## What changed
Add `ReviewInput` to bundle an approval request with its captured parent history and review context. Its `assess_isolated` method runs a fresh reviewer without publishing a live approval, honors deadlines and cancellation, and returns a `GuardianAssessment` or review error.
Share review parameter construction with the live review path and extract assessment parsing and error conversion into `GuardianReviewSessionOutcome::into_assessment`.
## Testing
Add a mock-backed test that verifies the isolated assessment is returned and the reviewer receives the captured history without instructions added after capture.

## Why
Source-built V8 archives and generated Rust bindings must agree on their ABI, including in debug builds. Linux platform selection must also distinguish GNU and musl when using LLVM platforms.
## What changed
- Always select source-built V8 archives and matching bindings for Bazel consumers, removing the `rusty_v8_from_source` flag.
- Use LLVM's libc-aware Linux selectors for archives, bindings, headers, and bindgen targets.
- Keep `V8_ENABLE_CHECKS` undefined in debug builds to match Rust's `HandleScope` layout, and set `V8_LOGGING_LEVEL=2` for debug verification.
- Include compiler-rt builtins in the ARM64 GNU/Linux archive to provide `__clear_cache`.
- Set the Intel macOS minimum version for `codex-code-mode-host` releases to 10.13 for V8's aligned allocation support.
## Testing
Always include the existing ignored CppGC ABI checks in Bazel `v8-poc` unit tests and remove their exclusion from Bazel CI.

## What changed
When a diagnostic upload includes logs, include saved threads attached directly to the reported thread in the existing bounded thread selection alongside spawned descendants. Query attached threads from the state database, including archived threads, so their rollouts, logs, and retained review failures can be collected. Log a warning and continue if the lookup fails.
## Testing
Extend the upload integration test to verify that the rollout archive includes the reported and directly attached threads, excludes unrelated threads, and is omitted when logs are disabled.

## What changed
- Allow `/quit` and `/exit` while offline, exiting without waiting for server shutdown.
- Retry `thread/resume` when the server reports `serverShuttingDown` instead of treating the conversation as unavailable.
- Report which reconnect stage timed out, including waiting for server shutdown, without exposing transport errors that may contain endpoint credentials.
## Testing
Extend regression coverage for offline quit commands and stage-specific timeout messages. Update the reconnect integration test to exercise a draining server and verify that local approval and sandbox settings are preserved.

## What changed
Overlap independent plugin filesystem and skill loads with a concurrency limit of eight. Consume results in configured-name order to preserve duplicate MCP server precedence and startup hook warning order.
## Testing
Add a regression test with ten plugins that verifies skill loads overlap within the concurrency limit, results match sequential loading, and MCP precedence and hook warnings are preserved.

## What changed
- Skip path resolution for empty TOML tables and apply resolved values in place, preserving unknown keys and omitting defaults introduced by deserialization.
- Avoid scanning project trust entries after an exact lookup misses on non-Windows platforms. On Windows, select the smallest case-insensitive matching key without allocating and sorting a list.
- Only build the effective configuration for credential-origin filtering when credential origins are present.

## Why
Stopping a sideband observer should close its WebSocket while preserving the frontend-owned realtime session. The existing frameless session close also sends `session.close`, so observer shutdown needs a transport-only close.
## What changed
- Add `close_transport()` to send a normal WebSocket close without `session.close`.
- Close the sideband transport on cancellation or when the call becomes inactive, including an existing connection awaiting adoption by the input task. Bound the close attempt to five seconds.
- Thread an optional sideband base URL through realtime start processing and allow nested thread ID fields in request serialization scopes.
## Testing
Add WebSocket and core conversation regression tests covering observer detachment for both realtime parsers and conversation versions. Verify that explicit frameless session close still sends `session.close`.

## Why
`codex exec` recovers turn items at completion because item notifications can be dropped under backpressure. This recovery needs to respect the thread's history mode.
## What changed
For paginated threads, fetch every page from `thread/items/list`, scoped to the completed turn. Preserve items already present in the completion notification when merging the recovered history, and stop recovery if a cursor repeats. Keep `thread/read` recovery for other history modes.
## Testing
Add regression coverage for recovering a final answer across multiple item pages and for a failed fork recovering only its own turn.

## What changed
- Separate evidence collection from prompt preparation and pass reviewer history, model limits, and image preparation policy through shared `guardian-context` APIs.
- Detect captured images before composition and measure reviewer history tokens only when images and a valid context window require it.
- Share instruction and native-message delivery filtering across budget checks, including restoration of retained evidence after history changes.
GitOrigin-RevId: 65ef53cf0bc22e1bde9abfb7a9ffce65cfab6f1c
## Why
When `codex-auto-review` is missing from the model catalog, Guardian falls back to the parent model. Refresh the catalog before accepting that fallback so reviews and checkpoint compatibility checks can use the default reviewer when it becomes available.
## What changed
- Share one refresh attempt per parent session, preserving it across cancelled waiters and retaining the parent fallback if recovery fails.
- Respect explicit reviewer overrides and custom catalogs, and skip recovery for basic sessions.
- Resolve reviewers from the in-memory catalog so a failed disk cache write does not discard a recovered reviewer.
- Apply review deadlines and cancellation to configuration setup, and avoid reviewer resolution during history replay unless checkpoint compatibility requires it.
## Testing
Add coverage for recovery, failed fetches and cache writes, overrides and custom catalogs, checkpoint compatibility, cancelled prewarming, review deadlines, and interruption.

## Why
Waiting for the event channel to close after `ShutdownComplete` retains the thread runtime because the thread owns its event sender. Remaining subscribers still need late notifications after shutdown.
## What changed
- Release runtime references after `ShutdownComplete` while keeping the listener command queue available for ordered notifications and late request resolutions.
- Preserve other subscribers when a client unsubscribes or disconnects from an unloaded thread, and serialize terminal cleanup with cold resumes.
- Return the thread-closing error when `thread/resume` encounters a terminated runtime or a listener that cannot complete the request.
- Close the status watch when removing a thread so a later load gets a fresh watch.
## Testing
Add regression coverage for listeners attached before and after shutdown, verifying runtime release, preservation of remaining subscribers, late notification delivery, and rejection of resumes against a terminated runtime. Add a test that removing and reloading a thread closes the old status watch.
GitOrigin-RevId: 6dda8f83a0fbe06c9a713e9801356753a0de172e
## Why
A thread runtime can shut down while listener setup is awaiting I/O, and a cold resume can start before the previous listener forwards `ShutdownComplete`. Listener replacement must preserve those buffered terminal events.
## What changed
- Coordinate listener installation and connection subscriptions with pending unloads, and reject closing or replaced runtimes.
- Allow a newly created runtime that has already shut down to drain its queued events, while enforcing one event reader per runtime.
- Track shutdown draining with a shared watch channel so cold resume and runtime replacement wait for terminal events to be processed.
- Serialize idle unloading with goal edits and cold resume, and recheck thread state after acquiring the lock.

## Why
Automatic gRPC reconnection could spawn a replacement host while dispatching an RPC for an existing session. Host loss must remain visible so recovery can report lost state without replaying execution or completed external effects.
## What changed
- Bind each local host channel to one child process and allow only `OpenSession` to replace a dead host.
- Add caller-owned transport support to `GrpcCodeModeSessionProvider` and map local transport failures to `Unavailable` so initial connection failures use the existing bounded admission retries.
## Testing
Add regression coverage for child exit during initial connection and between channel selection and dispatch, concurrent session recovery, and child reaping after teardown. An integration test kills the host after a command completes and verifies that recovery reports the reset, clears stored state, and leaves the command's external effect unchanged.
Increase the remote MCP stalled-handshake test's initialization timeout and mock response delay to let requests reach the server on loaded CI workers.

## Why
Probing a pasted serial device or FIFO path for image dimensions can block the TUI input loop.
## What changed
Only probe regular files for image dimensions, leaving pasted device and FIFO paths as literal text.
## Testing
Add a Unix regression test that pastes a FIFO path and verifies it remains editable with Backspace.

## Why
A bundled reviewer entry does not establish that a custom gateway offers that model. Guardian should fall back to the parent model when the reviewer is neither advertised by the provider nor explicitly configured.
## What changed
Track model metadata origins as `Bundled`, `Provider`, or `ExplicitConfig`, including across catalog merges and cache reloads. Exclude bundled entries from Guardian reviewer selection unless `auto_review_model_override` is set. Continue selecting reviewers from provider and explicitly configured catalogs.
## Testing
Add coverage for gateway reviewer selection with each catalog origin, plus origin tracking across cache reloads, empty provider responses, and authentication changes.

## Why
Guardian's `write_stdin` approval requests lacked the command used to launch the receiving terminal, leaving reviews without that context.
## What changed
- Carry `original_command` from the terminal's launch command into the approval request and Guardian action JSON. This describes the original launch, not the terminal's current state.
- Exclude `original_command` from the 8,000-byte stdin approval size check so launch context does not consume the budget for input, permission details, and the approval reason. Launch context remains subject to Guardian's normal request budget.
## Testing
Extend serialization coverage to check `original_command` and the strict stdin review integration test to verify that a launch command longer than 8,000 bytes reaches Guardian intact.

## What changed
Decode `response.metadata` Guardian assessments into typed `GuardianAssessment` events over SSE and WebSocket, carrying `response_id`, `item_id`, and `requires_review`. Ignore assessments with missing, empty, or incorrectly typed required fields while preserving independent delivery of other metadata.
Expose the assessment type to request-scoped extension interceptors. Keep assessments out of conversation history; they do not authorize tool execution.
## Testing
Add SSE coverage for assessment ordering, malformed metadata, and mixed metadata delivery. Extend model request tests to cover SSE and WebSocket interceptor delivery and exclusion from subsequent model input.

## What changed
Extract MCP approval elicitation parsing and review-decision response conversion from `core/src/session/mcp.rs` into `codex-guardian-reviewer`. Export the helpers and `GuardianElicitationReview` for the session code to use, while keeping session lookup, provenance checks, and approval enforcement in the host.
## Testing
Move the existing elicitation tests into the reviewer crate, retaining coverage for request construction, opt-in requirements, unsupported forms, decision mapping, and model-specific timeout instructions.

## Why
Runtime fixtures and snapshots should not invalidate integration-test compilation.
## What changed
Limit integration-test `compile_data` to Rust modules and explicitly declared embedded resources, while keeping all test files available as runtime data. Declare the embedded patch fixture in `exec` and PEM fixtures in `http-client` through `integration_compile_data_extra`.

## Why
The short-lived schema exporter sits on the critical path of application builds. Optimizing its full test harness adds unnecessary compilation work.

## What changed
Export `ReviewAction` and its conversion from `GuardianApprovalRequest` from
`codex-guardian-reviewer`. Keep host-specific approval preparation and environment

## Testing
Move the `write_stdin` serialization and tool-call provenance test into the
reviewer crate, preserving coverage for input, Windows working directories, and
sandbox permission modes.
GitOrigin-RevId: 9a1caf9bbb67ba017e4950a2da9ef2543b013007
## Why
Shared routing cookies can change after a parent connection is established. Guardian reviews need the routing affinity captured with their parent response so unrelated traffic does not replace that hint.
## What changed
- Capture `__oailb` and `__cflb` cookies from successful ChatGPT WebSocket handshakes and pair them with the parent response ID.
- Retain the snapshot throughout a review and its retries, and apply it only to Guardian requests with the matching parent response. Reconnect cached reviewer sockets when the parent affinity changes.
- Preserve cookie expiry and restrict reuse to the original secure origin and endpoint path. Replace routing cookies while retaining unrelated cookies, and keep the captured affinity out of prompts, history, and serialized metadata.
## Testing
Add coverage for cookie scope, expiry, duplicate rejection, and merging; replaying a captured pin after shared cookies change or are deleted; and retaining the matching parent snapshot through compaction without exposing cookies in review prompts or metadata.

## What changed
Move `GuardianContextMode`, parent compaction checkpoint policy, and reviewer instruction assembly into `codex-guardian-reviewer`. Export the conversation-history prompt and follow-up reminder from the same crate, and update core to use these shared definitions.
Keep session-specific authorization version lookup in core while preserving existing context selection, checkpoint validation, and prompt behavior.

## Why
`ShutdownComplete` can arrive before detached task cleanup records and exports `codex.turn_profile` events, causing the turn phase trace test to read incomplete profile counts.
## What changed
Poll exported spans until the expected number of profiles is available, with a 10-second timeout, before asserting sampling request and retry counts for reply and interrupt cases.
GitOrigin-RevId: 43cdcdde71bbcd16ed2e6c8b2accaa2a5de03001
## What changed
`review/start` now supports only inline reviews on the supplied thread and returns that thread's id as `reviewThreadId`. Clients can omit `delivery`, send `null`, or specify `"inline"`; `"detached"` is no longer supported.
For a separate review thread, call `thread/start` followed by `review/start`. To preserve conversation history with custom review instructions, use `thread/fork` followed by `turn/start`.
Remove detached review execution and its app-server dependency on `codex-agent-extension`, and update the protocol schemas and generated clients. Preserve `ReviewDelivery.inline` as a Python enum member.
## Testing
Add a Python regression test verifying that inline review delivery parses as `ReviewDelivery.inline` and round-trips to the original JSON.

## Why
Managed OpenTelemetry requirements could not specify span attributes or tracestate metadata.
## What changed
- Accept `otel.span_attributes` and `otel.tracestate` in requirements, merge them by attribute or member field, and expose them through app-server configuration and requirements reads.
- Protect required entries and their ancestors from configuration writes while preserving unmanaged sibling values and their origins. For example, requiring `otel.tracestate.vendor.tenant` leaves `vendor.region` writable. Empty metadata maps impose no requirements.
- Select Rustls when configuring an OTLP HTTP client certificate and private key so PEM client identities are accepted.
## Testing
Add coverage for metadata precedence, effective configuration, field-level write protection, and preservation of unmanaged values. Add blocking and asynchronous HTTP client construction tests using generated PEM certificates and keys.

## Why
When the app server started with tracing disabled, its subscriber lacked the trace bridge needed to export spans after an account switch enabled tracing. Disabling tracing also left the previous global tracer provider installed.
## What changed
- Always install the reloadable trace layer so account switches can enable trace export without restarting the app server.
- Replace the global tracer provider with a no-op provider when trace export is disabled, including when all telemetry exporters are disabled.
## Testing
Add an app-server regression test that starts with tracing disabled, enables a requirements-managed trace exporter on account switch, and verifies export with the parent trace ID preserved. Add a provider unit test verifying that disabling telemetry stops new spans from recording.

## Why
OTEL exporter headers can contain credentials. Configuration responses exposed these values in effective config, source layers, managed requirements, and write-override metadata.
## What changed
Replace header values with `[redacted]` across these responses for log, trace, and metrics exporters while preserving header names. Keep the original values available to runtime exporters, and redact them in debug output. Update protocol schemas and generated client documentation to describe the redacted values.
## Testing
Add coverage for effective config and source-layer redaction, managed requirements, and single and batch write-override metadata, including header names containing dots. Extend telemetry integration coverage to verify that runtime requests still carry the configured header value.
GitOrigin-RevId: ad24e8b91e615bd36c03c4c0ca205ac64bbd8cbf
## What changed
- Allow `requirements.toml` to manage OpenTelemetry logging flags, tool-result byte limits, environment, and log, trace, and metrics exporters.
- Apply higher-priority managed values over user configuration, replacing exporters atomically and warning when configured values conflict. Preserve unmanaged settings, including `span_attributes` and `tracestate`.
- Expose managed `otel` settings through `configRequirements/read`, apply them in `config/read` without ordinary field origins, and reject configuration writes that overlap managed fields.
- Distinguish omitted tool-result limits from explicit values while preserving the runtime default.
## Testing
Add coverage for requirement precedence, relative TLS paths, override warnings, managed write protection, effective configuration reads, and exporter serialization through the app-server API.

## Why
Pre-generated bindings can disagree with native V8 layouts, including CppGC member storage and the inline typed-array size. Source archives need bindings generated with the same headers, defines, and target configuration.
## What changed
- Generate bindings through Bazel using the archive's C++ compilation metadata, and include them in Darwin and Linux release pairs and source builds.
- Select native Windows MSVC archives and matching bindings together with `--//:rusty_v8_from_source=true`, and generate target-specific Windows GNU bindings.
- Pin a corrected `rusty_v8` revision for Cargo and Bazel, and track native Windows LLVM tools and resources as action inputs so pin changes invalidate cached actions.
- Run release and canary Cargo smoke tests with explicit target triples, required Linux linkers, and `--include-ignored`.
## Testing
Add `codex-v8-poc` coverage for CppGC member size, alignment, tracing, and weak references; typed-array contents and backing-store promotion; and microtask queues across garbage collection and context reassignment. Enable the otherwise ignored CppGC ABI checks for Bazel source builds and staged release smoke tests.

## What changed
Add optional `otel` requirements to `ConfigRequirements`, with types for logging controls, tool-result byte limits, environment, and telemetry exporters, including OTLP HTTP/gRPC headers and TLS settings.
Update the JSON schemas, precomputed exports, and generated TypeScript and Python bindings. The app-server requirements mapper currently returns `otel: None`.

## Why
Archiving a thread also archives its spawned descendants, but unarchiving it previously restored only the parent.
## What changed
- Restore archived spawned descendants when handling `thread/unarchive`, including descendants archived independently of the parent. Skip active descendants and continue past descendant restoration failures; fall back to restoring only the parent if descendant discovery fails.
- Emit `thread/unarchived` for each successfully restored thread. Hold the thread-list mutation permit through archive and unarchive responses and notifications to preserve ordering with other list mutations.
- Add `ThreadStore::unarchive_threads` and share rollout discovery across the subtree in the local store.
## Testing
Extend coverage for subtree restoration, independently archived children, descendant restoration failures, and failed descendant discovery. Store tests cover skipping active descendants and restoring archived descendants without hydrating their metadata.

## What changed
- Remove the standalone executor from CLI and app-server packages, release binary lists, and Bazel runtime archives.
- Remove the package builder's `--exec-server-bin` option and `execServer` metadata, along with executor-specific installer checks, signing configuration, and package smoke tests.
- Keep the standalone executor available to build from source.

## Why
Setting `model_provider = "openai"` hid the Amazon Bedrock setup option during onboarding, even though leaving the provider implicit allowed it.
## What changed
Allow the Bedrock setup wizard when the effective provider is `openai`, whether selected explicitly or by default. Preserve the existing feature, authentication, login-method, and embedded app-server requirements.
## Testing
Update onboarding tests to cover explicit OpenAI configuration, continued exclusion of custom providers, and the Bedrock sign-in option with configuration loaded from `config.toml`.

## Why
Legacy notifications only need user text, but retaining the full sampling input keeps image and audio payloads alive while the response streams.
## What changed
Extract notification text from the first request attempt only when `notify` is configured, and retain those strings across retries. Consume and drop the prompt after setting up the response stream, excluding attachments and media wrapper labels from notification text.
## Testing
Extend the notification integration test to verify that `input-messages` contains only user text while the model request still includes image and audio attachments.

## Why
Reloading an evicted sub-agent disabled its rollout tracing, leaving later turns out of the trace. Replacement Code Mode sessions could also reuse cell IDs from earlier sessions.
## What changed
- Reattach reloaded children to the root rollout trace and emit `ThreadResumed` without duplicating `ThreadStarted`.
- Reopen existing threads during replay, preserving their identity and history while clearing stale runtime cell and terminal process mappings.
- Allocate Code Mode cell generations across sessions for both gRPC and stdio hosts to prevent ID reuse after reload.
- Record an aborted trace when session startup fails before the session loop begins.
## Testing
Add regression coverage for child eviction and reload, including a failed reload, continued inference tracing, distinct cell IDs, and preserved mail and follow-up context. Extend replay tests for history preservation and reused runtime handles, and host tests for cell IDs across session replacement and reconnection.

## Why
VTE versions before 0.54 lack the parser that safely ignores unknown OSC sequences. Terminal status reporting needs to account for the VTE backend even when the terminal identifies itself as GNOME Terminal or another application.
## What changed
- Track VTE backend versions independently of terminal application identity.
- Disable `OSC 7501` and `OSC 21337` status reporting when the detected VTE version is below 0.54. Preserve existing behavior for newer or unknown versions.
- Prefer `VTE_VERSION` over the terminal application version, treating empty or malformed values as unknown.
## Testing
Add regression tests for GNOME identity with VTE metadata, version precedence, and status protocol selection around the VTE 0.54 boundary, including unknown versions.

## Why
Model catalog refreshes need to use the same registered actor and credential owner as model requests when runtime authentication is enabled.
## What changed
- Add `ProviderAuthContext` to carry credentials and optional runtime authentication into model providers and catalog clients.
- Select runtime authentication independently for inference and catalog destinations. Reject unsupported OpenAI origins while preserving provider authentication for other destinations.
- Include API organization and project selectors in runtime registration. Reuse registered runtimes across projects within an organization and replace them when the organization changes.
- Preserve transport restrictions and account bindings, and strip sensitive headers on cross-origin redirects.
- Apply the catalog timeout to authentication and fetching while allowing credential refresh and registration to finish safely.
## Testing
Add coverage for runtime-authenticated catalogs, provider isolation, resource selection, gateway authentication, cross-origin redirects, and timeout recovery that preserves registration and refreshed tokens.

## What changed
Limit `--network-only` reports to effective configuration, network environment, provider HTTP reachability, and Responses WebSocket checks, skipping unrelated diagnostics such as database and MCP scans. Preserve configuration warnings and report configuration load timing.
If configuration loading fails, report the redacted configuration error and stop without probing guessed network targets.
## Testing
Add integration tests for check selection, recoverable configuration warnings, and configuration failures that send no probe requests or expose configuration values. Add snapshots for human-readable success and failure reports.

## What changed
- Add `permissions.approved_command_prefix_saved` to model messages and use valid plain-text headings in saved-prefix notices, with or without full permissions instructions. Reject empty, oversized, control-character, and markup-like headings.
- Keep `Approved command prefix saved:` as the leading marker for older clients and recognize saved-prefix content kinds when trimming rolled-back context.
- Shut down analytics test exporters asynchronously and serialize workspace-switching tests that share authentication environment state.
## Testing
Extend coverage for heading validation, sparse wire values, single-notice emission, rollback with and without metadata, and saved-prefix behavior across model switches.

## Why
Repeated repository skill discovery performs remote metadata requests even when no `.agents/skills` directories exist. Reuse verified empty results to avoid those round trips while watching for changes that require a new scan.
## What changed
- Add the Linux executor capability `fs_watch_missing`, the `fs/watchMissing` request, and `fs/missingChanged` notifications.
- Register watches in the background and reuse empty results only after the executor acknowledges coverage. Key reuse by filesystem, working directory, and project root markers.
- Invalidate observations on relevant filesystem or mount changes, watch supersession, or connection loss. Clear cached observations on forced reloads and cache resets.
- Retain live discovery while registration is pending or watching is unsupported, unavailable, or cannot verify the observation.
## Testing
Add tests for empty-result reuse and invalidation, skill directory creation before watch acknowledgement, unsupported executors, idle WebSocket notifications, reconnection, case aliases, and bind mounts and unmounts. Include an ignored manual latency comparison for repeated empty remote scans.

## Why
Handshake probes can lose upgrade evidence when a subsequent read fails, and diagnostic output can expose URLs, server error messages, and close reasons containing sensitive values.
## What changed
- Retain successful upgrade status and allowlisted tracking headers, including on post-upgrade read failures.
- Return structured probe errors that distinguish typed TLS failures from failures with an unknown stage while preserving the underlying API error.
- Recognize empty close frames as immediate closes with code `1005`.
- Redact wire values from probe debug output and doctor error details, and remove URLs and raw errors from connection logs.
## Testing
Add local TLS and WebSocket fixtures covering certificate rejection, ordinary I/O failures, successful and rejected upgrades, immediate closes, malformed frames, header retention, and redaction. Add doctor output snapshots and error-detail redaction assertions.

## What changed
- Build and bundle the standalone executor in `bin/` for both package variants, and advertise its path through `execServer` in `codex-package.json`.
- Include the executor in Cargo and Bazel release packaging, macOS signing and notarization, and Windows signing verification. Preserve prebuilt signed binaries during packaging.
- Make installers detect a missing advertised executor while continuing to accept older packages without it.
## Testing
Add coverage for executor archive contents and executable permissions, signed binary preservation, and macOS entitlement verification. Add package smoke tests for initialization, version reporting, filesystem and process sandbox enforcement, and startup error parity with `codex exec-server`.
