## Why
External tool events may need to enter thread history without a preceding function call and therefore do not have a `call_id`.
## What changed
- Allow `function_call_output` items to omit `call_id` and carry optional `name` and `namespace` fields.
- Preserve named standalone outputs during history normalization and agent forks while retaining existing pairing behavior for outputs with a `call_id`.
- Accept, persist, and forward these outputs through `thread/inject_items`, and update the app-server schemas and documentation.

- Cover paired and standalone JSON round trips, history normalization, agent forks, and injected thread history.

## Why
App-server request deserialization otherwise ignores unknown fields, so clients using the removed `permissionProfile` field could have their requested permission settings silently ignored.
## What changed
- Reject `permissionProfile` with an invalid-params error on `thread/start`, `thread/resume`, `thread/fork`, and `turn/start`.
- Direct clients to select a named profile through `permissions` instead.
- Continue accepting unrelated unknown fields for forward compatibility.
## Testing
- Cover rejection of the obsolete field on all four affected methods and verify that the connection remains usable.
- Cover successful `thread/start` requests that combine named `permissions` with an unknown field.

- Add a composable `MockResponsesConfig` builder while preserving the existing

assembly.

## Why
Topology-neutral app-server integration tests should exercise automatic
environment selection so the same setup covers local and remote
executors.
## What
Migrate eligible tests to `TestAppServer::new_with_auto_env()` and
`send_thread_start_request_with_auto_env()`. Leave explicit-topology
tests unchanged, and skip the request-permissions case on Windows with a
TODO for cross-platform tool routing.

unsuccessful text fallback; the model receives the validation message as
the function output
