reia-oai 14c8b7771a Attribute compaction analytics to the correct voice session (#51963)
## Why

Compaction analytics lack a voice session ID. A compaction event can arrive after its voice session has closed or another session has started, so attribution needs to account for when compaction began.

## What changed

Add `voice_session_id` to compaction events. Track realtime session starts, ID updates, and closures, retaining up to 16 recent closed session windows. Resolve the session from the compaction start timestamp, falling back to the turn's voice session ID.

Forward realtime session ID updates from the app server to analytics and update the active turn's attribution.

## Testing

Add coverage for realtime session boundary facts, compaction event serialization, and delayed compaction events across consecutive voice sessions.

Adam Perry @ OpenAI c9870d0157 Use PathUri for skill identity and path matching (#51482)
## Why

Skill selection and disabled-path matching need to recognize equivalent Windows paths across case and separator differences while preserving literal spaces, `%`, and `#` in native filenames.

## What changed

- Carry `PathUri` through skill metadata, discovery mappings, filesystem lookups, disabled-path sets, and invocation analytics.
- Match linked skill mentions and TUI selections by parsed path identity.
- Use `LegacyAppPathString` for app-server skill paths and regenerate schemas and SDK types, preserving native path strings on the wire.
- Preserve native catalog display paths and ordering. Keep host filesystem probes and configuration writes restricted to host-compatible paths.

## Testing

Add regression tests for Windows path identity and disabled-skill matching, literal filename characters, catalog resource reads, native path ordering, and foreign-path analytics normalization.

Steve Coffey b0a6b8d86f Record resolved model and reasoning effort in sub-agent activity (#51463)
## What changed

Add `model` and `reasoningEffort` to `subAgentActivity` items for multi-agent v2 spawns. Capture startup settings before the child begins its turn and resolve the reported effort against model metadata, including model defaults, while preserving the child's configured effort.

Carry these fields through live notifications, persisted activity, thread history, and legacy rollout migration. Update the JSON schemas and generated TypeScript and Python types. Older records and other activity kinds retain null values.

## Testing

Add coverage for resolved spawn settings matching the child's request across legacy and paginated history, preserving configured effort, forwarding metadata through events and history, and reading and migrating older records without the new fields.

Eddie Chen cfc946f4e1 Expose and persist turn lineage across the app server (#51415)
## Why

Clients starting delegated work need to attribute descendant turns to the original turn and retain that attribution through completion and history reads.

## What changed

- Accept optional `parentTurnId` and `rootTurnId` in `turn/start`, and expose `rootTurnId` on returned turns and lifecycle notifications.
- Default new turns to their own root when none is supplied. Preserve the active turn's root when steering, ignoring incoming lineage fields.
- Carry root IDs through completion, interruption, recovery, and fork boundaries, and persist them for paginated history reads. Leave unknown roots null in older history.
- Update protocol schemas and generated TypeScript and Python bindings.

## Testing

Extend coverage for default and explicit roots, steering without changing lineage, interrupted and recovered turns, history reads after restart in both history modes, nested fork pagination, and compatibility with older database writers.

peilin-openai c19525e55e Record initialization analytics for resumed subagents (#51206)
## Why

Reloaded subagent threads did not emit initialization analytics, and subagent initialization events always reported `new`.

## What changed

Emit initialization events for resumed spawned subagents before notifying thread listeners. Inherit parent client metadata before reserving a residency slot can evict the parent, preserve the thread's original `created_at`, and report `initialization_mode` as `resumed`. Refresh stored analytics metadata when a subagent resumes; newly created subagents continue to report `new`.

## Testing

Extend the cold-resume integration test to verify a single worker initialization event with the expected session, parent, source, resume mode, and original creation timestamp. Update serialization and session tests to check initialization modes.

Ahmed Ibrahim 4ad985e2ca Track inference tool changes in turn analytics (#50964)
## What changed

Add `tools_change_count` to turn profiles and turn analytics events. Compare the full model-visible tool list before each sampling request, retaining the previous list across turns and connection resets. The first list establishes a baseline; subsequent changes increment the current turn's count.

## Testing

Add coverage for schema changes, tool additions, ordering changes, empty lists, and tracking across sessions and transport fallback. Extend the turn event serialization test to include `tools_change_count`.

felixxia-oai d79a95bdf8 Record Guardian context mode in review and classification telemetry (#49294)
## Why

Guardian review and classification telemetry lacks the context mode needed to distinguish `legacy`, `independent`, and `thread_owned` reviews.

## What changed

- Add `guardian_context_mode` to review and classification analytics and `context_mode` to their metrics.
- Capture the mode from the conversation history snapshot before review preparation or classification setup so failure metrics retain it, including when review preparation or reviewer startup fails.
- Use `unknown` for review metric tags when no context mode is available.

## Testing

Update metric assertions to cover `thread_owned` classification tags and the `unknown` review fallback.

papayo-oai 2e6cc4ed8d Attribute analytics requests to each thread's product SKU (#49117)
## Why

Sessions can share an analytics client while using different product SKUs. Analytics requests need per-thread attribution to reflect each session's configuration.

## What changed

- Register each thread's `apps_mcp_product_sku` and send `X-OpenAI-Product-Sku` on analytics requests grouped by product, without changing authentication.
- Send product groups concurrently while preserving event order within each group.
- Flush buffered tool events before changing or evicting their product attribution, and clear attribution when a session disables analytics.
- Bound retained thread registrations and concurrent product groups. Drop attribution when registration cannot be queued or a batch exceeds the product-group limit.

## Testing

Add tests for shared-session attribution, clearing attribution on resume with analytics disabled, buffered events across product changes, queue overflow recovery, registry limits, and concurrent delivery with preserved event order.

Tamir Duberstein 011f803f3c Avoid collecting unused Git metadata in skill analytics (#49076)
## Why

Skill invocation analytics only needs the repository's origin URL, but
`collect_git_info` also checks the repository and reads its commit and branch.

## What changed

Extract and export `get_git_origin_url` and use it directly in skill analytics
to skip the extra Git commands. Reuse the helper in `collect_git_info`,
preserving URL sanitization and command timeouts.

rhan-oai 44a9bfa145 Preserve usage limit windows in turn and compaction analytics (#48174)
## Why

Usage limit errors discarded the server-selected window responsible for the limit, leaving turn and compaction analytics unable to distinguish five-hour limits from weekly limits.

## What changed

Preserve optional `limit_window_minutes` from usage limit error responses in `UsageLimitReachedError` and report it as `usage_limit_window_minutes` in turn and compaction events. Accept unsigned integers that fit in `u16`; treat missing, null, malformed, or out-of-range values as unknown. Events for other error kinds report `null`.

## Testing

Add coverage for HTTP and wrapped WebSocket error mapping, analytics serialization and reduction, and app-server propagation to turn and local/remote compaction events. Cases include five-hour and weekly windows, missing or invalid values, and unrelated errors.

teddywyly-oai 49862b62be Retry Guardian reviews when authorization changes (#47819)
## Why

New user input during a Guardian review invalidated an in-flight approval and aborted the pending action, even when the input was only a status inquiry. Review the action again against the latest authorization evidence so it can proceed or be denied based on the updated instructions.

## What changed

- Treat stale authorization as a retryable review error within the existing attempt budget and shared deadline.
- Refresh authorization evidence for each attempt while keeping the pending action fixed, and record evidence from the completed attempt.
- Preserve cancellation and history-reset aborts, and deny approval if stale authorization exhausts the retry budget.

## Testing

Add and update coverage for parent status updates, authorization revocation, repeated updates, cancellation, history resets, and retry deadlines. Verify that refreshed reviews see the latest input and that the pending action executes exactly once only after approval.

iceweasel-oai 6f6af0fce4 Record sandbox backends in command execution analytics (#47568)
## Why

Command execution analytics lack the observed sandbox backend. Older execution servers may omit this information, so an unknown backend must remain distinguishable from an explicitly unsandboxed process.

## What changed

- Carry optional `sandbox_type` metadata through command execution items and emit its metric tag as `sandbox_backend` in analytics.
- Propagate the observed backend through unified execution completion and failure events, including asynchronous exits.
- Preserve missing backend information as `None` and mark executed user shell commands as `Some(SandboxType::None)`.
- Exclude the metadata from serialized command items and generated schemas and TypeScript types.

## Testing

Extend tests for missing and explicit executor backends, backend propagation on failed execution, and analytics payloads, including platform-specific backend tags in app-server integration coverage.

acrognale-oai 8f8ace78cb Enforce application network policy for AWS auth and telemetry (#47408)
## Why

AWS credential discovery, analytics, and telemetry used clients that did not share the application's network policy. These requests need to honor destination restrictions and permission revocation.

## What changed

- Route AWS credential and region HTTP requests through the shared HTTP client, and check the signing destination before loading credentials. Skip discovery for static access keys with an explicit region.
- Apply account-scoped policy to Bedrock authentication. Require unrestricted policy for credential exporters and reauthentication commands, and cancel active work when permission is revoked. Document that AWS profile `credential_process` network traffic remains outside the application's HTTP policy.
- Send analytics through the authenticated account's HTTP client factory.
- Guard OTLP log, trace, and metric exports with revocable permits, disable them under destination restrictions, and make managed HTTP exports cancellable. Suppress global Statsig settings while managed policy is active.

## Testing

Add coverage for metadata credential request cancellation, allowed and denied SigV4 destinations, AWS credential precedence and endpoint configuration, and blocked credential exporter recovery. Update telemetry tests for account transitions and managed-policy Statsig suppression.

alexsong-oai 064e701b0f Add explicit gateway OAuth sign-in to app-server (#47207)
## Why

Clients with a gateway sign-in UI need to control when browser authorization starts and inspect credential readiness before making authenticated requests.

## What changed

- Add `initialize.capabilities.explicitGatewayOauth` to require explicit login while allowing existing credentials to refresh. Preserve automatic authorization for clients that omit the capability, and prevent later connections from undoing explicit opt-in.
- Add `account/gatewayOAuth/read`, `account/gatewayOAuth/login`, and `account/gatewayOAuth/cancel`, plus `account/gatewayOAuth/changed` notifications. Send the authorization URL only to the initiating connection and cancel its login on disconnect.
- Check gateway authentication before returning cached results from `model/list`, with restart guidance when provider settings have changed.
- Update protocol schemas, TypeScript and Python bindings, and document capability probing and the sign-in lifecycle.

## Testing

Add tests for passive readiness reads, credential reuse after login, token exchange failures, cancellation and immediate retry, connection ownership, legacy versus explicit login, and model-list authentication checks.

Eddie Chen 6a601c00da Preserve originating turn metadata for yielded skill calls (#47106)
## Why

Code Mode callbacks can retain skill tools after another turn becomes active. Looking up the active turn when a delayed skill invocation completes can lose access to the originating turn's metrics and metadata.

## What changed

- Capture and retain the originating `SkillTurnMetrics` when constructing skill analytics.
- Expose turn analytics metadata through the turn extension store and carry it into skill telemetry.
- Add optional turn metadata to `TrackEventsContext` and pass it from app, skill, and plugin usage call sites.

## Testing

Add a regression test that yields a Code Mode cell, starts another turn, and releases both skill reads through a shared barrier. Assert that each `skill_invocation` event retains its originating `turn_id`.

Charlie Marsh 87bc50f9d4 Cache OS discovery for user agents and telemetry (#47038)
## Why

OS discovery can launch subprocesses on Linux. Repeating it for requests, analytics events, and metrics provider creation adds unnecessary work for stable metadata.

## What changed

Cache `os_info::get()` with a separate `LazyLock` in user-agent generation, analytics runtime metadata, and metrics OS resource attributes. Continue reading the user-agent originator and suffix on each call so updates remain effective.

## Testing

Add an isolated Linux regression test that races initial user-agent lookups and repeats calls, verifies `lsb_release` and `getconf` each run once, and checks that subsequent originator and suffix updates appear in the user agent.

Henry Levy 49e248d4c3 Add opt-in compaction after final responses (#46541)
## What changed

Add `model_post_turn_compact_threshold_percent` to trigger compaction after a final response when context usage reaches the configured percentage of the usable window or an existing auto-compaction limit. Accept values from `0` to `100`; omitted or zero disables the feature.

Skip post-turn compaction when input is pending, the turn is cancelled, or token-budget mode is enabled. Disable it for approval reviewers to avoid delaying approval completion, and record a distinct `PostTurn` analytics phase.

Buffer local post-turn summaries until compaction succeeds and require a nonempty assistant summary before replacing history. Log compaction failures without failing the completed turn, while still propagating interruptions and turn aborts.

marksteinbrick-oai a1efb59c4a Record active plugin inventory in turn analytics (#46323)
## What changed

Add `active_plugin_ids_at_turn_start` to turn analytics, combining active host plugins with selected plugin packages. Prefer remote plugin IDs when present and otherwise use validated package IDs. Sort and deduplicate the inventory.

Report `null` for unknown inventories, invalid IDs, IDs longer than 128 bytes, or inventories exceeding 512 distinct IDs; preserve `[]` for an observed empty inventory. Keep the first received inventory even when later resolved configuration updates arrive.

## Testing

Add coverage for ID selection and validation, deduplication, size limits, serialization, and preservation of the first inventory. Extend app-server tests to check inventories after plugin reconciliation and with selected plugins, including turns without explicit plugin mentions.

Chris Dong 8ace915ace Attribute analytics events to realtime voice sessions (#46058)
## What changed

Add `voice_session_id` to turn, app-use, skill-invocation, and MCP tool-call analytics events. Track realtime session lifecycle and handoff markers to preserve attribution when a handoff starts a turn after the voice session closes, or steers an active turn. Keep that attribution from carrying over to subsequent text turns.

Record handoff markers using only the thread ID, without retaining transcript content in analytics facts.

## Testing

victor-openai b71af39fe6 Preserve MCP App UI metadata in tool-call events and history (#45805)
## Why

Clients need widget presentation details to render MCP Apps without waiting for the full MCP catalog, including when replaying saved history.

## What changed

- Add `mcpToolCall.mcpAppUi` with the invoked descriptor's `resourceUri` and `preferredModelDisplayMode`, and carry it through tool-call events and saved history.
- Support `fullscreen` and default to `inline` when the display preference is missing or unsupported.
- Keep existing resource URI fields for compatibility. Leave `mcpAppUi` null for older history and tools that declare widgets only in result metadata, where clients still use catalog discovery.
- Update protocol schemas, TypeScript and Python types, and app-server documentation.

## Testing

Add parameterized integration coverage for fullscreen, missing and unsupported preferences, legacy URI metadata, and result-only widgets, verifying consistent tool events and preservation across session resume.

Krish Chainani 7b8b17b97a Support image references by file ID in inputs and tool outputs (#45794)
## What changed

- Accept `fileId` alongside the existing `url` form for app-server image inputs, and forward file references to the Responses API as `file_id`. Update generated schemas and client types.
- Preserve file references, image detail hints, and mixed inline/file image ordering through user-message events, thread history, and rollout migration. Retain file images when truncating tool output.
- Pass file references through image preparation without resolving them, while keeping resize-notice numbering correct. Omit them from unsupported TUI display and Guardian image context.
- Reject image-edit requests whose recent-image window includes a file reference, preventing selection of an older inline image instead.

## Testing

Add coverage for serialization, request and rollout preservation, mixed-image history ordering, incomplete ordering metadata, tool-output truncation, and rejection of unsupported image-edit selections.

Eric Traut 91d54f1667 Restore collaboration mode when resuming threads (#45519)
## Why

Resuming a thread initialized its collaboration mode to Default, losing the saved Plan mode and its developer instructions. Reconnecting clients also lacked a server-reported mode to reconcile changes made by another client.

## What changed

- Restore the saved collaboration mode from the latest matching `ThreadSettingsApplied` event, falling back to the last legacy `TurnContext`. Apply the effective model and reasoning effort while retaining the saved mode and developer instructions.
- Include `collaborationMode` in `thread/resume` responses and update the generated schemas and bindings.
- Use the restored mode in the TUI, including the first prompt after resume. Prefer the server's mode when restoring disconnected input, while preserving the local selection for older servers that omit it.

## Testing

jwang-openai 4d8eca1ff3 Attribute command and plugin analytics to the invoking model (#45445)
## Why

Command execution and plugin measurement events lack model and reasoning-effort labels. Attribution needs to reflect the step that invoked the command, even when model settings change before a background process finishes.

## What changed

- Add `model_slug` and `reasoning_effort` to command execution and plugin measurement analytics.
- Capture model context from resolved step settings and carry it through execution, approval, Guardian review, and plugin metrics collection.
- Preserve the first command-start model context when subsequent start notifications arrive.
- Keep the carried context out of serialized protocol items and generated schemas.

## Testing

Matthew Zeng c62d191c4c Expose disabled plugin settings in the app-server API (#44905)
## What changed

- Accept `disabledPluginIds` in `thread/settings/update` and `turn/start`. A supplied list replaces the saved selection; omission or `null` preserves it, and `[]` clears it.
- Return the selection in thread start, resume, and fork responses and `thread/settings/updated` notifications. Update generated schemas and client types.
- Restore disabled plugin IDs from the history retained at the requested fork boundary, preserving explicit overrides.

The selection persists across resume but does not yet filter plugin capabilities.

rka-oai 3dc1e2a584 Always use streamed remote compaction for supported providers (#44255)
## What changed

- Route manual and automatic remote compaction through the v2 implementation.
- Retire the `remote_compaction_v2` feature toggle while continuing to advertise it in the model client beta features header.
- Migrate compaction tests from `/responses/compact` to streamed `/responses` requests with `compaction_trigger` items and remove legacy parity coverage.

## Testing

Add streamed compaction coverage for tool search output trimming and session base instruction token estimates. Update app-server tests to verify started/completed notifications for both local and streamed remote compaction.

Chris Dong 929389f596 Preserve per-image generation IDs in image generation analytics (#43953)
## What changed

Parse optional `generation_id` values from image API responses and carry the selected image's ID through the image generation tool into analytics events. Keep the ID out of serialized extension items, JSON schemas, and TypeScript types. Responses without an ID remain supported.

## Testing

Add coverage for distinct IDs in multi-image responses and responses without IDs. Extend analytics and app-server tests to verify that the selected image's ID reaches analytics, and item tests to verify that it is omitted from serialization and TypeScript types.

Benjamin Carlsson 4b0f44d304 Add worktree classification to thread telemetry (#43621)
## What changed

Add `is_worktree` to thread initialization analytics and the `codex.thread.started` metric using validated Git repository metadata. Classify linked worktrees as `true` and primary checkouts as `false`; report an unavailable classification as `null` in analytics and `unknown` in metrics. Subagent initialization analytics leave the field unset (`null`).

## Testing

Add analytics coverage for primary checkouts, linked worktrees, unknown repositories, and event serialization. Extend the account-switch telemetry test to check that the `is_worktree` tag reaches the next account's collector.

faizan-oai 3921a30d6b Persist Daybreak preferences in thread metadata (#42854)
## Why

Clients need a per-thread Daybreak toggle that survives restarts and can be
restored on resume without changing the access program of an active or future
turn.

## What changed

- Add experimental `daybreakEnabled` support to `thread/metadata/update` and
expose the saved value on thread responses.
- Store the preference in SQLite, including for threads without a first turn,
and preserve it through reads, resumes, reconciliation, and restarts.
- Copy the preference when forking a thread while keeping subsequent updates
  independent.
- Keep `daybreakEnabled` separate from the per-turn `cyberAccessProgram` field;
app-server does not derive one from the other.

## Testing

- Cover persistence across restarts, cold reads, reconciliation, and forks.
- Verify metadata updates do not alter the access program of active or later
  turns.

iceweasel-oai 773f0b081d Preserve executor paths in Guardian approval reviews (#42838)
## Why

Guardian approval actions can contain paths from a remote executor whose path convention differs from the host, such as Windows paths reviewed on a POSIX host. Converting those paths to host-native absolute paths prevents the action from being reviewed.

## What changed

- Render command working directories using the executor's reported path convention, and preserve URI-backed paths for attribution.
- Carry foreign command and patch paths through Guardian events and app-server schemas as legacy path strings.
- Bound remote plugin attribution within the overall review deadline and reject Guardian action payloads that exceed the review byte limit.

## Testing

- Cover Windows and POSIX executor path rendering, foreign patch paths, protocol round trips, oversized payload rejection, and remote Guardian review context.

jif 1cd78651e2 Preserve response IDs for fast collaborator tool events (#42752)
## Why

Collaborator tools can finish before their sampling response arrives, causing
their analytics events to be emitted without response correlation.

## What changed

Hold completed collaborator tool events until the reducer can attach their
originating and subsequent response IDs.

## Testing

Add coverage for both orderings of collaborator completion and sampling
response arrival.

Can Sar 728cb12fe5 Expose thread originators through the app-server API (#42458)
## What changed

- Add the creation-time `originator` to app-server thread responses and `thread/started` notifications.
- Persist originators in thread metadata and preserve the first recorded value across list, read, resume, rollout, and SQLite paths.
- Add an `originators` allowlist to `thread/list` for hosted backends. The local app-server accepts omitted or empty values but rejects nonempty filters.

## Testing

- Cover originator persistence across server restarts and list, read, and resume requests.
- Verify local `thread/list` handling for nonempty, omitted, null, and empty originator filters.
- Heap-allocate large TUI dispatcher futures in affected tests to keep them within the Windows test-thread stack.

Anton Panasenko 2b554fd3f9 Expose loaded thread environments in app-server responses (#42386)
## What changed

- Add the experimental `Thread.environments` field with each selected environment's ID, working directory, and runtime workspace roots.
- Populate the field from live thread state in start, resume, read, list, and unarchive responses. Use `null` for unloaded threads and an empty list when a loaded thread has no environment selection.
- Document that the field reports selection rather than connection status and does not affect executor selection or resume behavior.

## Testing

- Cover selection updates, unloaded and resumed threads, loaded pathless unarchives, and clients that subscribe after environment connection notifications.

iceweasel-oai eb078b4f44 Preserve target-native cwd in permission approval requests (#42314)
## Why

Permission requests can originate in an environment whose path convention differs from the Codex host. Requiring the request working directory to be host-native prevents those requests from reaching interactive approval.

## What changed

- Carry the request `cwd` as a `LegacyAppPathString` through core events and the app-server protocol so the target environment's native representation is preserved.
- Convert the preserved value back to a `PathUri` when materializing permission grants.
- Regenerate the JSON and TypeScript protocol schemas for the updated `cwd` type.

## Testing

- Exercise the app-server permission request round trip with a target-native working directory across host path conventions.
- Verify remote permission requests retain their environment working directory and granted access still unblocks later execution.

pmccrary-oai bdfd769640 Track history notes thread hint outcomes (#42247)
## What changed

- Emit a `codex_thread_hint_status` analytics event for each native history-notes thread hint attempt.
- Report whether retrieval was successful or failed along with thread context and timing, without including hint contents.
- Treat valid empty responses as successful retrievals while continuing to omit them from the context window.

## Testing

- Extend the app-server history-notes tests to verify success, empty-result success, and backend failure statuses.

Brent Traut 986ff1cc7c Expose model settings in app-server thread metadata (#42151)
## What changed

- Add nullable `model` and `reasoningEffort` fields to the shared app-server `Thread` object and generated schemas.
- Report current settings for loaded threads and the latest persisted settings for unloaded threads across thread read, list, start, resume, rollback, metadata update, and notification paths.
- Keep unavailable legacy or filesystem-only settings nullable, and allow metadata reads without loading a thread or dispatching queued work.

## Testing

- Cover live-setting precedence, persisted settings after restart, null legacy values, and model metadata returned by thread lifecycle operations.

marksteinbrick-oai 307ce6cda9 Track Guardian reviewer turn and tool analytics (#41023)
## Why

Trusted Guardian reviewer sessions can emit events without an app-server listener, leaving their turns and tool usage absent from analytics.

## What changed

- Translate Guardian session lifecycle and canonical tool item events into analytics notifications while excluding messages, reasoning, and review content.
- Preserve Guardian lineage during prewarming even when inherited client metadata is not yet available, without emitting an incomplete thread initialization event.
- Limit direct session tracking to trusted Guardian children so resumed threads that have an app-server listener do not report duplicate events.

## Testing

- Cover completed, failed, and interrupted Guardian turns, tool counts and timing, lineage, content exclusion, prewarmed lineage, and resumed-thread deduplication.

rka-oai e56e4922eb Support standalone tool outputs in turn/start (#41002)
## What changed

- Add an optional `toolOutput` to `turn/start` for starting or steering a turn with a named function-call output instead of user input.
- Emit standalone outputs as `functionCallOutput` thread items, persist them in durable history, and include them in resumed threads.
- Validate that tool outputs have a name, are not combined with nonempty `input`, satisfy the text-size limit, and use supported image URLs.

## Testing

- Cover standalone tool-output notifications, model input, and resumed history in both legacy and paginated history modes.
- Cover the text-size limit for structured tool output.

## What changed

- Add the `write_stdin_approval` feature flag, disabled by default, to require a fresh approval before sending non-empty input to an escalated unified-exec terminal.
- Route these reviews through hooks, Guardian, app-server, and the TUI as `writeStdin` approvals while keeping the parent command item unchanged.
- Preserve the terminal environment, launch directory, and escalation state across turns, and revalidate the process identity after approval before writing input.

## Testing

Francis Chalissery 7276d67081 Expose resumable misalignment details through app server (#40952)
## Why

App-server clients need the explanation and steering instruction returned with a
misalignment policy violation to offer a user-confirmed continuation. Missing or
incomplete details must continue to behave as a terminal block.

## What changed

- Parse optional misalignment classification, explanation, and steering details
from streamed and HTTP Responses errors and propagate them through core errors.
- Include the details in live app-server `error` and `turn/completed` payloads and
export the corresponding protocol schemas and TypeScript types.
- Keep explanations and steering messages out of serialized rollout events and
  redact them from debug output.
- Document how clients can resume with `turn/start` after user confirmation.

## Testing

- Cover streamed, HTTP, and WebSocket-wrapped errors, including malformed and
  classification-only details.
- Verify live app-server notifications expose resumable details without writing
the explanation or steering message to the rollout.
