## Why
Direct Guardian review checked recorded `exec_command` calls without applying `PreToolUse` hooks, so approval decisions could miss hook rewrites or denials.
## What changed
Run the shared `PreToolUse` hook handling before evaluating approval requirements. Use the rewritten arguments for policy checks and the Guardian approval request, and return an error when a hook blocks the call, even if policy would otherwise skip review.
## Testing
Add regression tests covering rewritten commands that require approval and hook denials for policy-allowed commands. Verify the rewritten approval request, hook events and recorded denial context, and that reviewed commands are not executed.

## What changed
- Match final serving assessments to their response and output item, then retain them with tool invocations and code cells across waits.
- Use matching assessments instead of asynchronous scoring for adaptive review: low-risk actions can skip synchronous review, while elevated-risk actions require it.
- Revalidate captured authorization and policy before approval. Require fresh review for stale or incomplete authorization, preserve explicit review requirements, and discard assessments when hooks rewrite tool input.
- Keep asynchronous scoring as the fallback when an assessment is missing or does not match.
## Testing
Add integration coverage for direct and nested tool calls, hook rewrites, explicit escalation, interrupted responses, catalog-driven policy, incomplete authorization, and assessment retention across code-cell yields and waits.
GitOrigin-RevId: 4346a227204137f018c03c34b302d6f1e9592c96
## Why
Managed command policies need to require a human decision independently of automatic review, hook allowances, and saved approvals.
## What changed
- Support `decision = "prompt-user"` in managed `requirements.toml` prefix rules, requiring a fresh, one-time human decision for matching commands.
- Add experimental `humanCommand` approval requests for clients advertising both `experimentalApi` and `requiredHumanApproval`. Accept only `accept`, `decline`, or `cancel`; reject commands when no capable client is available.
- Restrict responses to capable clients that received the request, and replay pending prompts to compatible clients on reconnect.
- Enable the capability in the TUI and update protocol schemas and client bindings.
## Testing
Add coverage for fresh decisions overriding saved allowances, hook allowances, and automatic review, plus child-thread approval routing and rejection of responses from incompatible or unrelated clients.
GitOrigin-RevId: 0b95ff6aab7f8a04b7298c8a4329d1fddf6a603e
## Why
Guardian review needs the original function-call metadata even when the call is no longer in live conversation history. Looking it up in history can lose the evidence needed to review encrypted multi-agent messages.
## What changed
Capture native-review policy in the tool registry and retain the original call through dispatch for multi-agent tools that require review. Use that retained call for Guardian review, applying hook-rewritten arguments while preserving encrypted-argument metadata and clearing stale replay content. Limit `ToolCall` debug output to the tool name and call ID.
## Testing
Add coverage for selective call retention across review settings and namespaces. Extend Guardian tests to cover calls absent from live history and hook rewrites that preserve call identity and encrypted-argument metadata across incremental reviews.

## Why
`PermissionRequest` and `PostToolUse` hook inputs could report the turn's working directory instead of the local command's execution directory. This also affected commands whose results were collected in a later turn after the working directory changed.
## What changed
- Report the local command's execution directory in hook input `cwd`, consistent with `PreToolUse`.
- Preserve the original directory through `write_stdin` approvals and delayed completion, including network permission requests.
- Retain the host directory for remote commands and keep hook process working directories unchanged.
## Testing
Add coverage for relative and absolute workdirs, approval denials, command rewrites, completion and stdin approval across turns, remote command fallback, network approvals, and Unix and Windows path formatting.
GitOrigin-RevId: ef5dad2a0634dda9edb2f841ef4fe49ae0d1187d
## Why
`PreToolUse` hooks receive the host working directory even when a local
`exec_command` call specifies a different `workdir`. This prevents hooks from
seeing the directory in which the command will execute.
## What changed
- Share working-directory resolution between command execution and hook input,
using the selected environment and the command's `workdir`.
- Report the resolved local directory in the hook input's `cwd`. Keep hook
processes in the host directory and retain the host `cwd` for remote commands.
- Reject invalid environment selections or paths before running hooks and record
failures in tool-result telemetry.
## Testing
Add unit and integration coverage for default, empty, relative, and absolute
working directories; Windows path conventions; remote commands; hook denial and
input rewriting; and invalid arguments that fail without running hooks.
GitOrigin-RevId: 97f71f6d3e21a5d95a7abf5384f06f4993241797
## Why
Prompt telemetry can include full user text. Emitting it before hook acceptance logs prompts that `UserPromptSubmit` hooks subsequently block.
## What changed
Move prompt telemetry from input submission to hook acceptance, including queued and steered input. Preserve telemetry for accepted Guardian review prompts that are converted to response items during finalization.
## Testing
Extend hook regression tests to assert that blocked prompts are absent from telemetry and rollouts, while accepted queued prompts are logged exactly once. Extend Guardian review coverage to assert exactly one prompt event across inspection steps.
GitOrigin-RevId: aa71f98de2356dd08f62ff492e897ab3a43b54c9
## Why
Managed network policies need to require approval for selected hosts even when an exact or wildcard allow rule matches them.
## What changed
- Add `experimental_network.review_domains` to managed requirements and route matching hosts through the existing network approval flow. Explicit denials retain precedence, and approved connections still undergo DNS and local-address checks.
- Preserve review patterns across environment policy attachments and remote proxy configuration. Reject execution on an exec-server that lacks the `networkProxyReviewDomains` capability when review patterns are present.
- Expose `reviewDomains` through the app-server requirements API and generated schemas, and show `review_domains` in debug configuration output.
- Update approval reasons to say that network access requires approval rather than assuming the host is absent from the allowlist.
## Testing
Extend coverage for review precedence over exact and wildcard allows, explicit denials, DNS checks after local and remote approvals, approval hooks, policy merging, remote configuration round trips, and requirements API serialization.
GitOrigin-RevId: 4a861061a0ed80b3537d3b72f2caecb0db2a4f0b
## Why
Mailbox preemption can discard completed response items and lose the response ID needed for incremental continuation. Buffered tool calls must also wait for queued agent messages to be read.
## What changed
- Gracefully interrupt and drain Responses Lite streams, retaining completed items and the response ID for the next request.
- Cancel buffered tool calls before dispatch and suppress partial items that may be discarded during the drain.
- Keep user steering queued until draining finishes. Include pending mail in follow-up requests after disconnects and preserve content-filter recovery guidance.
## Testing
Add WebSocket regression coverage for buffered completion, interrupts without acknowledgement, buffered tools and partial items, user steering during draining, disconnects, and content-filter recovery.
GitOrigin-RevId: c9d3fb87a81850e5686e07f28103e2ff4378ddec
## What changed
- Add `PreModelRequest` and `PostModelRequest` hooks around model request attempts, including retries. Pass model, session, turn, and optional subagent context; report completion, failure, or cancellation with errors and token usage when available.
- Treat hook output as diagnostic data without allowing it to veto requests or alter model context. Wait for synchronous post-request hooks before dispatching tools, while asynchronous hooks allow streaming tool dispatch to continue.
- Apply memory-consolidation hook filtering and expose the new events through configuration, protocol schemas, generated clients, and the hooks browser.
## Testing
Add integration coverage for failures and retries, ignored control directives, cancellation, synchronous and asynchronous tool ordering, interrupted-turn resume and token accounting, and memory-consolidation filtering. Update hook schema fixtures and hooks browser snapshots.
GitOrigin-RevId: 6ed7b39fc8bb94cf467e350d07d97fcc729c21ce
## Why
A plugin's scripts can change without changing its hook declaration. Declaration-only approval therefore lets changed plugin code run under an existing hook approval.
## What changed
- Fingerprint plugin bundle contents and include the content hash in the approval value returned by `hooks/list`. Persist it as `trusted_content_hash`, keeping `trusted_hash` compatible with older clients.
- Verify contents before each command or MCP hook execution, within the hook's timeout, and require renewed approval when approved contents change.
- Migrate legacy declaration-only approvals to a shared, immutable content baseline per plugin and config home.
- Exclude generated Python bytecode, `.venv`, and `.egg-info` entries from fingerprints. Run app-server hook listing on blocking tasks.
## Testing
Extend integration coverage for plugin upgrades and rollbacks requiring approval, same-turn script changes preventing further hook execution, shared legacy approval baselines, and generated Python artifacts preserving trust.

## Why
When a container denies fresh `/proc` mounts, the existing sandbox fallback retains the caller's `/proc` while creating a new PID namespace. Process IDs inside the sandbox can then differ from those exposed by `/proc`, breaking process lookups.
## What changed
- Add `codex exec-server --linux-sandbox-pid-namespace=inherit` to reuse the caller's PID namespace and `/proc` for both process and filesystem helpers. Repository config and command environment variables cannot enable it.
- Keep `isolate` as the default, preserving the existing mount fallback and compatibility with older helpers. Inheritance requires an updated helper and omits `--unshare-pid` and `--as-pid-1`.
- Preserve other sandbox restrictions. With `:minimal`, bind the inherited `/proc` read-only with its container masks and apply explicit filesystem denials afterward.
Inheritance is intended for dedicated environments: it allows sandboxed commands to signal other same-UID processes, including the executor.
## Testing
Add unit and integration coverage for namespace flags, denied proc mounts, consistent process IDs, retained sandbox restrictions, and startup-only selection across process and filesystem helpers.

## Why
Saved model selections can outlive their catalog entries. Removing GPT-5.4 needs to preserve a migration path without applying it to other providers that may still support the same model slug.
## What changed
- Remove `gpt-5.4` from the bundled catalog and `openai.gpt-5.4` from the Bedrock catalogs, including GovCloud.
- Add fallback migration prompts to GPT-6 Sol for saved GPT-5.4 selections on OpenAI and Amazon Bedrock. Scope fallback migrations, including the existing GPT-5.4 Mini migration, to their owning providers while honoring catalog metadata.
- Update tests that depend on bundled GPT-5.4 metadata to use GPT-5.5 or explicit model overrides.
## Testing
Extend migration tests to cover catalog metadata precedence, acknowledged targets, missing or hidden replacements, and preserving the provider while saving the replacement model and its default reasoning effort. Update Bedrock catalog assertions and picker snapshots.

## What changed
- Make `LocalThreadStore` default to paginated history while retaining explicit legacy mode.
- Explicitly use paginated history for Guardian review sessions and allow in-memory stores to create paginated threads.
- Rename full-history resume, fork, and subagent helpers to identify them as legacy APIs, and document the paginated alternatives.
- Reject `thread/revert` for ephemeral threads and prevent the TUI from replaying or attaching to ephemeral agent threads.
## Testing
Add coverage for preserving the default and explicitly selected history modes and conversation context across restart, and for retaining context after rejected ephemeral reverts. Extend Guardian checkpoint coverage across both history modes and verify that saved reviews use paginated history.
GitOrigin-RevId: 870239eb3757018159ee4f230a009c97d38ac5b7
## What changed
Always use `GuardianContextMode::ThreadOwned` for session capture and subagent history forks, regardless of `guardianv2.thread_context`. Continue selecting reviewer policy from saved checkpoints, preserving legacy review for incompatible checkpoints and removing the feature gate from reviewer compaction compatibility checks.
## Testing
Update coverage for unconditional retained context across compaction, resume, and subagent forks. Exercise legacy review with saved checkpoints, and verify that local and managed opt-outs do not disable sender context capture or checkpoint migration.
GitOrigin-RevId: cd74e2029c83f12bc655b6563242d06f29dd46af
## Why
Ordinary user replies can depend on preceding assistant messages. Losing those messages during compaction can leave Guardian without the context needed to interpret a reply's scope.
## What changed
- Retain original assistant messages, including commentary, with their recorded order across checkpoints and resume. Exclude compaction output from this evidence.
- Include assistant context in local and delegated reviews as untrusted context, with separate storage limits and lower budget priority than user instructions.
- Prefer assistant context preceding ordinary replies when selecting bounded root evidence, and report unavailable context without inferring what a reply refers to.
- Preserve inherited context scope and remove assistant evidence beyond rollback boundaries, including during rollout migration.
## Testing
Extend coverage for compaction and resume, checkpoint migration, queued-input ordering and rollback, role labeling, and evidence budgets that preserve user restrictions while dropping assistant context.
GitOrigin-RevId: 886189b0be29ffc3d324b56f5d0b673d6b8d84da
## Why
Guardian's retained root context appended assistant messages after user evidence and omitted questions sent through User Messaging. This could leave subagent authorization reviews with missing or misplaced conversational context.
## What changed
- Record assistant messages and function calls in the same order sequence as accepted user inputs, then merge retained review context by recorded position while prioritizing user evidence within the message budget.
- Capture bounded User Messaging text after input hooks when a send succeeds, and match it to the original call for assistant context.
- Label assistant context without a recorded position explicitly and clear parent assistant and tool positions when forking agent history.
- Clarify that recorded order does not establish delivery order or pair ordinary replies with questions; assistant messages remain untrusted context.
## Testing
Add coverage for hook-rewritten messages, failed or unconfirmed sends, queued replies with delayed tool results, ordering across resume, and removal of inherited assistant positions from forked history.
GitOrigin-RevId: ac717cf0f5efe4b05c18f2d8370dd48453ac4e35
## Why
Model and reasoning effort updates during a turn can leave request metadata and tool hooks reporting the turn's initial settings. Metadata should describe the step that issued the request or tool call.
## What changed
- Share captured execution metadata across Responses, MCP, and extension tool calls, including model, reasoning effort, and automatic review and Node REPL flags.
- Build Responses tool inventory metadata from the issuing step's finalized tool router. Attach the finalized inventory separately for remote compaction.
- Use captured step settings for pre- and post-tool hooks, and captured review settings for permission-request hooks.
## Testing
Add and extend regression tests for model and effort changes during a turn, MCP metadata, pre-tool hook model attribution, captured review flags, and tool inventory matching the issuing request.
GitOrigin-RevId: 9dee46a8b4839de8b434cdad4ed441cc0bb6ff9b
## Why
Forked threads reported `startup`, causing startup hooks to run again even when their context was inherited from the parent. Resuming with supplied history also reported `startup` instead of `resume`.
## What changed
- Add `fork` as a `SessionStart` source and expose it in the hook input schema.
- Report `fork` for histories with a fork parent and `resume` for supplied histories without one.
- Continue dispatching `SubagentStart` for spawned subagents with either fresh or forked context.
## Testing
Add regression coverage for fork hook matching without duplicate startup context, resume hooks with supplied history, and `SubagentStart` context injection for fresh and forked subagents.
GitOrigin-RevId: 5ac1ae4ab8adcbbbceb8cb0ffb9b32b6402c585b
## Why
Guardian reviews need the original user instructions even when compaction or
transcript selection removes them from the active conversation window.
## What changed
- Capture complete user messages in host-owned retained context and include them
as ordered authorization evidence alongside verified answers.
- Omit oversized instructions atomically and emit an explicit incomplete-evidence
notice instead of exposing a potentially misleading partial permission.
- Mark user messages inherited by forked agents so they remain model-visible
without becoming child-local authorization.
## Testing
- Cover retained instructions across compaction, resume, rollback, and agent
forks, including oversized evidence and legacy context behavior.
GitOrigin-RevId: 97122509d818c90d2f87c6a1b29c99c05ad0316e
## Why
Queued prompts and `request_user_input` answers can be persisted in a different
order from when the host accepted them. Using persistence order can therefore
reorder retained instructions during replay or apply the wrong rollback
boundary.
## What changed
- Reserve an acceptance sequence for user prompts and elicitation responses
when retained thread context is enabled.
- Persist that sequence with user-message metadata and verified answers, and
use it when ordering, bounding, replaying, and rolling back retained context.
- Keep events without acceptance metadata compatible with legacy recording
order.
## Testing
- Cover delayed queued-input recording, rejected prompts, checkpoint replay,
compaction, rollback migration, and legacy retained-context behavior.
GitOrigin-RevId: 42d24465cc2033bc51c0c2da79d3c0884a8ad98a
## Why
Directly deserializing the flattened `RolloutLine` envelope can reject nested
decimal values, preventing affected paginated sessions from resuming.
## What changed
- Add canonical string, byte, and reverse-scanner helpers that decode rollout
records through `serde_json::Value` before decoding the flattened item.
- Route rollout readers across session discovery, history, migration, search,
thread storage, and transcript previews through those helpers.
- Remove `Deserialize` from `RolloutLine` so new readers cannot bypass the
canonical persistence decoder.
## Testing
Add coverage that resumes a paginated rollout after a token-count record with
a decimal rate-limit value and verifies that ordinal sequencing continues.
GitOrigin-RevId: 49abac1e0751c073daa5a93a840d8a483fd2d013
## What changed
- Centralize the allowlist for bundled MCP cleanup hooks and use it for both local and executor-discovered plugins, including `unified-computer-use` cleanup through `cua_repl`.
- Mark matching cleanup hooks as trusted built-ins so they run without saved hook trust and remain active when regular hooks or their per-hook state are disabled. Plugin enablement and managed-only policy still apply.
- Hide built-in cleanup hooks from hook listings and lifecycle notifications while retaining their metrics. Keep the built-in classification out of serialized protocol data.
## Testing
- Cover allowlist boundaries, trust and enablement behavior, inline and file-based hook declarations, MCP success and error responses, hook listing, lifecycle notifications, metrics, and protocol serialization.
GitOrigin-RevId: f93b7bc99f4ed9694f529def8ec383b45f31430e
## Why
Resumed threads need to continue per-turn and per-thread token totals without scanning arbitrarily far beyond the latest compaction checkpoint.
## What changed
- Add durable `TokenUsageRecord` rollout items with response, turn, thread, session, and root-turn attribution.
- Restore accumulated usage on resume and snapshot the latest record plus the compaction response ID in compaction checkpoints.
- Preserve root-turn lineage in persisted turn context, while ensuring forked child threads start with their own usage totals.
## Testing
- Cover usage accumulation across multiple responses and resumed turns.
- Cover local and remote compaction checkpoints, invalid remote compaction output, rollout reconstruction, and fork isolation.
GitOrigin-RevId: ef9e0c4a9102a08a2c382be4cdac68c84353c90a
## Why
Automatic goal continuations should remain attributable to the turn that created the goal. External input, hook context, or goal edits can make that attribution ambiguous and must not leave stale lineage metadata behind.
## What changed
- Carry the trusted root and previous parent turn through successive automatic goal continuations.
- Invalidate stored lineage when external context reaches an active turn or when a goal is edited or cleared.
- Preserve lineage for async hook results only when they belong to the receiving turn.
## Testing
- Cover goal continuations across intervening user turns and external objective edits.
- Cover active, buffered, and externally injected hook or response context.
GitOrigin-RevId: 22ccc07961cd0ed8f4b17912b7efa416b342bf50
## What changed
- Use `exec_command` and `write_stdin` as the shell tool surface.
- Treat legacy `default`, `local`, and `shell_command` model metadata as `unified_exec`.
- Remove obsolete shell-selection configuration and runtime paths while preserving the feature and policy gates for zsh fork execution.
GitOrigin-RevId: d743cbe598630d73052f1fecad680c4cde17977d
## What changed
- Remove the legacy `shell_command` handler and runtime, leaving `exec_command`
and `write_stdin` as the shell execution tools.
- Treat legacy `shell_command` model metadata as `unified_exec`, and normalize
legacy user opt-outs so they do not disable command execution. Managed feature
requirements and `shell_tool` can still disable it.
- Preserve shell approvals, sandboxing, zsh-fork support, and output truncation
through the unified execution path.
## Testing
- Cover legacy configuration and model-metadata compatibility.
- Exercise unified shell execution, approvals, truncation, and `apply_patch`
serialization across the app-server and core test suites.
GitOrigin-RevId: 5c2fd6164fc3519cdae4944cb9db276b8467311c
## Why
An `apply_patch` path can be replaced with a symlink after verification, allowing an unsandboxed patch operation to reach a different file than the one that was approved.
## What changed
- Add `follow_symlinks` options to executor filesystem reads, writes, metadata lookups, directory creation, and removal, including the corresponding `followSymlinks` protocol fields.
- Implement no-follow filesystem operations on Unix and Windows that reject links in any path component and restrict file access to regular files.
- Run `apply_patch` with symlink traversal disabled when an otherwise-required sandbox is bypassed, while retaining the existing follow-symlink default for standalone callers.
## Testing
- Cover leaf and ancestor symlinks across patch add, update, delete, and move operations, including a path swap after verification.
- Exercise local and remote no-follow filesystem behavior, concurrent directory creation, special-file rejection, and Windows reparse points.

## Why
Hooks supplied as managed requirements must not be silently skipped when their
handlers cannot be loaded.
## What changed
- Fail session and app-server thread startup when an enabled managed requirement
contains an invalid matcher, an empty command, or an unsupported handler type.
- Keep load failures for ordinary managed configuration hooks as warnings, and do
not enforce managed hook requirements while the hooks feature is disabled.
## Testing
Added hook-engine, core session, and app-server coverage for valid and invalid
managed hook requirements.
GitOrigin-RevId: 91a23b0c1d85e143aba6aabe874c5274e52fc6b2
## What changed
- Add `TurnInputRequest` and typed submission results for atomically starting a turn, steering the active turn, or declining input with a specific reason.
- Expose `start_or_steer_turn`, `start_turn_if_idle`, and `steer_turn` on `CodexThread`, and migrate Core consumers to these APIs.
- Make app-server `turn/start` steer an active regular turn and return that turn's ID. Reject incompatible output schemas and non-steerable turns without applying settings or enqueueing input.
## Testing
- Cover concurrent start-or-steer submissions, accepted and rejected settings updates, output-schema compatibility, idle-start rejection, and app-server steering.
GitOrigin-RevId: dd9b5528d76ec650c019e97af420bc13190ea86a
## What changed
- Wrap response items with optional harness-owned metadata while keeping the persisted response payload shape backward compatible.
- Store compacted-history metadata in an aligned sidecar and reject malformed sidecars.
- Carry metadata through history normalization, compaction, resume, fork, truncation, migration, and rollout recording without sending it to model providers.
## Testing
- Cover legacy rollout compatibility, metadata serialization and validation, compaction retention, and resume/fork reconstruction.
GitOrigin-RevId: 17a6127c9048ff984cb4df4d97c0751a38c29f5f
## What changed
- Add `codex-history` for model-history and persisted-rollout domain types, including `RolloutItem`, `RolloutLine`, `CompactedItem`, and initial/resumed history state.
- Re-export the persisted types from `codex-rollout` and update consumers to use the new crate boundary instead of `codex-protocol`.
- Preserve existing rollout serialization, including legacy numeric compacted-window IDs.
## Testing
- Add `codex-history` tests for rollout JSON round trips, compacted-history compatibility, persisted history modes, and multi-agent version selection.
GitOrigin-RevId: 944daa9297ddd231d3aebbdcb05fff4adf8b4e1b
## Why
Hook configurations can mark command handlers as asynchronous, but Codex previously skipped those handlers outside `SessionEnd`.
## What changed
- Run asynchronous command hooks in the background with a per-session concurrency limit, while keeping `SessionEnd` hooks synchronous.
- Prevent asynchronous hooks from blocking, stopping, rewriting, or otherwise controlling the operation that launched them.
- Deliver warnings and additional context at safe turn boundaries: inject results into an active turn after sampling, or buffer them ahead of the next user prompt when the session is idle.
- Preserve in-flight hooks across configuration reloads, scope spilled output to the thread, and abort outstanding work during session shutdown.
## Testing
Add unit and integration coverage for background scheduling, concurrency, output parsing, active and idle result delivery, configuration reloads, and shutdown cleanup.
GitOrigin-RevId: 8094552e4afe7b47b09a61bb575bb20f4e491d8d
## What changed
- Run `git init` to mark command execution in the affected pre-tool hook tests.
- Keep marker repositories in per-test temporary directories and assert against
their `.git` directories.
- Check `git version` output when a code-mode hook rewrites a nested command.
GitOrigin-RevId: 71c54ef525b08e1d592f69c8b88eed1ee6403e4f
## Why
Legacy thread histories did not use the cross-process writer ownership guard already applied to paginated histories.
## What changed
- Acquire and retain a writer lock whenever a legacy or paginated thread is created or resumed.
- Apply the same ownership checks when archiving or deleting either history mode.
- Release ownership when initialization fails or the active thread shuts down, allowing another process to resume it.
## Testing
- Cover competing create and resume operations for both history modes.
- Cover archive and deletion conflicts for legacy and paginated descendants.
GitOrigin-RevId: 1be79b01966ad2634ed79eb96c0aa0db8eafe2fa
## Why
Hooks can only consume a transcript path when the thread has a local rollout.
## What changed
- Resolve the local rollout path before materializing the transcript.
- Return no transcript path for non-local thread stores without persisting them.
- Continue materializing lazy local rollouts before passing their path to hooks.
## Testing
Added session and hook tests covering both non-local thread stores and lazy local rollouts.
GitOrigin-RevId: c742984d146f7a3e1048b848198198fc7a64f4de
## What changed
- Add `StartThreadOptions::new` to provide the standard configuration for a new thread.
- Make `ThreadManager::start_thread` the single thread-start entry point and migrate callers from the previous convenience methods.
- Derive default environment selections when `environments` is `None`, while preserving explicit selections, including an empty list.
GitOrigin-RevId: 8977dc11aed54c5e1215a81eaed2b2cf5fc6087a
## Why
Mid-turn auto-compaction queued `SessionStart` hooks but continued sampling
before running them. This delayed hook-provided context and ignored requests to
stop the continuation.
## What changed
Drain pending session-start hooks immediately after successful mid-turn
auto-compaction. End the turn when a hook requests a stop; otherwise include
its additional context in the next sampling request.
## Testing
Added coverage for repeated compactions in one turn, context delivery without
leaking hooks into the next user turn, and stop requests that block sampling.
GitOrigin-RevId: c57708a792fb47d98d95c38d7d91bcd9f235be84
## What changed
- Add `additionalContextLimit` to command hook configuration for events that can emit `additionalContext`.
- Apply the limit independently to each hook's context before it is sent to the model. Unset values retain the 2,500-token default, while `0` disables spilling for that hook.
- Expose configured limits through app-server hook and requirements responses, and show them in the TUI hooks browser.
## Testing
- Cover JSON and TOML parsing, discovery and hashing, per-hook spilling behavior, app-server responses, and TUI rendering.
GitOrigin-RevId: 55ddc2dbbdf9067dceee3e1968c03617bbf6bcea
## What changed
- Add the `SessionEnd` hook event to hook configuration, protocol schemas, analytics, and the hooks browser.
- Run matching hooks for root threads during shutdown, including app-server archive, delete, idle unload, and graceful shutdown. Flush the transcript first and provide the session ID, transcript path, working directory, and `reason: "other"`.
- Keep teardown bounded: hook output is advisory, the default timeout is one second, configured timeouts are capped at three seconds, and async hooks run synchronously with a warning.
## Testing
- Cover transcript availability, matcher selection, timeout normalization, ignored control output, subagent exclusion, archive/delete ordering, and graceful shutdown of multiple loaded threads.

## Why
Policy-blocked proxy requests must terminate the corresponding active tool call and preserve the correct approval result, including when multiple calls are running concurrently.
## What changed
- Resolve a blocked request's execution ID to its registered active network call before recording the policy denial.
- Preserve an outcome already recorded for that call, and cancel the owning call after storing a new denial.
## Testing
Expand network approval integration coverage for Guardian and user decisions, permission hooks, persisted policy amendments, concurrent calls, timeouts, cancellations, and unattributed proxy requests.
GitOrigin-RevId: 961901bbc07cc498309453ec78cda1a56ab00a4b
## What changed
- Run permission-request hooks before routing approvals to the automated reviewer or user, including when `strict_auto_review` is enabled.
- Centralize approval resolution so hook, automated-reviewer, and user decisions share rejection handling and report the correct telemetry source.
## Testing
- Add an integration test showing that an allow hook can approve a shell command during strict auto-review without invoking the automated reviewer.

## Why
Core tests should branch on the executor's operating system, not on
runner details such as Docker or Wine. This keeps platform behavior
stable as new test backends are added and reserves Wine-specific skips
for actual runner debt.
## What
- Add `TestTargetOs` and target/host-aware skip helpers while keeping
`TestEnvironment` internal.
- Replace topology enum access with remote predicates and a narrow
Docker accessor.
- Migrate OS-semantic Wine skips, preserve runner-specific gaps, and
document the skip taxonomy.
## Validation
- `just test -p core_test_support`
- `just test -p codex-core
remote_test_env_can_connect_and_use_filesystem`
- `bazel test //codex-rs/core:core-all-wine-exec-test
--test_output=errors` reached test execution; unrelated existing
view-image, path, and timing failures remain.
- `just test -p codex-core` and `just test` reached broad test
execution; this checkout has unrelated helper, sandbox, and timing
failures.
The workspace denies `clippy::expect_used` in production. Although
`clippy.toml` allows `expect` in tests, Bazel Clippy compiles
integration-test helper code in a way that does not receive that
exemption, which encouraged verbose `unwrap_or_else(... panic!(...))`
and equivalent `match`/`let else` forms.
This allows `clippy::expect_used` once at each integration-test crate
root (including aggregated suites and test-support libraries), then
replaces manual panic-based Result and Option unwraps with
`expect`/`expect_err`. Standalone `tests/*.rs` files remain their own
crate roots. Intentional assertion and unexpected-variant panics remain
unchanged, and the production `expect_used = "deny"` lint remains in
place.
The cleanup is mechanical and net-negative in line count.
## Summary
Make blocking hook behavior reliable for tools invoked from code mode.
Previously, a `PostToolUse` hook could block a completed tool result,
but code mode would still return the original typed result to
JavaScript. The hook appeared blocked in hook telemetry while the
running script continued with the result.
This change:
- rejects the nested JavaScript tool promise when `PostToolUse` blocks
- normalizes `decision: "block"` and exit code 2 to the same blocking
behavior
- surfaces the hook feedback as the rejected promise's error
- adds end-to-end coverage for the relevant PreToolUse and PostToolUse
interactions
## Hook semantics in code mode
| Hook behavior | Code-mode result |
|---|---|
| PreToolUse block | Reject the promise before the tool executes |
| PreToolUse `updatedInput` | Execute the rewritten invocation and
return its result |
| PostToolUse `decision: "block"` | Execute the tool, then reject the
promise with the hook reason |
| PostToolUse exit code 2 | Same behavior as `decision: "block"` |
| PostToolUse `continue: false` | Preserve the existing feedback-only
behavior; do not reject the promise |
## Test coverage
Added or strengthened end-to-end coverage proving that:
- a PreToolUse block rejects the JavaScript promise before execution
- a PreToolUse input rewrite executes only the rewritten command
- JavaScript receives the rewritten command's result
- PostToolUse `decision: "block"` rejects after the command executes
- PostToolUse exit code 2 has the same behavior
- the hook observes the original completed tool response
- the blocked original result does not reach JavaScript
- existing direct-mode replacement behavior remains intact
- `continue: false` without a reason produces deterministic fallback
feedback
## Why
Remote compaction v2 is ready to become the default for providers that
already support remote compaction. Leaving it behind an
under-development opt-in keeps eligible sessions on the legacy
remote-compaction path.
This does not broaden provider eligibility: OpenAI and Azure move to v2,
while Bedrock and OSS providers retain their existing local-compaction
behavior.
## What changed
- Mark `remote_compaction_v2` stable and enable it by default.
- Make tests that intentionally cover legacy remote compaction
explicitly disable v2.
- Update parity coverage so v2 exercises the production default and only
legacy mode opts out.
## Verification
- `just test -p codex-core
auto_compact_runs_after_resume_when_token_usage_is_over_limit
auto_compact_counts_encrypted_reasoning_before_last_user
auto_compact_runs_when_reasoning_header_clears_between_turns
responses_lite_compact_request_uses_lite_transport_contract`
## Why
Thread cwd and environment selections are a single logical setting in
core: updating one without the other can silently desynchronize the
next-turn execution context. This change makes that relationship
explicit in the internal thread settings flow while preserving the
existing app-server public API shape.
## What changed
- Moved the cwd/environment pair through internal
`ThreadSettingsOverrides.environment_settings` instead of a top-level
internal `cwd` field.
- Kept `thread/settings/update` public params unchanged, with app-server
translating top-level `cwd` into the paired internal settings shape.
- Moved `Op::UserInput` environment overrides into thread settings so
user turns and settings updates use the same core path.
- Updated core, app-server, MCP, memories, sample, and test callsites to
construct the paired settings shape.
## Verification
- `git diff --check`
- Local test run starting after PR creation.
## Summary
- Adapts the moved `codex-cloud-config` crate from the legacy cloud
requirements endpoint to the new config bundle endpoint.
- Switches runtime consumers from `CloudRequirementsLoader` to
`CloudConfigBundleLoader` so one shared bundle supplies cloud-delivered
config and requirements.
- Removes the legacy cloud requirements domain loader path.
## Details
This intentionally keeps `codex-cloud-config` monolithic for review
lineage: the previous PR establishes the crate move, and this PR shows
the behavior change against that moved implementation. A follow-up PR
splits the module back into focused files.
The new bundle path preserves the important cloud requirements loader
semantics where intended: account-scoped signed cache, 30 minute TTL, 5
minute refresh cadence, retry/backoff, auth recovery, and fail-closed
startup loading. The cached payload changes from a single requirements
TOML string to the backend-delivered bundle, and validation rejects
malformed config or requirements fragments before cache write/use.
