Matthew Zeng 60272096bc Enrich app/read connector metadata (#34417)
## What changed

- Add `iconUrlDark`, `distributionChannel`, `installUrl`, and
`pluginDisplayNames` to the experimental `app/read` connector metadata.
- Populate plugin display names from enabled plugins without starting their MCP
  servers.
- Accept both dark-icon field spellings from the batch API and keep the new
upstream fields optional for compatibility with older responses.

## Testing

- Cover the serialized response fields, legacy batch responses, and plugin
  display-name lookup without MCP startup.

jif 687f05cb94 Remove CSV-backed agent jobs (#34413)
## What changed

- Remove the `spawn_agents_on_csv` and `report_agent_job_result` tools and their agent-job runtime and state models.
- Drop the legacy `agent_jobs` and `agent_job_items` tables during state database migration.
- Keep `features.enable_fanout` and `agents.job_max_runtime_seconds` accepted as no-op compatibility settings while omitting them from the generated configuration schema.

## Testing

- Verify upgrades remove both legacy agent-job tables.
- Verify the removed feature and configuration keys still parse without taking effect.

stevenlee-oai 19940967bd Support threadless MCP connections without event channels (#34408)
## What changed

- Make the MCP connection manager's event sender optional.
- Skip startup notifications when a caller has no session event stream.
- Decline interactive MCP elicitations in that mode while allowing the
  underlying operation to continue.
- Update threadless resource reads, status snapshots, and connector discovery
  to use the channel-free path.

## Testing

- Extend the app-server MCP resource test to verify that a threadless resource
read declines an elicitation and still returns the resource contents.

Dylan Hurd e52c35b000 Propagate approval rejection reasons (#34400)
## What changed

- Change `ReviewDecision::Denied` to carry a rejection string and update the generated protocol schemas.
- Preserve specific rejection reasons through command, patch, network, MCP, delegated, and automatic approval flows so tool results can return them to the model.
- Distinguish invalid approval responses from user declines and truncate oversized rejection messages before adding them to model context.

## Testing

- Add coverage for denial serialization, invalid app-server approval responses, dropped automatic reviews, network rejection propagation, and rejection-message truncation.

Andrei Eternal e4836f998d Add configurable hook context spill limits (#34393)
## What changed

- Add `additionalContextLimit` to command hook configuration for events that can emit `additionalContext`.
- Apply the limit independently to each hook's context before it is sent to the model. Unset values retain the 2,500-token default, while `0` disables spilling for that hook.
- Expose configured limits through app-server hook and requirements responses, and show them in the TUI hooks browser.

## Testing

- Cover JSON and TOML parsing, discovery and hashing, per-hook spilling behavior, app-server responses, and TUI rendering.

nhamidi-oai 6f785632b0 Preserve audio across history and tool outputs (#34385)
## Why

Audio attachments could be dropped from tool outputs and reconstructed thread history, while inline base64 payload size could overstate their model context cost.

## What changed

- Preserve remote and local audio attachments in legacy user-message events and thread history.
- Keep audio from function and custom tool outputs when the model supports it and the output fits the truncation budget; otherwise replace it with an omission marker.
- Estimate inline audio cost from its duration, with a serialized-size fallback for malformed or unsupported data URLs, so compaction and truncation use model-visible cost instead of raw base64 size.
- Replace audio in messages and tool outputs with an explanatory placeholder for models without audio input support.

## Testing

Add coverage for history round trips, dynamic tool responses, modality filtering, duration-based context estimates, output-budget truncation, and remote compaction.

Won Park 7e51abbbd1 Avoid rendering generated images twice (#34378)
## What changed

- Tell the model that generated images are already displayed to the user and should not be repeated as Markdown images or file links.
- Keep generated-image artifact paths and output hints owned by the image generation extension.
- Centralize the availability check for the image generation tool across feature, provider, namespace-tool, model modality, and authentication requirements.

## Testing

- Verify artifact paths sanitize session and call identifiers.
- Verify the model-facing output hint reports that the generated image is already displayed.

Won Park bf3c1972b7 Migrate legacy exec policy allow rules (#34271)
## What changed

- On session startup, remove exact `allow` entries from `rules/default.rules`
for command prefixes that Codex no longer suggests as policy amendments.
- Record the migration in `.sandbox_migration` so it runs only once, preserving
  rules created after the migration.
- Skip the migration when user and project exec policy rules are ignored.
- Expand the protected prefix list across shells, interpreters, package runners,
  and destructive or privilege-related commands.

## Testing

- Cover selective removal, case-insensitive matching, one-time behavior, and the
startup path with ignored policy rules.

Brent Traut 5a208c1fc3 Persist names for paginated threads (#34229)
## Why

Paginated threads need explicit user-facing names to remain distinct from
derived titles and previews without requiring rollout metadata writes.

## What changed

- Add a nullable `name` column to persisted thread metadata and use it as the
  canonical name for paginated threads.
- Route paginated name updates through the state database while retaining the
name index as best-effort compatibility data and preserving legacy thread
  behavior.
- Return persisted names when reading, listing, searching, and resuming
paginated threads, and include them in state-database search matching.

## Testing

- Cover paginated name updates, reads, list/search results, and metadata-only
resume behavior, including stale or unavailable compatibility data.

Owen Lin 0fb559f0f6 Support legacy views for paginated thread history (#34085)
## Why

Clients that use full-history resume or request full items in turn pages should
work consistently across legacy and paginated threads.

## What changed

- Materialize complete turns and items from the paginated projection for
  `thread/resume` and `thread/turns/list` when requested.
- Support `initialTurnsPage` during paginated resume, including merging a live
active turn while preserving the requested page size and cursors.
- Keep metadata-only resume behavior and backwards cursors available through
  `excludeTurns: true`.

## Testing

- Extend app-server coverage for full paginated history, full item views, and
initial resume pages on running threads.

nhamidi-oai 643de86a19 Add audio output support to dynamic tools and code mode (#34080)
## What changed

- Add `inputAudio` content items to dynamic tool responses, app-server events, thread history, and generated protocol schemas.
- Add an `audio()` code-mode helper that accepts inline data URLs, audio URL objects, and MCP audio blocks.
- Convert MCP audio blocks into model input when audio is supported, and replace unsupported audio with an explanatory text item.
- Reject non-data audio URLs and track audio item counts in dynamic tool analytics.

## Testing

- Cover audio serialization, protocol round trips, thread-history conversion, MCP modality filtering, code-mode helper inputs, and invalid URL handling.

## What changed

- Add an optional `initialItems` field to `thread/realtime/start` for seeding
realtime V3 session history with `user`, `developer`, and `assistant` text.
- Encode the items in Frameless Bidi session bootstrap payloads for websocket
and WebRTC transports while preserving existing payloads when the list is
  omitted or empty.
- Reject non-empty initial items for V1 and V2, more than 128 items, any item
over 8,192 estimated tokens, or a combined total over 8,192 estimated tokens.

## Testing

- Cover protocol serialization, app-server forwarding, websocket and WebRTC
payloads, empty-list compatibility, version checks, and size limits.

## What changed

- Add `HistoryPosition` to identify a source thread and an exclusive rollout
prefix by ordinal and JSONL byte offset.
- Add optional `history_base` metadata to `SessionMeta` so a thread can record
an inherited prefix of another paginated rollout.
- Default missing `history_base` metadata to `None` and omit it when unset to
preserve compatibility with existing rollout metadata.

## Testing

jiayuhuang-openai 025db22058 Route realtime V3 handoffs by response channel (#33903)
## What changed

- Add `codexResponseHandoffMode` to `thread/realtime/start` for V3 sessions, with `thinking` as the default and `commentary` and `bemTags` routing modes.
- Map BEM `analysis` and `commentary` output to the commentary channel and `final` output to the speakable channel, while preserving the original BEM envelope. Unrecognized output falls back to speakable.
- Mark explicit speech appends as speakable and stop adding the legacy `"Agent Final Message"` prefix to V3 handoffs. V1 and V2 continue to ignore the new setting.

## Testing

- Cover channel selection for every routing mode, streamed BEM header parsing, fallback behavior, explicit speech, and V1 compatibility.

Abhinav 7bd44085e1 Add SessionEnd hooks for thread teardown (#33895)
## What changed

- Add the `SessionEnd` hook event to hook configuration, protocol schemas, analytics, and the hooks browser.
- Run matching hooks for root threads during shutdown, including app-server archive, delete, idle unload, and graceful shutdown. Flush the transcript first and provide the session ID, transcript path, working directory, and `reason: "other"`.
- Keep teardown bounded: hook output is advisory, the default timeout is one second, configured timeouts are capped at three seconds, and async hooks run synchronously with a warning.

## Testing

- Cover transcript availability, matcher selection, timeout normalization, ignored control output, subagent exclusion, archive/delete ordering, and graceful shutdown of multiple loaded threads.

## What changed

- Stream V3 agent message deltas to `delegation.context.append` at 200 ms intervals instead of waiting for each completed message.
- Send commentary without a prefix and label final or phase-less V1/V3 output with `"Agent Final Message":`.
- Bound streamed output while preserving both its beginning and final tail when truncation is required.
- Remove `codexResponseHandoffPrefix` from `thread/realtime/start`; unknown fields from older clients remain ignored.

## Testing

Bryan Ashley 08e30a2e4e Add batched executor capability discovery (#33852)
## Why

Selected capability roots can contribute plugins, MCP servers, connectors, and
skills. Discovering each contribution separately requires repeated access to the
executor filesystem.

## What changed

- Add the `capabilityRoots/discoverV1` exec-server RPC to scan selected roots and
materialize recognized plugin manifests, configuration files, skill
instructions, and skill metadata in one bounded request.
- Add the opt-in `executor_capability_discovery` feature, with a thread-scoped
cache and per-step snapshot shared by MCP and skill discovery.
- Parse MCP, connector, and skill contributions from the materialized snapshot,
including serving cached skill instructions without another filesystem read.

## Testing

- Cover discovery limits, manifest precedence, root-local failures, cache reuse,
plugin contributions, and parity with the existing environment skill loader.

jif 0f2321f244 Refresh host skill catalogs through world state (#33425)
## Why

Host skills can change after a thread starts. Subsequent turns need the updated
catalog without reinjecting an unchanged catalog on every turn.

## What changed

- Register the host skill provider in app-server and project its catalog through
  a dedicated World State section.
- Render host skills with the existing metadata budget and usage instructions,
  including skill-root aliases when available.
- Suppress the legacy host catalog and contextual fragment when World State owns
the catalog, while preserving host skills for explicit and shadow selection.

## Testing

Added an app-server test that installs a host skill at runtime and verifies the
catalog updates once and remains stable on the following turn.

charlesgong-openai 58e4ed17a0 Expose connector candidates from imported sessions (#33414)
## What changed

- Correlate MCP server attribution in imported session records with connector
metadata, and persist normalized connector names in the session import ledger.
- Extend `externalAgentConfig/import/readHistories` with connector candidates,
including each connector's display name, imported-session count, and detection
  source.

## Testing

- Cover connector detection through the session import and history-read flow.
- Verify candidate aggregation uses the latest import for each source session.

Owen Lin da61f7d8e1 Enable paginated thread history in app-server (#33364)
## What changed

- Allow `thread/start` to select experimental `historyMode: "paginated"` when the thread store supports turn and item pagination.
- Resume paginated threads with bounded model context and require `excludeTurns: true` instead of loading full history.
- Return `turnsBackwardsCursor` and `itemsBackwardsCursor` from `thread/resume` so clients can hydrate durable history through `thread/turns/list` and `thread/items/list` while newer records arrive through live notifications.
- Reject operations that still require full history, including `initialTurnsPage`, `thread/read` with `includeTurns: true`, `thread/rollback`, and detached review.
- Make item cursors independent of the optional `turnId` filter.

## Testing

- Cover paginated thread start, cold and live resume, backwards cursor hydration, metadata reads, unsupported operations, and stores without pagination support.

jif 1bbdb32789 Expand MCP tool catalog cache regression coverage (#33308)
## Why

Cached MCP tool catalogs must not substitute stale session state for the live
server connection.

## What changed

- Verify `mcpServerStatus/list` waits for replacement stdio server metadata
before combining it with cached tools.
- Verify configurations with remotely sourced environment variables bypass the
  tool catalog cache.
- Assert that calls made from cached definitions return output from the live
  replacement process.
- Make the stdio test server available to Windows Bazel integration tests.

jiayuhuang-openai 2e1607ee2f Add Frameless Bidi support for realtime conversations (#33261)
## What changed

- Add realtime conversation version `v3`, which preserves the V1 Codex Voice behavior while using Frameless Bidi `delegation.*` events.
- Translate audio, transcripts, handoffs, session context, and lifecycle events between the app server and the Frameless Bidi wire protocol.
- Support `v3` over WebSocket and WebRTC, including the Frameless `/live` endpoint, session configuration, headers, and default model selection.
- Update the app-server protocol schemas and documentation for the new version.

## Testing

- Add unit coverage for Frameless event parsing, outbound messages, context chunking, URL construction, and call creation.
- Add app-server end-to-end coverage for WebSocket delegation and WebRTC session startup.

sayan-oai 3afbd8dd45 Report selected environment connection transitions (#33251)
## What changed

- Track connected and disconnected states across initial remote exec-server connections and reconnection attempts.
- Emit experimental `thread/environment/connected` and `thread/environment/disconnected` app-server notifications for each thread selecting the environment. Each payload identifies the thread and environment; current state is not replayed when a thread starts.
- Stop forwarding connection events when an environment selection is removed or replaced.

## Testing

- Cover connection, disconnection, reconnection, shared-environment notifications, and replacement of a selected environment.

Felipe Coury d88db19144 Preserve thread context when retrying or editing turns (#33211)
## What changed

- Add experimental `beforeTurnId` support to `thread/fork` for copying history strictly before a turn, including an in-progress turn.
- Use before-turn forks for TUI backtracking and safety retries so the replacement prompt retains thread context instead of recreating the branch from the preceding turn or a new thread.
- Add experimental `deferGoalContinuation` support so safety-retry forks can inherit an active goal while waiting for the replacement turn before automatic continuation resumes.
- Hide duplicated nested-review prompts when replaying history or selecting a prompt to edit.

## Testing

- Cover before-turn truncation, fork validation, active-goal inheritance, TUI safety retries, backtracking, and nested-review replay.

stefanstokic-oai 4df8027a97 Honor workspace spend controls in rate-limit handling (#33187)
## Why

Sparse and out-of-order rate-limit updates can omit account metadata or let an
older full read overwrite a newer workspace hard stop. Credit availability also
does not always include a displayable balance.

## What changed

- Propagate the backend's `spendControlReached` state through rate-limit
snapshots and the app-server protocol, preserving it across sparse updates.
- Keep workspace hard-stop classifications for error routing and ignore stale
  full-read results after a newer hard-stop notification.
- Base TUI warnings and model-switch prompts on credit availability and hard-stop
state, and show `Credits: Available` when a finite balance is hidden.

## Testing

Added coverage for spend-control mapping, snapshot merging, out-of-order reads,
workspace error routing, warning and prompt behavior, and credit status display.

Celia Chen 6e215e0db5 Handle Amazon Bedrock credentials during logout (#33175)
## Why

Amazon Bedrock can use either a Codex-managed API key or credentials managed by
AWS, so logout must not remove or misrepresent credentials that Codex does not
control.

## What changed

- Remove the managed Bedrock key on logout and clear `model_provider` only when
its user-config value is still `amazon-bedrock`.
- Reject logout for AWS-managed Bedrock credentials without changing existing
  authentication or configuration.
- Add a `test-logout` app-server test-client command that waits for the
  resulting `account/updated` notification.

## Testing

Add coverage for managed and AWS-managed Bedrock logout, concurrent provider
changes, config reload failures, and conditional user-config cleanup.

Celia Chen 518085387b Support Amazon Bedrock login in the app server (#33170)
## What changed

- Handle experimental `account/login/start` requests with `type: "amazonBedrock"`.
- Validate the API key and Mantle region, persist the managed credential, select the `amazon-bedrock` model provider, and emit the standard login and account-update notifications.
- Reload configuration for account and authentication status reads so the newly selected provider is visible immediately, with the startup configuration as a fallback.
- Extend the app-server test client with an Amazon Bedrock login mode that redacts API keys from request logs.

## Testing

- Add app-server coverage for successful login, credential and provider persistence, notifications, refreshed account state, invalid inputs, conflicting configuration, forced ChatGPT login, external authentication, and configuration reload failure.

pakrym-oai 5d325ba223 Move sleep items to the extension-owned lifecycle path (#33159)
## What changed

- Define `SleepItem` in `codex-extension-items` and carry `clock.sleep` events through `TurnItem::Extension`.
- Preserve sleep persistence, thread-history reconstruction, and the app-server's `type: "sleep"` schema while reusing the extension-owned type.

## Testing

- Add serialization coverage for the `clock.sleep` extension wire shape and its protocol wrapper.
- Update sleep lifecycle and thread-history tests to exercise the extension-owned item.

jif 83a4187837 Run detached reviews as review-agent turns (#33156)
## Why

Detached reviews should behave like ordinary forked turns so clients get the normal steering, tool, permission, and item-stream behavior.

## What changed

- Add a bundled `$review-agent` skill with read-only, defect-first review guidance.
- Start detached reviews through `AgentRunner` with a target-specific prompt that explicitly references the bundled skill. Keep inline reviews on the existing review-mode flow.
- Continue emitting the detached review thread and turn identifiers through the app-server API.

## Testing

- Update the detached review integration test to verify the forked turn prompt, ordinary turn completion, and selection of the bundled skill when a user skill has the same name.

Owen Lin e0a3641e6c Support paginated thread history in app-server list APIs (#33152)
## Why

Clients could discover threads with paginated history, but could not page their
turns through `thread/turns/list`.

## What changed

- Route `thread/turns/list` for paginated threads through the thread store,
preserving cursors, sort direction, turn status, timing, errors, and the
requested `summary` or `notLoaded` item view.
- Reject the `full` turn item view for paginated threads and direct clients to
  `thread/items/list` for complete item pagination.
- Return each item page entry with both its containing `turnId` and `item`, so
clients can regroup thread-wide pages into turns.
- Expose local thread-store turn and item pagination through the `ThreadStore`
  implementation.

## Testing

Added app-server coverage for paging projected turns and items, item views,
cursors, turn metadata, and rejection of the unsupported `full` view.
