62b458c931 Block goals after repeated execution host failures (#41454)
## What changed

- Track failed `exec` attempts whose handler ran for each active goal.
- Mark the goal as blocked after three qualifying failure turns.
- Reset the failure streak when any tool succeeds, and keep failures from carrying over to a replacement goal.

## Testing

- Cover failure classification, streak resets, goal replacement, and end-to-end goal blocking through the app server.

48e22a5fa0 Report code mode host request durations (#41452)
## Why

Code mode wall time should measure the host operation itself, without including
client-side response delays or idle time between requests.

## What changed

- Measure each execute, wait, and terminate request in the code mode host.
- Carry the duration through the stdio and gRPC protocols and use it for
  model-visible wall time.
- Emit a structured `codex.code_mode.host_timing` event correlated with the
  conversation, turn, tool call, and cell.

## Testing

- Cover successful and failed execution timing, delayed response reads,
  repeated waits, termination, and missing cells across stdio and gRPC.
- Verify timing survives protocol serialization and is reflected in app-server
  model output and structured telemetry.

725b3a44f8 Remove WebSocket transport from code-mode hosts (#40692)
## What changed

- Accept only `http://` and `https://` gRPC endpoints for app server
  `--code-mode-host` connections.
- Limit the standalone code-mode host to stdio and gRPC listeners.
- Remove the WebSocket session provider, dual-WebSocket negotiation, transport
  lanes, and related dependencies and tests.
- Keep coverage for shared and prewarmed gRPC hosts and reject WebSocket host
  URLs in CLI parsing tests.

4b24638e1c Prewarm code-mode host connections at session startup (#40678)
## What changed

- Add the opt-in `code_mode_prewarm` feature to establish the code-mode host session during startup, before the first turn.
- Make in-progress host initialization cancellable so a stalled prewarm does not block shutdown.
- Share a failed connection attempt with concurrent callers instead of immediately starting another attempt.

## Testing

- Verify app-server contacts the configured host before the first turn and can shut down while that connection is stalled.
- Exercise shared remote-host behavior with prewarming enabled.

5104cb649e Support gRPC code-mode hosts in app server (#38288)
## What changed

- Accept root `http://` and `https://` URLs in `--code-mode-host` and use the
  shared gRPC session provider for those endpoints.
- Keep `ws://` and `wss://` URLs on the existing WebSocket transport.
- Reject paths, queries, fragments, and credentials where unsupported, without
  exposing gRPC URL credentials in command-line validation errors.

## Testing

- Cover argument parsing and transport selection for both remote protocols.
- Exercise a gRPC host shared across app-server threads and verify credential
  rejection does not disclose usernames or passwords.

f61b51ddd9 Support remote code-mode hosts in app-server (#35098)
## What changed

- Add `--code-mode-host ws://...` and `wss://...` support to `codex app-server`, gated by the `code_mode_host` feature. When omitted, app-server continues to start a local host.
- Share one remote WebSocket connection across the process's threads, using the configured HTTP client's proxy and TLS policy and preserving the existing framed host protocol.
- Reject invalid host URLs, bound WebSocket frame sizes, close connections cleanly, and return an error when a connection exceeds 1,024 pending delegate calls without disconnecting it.

## Testing

- Cover CLI validation, WebSocket protocol execution and shutdown, connection sharing across app-server threads, and delegate-call capacity recovery.
