942af8447b Retire the untrusted approval policy (#39630)
## What changed

- Remove `untrusted` from the CLI, configuration schema, and MCP tool interface. Explicit `approval_policy = "untrusted"` settings now fail with an actionable error.
- Remove the known-safe command allowlist. Projects marked untrusted now request approval for every command unless an explicit exec policy rule allows it.
- Keep command parsing conservative by treating in-place `sed` forms as mutating and ignoring unrecognized commands when recording memory usage.

## Testing

- Cover rejection of the retired configuration value and approval requests for commands in untrusted projects.

e2c0837923 Make user input blocking behavior explicit (#36410)
## Why

Clients need to know whether a `request_user_input` request must wait for an
explicit response or may auto-resolve. Using `autoResolutionMs` as that signal
conflated the blocking decision with timeout policy.

## What changed

- Add required `isBlocking` fields to user input protocol and app-server
  request payloads. Plan-mode requests are blocking, while requests from other
  enabled modes are non-blocking.
- Drive the TUI's auto-resolution behavior from `isBlocking` and remove
  `autoResolutionMs` from the model-facing tool schema.
- Deprecate `autoResolutionMs` while retaining it for compatibility, and treat
  legacy payloads without `isBlocking` as blocking.

## Testing

- Cover mode-derived blocking behavior, legacy deserialization, app-server
  forwarding, delegated requests, and TUI auto-resolution.

10cc57c95c Simplify app-server integration test setup (#34786)
## What changed

- Add typed helpers for initialized app-server instances, JSON-RPC requests,
  responses, notifications, and thread startup.
- Add a composable `MockResponsesConfig` builder while preserving the existing
  mock config writers.
- Migrate the app-server integration suite to the shared helpers, removing
  repeated request ID management, deserialization, initialization, and config
  assembly.

## Testing

- Add unit coverage for composing mock provider, feature, and extra config, and
  for compatibility of the existing mock config writer.

dbd2df27d1 test: migrate TestAppServer callers to builder (#31451)
## Why

Keep the TestAppServer builder API change reviewable by moving the
repository-wide caller migration into a mechanical follow-up.

## What

- replace TestAppServer constructor callsites with equivalent builder
chains
- preserve each caller's automatic-environment, args, program, env,
managed-config, plugin-startup, and JSON-logging behavior
- make no TestAppServer implementation changes

## Validation

- cargo check -p codex-app-server --tests

c2b3e3b4f5 test: use automatic environments in app-server integration tests (#29789)
## Why

Topology-neutral app-server integration tests should exercise automatic
environment selection so the same setup covers local and remote
executors.

## What

Migrate eligible tests to `TestAppServer::new_with_auto_env()` and
`send_thread_start_request_with_auto_env()`. Leave explicit-topology
tests unchanged, and skip the request-permissions case on Windows with a
TODO for cross-platform tool routing.

## Validation

- `just test -p codex-app-server`
- `bazel test //codex-rs/app-server:app-server-all-wine-exec-test
--test_output=errors`

Stacked on #29788.

216ce03031 Add request_user_input auto-resolution window contract (#27256)
## Why

`request_user_input` is moving beyond its original plan-mode-only
workflow, and future default/goal-mode usage needs a way for the model
to ask helpful but non-blocking questions without forcing the turn to
wait forever. This PR adds an explicit `autoResolutionMs` contract so a
later client/runtime change can auto-resolve unanswered prompts after a
bounded window while leaving truly blocking questions unchanged.

This is contract plumbing only; it does not implement the client-side
timer or auto-selection behavior, and the model-facing description
treats the field as reserved unless the current runtime explicitly
supports auto-resolution.

## What Changed

- Added optional `autoResolutionMs` to the model-facing
`request_user_input` args and core `RequestUserInputEvent`.
- Added model-facing schema text for `autoResolutionMs` while marking it
reserved for runtimes that explicitly support auto-resolution.
- Bounds `autoResolutionMs` to `60_000..=240_000` ms during argument
normalization by clamping out-of-range model-provided values.
- Propagated the field through app-server v2
`ToolRequestUserInputParams`, app-server request forwarding, generated
TypeScript, and JSON schema fixtures.
- Updated app-server, core, protocol, and TUI call sites/tests so
omitted values preserve existing `None`/`null` behavior and coverage
verifies a `Some(60_000)` round trip.

## Verification

- `just test -p codex-app-server-protocol`
- `just test -p codex-core request_user_input`
- `just test -p codex-app-server request_user_input_round_trip`

6536841d89 fix: rename McpServer to TestAppServer (#25701)
This PR brought to you via VS Code rather than Codex...

- opened `codex-rs/app-server/tests/common/mcp_process.rs`
- put the cursor on `McpServer`
- hit `F2` and renamed the symbol to `TestAppServer`
- went to the file tree
- hit enter and renamed `mcp_process.rs` to `test_app_server.rs`
- ran **Save All Files** from the Command Palette
- ran `just fmt`

The End

(Admittedly, most of the local variables for `TestAppServer` are still
named `mcp`, though.)

e92c952b2e [codex] Add user input client ids (#24653)
## Summary

Adds an optional `clientId` field to app-server v2 `UserInput` and
carries it through the core `UserInput` model so clients can correlate
echoed user input items without relying on payload equality.

## Details

- Adds `client_id: Option<String>` to core `UserInput` variants.
- Exposes the v2 app-server field as `clientId` on the wire and in
generated TypeScript.
- Preserves the id when converting between app-server v2 and core
protocol types.
- Regenerates app-server schema fixtures.

## Validation

- `just fmt`

69d7a456bb app-server: Replay pending item requests on thread/resume (#12560)
Replay pending client requests after `thread/resume` and emit resolved
notifications when those requests clear so approval/input UI state stays
in sync after reconnects and across subscribed clients.

Affected RPCs:
- `item/commandExecution/requestApproval`
- `item/fileChange/requestApproval`
- `item/tool/requestUserInput`

Motivation:
- Resumed clients need to see pending approval/input requests that were
already outstanding before the reconnect.
- Clients also need an explicit signal when a pending request resolves
or is cleared so stale UI can be removed on turn start, completion, or
interruption.

Implementation notes:
- Use pending client requests from `OutgoingMessageSender` in order to
replay them after `thread/resume` attaches the connection, using
original request ids.
- Emit `serverRequest/resolved` when pending requests are answered
or cleared by lifecycle cleanup.
- Update the app-server protocol schema, generated TypeScript bindings,
and README docs for the replay/resolution flow.

High-level test plan:
- Added automated coverage for replaying pending command execution and
file change approval requests on `thread/resume`.
- Added automated coverage for resolved notifications in command
approval, file change approval, request_user_input, turn start, and turn
interrupt flows.
- Verified schema/docs updates in the relevant protocol and app-server
tests.

Manual testing:
- Tested reconnect/resume with multiple connections.
- Confirmed state stayed in sync between connections.

2f4d6ded1d Enable request_user_input in Default mode (#12735)
## Summary
- allow `request_user_input` in Default collaboration mode as well as
Plan
- update the Default-mode instructions to prefer assumptions first and
use `request_user_input` only when a question is unavoidable
- update request_user_input and app-server tests to match the new
Default-mode behavior
- refactor collaboration-mode availability plumbing into
`CollaborationModesConfig` for future mode-related flags

0523a259c8 Reject ask user question tool in Execute and Custom (#9560)
## Summary
- Keep `request_user_input` in the tool list but reject it at runtime in
Execute/Custom modes with a clear model-facing error.
- Add a session accessor for current collaboration mode and enforce the
gate in the request_user_input handler.
- Update core/app-server tests to use Plan mode for success and add
