## Why
A plugin's scripts can change without changing its hook declaration. Declaration-only approval therefore lets changed plugin code run under an existing hook approval.
## What changed
- Fingerprint plugin bundle contents and include the content hash in the approval value returned by `hooks/list`. Persist it as `trusted_content_hash`, keeping `trusted_hash` compatible with older clients.
- Verify contents before each command or MCP hook execution, within the hook's timeout, and require renewed approval when approved contents change.
- Migrate legacy declaration-only approvals to a shared, immutable content baseline per plugin and config home.
- Exclude generated Python bytecode, `.venv`, and `.egg-info` entries from fingerprints. Run app-server hook listing on blocking tasks.
## Testing
Extend integration coverage for plugin upgrades and rollbacks requiring approval, same-turn script changes preventing further hook execution, shared legacy approval baselines, and generated Python artifacts preserving trust.

## Why
Background helpers launched from a detached Windows process can allocate console windows. Job Object launches and containment fallbacks need to preserve console suppression as well.
## What changed
- Add `codex_utils_process::background_command` to set `CREATE_NO_WINDOW` on Windows, and use it across background subprocess launches, including Git, plugins, authentication, diagnostics, and sandbox helpers.
- Centralize console suppression in Job Object launches. Keep `CREATE_NO_WINDOW` when retrying without containment, and clear `CREATE_SUSPENDED` so the fallback child can run.
- Preserve console inheritance for interactive shell launches and interactive AWS authentication refreshes.
## Testing
Add Windows regression tests covering console-free launches from detached parents through standard and Tokio commands, stdout/stderr capture, successful containment, Job Object creation and assignment failures, and sandbox setup refresh helpers.

## Why
Unix command hooks use a `pre_exec` callback to detach from the controlling terminal, forcing the launcher to fork. Native spawn attributes can create the detached session without that callback.
## What changed
- Route Unix hooks through `codex_utils_pty::Command` with `ProcessMode::NewSession`.
- Extend the native `posix_spawn` launcher to detached Linux sessions, retaining compatibility fallbacks for unsupported libc features and executable handling.
- Preserve the session environment snapshot, hook overrides, and restricted-variable filtering.
## Testing
Add regression coverage for launches without parent forks, terminal detachment, environment and argument handling, Linux `PATH` lookup, and executable compatibility. Cover process-group cleanup on cancellation and timeout, including descendants holding output pipes open, and preservation of background descendants after completed hooks.

## What changed
- Discover inline hooks from executor-provided plugin manifests and accept only
the bundled Computer Use `Stop` hook for `node_repl.turn_ended`.
- Add the accepted hook to the current step's hook engine with
executor-scoped provenance.
- Run executor-scoped hooks in the background after regular stop-hook
decisions, without delaying turn completion, applying control effects, or
exposing them in hook summaries. Skip them when regular hooks only block
continuation.
## Testing
- Added coverage for manifest filtering and option preservation.
- Added coverage for background execution, interaction with regular stop
hooks, and non-blocking turn completion.

## Why
Hook configurations can mark command handlers as asynchronous, but Codex previously skipped those handlers outside `SessionEnd`.
## What changed
- Run asynchronous command hooks in the background with a per-session concurrency limit, while keeping `SessionEnd` hooks synchronous.
- Prevent asynchronous hooks from blocking, stopping, rewriting, or otherwise controlling the operation that launched them.
- Deliver warnings and additional context at safe turn boundaries: inject results into an active turn after sampling, or buffer them ahead of the next user prompt when the session is idle.
- Preserve in-flight hooks across configuration reloads, scope spilled output to the thread, and abort outstanding work during session shutdown.
## Testing
Add unit and integration coverage for background scheduling, concurrency, output parsing, active and idle result delivery, configuration reloads, and shutdown cleanup.

## Why
Timing out a command hook can leave descendant processes running after the hook itself is stopped.
## What changed
- Run hook commands in a process group on Unix and a job object on Windows.
- Terminate the full process tree when hook execution is cancelled or fails, with `taskkill` as a Windows fallback.
- Preserve descendants when a hook completes successfully so intentionally detached helpers can continue running.

## Why
Large hook outputs can enter model-visible context through hook-specific
paths such as `additionalContext` and `Stop` continuation prompts.
Without a dedicated cap, one hook can inject a large blob directly into
conversation history instead of leaving a bounded preview for the model
and preserving the full text elsewhere.
## What
- spill hook text once it exceeds a fixed `2_500`-token budget,
preserving the full output on disk and leaving a head/tail preview plus
saved path in context
- add shared hook-output spilling under
`CODEX_HOME/hook_outputs/<thread_id>/<uuid>.txt`
- apply the cap to both `additionalContext`, `feedback_message`, and
`Stop` continuation fragments
## Why
Plugins can bundle lifecycle hooks, but Codex previously only discovered
hooks from user, project, and managed config layers. This adds the
plugin discovery and runtime plumbing needed for plugin-bundled hooks
while keeping execution behind the `plugin_hooks` feature flag.
## What
- Discovers plugin hook sources from each plugin's default
`hooks/hooks.json`.
- Supports `plugin.json` manifest `hooks` entries as either relative
paths or inline hook objects.
- Plumbs discovered plugin hook sources through plugin loading into the
hook runtime when `plugin_hooks` is enabled.
- Marks plugin-originated hook runs as `HookSource::Plugin`.
- Injects `PLUGIN_ROOT` and `CLAUDE_PLUGIN_ROOT` into plugin hook
command environments.
- Updates generated schemas and hook source metadata for the plugin hook
source.

## Reviewer Notes
- Core logic is in `codex-rs/core-plugins/src/loader.rs` and
`codex-rs/hooks/src/engine/discovery.rs`
- Moved existing / adding new tests to
`codex-rs/core-plugins/src/loader_tests.rs` hence the large diff there
- Otherwise mostly plumbing and minor schema updates
### Core Changes
The `codex-rs/core` changes are limited to wiring plugin hook support
into existing core flows:
- `core/src/session/session.rs` conditionally pulls effective plugin
hook sources and plugin hook load warnings from `PluginsManager` when
`plugin_hooks` is enabled, then passes them into `HooksConfig`.
- `core/src/hook_runtime.rs` adds the `plugin` metric tag for
`HookSource::Plugin`.
- `core/config.schema.json` picks up the new `plugin_hooks` feature
flag, and `core/src/plugins/manager_tests.rs` updates fixtures for the
added plugin hook fields.

(Experimental)
This PR adds a first MVP for hooks, with SessionStart and Stop
The core design is:
- hooks live in a dedicated engine under codex-rs/hooks
- each hook type has its own event-specific file
- hook execution is synchronous and blocks normal turn progression while
running
- matching hooks run in parallel, then their results are aggregated into
a normalized HookRunSummary
On the AppServer side, hooks are exposed as operational metadata rather
than transcript-native items:
- new live notifications: hook/started, hook/completed
- persisted/replayed hook results live on Turn.hookRuns
- we intentionally did not add hook-specific ThreadItem variants
Hooks messages are not persisted, they remain ephemeral. The context
changes they add are (they get appended to the user's prompt)
Summary
- move `core/src/hooks` implementation into a new `codex-hooks` crate
with its own manifest
- update `codex-rs` workspace and `codex-core` crate to depend on the
extracted `hooks` crate and wire up the shared APIs
- ensure references, modules, and lockfile reflect the new crate layout
Testing
- Not run (not requested)
