Won Park 6143217c67 Cancel Guardian reviews with their tool calls (#40021)
## What changed

Propagate tool cancellation tokens into Guardian approval reviews so interrupting a tool also aborts its pending review. Apply the same cancellation behavior to server-initiated MCP approval elicitation.

## Testing

Add integration coverage for cancellation from direct tool calls, code-mode turns, background code-mode cells, and MCP elicitation.

malsamiri-oai be6ebb1f6d Trace turn context creation and realtime state checks (#40017)
## What changed

- Add trace-level `turn_context.make` instrumentation around turn context
  construction.
- Add a trace-level `realtime_conversation.running_state` span around realtime
  conversation state checks.
- Skip recording function arguments in both spans.

Krish Chainani ab8768306f Preserve executor context for MCP stop hooks (#40012)
## What changed

- Scope executor-provided stop-hook calls to the MCP server environment that registered the hook, and reject calls when that environment does not match.
- Forward turn metadata with executor stop-hook requests while retaining the hook call's `threadId` metadata.

## Testing

- Cover environment matching for MCP tool calls and verify executor stop hooks receive their environment ID and request metadata.

Krish Chainani 8b61c50ebe Run allowlisted executor plugin stop hooks (#40009)
## What changed

- Discover inline hooks from executor-provided plugin manifests and accept only
the bundled Computer Use `Stop` hook for `node_repl.turn_ended`.
- Add the accepted hook to the current step's hook engine with
  executor-scoped provenance.
- Run executor-scoped hooks in the background after regular stop-hook
decisions, without delaying turn completion, applying control effects, or
exposing them in hook summaries. Skip them when regular hooks only block
  continuation.

## Testing

- Added coverage for manifest filtering and option preservation.
- Added coverage for background execution, interaction with regular stop
  hooks, and non-blocking turn completion.

sayan-oai f580dd886f Enforce environment network policies for remote execution (#39980)
## Why

`EnvironmentConfig.network_policy` could describe attachment-owned traffic
restrictions, but core rejected every configured policy because execution did
not enforce it.

## What changed

- Resolve each remote environment's network policy for the selected command
and apply it to the execution-scoped proxy.
- Compose owner rules with controller constraints and saved network decisions
while preserving inherited domain and Unix-socket denials.
- Keep strict allowlists non-expandable, allow reviewable policies to use
network approvals, and reject sandbox escalation that would bypass an owner
  policy.
- Reject policies for local execution, disabled managed enforcement, or a
  disabled controller proxy.

## Testing

Added coverage for policy composition, scoped remote proxy behavior, approval
and denial flows, offline execution, and unsupported environment authority.

jif e482cc66ae Keep Guardian reviews isolated from executor MCP servers (#39962)
## Why

Executor-owned MCP configuration can include required servers that are unavailable. Guardian review sessions should not attempt to discover those servers or let them block an approval review.

## What changed

- Skip projecting MCP servers from selected executors when the session source is a Guardian reviewer.
- Preserve executor MCP discovery for regular thread sessions.

## Testing

Added an app-server integration test that configures an unreachable required executor MCP server and verifies that Guardian can still review an escalated command and return its decision.

jif 3882ced09c Add in-memory shell snapshots to unified exec (#39957)
## What changed

- Add the under-development `shell_snapshot_v2` feature and advertise executor support through environment capabilities.
- Use executor-managed, in-memory snapshots for eligible direct `bash`, `zsh`, and `sh` login commands, while applying the configured shell environment policy and avoiding duplicate automatic startup-file effects.
- Keep file-backed snapshots available for user-shell commands and fall back to the existing execution path when in-memory snapshots are unsupported or inapplicable.

## Testing

- Cover policy filtering, snapshot reuse without snapshot files, automatic startup files, local and remote execution, and legacy user-shell snapshots.

jif 00a7b888b2 Discover HTTP MCP servers from selected executors (#39941)
## What changed

- Read `mcp_servers` configuration and requirements from each selected remote
executor and add eligible HTTP servers to the thread's MCP runtime.
- Bind discovered servers to the thread's concrete executor snapshot and apply
environment MCP policy and requirements. Discovery is best effort, and
executor-local servers are not treated as required at startup.
- Ignore unsupported stdio servers and HTTP configurations that depend on
  environment-provided headers or header helpers.

## Testing

- Added an app-server integration test covering discovery, authenticated HTTP
tool invocation, requirements enforcement, and exclusion of stdio servers.

Brandon Zhang 27c05a52e0 Include context window IDs in response metadata (#39847)
## What changed

- Add `context_window_id` to turn metadata for sampling and compaction requests, using the UUID for the model-visible context window.
- Keep the ID stable for requests in the same context window and advance it after compaction, independently of the existing thread-generation `window_id`.
- Reserve `context_window_id` so client metadata cannot override it.

## Testing

- Cover HTTP and WebSocket metadata, remote compaction, token-budget context alignment, resume behavior, and post-compaction window changes.

pmccrary-oai daa48072f4 Add history and notes tools for token-budget sessions (#39827)
## Why

Token-budget sessions need a way to recover prior conversation context and preserve working state across context-window transitions.

## What changed

- Add direct-model `history` tools for listing windows and items, reading items, and searching conversation contents.
- Add direct-model `notes` tools for listing, reading, searching, appending, and writing persistent notes.
- Route tool calls through the configured Codex backend with trusted session and agent context, bounded request arguments, and truncation-aware output handling.
- Expose the extension when `features.token_budget.use_history_notes_history` is enabled with an OpenAI provider and Codex backend authentication.

## Testing

- Cover tool registration, configuration changes, provider and authentication requirements, backend request context, encrypted output preservation, and request and response limits.

rka-oai aead844f64 Handle standalone tool outputs as external context (#39791)
## What changed

- Treat standalone `function_call_output` items without a `call_id` as external context, including items injected into new and forked threads, and mark the thread memory mode polluted when `memories.disable_on_external_context` is enabled.
- Include these outputs in guardian transcripts with their namespaced tool name and a placeholder for non-text content.
- Allow image generation to reuse images from standalone and otherwise unpaired function or custom-tool outputs.

## Testing

## Why

External tool events may need to enter thread history without a preceding function call and therefore do not have a `call_id`.

## What changed

- Allow `function_call_output` items to omit `call_id` and carry optional `name` and `namespace` fields.
- Preserve named standalone outputs during history normalization and agent forks while retaining existing pairing behavior for outputs with a `call_id`.
- Accept, persist, and forward these outputs through `thread/inject_items`, and update the app-server schemas and documentation.

## Testing

- Cover paired and standalone JSON round trips, history normalization, agent forks, and injected thread history.

text included in `codex.tool_result` logs independently of model-visible
  output limits.
- Apply truncation centrally when emitting telemetry, preserve UTF-8
boundaries, and report whether the logged output was truncated.
- Record typed tool names and namespaces, agent names, and process-local result

and `write_stdin` as the shell execution tools.
- Treat legacy `shell_command` model metadata as `unified_exec`, and normalize
legacy user opt-outs so they do not disable command execution. Managed feature
requirements and `shell_tool` can still disable it.
- Preserve shell approvals, sandboxing, zsh-fork support, and output truncation

sayan-oai 2e1f18e0db Refresh resumed thread capability roots from executors (#39746)
## Why

Resumed threads can carry a persisted capability-root location that no longer
matches the location reported by the attached executor.

## What changed

- Refresh matching thread-owned capability roots from the live environment while
preserving persisted roots when the executor reports none.
- Use the same merge behavior for capability-root inspection and per-step MCP
resolution, without changing owner-configured root precedence.

## Testing

Extend the remote-environment resume test to cover both an executor-provided

Adam Perry @ OpenAI 7ece061767 Enforce filesystem permissions when loading AGENTS.md (#39653)
## Why

Project instructions must respect the selected environment's filesystem read
permissions. Tightening those permissions for a later turn must not allow
previously cached instructions to reach the model.

## What changed

- Apply each environment's filesystem sandbox while discovering and reading
  `AGENTS.md` files.
- Fail thread or turn setup when sandboxing blocks a discovered instruction
file, while allowing a restricted project with no instructions to start.
- Clear cached instructions before refresh and include the Windows sandbox
  level in the cache key.
- Ignore inaccessible ancestor marker probes so readable instructions in the
selected working directory can still load.

## Testing

Added coverage for restricted projects, denied instruction files, cache
invalidation after permissions tighten, and unreadable ancestor markers.

jif 942af8447b Retire the untrusted approval policy (#39630)
## What changed

- Remove `untrusted` from the CLI, configuration schema, and MCP tool interface. Explicit `approval_policy = "untrusted"` settings now fail with an actionable error.
- Remove the known-safe command allowlist. Projects marked untrusted now request approval for every command unless an explicit exec policy rule allows it.

App widgets need to read resources using the app and account context of the
tool call that produced them, including after a thread is restored.

## What changed

- Add an optional `originCallId` to `mcpServer/resource/read` requests and
return it on successful scoped reads.
- Track bounded provenance for successful app tool calls in thread history and
use the current tool binding, account link, and app policy when reading the
  associated widget resource.
- Reject missing, mismatched, failed, or ambiguous origins, while keeping
app-only tools available for widget reads without exposing them to the model.

## Testing

Add app-server coverage for scoped widget reads across both history modes,
ephemeral and persistent threads, server restarts, app-only visibility, and
invalid origin cases.

Shijie Rao 230791fd1f Persist active permission profiles in turn context (#39145)
## What changed

- Add the optional `active_permission_profile` field to `TurnContextItem` to record the built-in or named profile that produced the effective permission profile.
- Populate the field when serializing a turn context while preserving compatibility with records where it is absent.

## Testing

- Add coverage verifying that turn-context serialization stores the active permission profile.

guinness-oai 13dfaab446 Surface interactive requests in realtime conversations (#39113)
## What changed

- Mirror execution, permission, and patch approval requests into active realtime conversations with a prompt to review them in the app.
- Mirror user-input and elicitation requests with a prompt to respond in the app.
- Include the serialized request event so the realtime handoff retains the request details.

willwang-openai fc6268ad38 Read plugin authentication state from AuthManager (#39087)
## What changed

- Give `PluginsManager` a shared `AuthManager` instead of a separately mutable authentication-mode snapshot.
- Read the current authentication mode and credentials from that shared manager for plugin discovery, startup tasks, CLI commands, MCP setup, and external-agent migration.
- Update test helpers and coverage to exercise plugin projections and curated marketplace selection as authentication changes.

iceweasel-oai 2013e04354 Preserve filesystem permission path conventions (#39084)
## Why

Filesystem permission paths can use a convention that differs from the host
running Codex. Converting them immediately to native absolute paths can change
the meaning of ambiguous paths such as `/C:/secret` or Windows UNC paths.

## What changed

- Store literal filesystem permission paths as `PathUri` values through the
  runtime policy and execution protocol.
- Keep legacy string-based serialization at explicit protocol boundaries and
reject conversions that cannot be represented losslessly.
- Encode native paths as opaque URIs when a normal file URI would imply the
  wrong path convention.

## Testing

Added coverage for cross-platform and ambiguous path round trips, UNC path
variants, permission-profile serialization, and deny-policy enforcement.

sayan-oai 4a7b51c560 Add network policy metadata to environment configuration (#39055)
## What changed

- Add `EnvironmentNetworkPolicy` as a serializable view of portable domain,
  Unix-socket, upstream-proxy, and local-binding restrictions without exposing
  controller-owned proxy runtime settings.
- Add an optional `network_policy` field to `EnvironmentConfig` and re-export
the policy and related permission types through the core APIs.
- Reject owner-provided network policies during environment preview and
readiness until runtime enforcement is implemented, while preserving the
existing controller policy when the field is absent.

## Testing

- Cover rejection through both environment preview and readiness, including
preservation of the existing environment selection.

felixxia-oai 9dd3d6a13e Restore Guardian risk scores across thread lifecycles (#39038)
## Why

Resumed and forked threads need their persisted Guardian risk state available when routing later approval requests.

## What changed

- Initialize a session's `SecurityRiskScore` from the latest score in its rollout history when no score was supplied explicitly.
- Preserve the inherited score when resuming or forking a thread so low-risk actions can skip redundant Guardian reviews.

## Testing

- Add app-server coverage for inheriting the latest Guardian score when resuming and forking threads.

sayan-oai e38290846c Enforce environment-specific command policies (#38942)
## What changed

- Add an optional restrictive execution policy to `EnvironmentConfig` and merge it over the active command policy for shell execution and approval decisions.
- Reject environment policies that contain allow rules, so environment configuration can only tighten command access.
- Include the environment policy fingerprint in cached approval keys so policy changes require fresh session approvals.

## Testing

- Cover restrictions overriding saved prefix approvals, policy changes invalidating session approvals, and environment overlays remaining effective when model-specific prefix filtering is active.

sayan-oai 6c108912ee Honor per-environment shell variable policies (#38902)
## What changed

- Carry `ShellEnvironmentPolicy` in each resolved `EnvironmentConfig` and use
the selected turn environment's policy for shell commands, user shell tasks,
  and unified exec.
- Infer the policy from the thread configuration when an environment does not
  provide its own resolved configuration.
- Redact the policy from `EnvironmentConfig` debug output because it can contain
  explicit environment variable values.

## Testing

- Verify shell handlers and unified exec filter inherited variables according
to the selected environment while preserving its explicit overrides.

Anton Panasenko fcdae21073 Support metadata staging for reserved thread IDs (#38819)
## Why

Callers need to associate host-owned state with a thread before Core starts it.

## What changed

- Add `ThreadManager::reserve_thread_id` and allow new threads to start with the reserved ID. Reject reserved IDs when resuming an existing thread.
- Let thread stores stage metadata for a reserved ID. The local store merges staged values into the first successful metadata update and then removes them.
- Clear staged metadata when an unmaterialized thread shuts down or is discarded, and require the state database for staging.

## Testing

- Cover reserved ID generation and resume validation.
- Cover metadata persistence, merging, rollout-compatible updates, cleanup, and invalid staging requests.

joeflorencio-openai 00f6a8a60e Keep active-turn model settings stable across updates (#38785)
## Why

Thread settings can change while a turn is still active, including between sampling requests. Those updates should apply to the next turn instead of changing the model configuration partway through the current turn.

## What changed

- Snapshot the model, reasoning settings, service tier, approval settings, and model-attributed telemetry in `StepContext`.
- Build prompts, model requests, world state, tracing, and startup prewarms from that step-scoped snapshot.
- Share immutable model metadata between turn and step contexts.

## Testing

Added an integration test that pauses an active turn, updates its thread settings, and verifies that all requests in the active turn retain the original settings while the next turn uses the updates.

Abhinav 85fc4def35 Add MCP tool handler support to the hooks engine (#38705)
## What changed

- Discover synchronous `mcp_tool` hook handlers and invoke their configured MCP server and tool through a supplied executor.
- Expand nested hook-event placeholders in MCP tool inputs while preserving JSON types, and process tool output through the existing hook output contract.
- Represent hook details as handler-specific metadata in `hooks/list`, including MCP server and tool fields, and show those details in the TUI hooks browser.
- Skip unsupported `SessionEnd` MCP hooks and runtimes without MCP invocation support with startup warnings.

## Testing

- Cover argument expansion, missing placeholders, MCP invocation and hook decisions, discovery warnings, `hooks/list` metadata, and TUI rendering.
