caches after remote plugin sharing changes.

## Testing

- Cover cache freshness, scope isolation, legacy global cache compatibility, identity requirements for private catalogs, forced refetches, and non-blocking startup refreshes.

sayan-oai 0f9fb40fa9 Allow custom providers to opt into standalone web search (#34846)
## What changed

- Add the `supports_standalone_web_search` model-provider setting, defaulting to `false`, and preserve it in remote thread configuration.
- Enable the standalone `web.run` tool for opted-in custom Responses providers when web search is enabled and the runtime provider supports it.
- Send standalone search requests through the custom provider's endpoint and authentication.

## Testing

- Cover opt-in, default-off, disabled-search, and unsupported-provider behavior.
- Verify custom-provider search request routing and authorization through the app server.

## What changed

- Add `isPinned` to thread responses and allow `thread/metadata/update` to pin or unpin stored threads.
- Add an `isPinned` filter to `thread/list`, including cursor-based pagination and combinations with relationship filters.
- Persist pin state in SQLite without modifying rollout files, default existing threads to unpinned, and preserve pins through reconciliation and archive transitions.

## Testing

- Cover protocol serialization, database migration and indexing, pin updates, filtered pagination, rollout reconciliation, and archive/unarchive behavior.

Ben Romano 88f1cd9664 Flush analytics before in-process app server shutdown (#34831)
## Why

The in-process app server can shut down while analytics for a completed turn are
still queued, preventing the completed-turn and accepted-line events from being
delivered.

## What changed

- Add a bounded analytics queue flush that waits for all preceding facts to be
  processed.
- Flush analytics after the in-process runtime drains its outbound work and
  before acknowledging shutdown.
- Extend the in-process shutdown timeout budgets to cover runtime draining and
  the best-effort analytics flush.

## Testing

- Verify that the flush waits for preceding facts and is a no-op when analytics
  is disabled.
- Verify that shutdown waits for delayed analytics delivery, including the
completed-turn and accepted-line events produced by a file change.

jay 88eb3a2b8a Enable git attribution across Codex entry points (#34819)
## What changed

- Install the git attribution extension in the app server, MCP server, and `codex debug prompt-input` so authenticated workspace policy controls the commit and pull request attribution instructions sent to the model.
- Resolve attribution settings from the process-level ChatGPT base URL, independent of per-thread or per-tool configuration overrides.
- Treat git attribution as contextual developer content when mapping model events.

## Testing

- Cover policy fetch retries, workspace switches, rollbacks, cold thread resumes with legacy instructions, MCP tool calls, and prompt-debug output.

jiayuhuang-openai 4ebd976312 Support configurable realtime BEM channel prefixes (#34816)
## What changed

- Route realtime V3 `bemTags` handoffs using `[ANALYSIS]`, `[COMMENTARY]`, and `[FINAL]` prefixes by default.
- Add `codexResponseHandoffChannelPrefixes` to realtime start requests so clients can replace the accepted prefixes for individual channels. Each channel may accept multiple prefixes, and omitted channels retain their defaults.
- Apply the configured prefixes to both complete and streamed handoff messages, without treating empty prefixes as matches.

## Testing

- Cover request deserialization, default and custom routing, partial streamed prefixes, empty-prefix handling, and end-to-end V3 handoffs.

Eric Traut 80f3c3141e Include the final agent message in turn completion summaries (#34777)
## Why

Streamed message deltas can be dropped when the transport is saturated, leaving
clients with a truncated final response.

## What changed

- Include the last non-empty final agent message in successful `turn/completed`
notifications and mark the items as a summary.
- Treat completed message content as authoritative in the TUI, repairing an
incomplete stream while avoiding duplicate messages when `item/completed` was
  already received.
- Continue backfilling full turn items for persisted `codex exec` threads when a
completion contains only the summary view.

## Testing

Added coverage for completion summaries, dropped stream deltas, duplicate item
handling, and directive-only responses.

jif f69dc49b15 Reduce app-server JSON serialization overhead (#34761)
## What changed

- Decode `JSONRPCRequest` into `ClientRequest` without serializing the entire request back into an intermediate JSON value.
- Define every client method's wire name explicitly and use it directly for request, response, and tracing method lookups.
- Serialize outgoing transport messages directly to JSON strings instead of first building a `serde_json::Value`.

## Testing

- Add regression coverage that compares direct request conversion with Serde decoding, including omitted and null parameters, legacy methods, and unknown methods.
- Verify that direct outgoing-message serialization preserves the notification wire shape.

Dylan Hurd cefcffd692 Preserve approvals reviewer when forking threads (#34664)
## Why

Forking a persisted thread could lose its approvals reviewer and fall back to
configuration defaults.

## What changed

- Restore the latest persisted approvals reviewer from the full source history
when creating a fork, while continuing to honor explicit overrides.
- Keep boundary-based history truncation independent from reviewer restoration,
so a fork at an earlier turn retains the source thread's latest setting.

## Testing

- Add an app-server test that sets auto-review, forks at an earlier turn, and
  verifies the fork reports auto-review.

Michael Bolin d4fcb2873b Honor configured proxy routes for auth refreshes (#34655)
## Why

ChatGPT token refresh requests need to follow the same configured routing policy as other authentication traffic, including environments that use the system proxy.

## What changed

- Require an `AuthRouteConfig` throughout login, logout, token refresh, personal access token, and agent identity flows.
- Build auth HTTP clients directly from that configuration instead of falling back to a default HTTP client when routing configuration is absent.

## Testing

- Add an integration test that refreshes a token through a cached system-proxy route and verifies the proxy receives the expected request.

Michael Bolin f899a79c03 Propagate resolved proxy policy through auth routing (#34649)
## Why

Auth routing represented the default proxy policy as an absent configuration,
leaving consumers to reconstruct the fallback HTTP client independently. Carry
the application's resolved policy explicitly so auth and related requests use
the same client configuration.

## What changed

- Build `AuthRouteConfig` from the resolved `HttpClientFactory` for both
  `ReqwestDefault` and `RespectSystemProxy` policies.
- Pass that route configuration through login, auth, cloud task, and cloud
configuration flows, and reuse its factory when loading cloud configuration.
- Log cloud-task configuration load failures before falling back to the
  transport-default proxy policy.

## Testing

Extended configuration tests to verify that effective and bootstrap auth routes
select the expected proxy policy when system-proxy support is enabled or
disabled by feature requirements.

iceweasel-oai 87f71e35b8 Skip missing paths in filesystem sandbox entries (#34598)
## Why

Default read-only protections for project metadata should apply when paths such
as `.git`, `.agents`, and `.codex` exist, without causing sandbox setup to
materialize missing paths as ACL targets.

## What changed

- Add an optional `missing_path_behavior` to filesystem sandbox entries and
mark default project-metadata protections with `skip`.
- Preserve the behavior through permission transforms and exec/MCP protocol
serialization while keeping existing path wire variants stable.
- Ignore skip-missing entries when projecting configuration or Windows sandbox

## Testing

- Cover protocol round trips for path and special-path entries.

Abhinav ee71c4a90f Enforce exact values from managed config requirements (#34597)
## What changed

- Apply managed values for `sqlite_home`, `log_dir`, `model_catalog_json`,
  `check_for_update_on_startup`, `allow_login_shell`, the report-submission
opt-in, and `windows.sandbox_private_desktop` to the runtime configuration,
with source-aware warnings when they override configured values.
- Expose these values through `configRequirements/read`, return them from
`config/read`, include them in session config locks, and show their sources in
  the TUI config debug view.
- Reject app-server writes that overlap an exact managed requirement with
`configRequirementReadonly` while leaving unrelated sibling keys writable.

## Testing

- Cover runtime overrides, requirement API mapping, config reads and writes,
session lock serialization, cross-platform path URIs, and debug output.

Abhinav 7442f5f932 Add keyed shell environment policy filters (#34590)
## What changed

- Add `shell_environment_policy.filters`, mapping environment-variable patterns to `include` or `exclude`, while continuing to accept the legacy `exclude` and `include_only` arrays.
- Merge filter keys case-insensitively across config layers so higher-precedence entries can override individual patterns. Switching representations replaces the other representation instead of combining them.
- Reject mixed, duplicate, or malformed filter definitions in enabled layers, config writes, and reloads, while retaining the previous session configuration when a reload is invalid.
- Make config-manager reads and writes representation-aware and preserve existing formatting when updating individual policy values.

Owen Lin 0b175e6439 Protect fork history references during rollout cleanup (#34566)
## Why

Paginated forks can retain a `history_base` pointer to another rollout. Deleting or compressing either side of that relationship can leave the fork without the rollout history it references.

## What changed

- Add a rollout reference index that scans active, archived, and compressed rollout metadata.
- Reject deletion when a rollout is still referenced by a fork outside the requested deletion set.
- Delete spawned thread subtrees as one store operation so internal references do not block deleting the complete subtree, while external references preflight the request before any files are removed.
- Skip referenced rollouts, fork-pointer rollouts, and rollouts with unreadable metadata during background compression.

## Testing

- Cover reference indexing across active, archived, compressed, duplicate, and self-referencing rollouts.
- Verify deletion rejects external references without partially deleting a subtree and permits batch deletion of internally referenced threads.

## Why

Sandbox wrappers replaced the process launch command and did not carry an

Dylan Hurd 1836ae0612 Preserve thread settings for goal-first and forked threads (#34469)
## Why

Goal-first and forked threads could create rollouts without recording their
effective thread settings. Resuming those threads after a restart could then
use current configuration instead of the settings selected when the thread was
created.

## What changed

- Add an optional opaque `providerId` to `externalAgentConfig/import` for analytics attribution, independently of the `migrationSource` selector.
- Include the provider ID in completed-import and import-failure analytics events.
- Have TUI imports identify their selected migration source as the provider.

## Testing

- Cover provider attribution in app-server import completion and failure tests and analytics serialization tests.

charlesgong-openai 3bc49e1721 Make external session detection limits configurable (#34449)
## What changed

- Add `maxSessionAgeDays` and `maxSessions` to the external-agent config detection request.
- Apply the requested limits when discovering sessions from supported migration sources.
- Preserve the existing defaults of a 30-day maximum age and 50 sessions when either option is omitted.

viyatb-oai 88fac6fe10 Honor managed permission profiles in network proxy resolution (#34436)
## Why

Permission profiles defined by `requirements.toml` can be selected as active,
but their network configuration was not included when resolving the network
proxy specification for that selection.

## What changed

Merge managed permission profiles from `requirements.toml` with configured
profiles before looking up the active profile's network proxy settings.

## Testing

- Verify config loading retains the proxy settings of an active managed
  profile.
- Verify `turn/start` accepts a managed profile with network settings.

iceweasel-oai 35c2278dd5 Support Windows sandboxing in the exec server (#34423)
## Why

Sandboxed process launch through the exec server was unsupported on Windows.

## What changed

- Add a shared native process launcher that selects the Windows sandbox session backend when required and otherwise launches PTY or pipe-based processes.
- Pass Windows permission, workspace-root, proxy, filesystem-override, and desktop settings through exec-server process preparation.
- Preserve piped stdin and inherited file descriptors across the shared launch path.

## Testing

- Verify that a remote Windows sandbox process accepts stdin while blocking a write under a read-only policy.
- Enable workspace-root sandbox tests on native Windows while continuing to skip environments that cannot emulate Windows sandbox semantics.

Matthew Zeng 60272096bc Enrich app/read connector metadata (#34417)
## What changed

- Add `iconUrlDark`, `distributionChannel`, `installUrl`, and
`pluginDisplayNames` to the experimental `app/read` connector metadata.
- Populate plugin display names from enabled plugins without starting their MCP
  servers.
- Accept both dark-icon field spellings from the batch API and keep the new
upstream fields optional for compatibility with older responses.

## Testing

- Cover the serialized response fields, legacy batch responses, and plugin
  display-name lookup without MCP startup.

jif 687f05cb94 Remove CSV-backed agent jobs (#34413)
## What changed

- Remove the `spawn_agents_on_csv` and `report_agent_job_result` tools and their agent-job runtime and state models.
- Drop the legacy `agent_jobs` and `agent_job_items` tables during state database migration.
- Keep `features.enable_fanout` and `agents.job_max_runtime_seconds` accepted as no-op compatibility settings while omitting them from the generated configuration schema.

## Testing

- Verify upgrades remove both legacy agent-job tables.
- Verify the removed feature and configuration keys still parse without taking effect.

stevenlee-oai 19940967bd Support threadless MCP connections without event channels (#34408)
## What changed

- Make the MCP connection manager's event sender optional.
- Skip startup notifications when a caller has no session event stream.
- Decline interactive MCP elicitations in that mode while allowing the
  underlying operation to continue.
- Update threadless resource reads, status snapshots, and connector discovery
  to use the channel-free path.

## Testing

- Extend the app-server MCP resource test to verify that a threadless resource
read declines an elicitation and still returns the resource contents.

Dylan Hurd e52c35b000 Propagate approval rejection reasons (#34400)
## What changed

- Change `ReviewDecision::Denied` to carry a rejection string and update the generated protocol schemas.
- Preserve specific rejection reasons through command, patch, network, MCP, delegated, and automatic approval flows so tool results can return them to the model.
- Distinguish invalid approval responses from user declines and truncate oversized rejection messages before adding them to model context.

## Testing

- Add coverage for denial serialization, invalid app-server approval responses, dropped automatic reviews, network rejection propagation, and rejection-message truncation.

Andrei Eternal e4836f998d Add configurable hook context spill limits (#34393)
## What changed

- Add `additionalContextLimit` to command hook configuration for events that can emit `additionalContext`.
- Apply the limit independently to each hook's context before it is sent to the model. Unset values retain the 2,500-token default, while `0` disables spilling for that hook.
- Expose configured limits through app-server hook and requirements responses, and show them in the TUI hooks browser.

## Testing

- Cover JSON and TOML parsing, discovery and hashing, per-hook spilling behavior, app-server responses, and TUI rendering.

nhamidi-oai 6f785632b0 Preserve audio across history and tool outputs (#34385)
## Why

Audio attachments could be dropped from tool outputs and reconstructed thread history, while inline base64 payload size could overstate their model context cost.

## What changed

- Preserve remote and local audio attachments in legacy user-message events and thread history.
- Keep audio from function and custom tool outputs when the model supports it and the output fits the truncation budget; otherwise replace it with an omission marker.
- Estimate inline audio cost from its duration, with a serialized-size fallback for malformed or unsupported data URLs, so compaction and truncation use model-visible cost instead of raw base64 size.
- Replace audio in messages and tool outputs with an explanatory placeholder for models without audio input support.

## Testing

Add coverage for history round trips, dynamic tool responses, modality filtering, duration-based context estimates, output-budget truncation, and remote compaction.

Won Park 7e51abbbd1 Avoid rendering generated images twice (#34378)
## What changed

- Tell the model that generated images are already displayed to the user and should not be repeated as Markdown images or file links.
- Keep generated-image artifact paths and output hints owned by the image generation extension.
- Centralize the availability check for the image generation tool across feature, provider, namespace-tool, model modality, and authentication requirements.

## Testing

- Verify artifact paths sanitize session and call identifiers.
- Verify the model-facing output hint reports that the generated image is already displayed.

Charlie Marsh 1e20272fa5 Avoid cloning thread history for token usage replay (#34361)
## What changed

- Resolve the turn ID for restored token usage before sending resume and fork responses, avoiding a clone of the full thread history.
- Add `ThreadHistoryBuilder::active_turn_id` so attribution can inspect the active turn without materializing its items.
- Stop history reconstruction at the latest persisted `TokenCount` and verify that a later tail turn does not receive the replayed usage.

Charlie Marsh 86102db5a1 Reject unsupported history modes when loading rollouts (#34344)
## Why

The first session metadata record defines a rollout's history mode. If that
record uses an unsupported mode, skipping it as a parse error can cause later
metadata copied from fork history to be treated as canonical.

## What changed

- Validate session metadata history modes before deserializing rollout lines,
while continuing to tolerate unknown modes in copied fork history.
- Simplify thread status registration to pass only the thread ID instead of a
  fully materialized `Thread`.

Won Park bf3c1972b7 Migrate legacy exec policy allow rules (#34271)
## What changed

- On session startup, remove exact `allow` entries from `rules/default.rules`
for command prefixes that Codex no longer suggests as policy amendments.
- Record the migration in `.sandbox_migration` so it runs only once, preserving
  rules created after the migration.
- Skip the migration when user and project exec policy rules are ignored.
- Expand the protected prefix list across shells, interpreters, package runners,
  and destructive or privilege-related commands.

## Testing

- Cover selective removal, case-insensitive matching, one-time behavior, and the
startup path with ignored policy rules.
