6af345407d Gate experimental context by model capability at session startup (#43147)
## Why

Experimental context activation previously checked the provider and account eligibility without checking model support. Child sessions also inherited token-budget activation from their parent, even when starting fresh with a different model.

## What changed

- Add `ModelInfo.supports_experimental_context`, defaulting to `false`, and enable it for the bundled `gpt-6-astra` model. Require this capability when activating experimental context.
- Snapshot configured token-budget preferences before startup activation. Restore them for fresh child sessions before applying their starting model's defaults, while history forks retain their parent's activation.
- Pass unresolved token-budget preferences to child sessions so they can use their own model's prompts.

## Testing

Extend coverage for unsupported models, model-switch guidance under explicit and experimental activation, and child configuration from both active and inactive parents. Verify that omitted capability metadata defaults to `false`.

## What changed

- Add optional per-model Guardian modes for computer use, shell, code mode,
  file changes, MCP, network, and permission requests.
- Apply disabled, synchronous, or adaptive review by action category while
  preserving legacy configuration when model policy is absent.
- Bind cached risk scores to the active model policy, thread settings, and
  environment selections so outdated scores cannot approve later actions.
- Keep unknown review modes on the synchronous path and ignore unknown policy
  fields for forward compatibility.

## Testing

- Cover policy serialization, legacy fallback, scoped tool classification,
  disabled categories, and score invalidation after policy changes.

88912c04cd Enable coordinated MCP OAuth refresh (#42413)
## What changed

- Honor `McpOAuthRefreshMode::Coordinated` for streamable HTTP MCP connections by letting RMCP refresh and persist credentials through the pinned credential store.
- Reread credentials while holding the refresh lock, persist refreshed tokens before retrying requests, and serialize login and logout writes with in-flight refreshes.
- Keep OAuth preparation outside MCP operation and handshake deadlines, and allow refresh persistence to finish if the initiating caller is cancelled.

## Testing

- Cover coordinated startup refresh, concurrent refreshes, `401` retries, credential mutation races, provider timeouts, and caller cancellation.

- Add the under-development `mcp_oauth_refresh_coordination` feature and pin its selected refresh mode to each MCP connection.
- Treat the refresh mode as part of OAuth connection identity so runtime and MCP configuration refreshes reconnect when the mode changes.
- Keep legacy refresh behavior in builds where coordination is unavailable and emit a warning when coordinated mode is selected.

- Allow MCP server names to contain `:`, `@`, `/`, and `.`, enabling names such as `npm:@modelcontextprotocol/server-sequential.thinking`.
- Preserve these names across `mcp add`, `get`, `list`, and `remove`, runtime tool namespaces, and OAuth credential lookup.
- Quote non-bare server names in generated `config.toml` recovery hints and keep similarly escaped OAuth credential names isolated.

- Re-run the HTTP headers helper after a same-origin `POST` receives a `401` or `403`, and retry the request once when the effective headers changed.

4213b38f3c Honor attachment-owned permissions for MCP servers (#40728)
## Why

MCP servers attached to executor environments must retain their owner's
permission profile instead of inheriting the thread-wide sandbox authority.

## What changed

- Resolve and capture a permission profile for each enabled MCP server when
  publishing the runtime, and reject calls or elicitations when that authority
  is unavailable.
- Use the captured server profile for tool approval decisions, elicitation, and
  sandbox metadata, including after runtime refreshes.
- Materialize `:workspace_roots` from `PathUri` values so permissions preserve
  the path convention of remote executor environments.
- Give threadless app discovery and resource reads an explicit default
  permission profile.

## Testing

Added coverage for per-server elicitation authority, unresolved attachments,
runtime refreshes, restricted tool calls, and foreign-platform workspace roots.

e3e5ad2847 Harden unsandboxed patch filesystem access (#39659)
## Why

An `apply_patch` path can be replaced with a symlink after verification, allowing an unsandboxed patch operation to reach a different file than the one that was approved.

## What changed

- Add `follow_symlinks` options to executor filesystem reads, writes, metadata lookups, directory creation, and removal, including the corresponding `followSymlinks` protocol fields.
- Implement no-follow filesystem operations on Unix and Windows that reject links in any path component and restrict file access to regular files.
- Run `apply_patch` with symlink traversal disabled when an otherwise-required sandbox is bypassed, while retaining the existing follow-symlink default for standalone callers.

## Testing

- Cover leaf and ancestor symlinks across patch add, update, delete, and move operations, including a path swap after verification.
- Exercise local and remote no-follow filesystem behavior, concurrent directory creation, special-file rejection, and Windows reparse points.

250b5ea2bf Bind MCP OAuth refresh tokens to their issuer (#39615)
## Why

An authorization server discovered for an MCP server can change. A stored refresh token must not be sent to a different issuer than the one that originally granted it.

## What changed

- Persist the discovered authorization server issuer with new OAuth credentials and preserve it across refreshes.
- Validate that issuer against a single pinned metadata snapshot before refreshing or adopting stored refresh credentials.
- Require reauthentication when refresh credentials have no issuer or the issuer changed. An unexpired access token can still be used without exposing its refresh token.

## Testing

Add coverage for issuer persistence, missing and changed issuers, metadata changes during startup, and refresh-token preservation.

f8a3db0b99 Clarify MCP OAuth reauthentication errors (#38418)
## Why

When an MCP server rejects existing OAuth credentials, reporting that the server
is merely not logged in obscures that the user needs to authenticate again.

## What changed

Pass the MCP startup failure reason into error formatting and report that the
server requires OAuth reauthentication when appropriate. Preserve the existing
recovery hint for either `codex mcp login` or the client's OAuth sign-in flow.

## Testing

Cover the reauthentication message for both login flows and verify it in the
streamable HTTP OAuth round-trip test.

- Add `TurnInputRequest` and typed submission results for atomically starting a turn, steering the active turn, or declining input with a specific reason.

b3ffe3d001 Recover MCP servers after OAuth reauthentication (#37337)
## Why

An OAuth-backed Streamable HTTP MCP server can fail startup when its stored credentials are rejected. If a client then completes OAuth sign-in and replaces those credentials, the failed server needs to become available without restarting the Codex session.

## What changed

- Track the credential store and OAuth tokens used for each MCP connection, and detect replacements after authentication failures.
- Refresh affected MCP servers before the next turn and include their recovered tools in the model request.
- Classify HTTP authentication challenges and rejected refresh tokens as requiring reauthentication, with sign-in guidance appropriate to local and client-managed environments.

## Testing

- Cover recovery from expired credentials through an externally written replacement token.
- Cover authentication-error classification, startup messaging, and `Auto` store migration from file credentials to the keyring.

App-server clients need to advertise structured MCP extension settings, including supported MCP App UI MIME types, rather than only opting into OpenAI form elicitation with a boolean.

## What changed

- Add an `extensions` map to initialize capabilities and preserve `mcpServerOpenaiFormElicitation` as a legacy alias for `openai/form`.
- Capture the declared extension profile when a thread is started, resumed, or forked, propagate it to subagents, and advertise it to downstream MCP servers during initialization.
- Keep the profile stable for the lifetime of the loaded session instead of changing it on later turns or direct tool calls.

## Testing

- Cover extension conversion, downstream MCP initialization, session isolation, legacy form support, and subagent inheritance.

df72fdb415 Consolidate model instructions in ModelMessages (#36787)
## What changed

- Remove `ModelInfo.base_instructions` as an in-memory instruction source and use `model_messages.instructions_template` consistently for bundled, remote, fallback, and overridden model metadata.
- Preserve compatibility by promoting legacy `base_instructions` values when reading model responses and caches, and by including rendered legacy instructions when serializing `ModelsResponse` for older clients.
- Treat templates without instruction variables as literal text and retain the other model-message fields when applying instruction overrides.

This completes the consolidation proposed in https://github.com/openai/codex/pull/31302.

## Testing

- Cover legacy response and cache migration, canonical-template precedence, fallback instructions, personality rendering, overrides, and model switching.

51c9ed6d4f Add per-surface MCP tool exposure controls (#36781)
## Why

MCP tools can be exposed directly, discovered through tool search, or called
from Code Mode. Servers need to be able to opt out of any of these surfaces
without disabling their tools everywhere.

## What changed

- Add `omit_tools_from` to MCP server configuration, accepting any combination
  of `direct`, `deferred`, and `code_mode`.
- Apply the exclusions independently when building direct, deferred, and Code
  Mode tool surfaces while keeping omitted tools registered for permitted uses.
- Remove client-private `_meta` fields from MCP results returned to Code Mode.
- Include the new setting in the configuration schema and MCP config
  serialization.

## Testing

- Cover every exposure combination across Code Mode, Code-Mode-only sessions,
  tool search, direct-only namespaces, and prefixed and unprefixed MCP names.
- Verify direct and nested execution paths, parallel-call support, config

164b3bfeab Isolate MCP OAuth credentials by environment (#36310)
## Why

MCP servers running in an executor must not reuse host-owned OAuth credentials,
even when they have the same configured server name and URL.

## What changed

- Derive environment-scoped credential names for executor-owned MCP servers and
  use them consistently for login, logout, authentication, and connection setup.
- Mark executor-owned entries in the file credential store and fail closed when
  loading or saving an entry could cross the host/executor boundary.
- Preserve compatibility with existing local OAuth credentials, including local
  server names that overlap the new reserved prefixes.

## Testing

- Extend the executor MCP integration test to verify that executor requests use
  the executor token, never send the host token, and persist both credentials
  separately.

3e3ae08839 Limit MCP catalog pagination (#36039)
## Why

MCP catalog discovery must not allow a server to keep pagination running or
growing without bounds.

## What changed

- Apply the shared pagination collector to tool, resource, and resource-template
  discovery.
- Limit each catalog to 100 pages and 1,024 items, and reject pagination cursors
  larger than 64 KiB or any repeated cursor.
- Bound the entire pagination operation by the configured tool timeout, falling
  back to 30 seconds when no timeout is configured.

## Testing

Add unit coverage for every limit and an end-to-end MCP test that preserves
valid multi-page tools while excluding a server that returns an oversized
cursor.

## Why

A pending optional MCP server can delay the first model request even when the
turn does not need that server.

## What changed

- Give optional MCP servers a shared one-second startup grace, then omit servers
  that are still pending from the captured tool catalog.
- Continue waiting when the turn explicitly requires a server through a plugin,
  skill dependency, or `mcp://` mention, and preserve this behavior for input
  received between sampling steps.
- Route direct resource requests to the live connection set when a pending
  server was omitted from the binding, while keeping all-server resource
  discovery non-blocking.

## Testing

- Cover the shared grace period and resource behavior for pending optional
  servers.
- Verify plugin, plugin-skill, and direct MCP mentions wait for startup.
- Verify an Apps-enabled turn proceeds without tools from an unrelated pending
  optional server.

d7e8f4c3dc Preserve user input when MCP startup is interrupted (#34839)
## Why

Interrupting a turn while MCP tools were still starting could abort before the
submitted user input was recorded in conversation history.

## What changed

- Build and retain the MCP tool list and tool router as part of each step
  snapshot, using the turn cancellation token during tool discovery.
- Record turn hooks and submitted input when cancellation occurs during startup
  prewarming, pre-sampling compaction, or first-step capture.
- Reuse the captured router for sampling, compaction, and prompt debugging so
  advertised and executable tools share the same request-scoped view.

## Testing

Add coverage that interrupts each affected MCP startup phase and verifies that
the user prompt remains in history before the turn-aborted marker.

42c5d3c80d Reuse MCP tool catalogs across sessions (#33184)
## Why

Starting a new session should not have to wait for an unchanged stdio MCP
server to initialize before its tools can be presented to the model.

## What changed

- Cache recent tool catalogs for matching stdio MCP server configurations and
  use them while a new live connection starts in the background.
- Bound the process-wide cache to 32 entries with a 30-minute TTL, and key
  entries by the server configuration, environment, working directory, and
  elicitation capabilities that affect the catalog.
- Keep connection-specific instructions and tool annotations out of cached
  catalogs. Resolve tool calls against the live connection so tools that are
  unavailable or hidden in the new session cannot be invoked from stale data.
- Publish only the newest completed catalog fetch and do not cache HTTP servers
  or stdio configurations that depend on remotely sourced environment values.

## Testing

- Add coverage for cache expiration, environment changes, catalog sanitization,
  and out-of-order refreshes.
- Add an end-to-end test that starts inference from cached definitions while a
  replacement MCP process initializes, then verifies calls and visibility
  against the live process.
