28327355b8 Update tests for default-enabled update_plan (#41630)
## What changed

- Cover the default, explicitly enabled, and explicitly disabled states of
  `tools.update_plan.enabled`.
- Verify prompt tool lists remain consistent across requests, including when
custom base and developer instructions are configured.
- Explicitly enable `update_plan` in existing fixtures that depend on the tool.

b836aecd4d Preserve one-shot exec when unified exec is disabled (#41393)
## Why

Managed configuration can disable resumable unified execution while leaving shell tools enabled. This should not remove command execution entirely or grant access to retained processes.

## What changed

- Keep `exec_command` available in completion-only mode when `unified_exec` is disabled.
- Remove `tty`, `yield_time_ms`, `session_id`, and `write_stdin` from that mode, and add a `timeout_ms` argument.
- Terminate one-shot commands on timeout or turn cancellation, reporting timed-out commands with exit code 124.

## Testing

Added integration coverage for tool availability, timeout cleanup, and interruption cleanup under managed configuration.

8a40095ea3 Standardize shell execution on unified exec (#39757)
## What changed

- Remove the legacy `shell_command` handler and runtime, leaving `exec_command`
and `write_stdin` as the shell execution tools.
- Treat legacy `shell_command` model metadata as `unified_exec`, and normalize
legacy user opt-outs so they do not disable command execution. Managed feature
requirements and `shell_tool` can still disable it.
- Preserve shell approvals, sandboxing, zsh-fork support, and output truncation
  through the unified execution path.

## Testing

- Cover legacy configuration and model-metadata compatibility.
- Exercise unified shell execution, approvals, truncation, and `apply_patch`
serialization across the app-server and core test suites.

cbb7e82a8b Unify turn input submission and routing (#38275)
## What changed

- Add `TurnInputRequest` and typed submission results for atomically starting a turn, steering the active turn, or declining input with a specific reason.
- Expose `start_or_steer_turn`, `start_turn_if_idle`, and `steer_turn` on `CodexThread`, and migrate Core consumers to these APIs.
- Make app-server `turn/start` steer an active regular turn and return that turn's ID. Reject incompatible output schemas and non-steerable turns without applying settings or enqueueing input.

## Testing

- Cover concurrent start-or-steer submissions, accepted and rejected settings updates, output-schema compatibility, idle-start rejection, and app-server steering.

361fe2d202 Stamp conversation history items with creation times (#38272)
## What changed

- Add fractional Unix creation times to locally authored user, developer, agent, and tool-output items when they enter durable conversation history.
- Preserve any creation time already supplied with an item across subsequent requests, remote compaction, and resumed history.

## Testing

- Cover timestamp assignment and preservation in session, client, tool, agent-spawn, and remote-compaction tests.

3171166881 Reject conflicting namespace descriptions in strict tool mode (#37053)
## What changed

- Treat different non-empty descriptions for the same tool namespace as a
collision when `error_on_tool_collisions` is enabled, including deferred
  tools.
- Continue allowing multiple tools to share a namespace when their descriptions
  match or are empty.
- Preserve the first non-empty namespace description when strict collision
  checking is disabled.

## Testing

- Cover strict collisions across direct and deferred tools, valid shared
namespaces, relaxed description merging, and failure before model sampling.

1e489adad0 Enforce strict tool name collision errors (#37020)
## What changed

- Track the first duplicate effective tool name while assembling the tool registry, including collisions introduced by external tools, code mode, and tool search.
- When `[features.tool_registry].error_on_tool_collisions` is enabled, fail the turn with a `duplicate tool: <namespace>.<name>` error before sending a model request.
- Continue allowing identical tool names in different namespaces, and preserve the existing collision behavior when strict checking is disabled.

## Testing

- Cover registry collision tracking, strict tool planning across tool sources, namespace isolation, failure before sampling, and pre-sampling compaction.

6a828ca26f Read approval policy from the current turn configuration (#36912)
## Why

Thread settings can update the approval policy after a turn context is created.
Keeping a separate copy on `TurnContext` could leave tool approval checks using
the previous policy.

## What changed

- Remove the duplicated approval-policy field from `TurnContext`.
- Resolve the policy through the turn's current configuration everywhere it is
needed, including tool execution, Guardian routing, MCP handling, and
  permission requests.

## Testing

- Update the shell escalation test to apply a thread-level policy override and
verify that the next turn rejects an escalation using the updated policy.

a1dd74b535 Retain attempted tool metadata across prompts (#36507)
## What changed

- Reattach recorded `executed_tool_calls` metadata when an output is included in a subsequent prompt.
- Bound retained metadata to 32 KiB, prioritizing recent calls and reporting omitted calls in truncation metadata.
- Drop retained entries after their corresponding outputs leave the prompt history.

## Testing

- Cover metadata replay, cleanup after compaction, bounded retained history, and propagation through later tool requests.

ceb4bc72c4 Record attempted tool calls in response metadata (#36181)
## What changed

- Add the opt-in `executed_tool_call_metadata` feature to record model-attempted direct and code-mode tool calls.
- Attach recorded names and arguments to the matching tool output in the next Responses request, preserving the metadata across sampling retries.
- Bound pending calls and argument sizes, and emit truncation metadata when limits are exceeded.

## Testing

- Cover disabled-by-default behavior, namespaced and nested calls, blocked or failed attempts, retry handling, and recorder limits.

4a443994bd Always assign response item IDs (#34645)
## What changed

- Assign IDs to client-created response items in every session, including streamed items, forked history, compaction results, and non-OpenAI provider requests.
- Preserve server-provided IDs and stable generated IDs across persisted and resumed conversations while continuing to omit unprefixed server IDs from outbound requests.
- Retire `features.item_ids` as a configurable feature. Existing configuration and CLI toggles remain accepted as compatibility no-ops.

## Testing

- Cover default ID assignment, persistence and resume behavior, remote compaction, provider requests, WebSocket requests, and removed-feature compatibility.

87f71e35b8 Skip missing paths in filesystem sandbox entries (#34598)
## Why

Default read-only protections for project metadata should apply when paths such
as `.git`, `.agents`, and `.codex` exist, without causing sandbox setup to
materialize missing paths as ACL targets.

## What changed

- Add an optional `missing_path_behavior` to filesystem sandbox entries and
mark default project-metadata protections with `skip`.
- Preserve the behavior through permission transforms and exec/MCP protocol
serialization while keeping existing path wire variants stable.
- Ignore skip-missing entries when projecting configuration or Windows sandbox
overrides, while retaining explicit metadata carveouts.

## Testing

- Cover protocol round trips for path and special-path entries.
- Verify default metadata protections and Windows explicit carveout handling.

328e95110c Preserve namespaces on custom tool calls (#30302)
## Summary

- Preserve the optional namespace on custom tool calls during response
deserialization and app-server replay.
- Use the namespaced tool identifier for streaming argument handling and
tool dispatch.
- Regenerate app-server protocol schemas.
- Add regression tests covering namespace serialization and routing.

## Testing

- Ran affected protocol and app-server test suites.
- Ran the full core test suite; two load-sensitive timing tests passed
when rerun individually.
- Ran Clippy and formatting checks.
- Verified with a local end-to-end app-server replay that the namespace
is preserved through the complete request/response flow.

e752f7b4ae [codex] Use expect in integration tests (#28441)
The workspace denies `clippy::expect_used` in production. Although
`clippy.toml` allows `expect` in tests, Bazel Clippy compiles
integration-test helper code in a way that does not receive that
exemption, which encouraged verbose `unwrap_or_else(... panic!(...))`
and equivalent `match`/`let else` forms.

This allows `clippy::expect_used` once at each integration-test crate
root (including aggregated suites and test-support libraries), then
replaces manual panic-based Result and Option unwraps with
`expect`/`expect_err`. Standalone `tests/*.rs` files remain their own
crate roots. Intentional assertion and unexpected-variant panics remain
unchanged, and the production `expect_used = "deny"` lint remains in
place.

The cleanup is mechanical and net-negative in line count.

f3c1283411 Pair thread environment settings (#26687)
## Why

Thread cwd and environment selections are a single logical setting in
core: updating one without the other can silently desynchronize the
next-turn execution context. This change makes that relationship
explicit in the internal thread settings flow while preserving the
existing app-server public API shape.

## What changed

- Moved the cwd/environment pair through internal
`ThreadSettingsOverrides.environment_settings` instead of a top-level
internal `cwd` field.
- Kept `thread/settings/update` public params unchanged, with app-server
translating top-level `cwd` into the paired internal settings shape.
- Moved `Op::UserInput` environment overrides into thread settings so
user turns and settings updates use the same core path.
- Updated core, app-server, MCP, memories, sample, and test callsites to
construct the paired settings shape.

## Verification

- Local test run starting after PR creation.

3c76081876 Make deny canonical for filesystem permission entries (#23493)
## Why
Filesystem permission profiles used `none` for deny-read entries, which
is less direct than the action the entry actually represents. This
change makes `deny` the canonical filesystem permission spelling while
preserving compatibility for older configs that still send `none`.

## What changed
- rename `FileSystemAccessMode::None` to `Deny`
- serialize and generate schemas with `deny` as the canonical value
- retain `none` only as a legacy input alias for temporary config
compatibility
- update filesystem glob diagnostics and regression coverage to use the
canonical spelling
- refresh config and app-server schema fixtures to match the new wire
shape

## Validation
- `cargo test -p codex-protocol`
- `cargo test -p codex-app-server-protocol`
- `cargo test -p codex-core config_toml_deserializes_permission_profiles

- `cargo test -p codex-core
read_write_glob_patterns_still_reject_non_subpath_globs --lib`

Earlier in the session, a broad `cargo test -p codex-core` run reached
unrelated pre-existing failures in timing/snapshot/git-info tests under
this environment; the targeted surfaces touched by this PR passed
cleanly.

83decfa300 [codex] Remove unused legacy shell tools (#22246)
## Why

Recent session history showed no active use of the raw `shell`,
`local_shell`, or `container.exec` execution surfaces. Keeping those
handlers/specs wired into core leaves duplicate shell execution paths
alongside the supported `shell_command` and unified exec tools.

## What changed

- Removed the raw `shell` handler/spec and its `ShellToolCallParams`
protocol helper.
- Removed the legacy `local_shell` and `container.exec` handler/spec
plumbing while preserving persisted-history compatibility for old
response items.
- Normalized model/config `default` and `local` shell selections to
`shell_command`.
- Pruned tests that exercised removed raw-shell/local-shell/apply-patch
variants and kept coverage on `shell_command`, unified exec, and
freeform `apply_patch`.

2304ec45ca Remove unavailable MCP placeholder tool backfill (#22439)
## Why

`UnavailableDummyTools` kept synthetic placeholder tools alive for
historical tool calls whose backing MCP tool was no longer available.
That path adds stale model-visible tool specs and special routing at the
point where unavailable MCP calls should use ordinary current-tool
handling. This removes the runtime backfill instead of preserving a
second compatibility lane.

## Is it safe to remove?

The unavailable tools were added in #17853 after a CS issue when a
previously-called MCP tool failed to load and was omitted from the CS
spec. Now that we have tool search, I think this is resolved:
- API merges tools from previous TST output into effective tool set so
theyre always in CS spec
- if an MCP tool surfaced by TST later becomes unavailable, the model
can still call it and it will just return model-visible error
- both TST output and function call output are dropped on compaction so
model will not remember old calls to MCP post compaction

## What changed

- Delete unavailable-tool collection, placeholder handler, router/spec
plumbing, and obsolete placeholder coverage.
- Keep `features.unavailable_dummy_tools` as a removed no-op feature
tombstone so existing configs still parse cleanly.
- Add an integration-style `tool_search` regression test showing that a
deferred MCP tool surfaced through `tool_search` still routes through
MCP and returns a model-visible tool-call error rather than `unsupported
call`.

## Verification

- `cargo test -p codex-core tool_search`

e783341b70 [codex] Delete function-style apply_patch (#21651)
## Why

`apply_patch` is now a freeform/custom tool. Keeping the old
JSON/function-style registration and parsing path left another way for
models and tests to invoke `apply_patch`, which made the tool surface
harder to reason about.

## What changed

- Removed the `ApplyPatchToolType::Function` variant, JSON `apply_patch`
spec, and handler support for function payloads.
- Kept `apply_patch_tool_type = freeform` as the supported model
metadata path, including Bedrock catalog metadata.
- Migrated `apply_patch` tests and SSE fixtures to custom/freeform tool

3ef09c71d3 core tests: migrate tools tests to permission profiles (#20027)
## Summary

This continues the test-side migration away from `SandboxPolicy` by
removing the remaining legacy policy setup in
`core/tests/suite/tools.rs`. The affected test was already modeling a
profile-backed filesystem policy with a deny-read glob, so configuring
the test through `Permissions::set_permission_profile()` is a better
match for the behavior being exercised.

## Changes

- Drops the `SandboxPolicy` import from `core/tests/suite/tools.rs`.
- Configures the glob deny-read shell test directly with a
`PermissionProfile` instead of creating a legacy read-only policy first.
- Submits the test turn with the session permission profile so the
deny-read glob remains active for the command under test.

## Verification

- `cargo check -p codex-core --tests`

158b2a4201 core tests: configure profiles directly (#20015)
## Summary
- Replace legacy sandbox config setup in delegate and telemetry tests

- Move no-sandbox and read-only test turns in `tools.rs`,
`code_mode.rs`, `user_shell_cmd.rs`, and `model_visible_layout.rs` from
legacy `SandboxPolicy` values to `PermissionProfile` helpers, while
leaving the deny-glob read-only compatibility case for a later targeted
cleanup.
- Use `PermissionProfile::read_only()` where tests need managed
read-only behavior and `PermissionProfile::Disabled` where they
intentionally need no sandbox.
- Reduce `SandboxPolicy` references in `codex-rs/core/tests` from 27
files after #20013 to 22 files.

deaa307fb2 permissions: derive compatibility policies from profiles (#19392)
## Why

After #19391, `PermissionProfile` and the split filesystem/network
policies could still be stored in parallel. That creates drift risk: a
profile can preserve deny globs, external enforcement, or split
filesystem entries while a cached projection silently loses those
details. This PR makes the profile the runtime source and derives
compatibility views from it.

## What Changed

- Removes stored filesystem/network sandbox projections from
`Permissions` and `SessionConfiguration`; their accessors now derive
from the canonical `PermissionProfile`.
- Derives legacy `SandboxPolicy` snapshots from profiles only where an
older API still needs that field.
- Updates MCP connection and elicitation state to track
`PermissionProfile` instead of `SandboxPolicy` for auto-approval
decisions.
- Adds semantic filesystem-policy comparison so cwd changes can preserve
richer profiles while still recognizing equivalent legacy projections
independent of entry ordering.
- Updates config/session tests to assert profile-derived projections
instead of parallel stored fields.

0d0abe839a feat(sandbox): add glob deny-read platform enforcement (#18096)
## Summary
- adds macOS Seatbelt deny rules for unreadable glob patterns
- expands unreadable glob matches on Linux and masks them in bwrap,
including canonical symlink targets
- keeps Linux glob expansion robust when `rg` is unavailable in minimal
or Bazel test environments
- adds sandbox integration coverage that runs `shell` and `exec_command`
with a `**/*.env = none` policy and verifies the secret contents do not
reach the model

## Linux glob expansion

```text
Prefer:   rg --files --hidden --no-ignore --glob <pattern> -- <search-root>
Fallback: internal globset walker when rg is not installed
Failure:  any other rg failure aborts sandbox construction
```

```
[permissions.workspace.filesystem]
glob_scan_max_depth = 2
[permissions.workspace.filesystem.":project_roots"]
"**/*.env" = "none"
```

This keeps the common path fast without making sandbox construction
depend on an ambient `rg` binary. If `rg` is present but fails for
another reason, the sandbox setup fails closed instead of silently
omitting deny-read masks.

## Platform support
- macOS: subprocess sandbox enforcement is handled by Seatbelt regex
deny rules
- Linux: subprocess sandbox enforcement is handled by expanding existing
glob matches and masking them in bwrap
- Windows: policy/config/direct-tool glob support is already on `main`
from #15979; Windows subprocess sandbox paths continue to fail closed
when unreadable split filesystem carveouts require runtime enforcement,
rather than silently running unsandboxed

## Stack
1. #15979 - merged: cross-platform glob deny-read
policy/config/direct-tool support for macOS, Linux, and Windows
2. This PR - macOS/Linux subprocess sandbox enforcement plus Windows
fail-closed clarification
3. #17740 - managed deny-read requirements

61dfe0b86c chore: clean up argument-comment lint and roll out all-target CI on macOS (#16054)
## Why

`argument-comment-lint` was green in CI even though the repo still had
many uncommented literal arguments. The main gap was target coverage:
the repo wrapper did not force Cargo to inspect test-only call sites, so
examples like the `latest_session_lookup_params(true, ...)` tests in
`codex-rs/tui_app_server/src/lib.rs` never entered the blocking CI path.

This change cleans up the existing backlog, makes the default repo lint
path cover all Cargo targets, and starts rolling that stricter CI
enforcement out on the platform where it is currently validated.

## What changed

- mechanically fixed existing `argument-comment-lint` violations across
the `codex-rs` workspace, including tests, examples, and benches
- updated `tools/argument-comment-lint/run-prebuilt-linter.sh` and
`tools/argument-comment-lint/run.sh` so non-`--fix` runs default to
`--all-targets` unless the caller explicitly narrows the target set
- fixed both wrappers so forwarded cargo arguments after `--` are
preserved with a single separator
- documented the new default behavior in
`tools/argument-comment-lint/README.md`
- updated `rust-ci` so the macOS lint lane keeps the plain wrapper
invocation and therefore enforces `--all-targets`, while Linux and
Windows temporarily pass `-- --lib --bins`

That temporary CI split keeps the stricter all-targets check where it is
already cleaned up, while leaving room to finish the remaining Linux-
and Windows-specific target-gated cleanup before enabling
`--all-targets` on those runners. The Linux and Windows failures on the
intermediate revision were caused by the wrapper forwarding bug, not by
additional lint findings in those lanes.

## Validation

- `bash -n tools/argument-comment-lint/run.sh`
- `bash -n tools/argument-comment-lint/run-prebuilt-linter.sh`
- shell-level wrapper forwarding check for `-- --lib --bins`
- shell-level wrapper forwarding check for `-- --tests`
- `just argument-comment-lint`
- `cargo test` in `tools/argument-comment-lint`
- `cargo test -p codex-terminal-detection`

## Follow-up

- Clean up remaining Linux-only target-gated callsites, then switch the
Linux lint lane back to the plain wrapper invocation.
- Clean up remaining Windows-only target-gated callsites, then switch
the Windows lint lane back to the plain wrapper invocation.

bfff0c729f config: enforce enterprise feature requirements (#13388)
## Why

Enterprises can already constrain approvals, sandboxing, and web search
through `requirements.toml` and MDM, but feature flags were still only
configurable as managed defaults. That meant an enterprise could suggest
feature values, but it could not actually pin them.

This change closes that gap and makes enterprise feature requirements
behave like the other constrained settings. The effective feature set
now stays consistent with enterprise requirements during config load,
when config writes are validated, and when runtime code mutates feature
flags later in the session.

It also tightens the runtime API for managed features. `ManagedFeatures`
now follows the same constraint-oriented shape as `Constrained<T>`
instead of exposing panic-prone mutation helpers, and production code
can no longer construct it through an unconstrained `From<Features>`
path.

The PR also hardens the `compact_resume_fork` integration coverage on
Windows. After the feature-management changes,
`compact_resume_after_second_compaction_preserves_history` was
overflowing the libtest/Tokio thread stacks on Windows, so the test now
uses an explicit larger-stack harness as a pragmatic mitigation. That
may not be the ideal root-cause fix, and it merits a parallel
investigation into whether part of the async future chain should be
boxed to reduce stack pressure instead.

## What Changed

Enterprises can now pin feature values in `requirements.toml` with the
requirements-side `features` table:

```toml
[features]
personality = true
unified_exec = false
```

Only canonical feature keys are allowed in the requirements `features`
table; omitted keys remain unconstrained.

- Added a requirements-side pinned feature map to
`ConfigRequirementsToml`, threaded it through source-preserving
requirements merge and normalization in `codex-config`, and made the
TOML surface use `[features]` (while still accepting legacy
`[feature_requirements]` for compatibility).
- Exposed `featureRequirements` from `configRequirements/read`,
regenerated the JSON/TypeScript schema artifacts, and updated the
app-server README.
- Wrapped the effective feature set in `ManagedFeatures`, backed by
`ConstrainedWithSource<Features>`, and changed its API to mirror
`Constrained<T>`: `can_set(...)`, `set(...) -> ConstraintResult<()>`,
and result-returning `enable` / `disable` / `set_enabled` helpers.
- Removed the legacy-usage and bulk-map passthroughs from
`ManagedFeatures`; callers that need those behaviors now mutate a plain
`Features` value and reapply it through `set(...)`, so the constrained
wrapper remains the enforcement boundary.
- Removed the production loophole for constructing unconstrained
`ManagedFeatures`. Non-test code now creates it through the configured
feature-loading path, and `impl From<Features> for ManagedFeatures` is
restricted to `#[cfg(test)]`.
- Rejected legacy feature aliases in enterprise feature requirements,
and return a load error when a pinned combination cannot survive
dependency normalization.
- Validated config writes against enterprise feature requirements before
persisting changes, including explicit conflicting writes and
profile-specific feature states that normalize into invalid
combinations.
- Updated runtime and TUI feature-toggle paths to use the constrained
setter API and to persist or apply the effective post-constraint value
rather than the requested value.
- Updated the `core_test_support` Bazel target to include the bundled
core model-catalog fixtures in its runtime data, so helper code that
resolves `core/models.json` through runfiles works in remote Bazel test
environments.
- Renamed the core config test coverage to emphasize that effective
feature values are normalized at runtime, while conflicting persisted
config writes are rejected.
- Ran `compact_resume_after_second_compaction_preserves_history` inside
an explicit 8 MiB test thread and Tokio runtime worker stack, following
the existing larger-stack integration-test pattern, to keep the Windows
`compact_resume_fork` test slice from aborting while a parallel
investigation continues into whether some of the underlying async
futures should be boxed.

## Verification

- `cargo test -p codex-config`
- `cargo test -p codex-core feature_requirements_ -- --nocapture`
- `cargo test -p codex-core
load_requirements_toml_produces_expected_constraints -- --nocapture`
- `cargo test -p codex-core
compact_resume_after_second_compaction_preserves_history -- --nocapture`
- `cargo test -p codex-core compact_resume_fork -- --nocapture`
- Re-ran the built `codex-core` `tests/all` binary with
`RUST_MIN_STACK=262144` for
`compact_resume_after_second_compaction_preserves_history` to confirm
the explicit-stack harness fixes the deterministic low-stack repro.
- `cargo test -p codex-core`
- This still fails locally in unrelated integration areas that expect
the `codex` / `test_stdio_server` binaries or hit existing `search_tool`
wiremock mismatches.

## Docs

`developers.openai.com/codex` should document the requirements-side
`[features]` table for enterprise and MDM-managed configuration,
including that it only accepts canonical feature keys and that
conflicting config writes are rejected.

1af2a37ada chore: remove codex-core public protocol/shell re-exports (#12432)
## Why

`codex-rs/core/src/lib.rs` re-exported a broad set of types and modules
from `codex-protocol` and `codex-shell-command`. That made it easy for
workspace crates to import those APIs through `codex-core`, which in
turn hides dependency edges and makes it harder to reduce compile-time
coupling over time.

This change removes those public re-exports so call sites must import
from the source crates directly. Even when a crate still depends on
`codex-core` today, this makes dependency boundaries explicit and
unblocks future work to drop `codex-core` dependencies where possible.

## What Changed

- Removed public re-exports from `codex-rs/core/src/lib.rs` for:
- `codex_protocol::protocol` and related protocol/model types (including
`InitialHistory`)
  - `codex_protocol::config_types` (`protocol_config_types`)
- `codex_shell_command::{bash, is_dangerous_command, is_safe_command,
parse_command, powershell}`
- Migrated workspace Rust call sites to import directly from:
  - `codex_protocol::protocol`
  - `codex_protocol::config_types`
  - `codex_protocol::models`
  - `codex_shell_command`
- Added explicit `Cargo.toml` dependencies (`codex-protocol` /
`codex-shell-command`) in crates that now import those crates directly.
- Kept `codex-core` internal modules compiling by using `pub(crate)`
aliases in `core/src/lib.rs` (internal-only, not part of the public
API).
- Updated the two utility crates that can already drop a `codex-core`
dependency edge entirely:
  - `codex-utils-approval-presets`
  - `codex-utils-cli`

## Verification

- `cargo test -p codex-utils-cli`
- `cargo check --workspace --all-targets`

a4cc1a4a85 feat: introduce Permissions (#11633)
## Why
We currently carry multiple permission-related concepts directly on
`Config` for shell/unified-exec behavior (`approval_policy`,
`sandbox_policy`, `network`, `shell_environment_policy`,
`windows_sandbox_mode`).

Consolidating these into one in-memory struct makes permission handling
easier to reason about and sets up the next step: supporting named
permission profiles (`[permissions.PROFILE_NAME]`) without changing
behavior now.

This change is mostly mechanical: it updates existing callsites to go
through `config.permissions`, but it does not yet refactor those
callsites to take a single `Permissions` value in places where multiple
permission fields are still threaded separately.

This PR intentionally **does not** change the on-disk `config.toml`
format yet and keeps compatibility with legacy config keys.

## What Changed
- Introduced `Permissions` in `core/src/config/mod.rs`.
- Added `Config::permissions` and moved effective runtime permission
fields under it:
  - `approval_policy`
  - `sandbox_policy`
  - `network`
  - `shell_environment_policy`
  - `windows_sandbox_mode`
- Updated config loading/building so these effective values are still
derived from the same existing config inputs and constraints.
- Updated Windows sandbox helpers/resolution to read/write via
`permissions`.
- Threaded the new field through all permission consumers across core
runtime, app-server, CLI/exec, TUI, and sandbox summary code.
- Updated affected tests to reference `config.permissions.*`.
- Renamed the struct/field from
`EffectivePermissions`/`effective_permissions` to
`Permissions`/`permissions` and aligned variable naming accordingly.

abbd74e2be feat: make sandbox read access configurable with ReadOnlyAccess (#11387)
`SandboxPolicy::ReadOnly` previously implied broad read access and could
not express a narrower read surface.
This change introduces an explicit read-access model so we can support
user-configurable read restrictions in follow-up work, while preserving
current behavior today.

It also ensures unsupported backends fail closed for restricted-read
policies instead of silently granting broader access than intended.

## What

- Added `ReadOnlyAccess` in protocol with:
  - `Restricted { include_platform_defaults, readable_roots }`
  - `FullAccess`
- Updated `SandboxPolicy` to carry read-access configuration:
  - `ReadOnly { access: ReadOnlyAccess }`
  - `WorkspaceWrite { ..., read_only_access: ReadOnlyAccess }`
- Preserved existing behavior by defaulting current construction paths
to `ReadOnlyAccess::FullAccess`.
- Threaded the new fields through sandbox policy consumers and call
sites across `core`, `tui`, `linux-sandbox`, `windows-sandbox`, and
related tests.
- Updated Seatbelt policy generation to honor restricted read roots by
emitting scoped read rules when full read access is not granted.
- Added fail-closed behavior on Linux and Windows backends when
restricted read access is requested but not yet implemented there
(`UnsupportedOperation`).
- Regenerated app-server protocol schema and TypeScript artifacts,
including `ReadOnlyAccess`.

## Compatibility / rollout

- Runtime behavior remains unchanged by default (`FullAccess`).
- API/schema changes are in place so future config wiring can enable
restricted read access without another policy-shape migration.

dc61fc5f50 feat: support allowed_sandbox_modes in requirements.toml (#8298)
This adds support for `allowed_sandbox_modes` in `requirements.toml` and
provides legacy support for constraining sandbox modes in
`managed_config.toml`. This is converted to `Constrained<SandboxPolicy>`
in `ConfigRequirements` and applied to `Config` such that constraints
are enforced throughout the harness.

Note that, because `managed_config.toml` is deprecated, we do not add
support for the new `external-sandbox` variant recently introduced in
https://github.com/openai/codex/pull/8290. As noted, that variant is not
supported in `config.toml` today, but can be configured programmatically
via app server.

73ed30d7e5 Avoid hang when tool's process spawns grandchild that shares stderr/stdout (#6575)
We've received many reports of codex hanging when calling certain tools.
[Here](https://github.com/openai/codex/issues/3204) is one example. This
is likely a major cause. The problem occurs when
`consume_truncated_output` waits for `stdout` and `stderr` to be closed
once the child process terminates. This normally works fine, but it
doesn't handle the case where the child has spawned grandchild processes
that inherits `stdout` and `stderr`.

The fix was originally written by @md-oai in [this
PR](https://github.com/openai/codex/pull/1852), which has gone stale.
I've copied the original fix (which looks sound to me) and added an
integration test to prevent future regressions.

273819aaae Move changing turn input functionalities to ConversationHistory (#5473)
We are doing some ad-hoc logic while dealing with conversation history.
Ideally, we shouldn't mutate `vec[responseitem]` manually at all and
should depend on `ConversationHistory` for those changes.

Those changes are:
- Adding input to the history
- Removing items from the history
- Correcting history

I am also adding some `error` logs for cases we shouldn't ideally face.
For example, we shouldn't be missing `toolcalls` or `outputs`. We
shouldn't hit `ContextWindowExceeded` while performing `compact`

This refactor will give us granular control over our context management.
