4c89139da9 Add CIMD support to MCP OAuth registration (#38089)
## What changed

- Make automatic MCP OAuth registration prefer Client ID Metadata Documents
(CIMD) when the authorization server advertises support for public clients and
Codex is using its native loopback callback. Fall back to advertised Dynamic
  Client Registration (DCR) otherwise.
- Add explicit `cimd` and `dcr` registration overrides to the CLI and app-server
OAuth login API. Validate CIMD metadata and callback URLs before starting the
  authorization flow.
- Use a callback-specific Codex client metadata URL for CIMD and retain the
exact redirect URI through authorization and token exchange.

## Testing

- Cover automatic and forced CIMD selection, DCR fallback, invalid metadata and
redirects, token refresh, authenticated MCP requests, and conformance
  regression checks.

67afc79674 Use ReviewDecision for MCP tool approvals (#38081)
## What changed

- Add `ApprovedMcpPolicyAmendment` to represent MCP approvals that persist across sessions.
- Route MCP approval responses directly through the shared `ReviewDecision` type while preserving session-only approvals, rejection reasons, timeouts, and cancellations.
- Reject the MCP-only policy decision if it reaches command, shell, network, or other tool approval paths.

## Testing

- Update MCP approval parsing, normalization, hook, and Guardian tests for the shared decisions.
- Verify generic tool approvals reject MCP policy amendments.

3a6f747d77 Preserve harness metadata across conversation history (#38058)
## What changed

- Wrap response items with optional harness-owned metadata while keeping the persisted response payload shape backward compatible.
- Store compacted-history metadata in an aligned sidecar and reject malformed sidecars.
- Carry metadata through history normalization, compaction, resume, fork, truncation, migration, and rollout recording without sending it to model providers.

## Testing

- Cover legacy rollout compatibility, metadata serialization and validation, compaction retention, and resume/fork reconstruction.

6dc3ac8721 Add per-login MCP OAuth client registration selection (#38052)
## What changed

- Add `--oauth-client-registration` with `auto` and `dcr` choices to `codex mcp add` and `codex mcp login`.
- Add the equivalent optional `clientRegistration` field to `mcpServer/oauth/login` and the generated protocol schemas.
- Keep automatic dynamic client registration as the default, prefer a configured OAuth client ID when present, and apply the selection only to the current login without persisting it in MCP configuration.

## Testing

- Cover dynamic registration and configured-client OAuth flows through the app server.
- Verify the CLI does not persist the per-login registration choice.

edcec13372 Expose image generation usage-limit failures (#38024)
## What changed

- Add structured `usageLimitExceeded` metadata to failed image-generation
items, including the image limit ID and optional reset timestamp.
- Preserve the failure metadata in completion events, app-server thread
history, thread reads and resumes, and migrated legacy rollouts.
- Export the new failure type in the generated JSON and TypeScript schemas.

## Testing

- Cover usage-limit responses through item completion, persistence, and thread
  resume.
- Verify legacy rollout migration retains image-generation failure metadata.

1549756b78 Add appearance metadata to thread sections (#37898)
## What changed

- Add optional `icon` and `color` appearance fields to custom thread sections and expose them through the app-server protocol.
- Persist appearance metadata in SQLite and include it when listing sections or returning threads.
- Let `threadSection/update` preserve an omitted appearance, clear it with `null`, or replace it with a new value.
- Reject appearance fields larger than 64 bytes.

## Testing

- Cover protocol compatibility, persistence across restart, update and clear behavior, and field-length validation.

63002bdb26 Extract persisted history types into a dedicated crate (#37871)
## What changed

- Add `codex-history` for model-history and persisted-rollout domain types, including `RolloutItem`, `RolloutLine`, `CompactedItem`, and initial/resumed history state.
- Re-export the persisted types from `codex-rollout` and update consumers to use the new crate boundary instead of `codex-protocol`.
- Preserve existing rollout serialization, including legacy numeric compacted-window IDs.

## Testing

- Add `codex-history` tests for rollout JSON round trips, compacted-history compatibility, persisted history modes, and multi-agent version selection.

34ecac1f2b Support packaged defaults in config layering (#37810)
## What changed

- Load an optional package-supplied config file as the lowest-precedence
configuration layer, so every existing layer can override its values.
- Report the layer as `packagedDefaults`, including its source path, through
config diagnostics and the app-server protocol.
- Return an error when a configured packaged defaults file is missing.

## Testing

- Cover precedence across packaged, system, user, and session layers.
- Cover the missing-file error path.

89a335ed50 Forward install attempt IDs for remote plugins (#37773)
## Why

Clients need to correlate a remote plugin installation request with a specific
installation attempt.

## What changed

- Add the optional `installAttemptId` field to `PluginInstallParams`.
- Forward the value as `install_attempt_id` in the remote plugin install POST
  body.
- Preserve the existing empty request body when the field is omitted.

## Testing

- Cover protocol serialization and remote install request bodies with and
  without an attempt ID.

f65ea998c7 Keep external agent detection from blocking config requests (#37528)
## Why

External agent detection can spend time reading session transcripts. Sharing the
configuration serialization queue causes unrelated configuration requests to
wait for detection to finish.

## What changed

- Give `externalAgentConfig/detect` its own global serialization queue.
- Keep configuration reads, requirements reads, writes, and external agent
imports responsive while detection is still running.

## Testing

Add a Unix app-server test that blocks transcript detection on a FIFO and
verifies the unrelated requests complete before detection is released.

208f05b233 Enforce automatic review for managed models (#37511)
## What changed

- Add the managed `auto_review.required_on_models` requirement, unioning model slugs across requirement layers and exposing the result through `configRequirements/read`.
- Force listed models to use `on-request` approvals with the `auto_review` reviewer. Downgrade Full Access to workspace-write when starting a protected session, and reject incompatible runtime overrides or use when automatic review is disabled.
- Preserve these protections across thread starts, resumes, forks, model changes, turn overrides, MCP approvals, and TUI session attachment.

## Testing

- Add coverage for layered model requirements and exact provider-alias matching.
- Add app-server tests for protected thread lifecycle operations, unsafe setting overrides, legacy thread upgrades, disabled automatic review, and requirements API output.

928bda82cf Preserve image transparency metadata in app-server items (#37178)
## What changed

- Add `transparentBackground` to image-generation items and legacy completion events.
- Map Images API backgrounds to `true` for transparent, `false` for opaque, and `null` for automatic or unavailable values.
- Preserve the metadata in live notifications and persisted or resumed thread history while continuing to deserialize older items that omit it.

## Testing

757c151a0e Add safer TUI defaults for cyber models (#37055)
## What changed

- Propagate the optional `modelSpecialty` field through the model catalog and
  app-server `model/list` response.
- When a newly selected model has the `cyber` specialty, default an active TUI
thread to workspace-write permissions with on-request approval. Use auto
review when available, otherwise keep the user as reviewer, while respecting
  configured permission requirements.
- Show a notice when auto review is applied and strengthen the full-access
  warning for cyber models.
- Preserve explicitly selected permissions when only reasoning settings change.

## Testing

- Cover auto-review defaults, requirement-aware fallback behavior, permission
preservation across reasoning changes, and the new warning notice.

ee46c5ba0e Negotiate MCP extensions per app-server session (#36910)
## Why

App-server clients need to advertise structured MCP extension settings, including supported MCP App UI MIME types, rather than only opting into OpenAI form elicitation with a boolean.

## What changed

- Add an `extensions` map to initialize capabilities and preserve `mcpServerOpenaiFormElicitation` as a legacy alias for `openai/form`.
- Capture the declared extension profile when a thread is started, resumed, or forked, propagate it to subagents, and advertise it to downstream MCP servers during initialization.
- Keep the profile stable for the lifetime of the loaded session instead of changing it on later turns or direct tool calls.

## Testing

- Cover extension conversion, downstream MCP initialization, session isolation, legacy form support, and subagent inheritance.

fcf636a41d Redact secrets from app-server command execution items (#36893)
## Why

Client-facing command execution items can include recognizable secrets in the
rendered command or parsed command actions.

## What changed

- Redact secrets in `commandExecution.command` and `commandActions` for live
items, completed items, and replayed thread history.
- Keep command approval requests backed by the original executable command and
parsed actions while using the redacted representation for display items.
- Document that execution item commands are redacted display values rather than
  executable commands.

## Testing

- Cover redaction for command conversion, parsed search actions, legacy replay,
approval flows, completion notifications, and rejected commands.

5157493c23 Expose onboarding hints in login completion notifications (#36635)
## What changed

- Accept the allowlisted `.onboarding_entrypoint=life_sciences` suffix on a valid OAuth state while continuing to reject unknown or malformed suffixes.
- Return the parsed callback metadata from the login server without including the suffix in the token exchange redirect URI.
- Add the optional `onboardingEntrypoint` field and `life_sciences` value to the v2 `account/login/completed` notification and generated protocol exports.

## Testing

- Cover valid, invalid, and repeated OAuth state suffixes.
- Verify the end-to-end login callback result, token redirect URI, and app-server notification payload.

6751b54cae Add a realtime delegation acknowledgement control (#36413)
## What changed

- Add the optional `delegationAckFiller` field to `thread/realtime/start`.
- Forward explicit `true` or `false` values to V3 Frameless Bidi session payloads as `delegation.ack_filler`; leave the field absent when unspecified so the Realtime API default is preserved.
- Document that V1 and V2 ignore this setting.

## Testing

e2c0837923 Make user input blocking behavior explicit (#36410)
## Why

Clients need to know whether a `request_user_input` request must wait for an
explicit response or may auto-resolve. Using `autoResolutionMs` as that signal
conflated the blocking decision with timeout policy.

## What changed

- Add required `isBlocking` fields to user input protocol and app-server
request payloads. Plan-mode requests are blocking, while requests from other
  enabled modes are non-blocking.
- Drive the TUI's auto-resolution behavior from `isBlocking` and remove
`autoResolutionMs` from the model-facing tool schema.
- Deprecate `autoResolutionMs` while retaining it for compatibility, and treat
legacy payloads without `isBlocking` as blocking.

## Testing

- Cover mode-derived blocking behavior, legacy deserialization, app-server
forwarding, delegated requests, and TUI auto-resolution.

a01a2d9146 Preserve executor paths in read command actions (#36223)
## Why

Read command actions were omitted when a selected environment used a path
convention foreign to the app-server host. Clients need these actions to refer
to files in the executor's filesystem, not paths interpreted on the host.

## What changed

- Resolve read paths against the executor's `PathUri` and serialize them using
the executor's native POSIX, Windows drive, or UNC convention.
- Accept same-drive Windows-relative paths while continuing to reject paths
that require another drive's executor-local current directory.
- Document the path convention used by `commandExecution` notifications.

## Testing

Cover native and cross-platform path serialization, Windows drive-relative
joins, and selected-environment `item/started` and `item/completed`
notifications.

acd540f158 Precompute app-server protocol exports (#36212)
## Why

Normal app-server protocol builds do not need the `ts-rs` and `schemars`

## What changed

- Embed compressed stable and experimental TypeScript and JSON schema exports,
and serve the existing export APIs from those artifacts.
- Use no-op schema derives in non-test builds while keeping the real generators
available for fixture regeneration and tests.
- Move schema fixture regeneration behind a Python helper and update

7ec480dda5 Distinguish unknown MCP authentication status (#36045)
## Why

OAuth discovery failures do not establish that an MCP server lacks OAuth
support. Reporting those failures as `unsupported` conflates an inconclusive
check with a confirmed result.

## What changed

- Add an `unknown` MCP authentication status across the protocol, app server,
  CLI, and TUI.
- Preserve OAuth discovery errors so callers can report `unknown`, while
retaining `unsupported` for servers known not to support OAuth.
- Document the distinction in the app server API.

## Testing

- Verify transient HTTP discovery errors are preserved.
- Verify `codex mcp list --json` reports `unknown` when discovery is rate
  limited.

9f23e97797 Include session titles in external agent import history (#35870)
## What changed

- Preserve the original title when importing an external-agent session and return it with successful session entries from `externalAgentConfig/import/readHistories`.
- Add dedicated history-record success types so externally completed imports can optionally supply a session title without changing the live import result type.

## Testing

- Verify imported session titles are returned by the app server and persisted when session import ledger entries are created or refreshed.

28f3f1f9ef Expose plugin installation timestamps in app-server summaries (#35859)
## What changed

- Add nullable `installedAt` metadata to `PluginSummary` and its generated schemas, expressed as a Unix timestamp in seconds.
- Preserve the backend installation time for remote plugins across plugin list, installed, read, and share-list responses. Return `null` when the timestamp is unavailable, including for local or uninstalled plugins and older backend responses.

## Testing

- Cover timestamp conversion in the remote plugin list integration test and the field's serialization and backward-compatible default in protocol tests.

101d6b8cb2 Preserve foreign paths in background terminal listings (#35850)
## Why

Background terminal working directories can use a path convention from a
different platform than the app-server host. Converting them to host-native
absolute paths can reject otherwise valid terminal entries and fail the list
request.

## What changed

- Represent `ThreadBackgroundTerminal.cwd` with `LegacyAppPathString` at the
  protocol boundary.
- Convert terminal `PathUri` values directly to their inferred native path
strings instead of validating them against the host platform.

## Testing

Add protocol round-trip coverage for POSIX, Windows drive, and Windows UNC
working directories.

12b961d4c5 Expose plugin eligibility metadata in app-server summaries (#35837)
## What changed

- Add nullable `disabledReason` and `eligiblePlanTypes` fields to v2
  `PluginSummary` responses and generated schemas.
- Preserve the remote catalog values across discovered, installed, and cached
plugin summary paths while returning `null` for local plugins and older
  remote responses.
- Treat unrecognized disabled reasons as `unknown` for forward compatibility.

## Testing

- Cover protocol round trips, remote summary propagation, unknown disabled
reasons, and app-server responses for admin-disabled and plan-ineligible
  plugins.

85c6da1c79 Add persisted sections for organizing threads (#35722)
## What changed

- Replace the `isPinned` thread metadata and filters with an optional persisted
  `section` and `sectionId`.
- Add the paginated `threadSection/list` app-server method so clients can
discover sections even when they contain no threads.
- Seed a stable `Pinned` section, validate section assignments, and support
filtering for a specific section or for unsectioned threads.

## Testing

- Cover section protocol serialization, listing and pagination, metadata
updates, filtering, persistence, migration compatibility, and operation
  without SQLite state.

61de0d8fe8 Upgrade rmcp to 3.0.0-beta.3 (#35720)
## What changed

- Update the `rmcp` model and transport integrations for `3.0.0-beta.3`.
- Preserve legacy `elicitation/create` form requests, schema defaults, metadata, and wire-format compatibility while adopting the new elicitation types.
- Harden OAuth discovery by using GET-first discovery without starting an MCP session, preventing cross-origin header redirects, validating authorization-server and callback issuers, and retaining compatibility with metadata that omits an issuer.
- Keep stored OAuth credentials readable and avoid requiring reauthorization after transient refresh failures.

## Testing

- Add coverage for legacy elicitation round trips, OAuth discovery variants, callback issuer validation, credential migration, and refresh failures.

8d6a91b1ff Preserve item timestamps in thread history projections (#35689)
## What changed

- Add optional start and completion timestamps to `ThreadHistoryItemChange`.
- Populate them from canonical `ItemCompleted` records during stateless thread history projection.
- Treat a zero completion timestamp as absent, and leave timestamps unset for legacy builder events that do not provide them.

## Testing

- Cover projected item timestamps, including missing start times and zero completion times.

9ca6df68da Preserve terminal turn errors in replayed history (#35524)
## Why

When rebuilding a thread from rollout events, errors embedded in turn completion
events were ignored. Failed retries could therefore be restored as completed
turns, causing warnings such as model-overload errors to disappear from the TUI
transcript.

## What changed

- Mark turns with terminal completion errors as failed and retain their message
  and structured error information.
- Apply late completion errors to their matching historical turn without
  interrupting the active turn.

## Testing

Added history-builder coverage for current and late completion errors, plus a
TUI replay snapshot covering repeated overload failures.

af7f6f4d34 Include item start times in completion events (#35363)
## What changed

- Add an optional `started_at_ms` field to `ItemCompletedEvent`, retaining
compatibility with older persisted events that lack it.
- Track the first start timestamp for each in-flight item and attach it when
emitting and persisting the completion event. If no start was recorded, use
the completion timestamp as a fallback.
- Emit a complete start/completion lifecycle for subagent activity items.

## Testing

- Cover concurrent item timing, repeated starts, turn-boundary cleanup, the
missing-start fallback, subagent activity, and persisted web-search events.

## What changed

- Add nullable `canPublishToWorkspace` metadata to plugin share contexts and `plugin/share/save` responses.
- Preserve the remote capability through catalog parsing and app-server responses so clients can decide whether to offer workspace-directory publishing.
- Document that clients should fail closed when the capability is unavailable.

## Testing

- Cover remote catalog parsing, share-save response propagation, and protocol serialization.

ce803c45ae Record externally completed agent config imports (#34981)
## Why

Clients that complete an external agent config import outside the app server need a way to include the result in import history.

## What changed

- Add `externalAgentConfig/import/recordHistory` to persist completed item results and return a generated import ID.
- Store the optional provider ID for regular imports and expose it when reading import history.
- Update the generated JSON and TypeScript protocol schemas for the new request, response, and history field.

## Testing

- Cover recording and reading an externally completed import through the app-server protocol.
- Cover provider ID persistence and updates in the state runtime.
