## Why
Private IP destinations always bypassed inherited upstream proxies, preventing their use for private networks reachable through an upstream VPN proxy.
## What changed
- Add `codex exec-server --proxy-private-ips-via-upstream`, also configurable with `CODEX_EXEC_SERVER_PROXY_PRIVATE_IPS_VIA_UPSTREAM=true`. The setting defaults to disabled.
- Allow permitted RFC 1918, carrier-grade NAT, and IPv6 unique-local destinations to use an applicable upstream proxy. Loopback and link-local destinations retain direct routing, and destination access policy still applies.
- Keep connections direct when no valid upstream proxy applies or `allow_upstream_proxy=false`. Errors after selecting an upstream proxy do not trigger a direct retry.
- Rename `ExecServerRuntimePaths` to `ExecServerRuntimeOptions` and carry the routing setting from executor startup into the managed network proxy.
## Testing
Add routing coverage for private address ranges, special-use addresses, and public targets with the option enabled and disabled. Verify that HTTP and CONNECT requests still enforce destination allowlists and denylists, and update the CLI help snapshot.

## Why
Application network restrictions need to cover clients created by the TUI and `codex exec` before embedded app-server startup, and remain effective when configuration is rebuilt.
## What changed
- Introduce `EmbeddedNetworkPolicy` to share live policy between caller-owned transports and the embedded app-server.
- Apply loaded requirements before creating telemetry, background HTTP clients, and executor connections. Limit authentication and cloud configuration bootstrap requests to exact endpoints under local policy.
- Preserve the shared policy across TUI session changes, permission-profile rebuilds, and worktree cloud loaders so existing clients observe policy reloads and failures.
- Document application network policy behavior and transport coverage.
## Testing
Add regression coverage for policy activation, reload failure and recovery, retained policy across TUI rebuilds, rejected OAuth and cloud-policy redirects, and workspace network restrictions with `CODEX_API_KEY`. Also cover retaining the caller-selected project during embedded startup.

## Why
`--worktree` previously supported only `codex exec` and `codex exec fork`. Interactive sessions need the same managed checkout support, with configuration and policy resolved for the destination before starting a turn.
## What changed
- Enable `codex --worktree` and `codex fork --worktree <session-id>` behind the `worktrees` feature, restricted to local sessions. Interactive forks require an explicit session selector.
- Resolve interactive and exec worktree forks from the session's latest saved working directory unless `--cd` is supplied. Keep relative `--add-dir` paths anchored to the invocation directory.
- Load interactive destination configuration before telemetry and login policy initialization, and bind checkout ownership before the first turn.
- Reject explicitly untrusted sources, including when destination cloud policy or refreshed configuration reveals distrust. Retain unbound interactive checkouts after startup failure and report manual recovery instructions.
## Testing
Add CLI integration and TUI tests for startup and named forks, destination instructions and configuration, ownership before the first request, trust enforcement, and retained-checkout recovery. Extend exec coverage for saved fork directories, explicit `--cd`, relative writable roots, and cloud policy rejection.
