986c60467b windows-sandbox: pass workspace roots to runner (#24108)
## Why

#23813 switches the Windows sandbox runner path to `PermissionProfile`,
but it still left one runtime anchor for resolving symbolic
`:workspace_roots` entries. That is not enough once a turn has multiple
effective workspace roots: exact entries and deny globs under
`:workspace_roots` need to be materialized for every runtime root before
the command runner chooses token mode or builds ACL plans.

## What Changed

- Replaces the Windows runner/setup `permission_profile_cwd` plumbing
with `workspace_roots: Vec<AbsolutePathBuf>`.
- Resolves Windows-local `PermissionProfile` data with
`materialize_project_roots_with_workspace_roots(...)` instead of the
single-cwd helper.
- Threads `Config::effective_workspace_roots()` through core execution,
unified exec, TUI setup/read-grant flows, app-server setup, app-server
`command/exec`, and `debug sandbox` on Windows.
- Preserves those workspace roots through the zsh-fork escalation
executor instead of rebuilding them from `sandbox_policy_cwd`.
- Makes `ExecRequest::new(...)` and the remaining
`build_exec_request(...)` helper path take
`windows_sandbox_workspace_roots` explicitly so new call sites cannot
silently fall back to `vec![cwd]`.
- Clarifies the `debug sandbox` non-Windows comment: remaining
cwd-dependent resolution still uses `sandbox_policy_cwd`, while
`:workspace_roots` entries are already materialized from config roots.
- Updates elevated runner IPC `SpawnRequest` to send `workspace_roots`
and bumps the framed IPC protocol version to `3` for the payload shape
change.
- Adds Windows-local resolver coverage for expanding exact and glob
`:workspace_roots` entries across multiple roots, plus core helper
coverage proving explicit roots are preserved.

A local macOS cross-check with `cargo check --target
x86_64-pc-windows-msvc ...` did not reach crate Rust code because native
dependencies require Windows SDK headers (`windows.h` / `assert.h`) in
this environment; Windows CI remains the real target validation.

Two local targeted filters compile but do not run assertions on macOS:
`env -u CODEX_NETWORK_PROXY_ACTIVE -u CODEX_NETWORK_ALLOW_LOCAL_BINDING
just test -p codex-app-server --lib command_exec_processor` matched zero
tests, and `just test -p codex-linux-sandbox landlock` matched zero
tests because the landlock suite is Linux-only.

bc10e5b390 runtime: prepend zsh fork bin dir to PATH (#23768)
## Why

#23756 makes packaged Codex builds include and default to the bundled
zsh fork. The important reason to put that fork's directory at the front
of `PATH` is to keep executable-level escalation working after a command
leaves the original shell and later re-enters zsh through `env`.

The expected chain is:

1. The zsh fork runs the top-level shell command.
2. That command launches another program, such as `python3`, while
inheriting the `EXEC_WRAPPER` environment and the escalation socket fd.
3. That program spawns a shell script whose shebang is `#!/usr/bin/env
zsh` rather than `#!/bin/zsh`, and it does not close the escalation fd.
4. `/usr/bin/env` resolves `zsh` through `PATH`, so it must find the
packaged zsh fork before the system zsh.
5. Commands inside that nested script are intercepted by the zsh fork
and can still request escalation from Codex.

If `PATH` resolves `zsh` to the system shell instead, the nested script
loses zsh-fork exec interception. Commands that should request
escalation can then run only in the original sandbox, or fail there,
without Codex ever receiving the approval request.

Shell snapshots make this slightly more subtle: a snapshot can restore
an older `PATH` after the child shell starts. This PR treats the zsh
fork `PATH` prepend as an explicit environment override so snapshot
wrapping preserves it.

## What Changed

- Added shared zsh-fork runtime helpers that prepend the configured zsh
executable parent directory to `PATH` without duplicate entries.
- Applied the zsh fork `PATH` prepend to both zsh-fork `shell_command`
launches and unified-exec zsh-fork launches before sandbox command
construction.
- Kept the shell-command zsh-fork backend API narrow: it derives the
configured zsh path from session services and rebuilds its sandbox
environment from `req.env`, rather than accepting a second, competing
environment map or a separately threaded bin dir.
- Kept Unix-only zsh-fork `PATH` mutation out of Windows clippy-visible
mutability.
- Added coverage for duplicate `PATH` entries, for preserving the zsh
fork prepend through shell snapshot wrapping, and for the nested
`python3` -> `#!/usr/bin/env zsh` escalation flow.

## Testing

- `just fmt`
- `just fix -p codex-core`

I left final test validation to CI after the latest review-comment
cleanup. Before that cleanup, `just test -p codex-core zsh_fork` passed
locally for the zsh-fork-focused tests.

ecb41fcb64 Add feature-gated standalone image generation extension (#24723)
## Why

Add a standalone image generation path that can be exercised
independently of hosted Responses image generation, while retaining the
hosted tool as fallback unless the extension is actually available to
the model.

## What changed

- Added the `codex-image-generation-extension` crate with standalone
generate/edit execution, prior-image selection for edits, model-visible
image output, and local generated-image persistence.
- Installed the extension in app-server behind the disabled-by-default
`imagegenext` feature and backend eligibility checks.
- Updated core tool planning so eligible `image_gen.imagegen` exposure
replaces hosted `image_generation`, while unavailable configurations
retain hosted fallback.
- Added coverage for extension behavior, edit history reuse, feature
gating, auth eligibility, and hosted-tool replacement.
- The extension is installed through app-server only in this PR; other
execution paths retain hosted image generation because hosted
replacement occurs only when the standalone executor is actually
registered and model-visible.
- The initial extension contract intentionally fixes the image model to
`gpt-image-2` and uses automatic image parameters.
- Native generated-image history/card parity and rollout persistence
cleanup are intentionally deferred follow-up work.

2264fdd4a2 Fix extension turn item emitter test event ordering (#24936)
## Why

PR #24813 added extension `TurnItemEmitter` coverage and introduced a
test that records a conversation history item before asserting
extension-emitted turn item events.

`record_conversation_items()` also emits a `RawResponseItem` event to
observers. The test was reading from the same event receiver and
expected the next event to be `ItemStarted`, so the test failed reliably
once the setup history item was present.

## What Changed

Update
`passes_turn_fields_and_scoped_turn_item_emitter_to_extension_call` to
consume and assert the expected setup `RawResponseItem` before checking
the extension `ItemStarted`, `WebSearchBegin`, `ItemCompleted`, and
`WebSearchEnd` events.

This is test-only and does not change extension runtime behavior.

e2551a5e36 Reap stale multi-agent slots (#24903)
## Summary

- Let `close_agent` clean up an agent that is still registered in
`AgentRegistry` even when its underlying thread is already missing.
- Preserve the explicit-close boundary: for known stale thread-spawn
agents, mark the persisted spawn edge `Closed`, then treat
`ThreadNotFound` / `InternalAgentDied` as a successful close so the
registry slot can be released.
- Add a regression for MultiAgentV2 task-name targets where
`close_agent("worker")` succeeds after the worker thread has already
disappeared.

## Motivation

A worker can disappear from `ThreadManager` while its metadata still
exists in the root `AgentRegistry`. Before this change, the close tool
failed while trying to subscribe to the missing thread status, so it
never reached the cleanup path that releases the registered agent slot.
With `agents.max_threads = 1`, an explicit close of that stale task-name
agent could fail and leave the session unable to spawn a replacement.

## Scope

This PR intentionally does not add automatic stale-agent reaping to
`spawn_agent`, `resume_agent`, or `list_agents`. A thread being missing
from `ThreadManager` is not the same as an explicit close: persisted
open spawn edges are still the durable source of truth for resume and
task-name ownership until `close_agent` is called.

## Validation

2066874415 extension-api: add TurnItemEmitter to tool calls (#24813)
## Why
Extension-contributed tools need to emit visible turn items through
Codex's normal event and persistence pipeline.

## What
- Add `TurnItemEmitter` to extension `ToolCall`s and route the core
implementation through `Session::emit_turn_item_*`.
- Hold weak session and turn references so retained tool calls cannot
keep host state alive.
- Provide a no-op emitter for extension test callers.

## Test Plan

910578792f Update rmcp to 1.7.0 (#24763)
WIll make it easier to uprev when the new draft spec is supported.

Also updates reqwest where needed for compatibility but doesn't update
it everywhere since this is already a large diff.

The new version of rmcp handles certain kinds of authentication failures
differently, this patch includes support for identifying the failing scope
in a WWW-Authenticate header.

9152ebd289 fix(linux-sandbox): preserve shell cleanup on interruption (#22729)
## Why
Interrupted `shell_command` calls can race with the outer tool-dispatch
cancellation path. When that happens, the runtime future may be dropped
before the spawned process gets a chance to run `SIGTERM` cleanup. For
bwrapd-backed Linux sandbox commands, that can leave synthetic
protected-path mount bookkeeping such as `.git/.codex` registrations
under `/tmp` behind after a TUI interruption.

The relevant cancellation points are the outer dispatch race in
[`core/src/tools/parallel.rs`](bd184ba847/codex-rs/core/src/tools/parallel.rs (L91-L132))
and the process shutdown logic in
[`core/src/exec.rs`](bd184ba847/codex-rs/core/src/exec.rs (L1367-L1393)).

## What changed
- Keep `shell_command` dispatch alive long enough for the runtime to
finish cancellation cleanup instead of immediately returning the
synthetic aborted response.
- Fold shell-turn cancellation into the existing `ExecExpiration` path
in
[`core/src/tools/runtimes/shell.rs`](bd184ba847/codex-rs/core/src/tools/runtimes/shell.rs (L267-L274)),
so cancellation and timeout behavior stay centralized.
- On cancellation, send `SIGTERM` first, wait briefly for cleanup to
run, then hard-kill any remaining descendants in the original process
group.
- Treat `ESRCH` as an already-gone process-group cleanup case in
`codex-utils-pty`, which keeps best-effort teardown from surfacing a
stale-process race as an error.

## Verification
- `cargo test -p codex-core cancellation`
- Added regression coverage for:
  - `shell_tool_cancellation_waits_for_runtime_cleanup`
  - `process_exec_tool_call_cancellation_allows_sigterm_cleanup`

07a930138f chore: enable namespace tools for Bedrock (#24713)
Client-side namespace tools are now supported by bedrock. Enable
`namespace_tools` for the Amazon Bedrock provider while continuing to
disable unsupported hosted tools such as image generation and web
search.

## Summary
- Bump the workspace Rust toolchain from `1.93.0` to `1.95.0` across
Cargo, Bazel, CI, release workflows, devcontainers, and the Codex
environment config.
- Refresh `MODULE.bazel.lock` so the Bazel Rust toolchain artifacts
match the new version.
- Leave purpose-specific toolchains unchanged, including the
`argument-comment-lint` nightly and the upstream `rusty_v8` `1.91.0`
build pin.
- Includes fixes for new lints from `just fix` and a few codex-authored
fixes for lints without a suggestion.

dc4e54d061 Restore legacy image detail values (#24644)
## Why

Older persisted rollouts can contain `input_image.detail` values of
`auto` or `low` from before `ImageDetail` was narrowed to
`high`/`original`. Current deserialization rejects those values, which
can make resume skip later compacted checkpoints and reconstruct an
oversized raw suffix before the next compaction attempt.

Confirmed Sentry reports fixed by this compatibility path:

## Background

[openai/codex#20693](https://github.com/openai/codex/pull/20693) added
image-detail plumbing for app-server `UserInput` so input images could
explicitly request `detail: original`. The Slack discussion behind that
PR was about ScreenSpot / bridge evals where user input images were
resized, while tool output images already had MCP/code-mode ways to
request image detail.

In review, the intended new API surface was narrowed to `high` and
`original`: default to `high`, allow `original` when callers need
unchanged image handling, and avoid encouraging new `auto` or `low`
usage. That policy still makes sense for newly emitted values.

The missing compatibility piece is persisted history. Older rollouts can
already contain `auto` and `low`, and resume reconstructs typed history
by deserializing those rollout records. Rejecting old values at that
boundary causes valid compacted checkpoints to be skipped. This PR
restores `auto` and `low` as real variants so old records deserialize
and round-trip without being rewritten as `high`, while product paths
can continue to default to `high` and avoid emitting `auto` for new
behavior.

## What changed

- Restored `ImageDetail::Auto` and `ImageDetail::Low` as first-class
protocol values.
- Preserved `auto`/`low` through rollout deserialization, MCP image
metadata, code-mode image output, and schema/type generation.
- Kept local image byte handling conservative: only `original` switches
to original-resolution loading; `auto`/`low`/`high` continue through the
resize-to-fit path while retaining their detail value.
- Added regression coverage for enum round-tripping and code-mode `low`
detail handling.

## Testing

finish green: 2559 tests run, 2467 passed, 55 flaky, 91 failed, 1 timed
out. The failures were broad timeout/deadline failures across unrelated
areas; targeted changed-path core tests above passed.

768848ab6f Add experimental turn additional context (#24154)
## Summary

Adds experimental `additionalContext` support to `turn/start` and
`turn/steer` so clients can provide ephemeral external context, such as
browser or automation state, without turning that plumbing into a
visible user prompt or triggering user-prompt lifecycle behavior.

## API Shape

The parameter shape is:

The keys are opaque and caller-defined.

## Context Injection

When provided, accepted entries are inserted into model context as
hidden contextual message items, not as visible thread user-message
items.

`kind: "untrusted"` entries are inserted with role `user`:

Values are not escaped. Each value is truncated to 1k approximate tokens
before wrapping.

For `turn/start`, accepted additional context is inserted before normal
user input. For `turn/steer`, additional context is merged only when the
steer includes non-empty user input; context-only steers still reject as
empty input.

## Dedupe Strategy

`AdditionalContextStore` lives on session state and stores the latest
complete additional-context map.

Each `turn/start` or non-empty `turn/steer` treats its
`additionalContext` as the current complete set of values. Entries are
injected only when the key is new or the exact entry for that key
changed, including `value` or `kind`. After merging, the store is
replaced with the provided map, so omitted keys are removed from the
retained set and can be injected again later if reintroduced.

Omitting `additionalContext`, passing `null`, or passing an empty object
resets the store to empty and injects nothing.

## What Changed

- Threads experimental v2 `additionalContext` through app-server into
core turn start and steer handling.
- Adds separate contextual fragment types for untrusted user-role
context and application developer-role context.
- Uses pending response input items so additional context can be
combined with normal user input without treating it as prompt text.
- Adds integration coverage for start/steer flow, role routing,
dedupe/reset behavior, deletion/re-add behavior, hook-blocked input
behavior, empty context-only steer rejection, external-fragment marker
matching, and truncation.

a22706dfae standalone websearch extension (#23823)
## Summary

Add the extension-backed standalone `web.run` tool so Codex can call the
standalone search endpoint through the `codex-api` search client and
return its encrypted output to Responses.

- gate the new tool behind `standalone_web_search`
- install the extension in the app-server thread registry and hide
hosted `web_search` when standalone search is enabled for OpenAI
providers so the two paths stay mutually exclusive
- build search context from persisted history using a small tail
heuristic: previous user message, assistant text between the last two
user turns capped at about 1k tokens, and current user message

ff7513cd83 Move MCP tool naming mode into manager (#21576)
## Why

The `non_prefixed_mcp_tool_names` feature should be applied where MCP
tools become model-visible, not by remapping names later in core.
Keeping the decision in `McpConnectionManager` construction makes
`ToolInfo` the single shaped view that spec building, deferred tool
search, routing, and unavailable-tool placeholders can consume directly.

This also preserves the existing external behavior while the feature is
off, and keeps the feature-on behavior for code mode and hooks explicit
at the manager boundary.

## What Changed

- Add `McpToolNameMode` to `codex-mcp` and flow it through `McpConfig`
into `McpConnectionManager::new`.
- Normalize MCP `ToolInfo` names in the manager using either
legacy-prefixed namespaces or non-prefixed namespaces; the legacy path
adds `mcp__` without restoring the old trailing namespace suffix.
- Remove the core-side MCP name remapping path so specs, tool search,
session resolution, and unavailable-tool placeholder construction use
the manager-provided `ToolName` values directly.
- Keep code mode flattening on the `__` namespace separator.
- Preserve hook compatibility by giving non-prefixed MCP hook names
legacy `mcp__...` matcher aliases.
- Add/adjust integration and unit coverage for non-prefixed code-mode
behavior, hook matching with the feature on and off, and manager-level
legacy prefixing.

b637fd26aa [codex] Make active turn task singular (#24105)
## Why

`ActiveTurn` already runs at most one task: starting a task requires
that no task is present, and replacement aborts existing work first.
Representing that state as an `IndexMap` leaves a multi-task shape for a
single-task invariant and makes each lifecycle lookup operate like a
collection lookup.

The slot remains optional because goal continuation uses an empty active
turn as a reservation while deciding whether to start continuation work.

## What changed

- Replace `ActiveTurn.tasks` with `task: Option<RunningTask>`.
- Update task abort/completion, session lookup and steering, input-queue
matching, goal reservation, and network-approval lookup to operate on
the singular slot.
- Mutate the singular task slot directly instead of retaining
collection-era add/remove/take helpers.
- Record token usage on the completing active task span without a
regular-task-only opt-in flag.

f94157a4b2 code-mode: merge stored values by key (#24159)
## Summary

Change code-mode stored value updates to merge writes by key instead of
replacing the session's complete stored-value map after each cell
completes.

Previously, each cell received a snapshot of stored values and returned
the complete resulting map. When multiple cells ran concurrently, a
later completion could overwrite values written by another cell because
it committed an older snapshot.

This change moves stored-value ownership into `CodeModeService`:

- Each runtime starts from the service's current stored values.
- Runtime completion reports only keys written by that cell.
- The service merges those writes into the current stored-value map on
successful completion.
- Core no longer replaces its stored-value state from a cell result.

As a result, concurrently executing cells can update different stored
keys without clobbering one another.

The move into CodeModeService is motivated by a desire to have this
lifetime tied to a new lifetime object on that side in a subsequent PR.

5c20513a1b Default function tools into tool hooks (#23757)
# Why

`PreToolUse`, `PostToolUse`, and `updatedInput` coverage for local
function tools currently depends on each handler remembering to wire up
the hook contract itself. That makes coverage easy to miss as new
function tools are added, even though most of them share the same basic
shape: a model-facing function call with JSON arguments.

# What

This makes `CoreToolRuntime` provide the default hook contract for
ordinary local function tools:

- build generic `PreToolUse` and `PostToolUse` payloads from the
function tool name and arguments
- apply `updatedInput` rewrites back into function-tool arguments
through the same default path
- let tool outputs override the post-hook input or response when they
have a more stable hook-facing contract

The exceptions stay explicit:

- hosted tools remain outside the generic local function path
- code-mode `wait` and `write_stdin` opt out for now
- `PostToolUse` feedback replaces only the model-visible response, so
code mode keeps its typed tool result

With the generic path in place, the MCP and extension-tool adapters no
longer need their own duplicate pre/post hook plumbing. The new coverage
exercises the registry default plus end-to-end local function behavior
for pre-hook blocking, `updatedInput` rewriting, and post-hook context.

e6c8371e4e refactor: centralize tool exposure planning (#23876)

Tool exposure is a planning concern, but the deferred MCP path and
dispatch-only legacy shell path were carrying those decisions in handler
constructors and a shell-only tool-family builder. Keeping those
decisions in `spec_plan` makes the core tool plan easier to follow and
keeps handlers focused on runtime behavior.

- add `PlannedTools` helpers for ordinary runtimes, exposure overrides,
dispatch-only runtimes, and hosted specs
- inline shell tool assembly into `core/src/tools/spec_plan.rs` and

516f134641 Make tool executor specs mandatory (#23870)
## Why

`ToolExecutor` is the runtime contract that keeps a callable tool and
its model-visible spec together. Leaving `spec()` optional lets a
registered runtime silently omit that half of the contract, and it also
overloads a missing spec as an exposure decision for tools that should
stay dispatchable without being shown to the model.

## What

- Make `ToolExecutor::spec()` required and update core, extension, and
test tool executors to return a concrete `ToolSpec`.
- Add `ToolExposure::Hidden` for dispatch-only tools. The legacy
`shell_command` runtime in unified-exec sessions now uses that explicit
exposure instead of hiding itself by omitting a spec.
- Build MCP tool specs when `McpHandler` is constructed so invalid MCP
specs are skipped before the handler is registered.
- Keep tool planning aligned with the new contract for direct, deferred,
hidden, code-mode, dynamic, and namespaced tool paths.

370b13afc9 Honor client-resolved service tier defaults (#23537)
## Why

Model catalog responses can now advertise a nullable
`default_service_tier` for each model. Codex needs to preserve three
distinct states all the way from config/app-server inputs to inference:

- no explicit service tier, so the client may apply the current model
catalog default when FastMode is enabled
- explicit `default`, meaning the user intentionally wants standard
routing
- explicit catalog tier ids such as `priority`, `flex`, or future tiers

Keeping those states distinct prevents the UI from showing one tier
while core sends another, especially after model switches or app-server
`thread/start` / `turn/start` updates.

## What Changed

- Plumbed `default_service_tier` through model catalog protocol types,
app-server model responses, generated schemas, model cache fixtures, and
provider/model-manager conversions.
- Added the request-only `default` service tier sentinel and normalized
legacy config spelling so `fast` in `config.toml` still materializes as
the runtime/request id `priority`.
- Moved catalog default resolution to the TUI/client side, including
recomputing the effective service tier when model/FastMode-dependent
surfaces change.
- Updated app-server thread lifecycle config construction so
`serviceTier: null` preserves explicit standard-routing intent by
mapping to `default` instead of internal `None`.
- Kept core responsible for validating explicit tiers against the
current model and stripping `default` before `/v1/responses`, without
applying catalog defaults itself.

c5bd131567 feat: add turn_id and truncation_policy to extension tool calls (#23666)
## Why

Extension-owned tools currently receive a stripped `ToolCall` with only
`call_id`, `tool_name`, and `payload`.
That makes extension work that needs turn-local execution context
awkward, especially web-search extension work that needs the active
`truncation_policy` at tool invocation time.

Reconstructing that value from config or `ExtensionData` would be
indirect and could drift from the actual turn context, so the cleaner
fix is to pass the needed turn metadata directly on the extension-facing
invocation type.

## What changed

- added `turn_id` and `truncation_policy` to `codex_tools::ToolCall`
- populated those fields when core adapts `ToolInvocation` into an
extension tool call
- added a focused adapter test that verifies extension executors receive
the forwarded turn metadata
- updated the memories extension tests to construct the richer
`ToolCall`
- added the `codex-utils-output-truncation` dependency to `codex-tools`

a52c91d8b5 [codex] Hide deferred tools from code mode prompt (#23605)
## Why

`code_mode_only_guides_all_tools_search_and_calls_deferred_app_tools`
was failing because code-mode prompt generation used the same nested
tool spec list for both the model-visible `exec` guide and the runtime
`ALL_TOOLS` surface. That allowed deferred MCP/app tools, such as
`calendar_timezone_option_99`, to leak into the `exec` description even
though they should only be discoverable through `ALL_TOOLS` at runtime.

## What changed

Split code-mode nested tool planning into two sets in
`core/src/tools/spec_plan.rs`:

- runtime nested tool specs still include deferred tools, so
`tools[...]` and `ALL_TOOLS` can call them
- `exec` prompt docs only render non-deferred tools, so deferred app
tools stay out of the model-visible guide

5a4202ad90 [codex] Preserve raw code-mode exec output by default (#23564)
## Why
Code mode can use nested unified exec calls as data sources. When those
calls omit `max_output_tokens`, code mode should receive raw command
output so the script can parse or summarize it itself. When code mode
does provide `max_output_tokens`, that explicit nested budget should be
respected, including values above the default unified exec limit, rather
than being capped before code mode sees the result.

## What
- Preserve direct unified exec truncation behavior, while letting
code-mode exec/write_stdin keep `max_output_tokens` as `None` unless
explicitly supplied.
- Make code-mode tool results use raw output when no explicit limit is
present, and use the explicit nested limit directly when one is
specified.
- Refactor unified exec output formatting so `truncated_output` takes
the caller-selected token budget.
- Add e2e integration coverage for explicit nested exec limits, omitted
nested exec limits, outer exec limit propagation, omitted-limit outputs
that exceed both the default and a small truncation policy, explicit
nested limits above those caps, and high explicit limits that still
compact larger command output.
- Reuse the code-mode turn setup helper while directly asserting the
exact exec output item in each test.

e43a2e297f Fix stale background terminal poll events (#23231)
## Why

Issue #23214 reports `/ps` showing no background terminals while the
status line still says it is waiting for a background terminal. The race
is in core: `write_stdin` can poll a process that exits before the
response returns. The process manager correctly returns `process_id:
None`, but the handler still emitted a `TerminalInteraction` event using
the requested session id, causing clients to believe a dead process was
still being polled.

Fixes #23214.

## What changed

- Suppress `TerminalInteraction` events for empty `write_stdin` polls
once `response.process_id` is `None`.
- Continue emitting interactions for non-empty stdin, even if that input
causes the process to exit before the response returns.
- Extend the unified exec integration test to assert completed empty
polls do not emit terminal interactions.

## Verification

- `cargo test -p codex-core --test all
unified_exec_emits_one_begin_and_one_end_event`
- `cargo test -p codex-core --test all
unified_exec_emits_terminal_interaction_for_write_stdin`

`cargo test -p codex-core` currently aborts in unrelated

c58c84d6ee test: fix multi-agent service tier assertion (#23576)
## Why

`openai/codex#22169` added a regression test that expects an invalid
child `service_tier` to be rejected, but the test used
`Result::expect_err` on `SpawnAgentHandler::handle`. That requires the
`Ok` type to implement `Debug`, and this handler returns `Box<dyn
ToolOutput>`, so Bazel failed while compiling `codex-core` tests before
it could run them.

## What changed

- Capture the handler result and assert on `result.err()` instead of
calling `expect_err`.
- Keep the same `FunctionCallError::RespondToModel` assertion for the
rejected service tier.

## Verification

Custom agent roles are ordinary config layers, so a role file can
already express `service_tier` just like other config values. The
spawned-agent tier path needs to preserve that effective role config and
follow the same precedence pattern as model/reasoning.

## What changed
- Apply an explicit spawn-time `service_tier` onto the child config
before role application, so a role config layer can override it just
like role-defined model/reasoning settings do.
- Validate the final effective child tier after the final child model is
known, while still falling back to the parent tier when no child tier
survives.
- Add focused integration coverage for both v1 and v2 proving role TOML
loads a service tier, spawned children keep that role-configured tier,
and a role tier wins over a conflicting spawn-time tier.

05e171094d Remove ToolsConfig from tool planning (#22835)
## Why

`codex-tools` is meant to hold reusable tool primitives, but
`ToolsConfig` had become a second copy of core runtime decisions instead
of a small shared contract. It carried provider capabilities, auth/model
gates, permission and environment state, web/search/image feature gates,
multi-agent settings, and goal availability from core into `codex-tools`
([definition](22dd9ad392/codex-rs/tools/src/tool_config.rs (L97)),
[stored on each
`TurnContext`](22dd9ad392/codex-rs/core/src/session/turn_context.rs (L87))).
Every session/context variant then had to build and mutate that snapshot
before assembling tools.

This PR removes that master object instead of renaming it. Tool planning
now reads the live `TurnContext`, where `codex-core` already owns those
decisions, while `codex-tools` keeps only reusable primitives and a
generic `ToolSetBuilder`/`ToolSet` accumulator.

## What Changed

- Removed `ToolsConfig` / `ToolsConfigParams` from `codex-tools`; the
crate keeps the shared helpers that still belong there, including
request-user-input mode selection, shell backend/type resolution,
`UnifiedExecShellMode`, and `ToolEnvironmentMode`.
- Replaced config-snapshot planning with `ToolRouter::from_turn_context`
and a `spec_plan` pipeline over `CoreToolPlanContext`, deriving provider
capabilities, auth gates, model support, feature gates, environment
count, goal support, multi-agent options, web search, and image
generation from the authoritative turn state.
- Added generic `codex_tools::ToolSetBuilder` / `ToolSet`, plus the
small core adapter needed to accumulate `CoreToolRuntime` values and
hosted model specs.
- Added the `tool_family::shell` registration module and moved
shell/unified-exec/memory accounting call sites to read the narrow
per-turn fields directly.
- Narrowed `TurnContext` to the remaining explicit per-turn fields
needed by planning: `available_models`, `unified_exec_shell_mode`, and
`goal_tools_supported`.
- Reworked MCP exposure and tool-search setup so deferred/direct MCP
behavior is driven by the current turn rather than a precomputed config
snapshot.
- Replaced the large expected-spec fixture tests with focused
behavior-level coverage for shell tools, environments, goal and
agent-job gates, MCP direct/deferred exposure, tool search,
request-plugin-install, code mode, multi-agent mode, hosted tools, and
extension executor dispatch.

## Summary

The first three PRs in this stack are a cleanup pass before the actual
thread settings API work.

Today, core has several overlapping "user input" ops: `UserInput`,
`UserInputWithTurnContext`, and `UserTurn`. They differ mostly in how
much next-turn state they carry, which makes the later queued thread
settings update harder to reason about and review.

This PR starts that cleanup by adding the shared
`ThreadSettingsOverrides` payload and allowing `Op::UserInput` to carry
it. Existing variants remain in place here, so this layer is mostly a
behavior-preserving API shape change plus mechanical constructor
updates.

## End State After PR3

By the end of PR3, `Op::UserInput` is the only "user input" core op. It
can carry optional thread settings overrides for callers that need to
update stored defaults with a turn, while callers without updates use
empty settings. `Op::UserInputWithTurnContext` and `Op::UserTurn` are
deleted.

By the end of PR5, core will have only two ops for this area:

- `Op::UserInput` for user-input-bearing submissions.
- `Op::ThreadSettings` for settings-only updates.

Pending model input was split across `Session`, `TurnState`, and the
agent mailbox. That made it easy for new paths to manage queued user
input or mailbox delivery outside the intended ownership boundary.

This PR consolidates the model-facing input lifecycle behind the session
input queue so turn-local pending input, next-turn queued items, and
mailbox delivery coordination are owned in one place.

## What Changed

- Added `session/input_queue.rs` to own pending input queues and mailbox
delivery coordination.
- Removed the standalone `agent/mailbox.rs` channel wrapper and store
mailbox items directly in the input queue.

c69cde3547 Add tool lifecycle extension contributor (#23309)
## Why

Extensions that need to track runtime progress currently have no typed
host signal for tool execution. The goal extension in particular needs
to observe tool attempts without inspecting tool payloads, owning tool
implementations, or staying coupled to core-only runtime plumbing.

This adds a narrow lifecycle contributor API for host-owned tool
execution: extensions can observe when an accepted tool call starts and
how it finishes, while policy hooks and tool handlers continue to own
payload rewriting, blocking, and execution.

Relevant code:

- Added `ToolLifecycleContributor` to `codex-extension-api`, including:

  - `ToolFinishInput`
  - `ToolCallSource`
  - `ToolCallOutcome`
- Added registration and lookup support on `ExtensionRegistryBuilder` /
`ExtensionRegistry`.
- Wired core tool dispatch to notify lifecycle contributors for:
  - accepted tool starts
  - completed tool calls, including the tool output success marker
  - pre-tool-use blocks
  - failures before or after the handler runs
  - cancellation/abort in the parallel tool path
- Registered the goal extension as a lifecycle contributor and added the
outcome filter it will use for goal progress accounting.

## Test Coverage

- Added `dispatch_notifies_tool_lifecycle_contributors` to cover
lifecycle notification ordering and outcomes for successful and
handler-failed tool calls.

4dbca61e20 fix: default unknown tool schemas to empty schemas (#22380)
## Why

Some tool providers, especially MCP servers and dynamic tool sources,
can supply schema nodes that omit `type` and have no recognized JSON
Schema shape hints. Previously, `sanitize_json_schema` filled those
unknown nodes in as `string`, which made the schema parseable but
invented a scalar constraint that the provider did not specify. For
description-only fields, that could incorrectly steer tool arguments
away from the provider's actual accepted shape.

The Responses API accepts permissive empty schemas such as `{}` at
nested property positions, so Codex should preserve that permissive
meaning instead of coercing unknown schema nodes into a misleading
scalar type.

## What Changed

- Changed the no-hints fallback in `codex-rs/tools/src/json_schema.rs`
to clear unrecognized object schema nodes to `{}`.
- Empty schemas now remain `{}` rather than becoming `type: "string"`.
- Description-only or otherwise metadata-only nested property schemas
now become `{}` while surrounding object/array/string/number inference
still applies when recognized hints are present.
- Updated `codex-tools` and `codex-core` tests to cover top-level empty
schemas, nested empty schemas, metadata-only malformed schemas, dynamic
tools, and MCP tool specs.

## Verification

- `cargo test -p codex-tools`
- `cargo test -p codex-core
test_mcp_tool_property_missing_type_defaults_to_empty_schema`
- Manually verified the real Responses API behavior for both
empty-schema positions:
- Top-level function `parameters: {}` is accepted and echoed back as
`{"type":"object","properties":{}}`; when forced to call the tool,
Responses emitted empty object arguments: `"arguments": "{}"`.
- Nested property schema `{}` is accepted and preserved as `{}`; when
forced to call a tool with `metadata.extra`, Responses emitted
`"arguments": "{\"metadata\":{\"extra\":\"codex schema sanitizer
behavior\"}}"`.

0d344aca9b goal: pause continuation loops on usage limits and blockers (#23094)
Addresses #22833, #22245, #23067

## Why
`/goal` can keep synthesizing turns even when the next turn cannot make
meaningful progress. Hard usage exhaustion can replay failing turns, and
repeated permission or external-resource blockers can keep burning
tokens while waiting for user or system intervention.

## What changed
- Add resumable `blocked` and `usageLimited` goal states. As with
`paused`, goal continuation stops with these states.
- Move to `usageLimited` after usage-limit failures.
- Allow the built-in `update_goal` tool to set `blocked` only under
explicit repeated-impasse guidance. Updated goal continuation prompt to
specify that agent should use `blocked` only when it has made at least
three attempts to get past an impasse.

Most of the files touched by this PR are because of the small app server
protocol update.

## Validation

I manually reproduced a number of situations where an agent can run into
a true impasse and verified that it properly enters `blocked` state. I
then resumed and verified that it once again entered `blocked` state
several turns later if the impasse still exists.

I also manually reproduced the usage-limit condition by creating a
simulated responses API endpoint that returns 429 errors with the
appropriate error message. Verified that the goal runtime properly moves
the goal into `usageLimited` state and TUI UI updates appropriately.
Verified that `/goal resume` resumes (and immediately goes back into
`ussageLImited` state if appropriate).


## Follow-up PRs

Small changes will be needed to the GUI clients to properly handle the
two new states.

82061660ae [codex] Remove legacy shell output formatting paths (#22706)
## Why

The client and tool pipeline still carried compatibility code for legacy
structured shell output. Current shell and apply_patch responses are
already plain text for model consumption, so keeping a
JSON-serialization path plus shell-item rewrite logic makes the request
formatter and tests preserve a format we do not need anymore.

## What Changed

- Removed the client-side shell output rewrite from
`core/src/client_common.rs`.
- Removed the structured exec-output formatter and the shell `freeform`
switch so tool emitters use one model-facing formatter.
- Collapsed apply_patch/shell serialization tests around the remaining
plain-text output expectations and removed duplicate one-variant
parameterized cases.
- Kept the `ApplyPatchModelOutput::ShellCommandViaHeredoc` compatibility
input shape, but no longer treats it as a separate output-format mode.

## Validation

- `cargo test -p codex-core client_common`
- `cargo test -p codex-core shell_serialization`

0a83353ca3 test: reduce core sandbox policy test setup (#23036)
## Why

`SandboxPolicy` is a legacy compatibility shape, but several core tests
still used it for ordinary turn setup even when the runtime path now
carries `PermissionProfile`. With the first cleanup PR merged, this
follow-up trims more core test scaffolding so remaining `SandboxPolicy`
matches are easier to classify as production compatibility,
legacy-boundary coverage, or explicit conversion tests.

## What Changed

- Updated apply-patch handler and runtime tests to pass
`PermissionProfile` directly.
- Changed sandboxing test helpers to build permission profiles without
first creating `SandboxPolicy` values.
- Converted request-permissions integration turns to pass
`PermissionProfile` through the test helper, leaving legacy sandbox
projection at the `Op::UserTurn` boundary.
- Converted unified exec integration helpers and direct turn submissions
to use `PermissionProfile` values instead of `SandboxPolicy` setup.
- Removed now-unused `SandboxPolicy` imports from the touched core
tests.

545ede569c Make multi-agent v2 tool namespace configurable (#23147)
## Summary
- Add `features.multi_agent_v2.tool_namespace` with config/schema
validation for Responses-compatible namespace values.
- Thread the resolved namespace into `ToolsConfig` for normal turns and
review turns.
- Wrap MultiAgentV2 tool specs and registry names in the configured
namespace when namespace tools are supported, while falling back to the
plain tool names when they are not.

## Why

The `spawn_agent` model override guidance is uncapped and bloating
context. We need to trim down each entry and cap total entries.

## What changed

- Cap the model override summaries shown in `spawn_agent` to the first 5
picker-visible models, preserving the existing priority ordering from
the models manager.
- Condense each rendered entry to the actionable pieces the model needs:
  - use the model slug as the label
  - render compact reasoning effort lists with the default marked inline
- render only service tier IDs, and omit the clause when no tiers are
available
- Update coverage so the compact formatter shape and the top-5 cap are
exercised, and keep the end-to-end request assertion aligned with real
model metadata.

## Example

Before:

d91bc15618 test: construct permission profiles directly (#23030)
## Why

`SandboxPolicy` is now a legacy compatibility shape, but several tests
still built a `SandboxPolicy` only to immediately convert it into
`PermissionProfile` for APIs that already accept canonical runtime
permissions. Those detours make it harder to audit where legacy sandbox
policy is still required, because boundary-only usages are mixed
together with ordinary test setup.

## What Changed

- Updated tests in `codex-core`, `codex-exec`, `codex-analytics`, and
`codex-config` to construct `PermissionProfile` values directly when the
code under test takes a permission profile.
- Changed exec-policy, request-permissions, session, and sandbox test
helpers to pass `PermissionProfile` through instead of converting from
`SandboxPolicy` internally.
- Left `SandboxPolicy` in place where tests are explicitly exercising
legacy compatibility or request/response boundaries.

941e7f825e Improve goal completion usage reporting (#22907)
## Why

Goal completion follow-up turns currently receive a preformatted English
usage sentence such as `time used: 2586 seconds`. That nudges the model
to echo an awkward raw seconds count in the final reply, even though the
tool result already exposes structured usage fields like
`goal.timeUsedSeconds`, `goal.tokensUsed`, and `goal.tokenBudget`.

## What changed

- Replace the preformatted completion usage sentence with guidance to
read the structured goal fields from the tool result.
- Preserve token-budget reporting while allowing the model to phrase
elapsed time in a concise, human-friendly way that fits the response
language.
- Update core coverage for both the generated completion guidance and
the session flow that forwards it back to the model.

## Verification

Previously, it would have output a final message indicating that it
"worked for 303 seconds". Now it shows the following:
