## Why
Background code-mode cells can continue running after a new turn starts. Looking up grants and strict review requirements through the active turn can make those cells lose their approved access or use another turn's permissions.
## What changed
- Keep turn-scoped grants and strict review state in `TurnContext`, retained by captured steps and background cells.
- Record permission responses against the requesting turn, including requests made after a later turn starts.
- Share live session-scoped grants across turns while keeping turn-scoped grants isolated.
- Use the originating turn's permissions and review requirements for tool execution, patches, MCP calls, file uploads, and terminal input.
## Testing
Add regression coverage for background cells retaining grants across turn changes, turn versus session scope, permission requests after the next turn starts, and strict review enabled while `write_stdin` is queued.
GitOrigin-RevId: 641502fbfd43553c2c42424d4aaa53ae8020c850
## What changed
Add `turn_extension_init` to thread startup and settings, and expose `WithTurnExtensionData` for turn input, recovery, and settings requests. Accept or reject data updates together with the request that supplies them.
Each new turn captures the configured data, including automatically started turns. Accepted steering updates the data for future turns while preserving the running turn's capture. Plain requests preserve the configured data; an empty initializer clears it. The data is not persisted and must be supplied again on cold resume.
Expose `current_turn_extension_data` to retrieve typed data only when the requested turn is still running and has not been cancelled.
## Testing
Add an integration test covering initial capture, accepted and rejected steering, inheritance by an automatic turn, clearing future data without changing the active capture, and rejection of lookups for an earlier turn.
GitOrigin-RevId: dd81eda3e32c7b62d783c772221858f4c0e68264
## What changed
- Store `Arc<dyn AgentControl>` in session services and route agent tools, turn admission, usage accounting, completion notifications, and shared settings through the trait.
- Add `AgentControlInit` so delegated and guardian sessions inherit the parent's controller and local runtime.
- Move local registry and context helpers to `LocalAgentRuntime`, keep resume, close, inspection, and status subscriptions on the local controller, and simplify synchronous and infallible trait methods.
GitOrigin-RevId: ad3f213eb483ac524bde56d769f20a78f20989a0
## What changed
- Add `environments` to `TurnSettingsUpdate` to replace the selection for subsequent steps without changing future turns.
- Capture model settings and environments together so combined updates apply or reject atomically, while existing steps retain their captured selection.
- Validate environment IDs, working directories, configuration ownership, and permission requirements against the running turn's selections.
- Preserve executor startup behavior when switching environments and use the step's local working directory for tool hooks.
## Testing
Add coverage for workspace switching and restoration on the next turn, atomic model and environment updates, permission validation, executor connection waits, and replacement of pending environment selections.
GitOrigin-RevId: 02b5c57a78fd594b2aed84fc09d9e99eb97be28c
## Why
Background processes can outlive their launching turn. Their approval reviews need the originating environment's filesystem restrictions, even when the active turn uses a different environment. Remote restrictions must also be resolved using executor paths rather than the local filesystem.
## What changed
- Capture settings and environment selections together in `StepInputs`. Preserve the captured selection during active model updates while allowing environment startup to finish.
- Carry environment IDs into Guardian network and `execve` requests, and use the owning process's environment with current review settings. Refresh environment readiness for network requests that fall back to the active turn.
- Resolve remote denied paths and globs using the executor's policy context. Decline automatic approval when the request's environment is unavailable or an explicitly denied temporary directory cannot be resolved.
## Testing
Add regression coverage for active model updates retaining their environment while the next turn adopts a new selection, background approvals using their owning environment, newly ready network environments, and remote denied paths and globs, including Windows paths and missing temporary-directory metadata.
GitOrigin-RevId: b620d4b030c597a4f430fe0500864fa5469db3d4
## What changed
- Wait for a request tagged with `x-openai-subagent: guardian` before triggering cancellation, disconnection, or timeout in network approval tests.
- Read the last yielded parent request in the tool cancellation test, with an explicit error if no request was captured.
- Disable WebSockets in the issuing-step permissions test to use its HTTP mock server.
GitOrigin-RevId: 35c021e3931ff4a66ca3970f8e39e1d20257be88
## What changed
- Accept `fileId` alongside the existing `url` form for app-server image inputs, and forward file references to the Responses API as `file_id`. Update generated schemas and client types.
- Preserve file references, image detail hints, and mixed inline/file image ordering through user-message events, thread history, and rollout migration. Retain file images when truncating tool output.
- Pass file references through image preparation without resolving them, while keeping resize-notice numbering correct. Omit them from unsupported TUI display and Guardian image context.
- Reject image-edit requests whose recent-image window includes a file reference, preventing selection of an older inline image instead.
## Testing
Add coverage for serialization, request and rollout preservation, mixed-image history ordering, incomplete ordering metadata, tool-output truncation, and rejection of unsupported image-edit selections.
GitOrigin-RevId: 6ca20a8577155cc934b720803c3b7b3bffdf972a
## What changed
Introduce `ReviewRequest` in `codex-guardian-reviewer` to own contributor routing, cached approvals, synchronous fallback, and review cancellation. Keep action validation and session-specific preparation in the host.
Track the complete approval operation through parent shutdown, driving cancellation through reporting and reviewer cleanup before releasing it. Preserve fresh-review requirements and validate cached approvals before recording their outcome.
## Testing
Extend coverage for cancellation before routing and during cached approval, parent shutdown cleanup, and required fresh review overriding a cached allow result with matching assessment events and a denial warning.
GitOrigin-RevId: 8dcf26cc3660e91ef94e28e0bee7327ebe34ae6c
## What changed
- Use cancellation guards to tie reusable reviewers and temporary forks to their lifetimes, including when a review future is dropped. Replace cancelled reusable reviewers before reuse.
- Share `ReviewerTasks` between the pool and `ThreadManager`, and wait for tracked cleanup during pool shutdown. Remove the separate session shutdown protocol.
- Move denial cleanup from core task handling into Guardian's turn start, stop, and abort hooks. Expose `install_reviewer` to register both thread and turn lifecycle contributors together.
## Testing
Add an integration test that exercises stale denial cleanup at turn start, completion, and interruption, including starting another turn after interruption.
GitOrigin-RevId: 85badb235309c1e1576547d1746f7ee9fc66d545
## What changed
Replace `AttachmentStore::persist` with `upload` and `resolve`. Uploads return inline bytes or a file ID; resolution returns optional file metadata and a download URL only when a minimum URL lifetime is requested. Add image dimensions, digest, size, MIME type, and categorized errors to the API.
Make `InlineAttachmentStore` return the original bytes and report `NotFound` when resolving file IDs. Redact attachment bytes and file URLs in debug output.
Pass the configured image store from the thread manager into sessions and inherit it in delegated sessions. Allow `TestCodexBuilder` to accept a custom image store.
## Testing
Update unit coverage for preserving PNG and JPEG bytes, redacting bytes and URLs in debug output, and rejecting file resolution in the inline store.
GitOrigin-RevId: f313e0048925998d84394c7e5a278e0d39d86a45
## Why
Inline delegates have no entry in the thread registry, so reviewer creation cannot depend on looking up the parent or waiting for its thread-ready notification.
## What changed
- Capture parent identity, authentication, shared agent control, originator, and inherited instructions in `StartThreadOptions` so `ThreadManager` can start a child without a registered parent.
- Route Guardian reviewer creation through this path, remove the standalone fallback and readiness gate, and require a Guardian extension host.
- Install explicit reviewer hosts in unit tests and the Guardian reviewer extension in the integration test harness, using `ExtensionRegistry::to_builder()` to preserve existing contributors.
## Testing
Extend the thread-manager regression test to remove the parent from the registry before starting a child, then verify inherited lineage, originator, session identity, and authentication, and exclusion from the public thread list.
GitOrigin-RevId: 468ded6fdce7520cb39d55c1a884dcfa5aaea2d9
## Why
Callers need to tie isolated threads to an explicit lifetime and wait for cleanup even when startup is cancelled or its result is never received.
## What changed
- Add `ThreadManager::start_thread_until` to run an isolated thread until a caller-provided future completes or the thread exits, with a `TaskTracker` covering cleanup and deregistration.
- Retain persistence and session resources across interrupted startup so cleanup can release partially initialized resources or shut down a running session using normal history rules.
- Reject resumed history and startup without explicit session isolation.
## Testing
Add integration tests for cancellation during stalled required MCP initialization, dropping an unconsumed startup result, and owner cancellation that preserves history and parent usability. Also verify that failed duplicate startup leaves the existing thread's writer intact.
GitOrigin-RevId: 47251c1b0c43821ab8f950816c6d341d39cdc823
## Why
Running root threads retained their startup global instructions, so edits to global `AGENTS.md` files did not take effect during an active session.
## What changed
- Reload global instructions when capturing model-request context, including after tools within the same turn. Apply changes without repeating unchanged instructions or rediscovering repository instructions when the environment and trust level are unchanged.
- Preserve the last successful global instructions on read failures, suppress recurring warnings until recovery, and clear instructions when their source is removed or blank.
- Serialize refreshes and allow cancellation without blocking subsequent requests.
- Give new subagents the parent's applied instruction snapshot and update Guardian reviewer reuse to account for refreshed instructions.
## Testing
Add regression coverage for live edits and removal, read failures and recovery, warning suppression, cancellation, subagent inheritance, and Guardian reviewer reuse. Update resume, fork, and compaction tests to verify refreshed instructions.
GitOrigin-RevId: f7e9b399740fa4f45e482c070f6e901ee7cf85cd
## What changed
- Move assessment event construction, metrics, and analytics tracking into `codex-guardian-reviewer` through `ReviewReport`.
- Store denial accounting in thread extension data through `ReviewDenials`, with core retaining turn interruption and lifecycle cleanup.
- Move failed-review record selection and bounded serialization into the extension, with core supplying captured review context.
## Testing
Add coverage that denial accounting clears on turn completion and interruption. Move the oversized-record test into the extension and exercise the new API.
GitOrigin-RevId: 6e5b3d9d4128b356f99b1d0e74a68111e40261ab
## Why
Session isolation relied on Guardian source attribution. An explicit policy lets callers control inherited capabilities independently of how a session is attributed.
## What changed
- Add `SessionIsolation` with default `Inherit` and opt-in `Isolated` modes, captured at session startup through `ExtensionDataInit`.
- Use the policy to control inherited instructions, extensions, execution rules, and MCP resources. Isolated sessions retain managed execution rules and omit executor-discovered MCP servers.
- Explicitly isolate Guardian reviewers while preserving source-based fallback for older callers and saved reviewers.
## Testing
Extend delegate tests to cover isolation independently of attribution, update execution-policy coverage to supply the explicit policy, and assert that managed reviewers do not inherit the parent's configured MCP tools.
GitOrigin-RevId: f0ab42fa2f0237860ec661e72693ab191216afbf
## What changed
- Add `disabled_plugin_ids` to thread startup options, settings overrides, snapshots, and persisted turn contexts. Omitted overrides preserve the current list; an empty list clears it.
- Restore the selection on resume and capture it before truncating history for a fork. Prefer the owning thread's latest settings snapshot, falling back only to the latest turn context so stale history cannot resurrect an older selection.
- Freeze the selection at turn admission and retain compatibility with histories that lack the new fields.
## Testing
Add coverage for owned settings, explicit clears, latest-turn fallback, and legacy snapshots. Extend settings tests to cover plugin selection during runtime restoration and compaction, and verify initial turn persistence without extra settings checkpoints in both history modes.
GitOrigin-RevId: 75ffbff5763cd93cc5ac69d73b08b237f59bc009
## What changed
- Route manual and automatic remote compaction through the v2 implementation.
- Retire the `remote_compaction_v2` feature toggle while continuing to advertise it in the model client beta features header.
- Migrate compaction tests from `/responses/compact` to streamed `/responses` requests with `compaction_trigger` items and remove legacy parity coverage.
## Testing
Add streamed compaction coverage for tool search output trimming and session base instruction token estimates. Update app-server tests to verify started/completed notifications for both local and streamed remote compaction.
GitOrigin-RevId: cea392a6166a9b116ff83708fd9126d5546476e4
## Why
A remote process can finish while a network policy review is still pending. Normal process cleanup should withdraw that review without turning the completed command into a review failure or losing its output.
## What changed
- Record whether a network policy request was withdrawn because the process finished, was cancelled, lost its executor connection, or timed out.
- Treat normal process completion as cleanup while retaining fail-closed behavior for other cancellation causes.
- Preserve explicit network denials before policy persistence so cleanup cannot replace the reported call outcome.
## Testing
Add an integration test that completes a remote process during a pending network review and verifies that the command reports its successful exit and output without approving the withdrawn request.
GitOrigin-RevId: 7f42d75631ee29eba43bf04cc953eea490f553fc
## Why
Full Access already combines `approvalPolicy: "never"` with unrestricted
permissions, so confirmation-only actions do not require a model review.
## What changed
- Detect Full Access consistently across the thread and every selected
environment. Pending, failed, or restricted environments are not treated as
Full Access.
- Approve confirmation-only Guardian and MCP requests without synchronous
review, sampler prewarming, or background scoring. Cancellation, explicit
denials, and forms that require user input keep their existing behavior.
- Re-evaluate the active permission state on each turn so an existing thread
can enter or leave Full Access safely.
## Testing
Added coverage for switching approval modes, strict sensitive MCP
confirmations, failed and pending environments, and suppression of Guardian
requests and background-scoring connections.
GitOrigin-RevId: 089ab4296dde473b8e33ab8324be79c5446c46f6
## Why
Git root discovery is optional metadata work, but filesystem probes can block. They should not exhaust Tokio's blocking pool, delay runtime shutdown, or prevent later turns from observing repository changes.
## What changed
- Add a shared `GitRootDiscovery` service that coalesces concurrent lookups for the same working directory and limits probes across directories.
- Run probes on detached threads, retain in-flight work across caller cancellation, and discard completed results instead of caching them.
- Use the service for turn and memory metadata enrichment, abort unused turn enrichment when its state is dropped, and limit memory metadata waits to one second.
## Testing
Add coverage for probe sharing, capacity limits, cancellation, fresh discovery, runtime shutdown, memory timeouts, and repositories restored after startup prewarming.
GitOrigin-RevId: bca46fc263e7a12a2f69146d8a0b3e7c7e0846cb
## What changed
- Add bundled proactivity and follow-up guidance when `ReasoningEffort::Persistent` is selected.
- Allow model metadata to override or disable the guidance with `persistent_instructions`.
- Track the instructions as world state so effort and model changes replace or retire stale guidance without duplicating unchanged context.
- Tailor approval guidance to `send_user_message_async` availability, rebuild it for forked agents and rolled-back history, and exclude Guardian sessions.
## Testing
- Cover default, overridden, disabled, replaced, and retired instructions.
- Cover model changes, history transitions, agent forks, Guardian sessions, and asynchronous-message tool availability.
GitOrigin-RevId: 78e05d0335a70b385bd069b45edcd4ba570cef91
## Why
Tool actions and approval retries can outlive the step that issued them. If approval handling reads newer turn settings, a delayed action can use the wrong approval policy or reviewer.
## What changed
- Capture the approval policy and reviewer in `GuardianReviewContext` from the issuing step.
- Use step settings for command, patch, permission, sandbox-retry, and remote-network approval decisions.
- Use the active task's published settings for background network approvals and long-lived Unix shell interception.
- Preserve explicit MCP reviewer overrides.
## Testing
Added regression coverage for permission requests, same-turn network policy changes, delayed commands after a new turn starts, sandbox retries, patch policy selection, and MCP reviewer overrides.
GitOrigin-RevId: b7e84a2e1e79a71502272b15b5ef5b0e5643f8b0
## What changed
- Add the experimental `turn/start.cyberAccessProgram` option with `standard`,
`daybreakBlue`, and `daybreakRed` values.
- Forward the selection as `access_programs.cyber` on Responses, WebSocket,
and remote-compaction requests made with ChatGPT authentication, while
omitting it for API-key and custom-provider requests.
- Preserve the per-turn selection across recovery, compaction, and child-agent
turns without making it a persistent thread setting.
## Testing
- Cover app-server forwarding, authentication boundaries, WebSocket reuse,
turn recovery, compaction, and child-agent inheritance.
GitOrigin-RevId: d2eb468f365b2214c5099bc21741cf30f8bd2eb5
## Why
Thread settings can change while a turn is running, and delegated review or
compaction steps can select a different model. Each request needs a consistent
set of settings resolved against the model that will execute it.
## What changed
- Capture immutable resolved settings for each model step, including model
metadata, reasoning options, service tier, approvals, and personality.
- Keep in-flight steps on their captured settings while applying thread updates
to subsequent work.
- Resolve inherited reasoning summaries and filter service tiers against the
selected model and feature configuration for review and compaction steps.
## Testing
Added coverage for settings updates during paused turns, model changes,
previous-model compaction, review-model defaults, and service-tier filtering.
GitOrigin-RevId: fca4b3b305379f1bf26a5cac75ee0dd1747ac1f6
## What changed
- Add a `GuardianPolicy` contextual fragment that renders as a standalone
developer message with the `guardian.policy` content kind.
- Use the fragment when rebuilding Guardian context and verify that its content
kind remains attached after history compaction.
GitOrigin-RevId: f7028fb29a48ad2453437f3d3a4405fcab7a2fd7
## What changed
- Remove the legacy `shell_command` handler and runtime, leaving `exec_command`
and `write_stdin` as the shell execution tools.
- Treat legacy `shell_command` model metadata as `unified_exec`, and normalize
legacy user opt-outs so they do not disable command execution. Managed feature
requirements and `shell_tool` can still disable it.
- Preserve shell approvals, sandboxing, zsh-fork support, and output truncation
through the unified execution path.
## Testing
- Cover legacy configuration and model-metadata compatibility.
- Exercise unified shell execution, approvals, truncation, and `apply_patch`
serialization across the app-server and core test suites.
GitOrigin-RevId: 5c2fd6164fc3519cdae4944cb9db276b8467311c
## What changed
- Give `PluginsManager` a shared `AuthManager` instead of a separately mutable authentication-mode snapshot.
- Read the current authentication mode and credentials from that shared manager for plugin discovery, startup tasks, CLI commands, MCP setup, and external-agent migration.
- Update test helpers and coverage to exercise plugin projections and curated marketplace selection as authentication changes.
GitOrigin-RevId: 600c94de5130eda2da5727e1a0b4d39083fefc56
## Why
Callers need to associate host-owned state with a thread before Core starts it.
## What changed
- Add `ThreadManager::reserve_thread_id` and allow new threads to start with the reserved ID. Reject reserved IDs when resuming an existing thread.
- Let thread stores stage metadata for a reserved ID. The local store merges staged values into the first successful metadata update and then removes them.
- Clear staged metadata when an unmaterialized thread shuts down or is discarded, and require the state database for staging.
## Testing
- Cover reserved ID generation and resume validation.
- Cover metadata persistence, merging, rollout-compatible updates, cleanup, and invalid staging requests.
GitOrigin-RevId: 3f9bc48f0f7764011cc90ea01349e497dda0a9e3
## Why
Thread settings can change while a turn is still active, including between sampling requests. Those updates should apply to the next turn instead of changing the model configuration partway through the current turn.
## What changed
- Snapshot the model, reasoning settings, service tier, approval settings, and model-attributed telemetry in `StepContext`.
- Build prompts, model requests, world state, tracing, and startup prewarms from that step-scoped snapshot.
- Share immutable model metadata between turn and step contexts.
## Testing
Added an integration test that pauses an active turn, updates its thread settings, and verifies that all requests in the active turn retain the original settings while the next turn uses the updates.
GitOrigin-RevId: c51352218e463dcc9b7338566d16742292c49fbf
## Why
Environment attachments can either inherit configuration from their thread or
provide their own. Later thread setting updates must refresh inherited
configuration without overwriting attachment-owned permissions and capability
roots.
## What changed
- Resolve each attachment's configuration when it is selected and retain
whether it came from the thread or the attachment owner.
- Apply subsequent thread configuration updates only to thread-owned
attachments, while preserving ownership across snapshots and child threads.
- Keep the resolved configuration with the environment selection so runtime
consumers use a single canonical value.
## Testing
- Extend remote-environment coverage to verify that thread-owned permissions
follow thread updates while owner-provided read-only permissions remain in
effect.
- Cover configuration inheritance, attachment replacement, and owner-configured
capability roots.
GitOrigin-RevId: fe70c4be5f151432b69bf4b141e316faa89036ae
## What changed
- Preserve valid image content from completed `node_repl` Code Mode responses alongside text evidence for Guardian reviews.
- Enable multimodal evidence for models that require automatic `node_repl` review, or when both `guardian_enhanced_node_repl_transcripts` and `guardian_node_repl_transcript_images` are enabled.
- Bound retained and rendered evidence, deduplicate images already present in review history, and fall back to text-only evidence when the reviewer lacks image support or prompt capacity.
- Keep review evidence out of the parent turn input.
## Testing
- Cover feature gating, mixed text and image ordering, truncation and eviction, unsupported reviewers, prompt pressure, image detail normalization, and reused review sessions.
GitOrigin-RevId: a3e482402f7b3484401b231dadc662e6e3504fdf
## Why
The model provider does not reliably identify which curated plugin catalog is
available. ChatGPT authentication can be used with a custom provider, while an
unauthenticated session should use the API-compatible catalog regardless of its
provider.
## What changed
- Select the ChatGPT curated catalog only for authentication modes that use the
Codex backend; use the API curated catalog for API-key and unauthenticated
sessions.
- Initialize standalone plugin managers with the current authentication mode
across CLI, app-server, MCP, and external-agent migration paths.
- Preserve authentication mode while detecting and importing migrated plugins.
## Testing
- Cover catalog and skill routing across ChatGPT, API-key, unauthenticated,
Bedrock, and custom-provider configurations.
- Verify authenticated plugin migration uses the ChatGPT curated marketplace.

## What changed
- Resolve user message admission when Core accepts the input as a new turn or a steer, without waiting for rollout persistence.
- Remove persistence- and hook-specific admission errors and task bookkeeping.
- Delete queued messages after Core accepts them for turn processing, including messages subsequently stopped by a prompt hook.
## Testing
- Update concurrent admission and queue service tests for acceptance-based admission.
GitOrigin-RevId: 7dcc311cfecd3d710733b420f2add546edb86153
## What changed
- Send Unix `execve` approvals intercepted by the zsh fork through the shared approval pipeline, including permission hooks, Guardian review, user prompts, and telemetry.
- Resolve the active turn and its auto-review setting when an intercepted command needs approval, so commands sent to persistent terminals use the current turn's reviewer.
- Give each intercepted command a distinct approval ID and propagate an aborted approval as a turn abort.
## Testing
- Cover Guardian review for intercepted `unified_exec` commands and persistent terminals across turns.
- Verify repeated identical intercepted commands receive separate user approvals.
GitOrigin-RevId: e6cccf160637e4246aff4714c22f08c90b65306d
## What changed
- Inject the host skill loader into `PluginsManager` so plugin discovery and agent turns use the same loading and product-policy behavior.
- Share plugin skill snapshots across those paths, preserving a consistent view of skills across workspaces.
- Apply migrated-command precedence after product filtering, allowing an eligible migrated command to replace a filtered native skill with the same name.
## Testing
- Add coverage for product-restricted plugin skills, native-versus-migrated command precedence, and the skills exposed to agent turns.
GitOrigin-RevId: f5ef0d0766ebeeb30d73ffaf044d003c2906ea4d
## What changed
- Derive sandboxing, approval, and permission-profile environment variables from the selected turn environment for shell, unified exec, `apply_patch`, image viewing, and network approval flows.
- Materialize project-root permissions against every workspace root in that environment, allowing `apply_patch` to write to secondary workspace roots while preserving protected paths.
- Carry the selected environment's permission profile into spawned or resumed agents and Guardian permission context.
## Testing
- Add an integration test covering `apply_patch` writes in a secondary workspace root.
- Extend tool, Guardian, and multi-agent tests to distinguish environment permissions from thread-level permissions.
GitOrigin-RevId: e22da8974bcd3de1e724978b1a7790307a51e4bc
## What changed
- Remove the cached permission profile from `TurnContext` and derive the
effective profile, filesystem policy, network policy, and legacy sandbox
policy from `config.permissions`.
- Update sandbox consumers and tests to use the current turn configuration.
- Verify that role-based agent spawning reapplies runtime permissions and that
cold-resumed agents retain their disabled permission profile.
GitOrigin-RevId: d67c5d2bcbe6dc76f15b56defb485b155fd1f138
## What changed
- Move host skill discovery, snapshot caching, and configuration handling from
`codex-core-skills` to `codex-skills-extension`.
- Rename `SkillsService` and `SkillsLoadInput` to `HostSkillsService` and
`HostSkillsLoadInput` to distinguish the host implementation from other skill
providers.
- Keep shared loading primitives and skill outcome modeling in
`codex-core-skills`.
GitOrigin-RevId: d81a21791d2ef8c066e157e7e538b8cb7ee4c24b
## Why
Thread settings can update the approval policy after a turn context is created.
Keeping a separate copy on `TurnContext` could leave tool approval checks using
the previous policy.
## What changed
- Remove the duplicated approval-policy field from `TurnContext`.
- Resolve the policy through the turn's current configuration everywhere it is
needed, including tool execution, Guardian routing, MCP handling, and
permission requests.
## Testing
- Update the shell escalation test to apply a thread-level policy override and
verify that the next turn rejects an escalation using the updated policy.
GitOrigin-RevId: e5966ba08f179d53fd76871ad904762958c0f5ea
## Why
App-server clients need to advertise structured MCP extension settings, including supported MCP App UI MIME types, rather than only opting into OpenAI form elicitation with a boolean.
## What changed
- Add an `extensions` map to initialize capabilities and preserve `mcpServerOpenaiFormElicitation` as a legacy alias for `openai/form`.
- Capture the declared extension profile when a thread is started, resumed, or forked, propagate it to subagents, and advertise it to downstream MCP servers during initialization.
- Keep the profile stable for the lifetime of the loaded session instead of changing it on later turns or direct tool calls.
## Testing
- Cover extension conversion, downstream MCP initialization, session isolation, legacy form support, and subagent inheritance.
GitOrigin-RevId: fbcedbb74ce788e574b0f884a4c45c4cedb9de54
## What changed
- Move the V8 implementation into a dedicated `codex-code-mode-runtime` crate used by `codex-code-mode-host`, removing the embedded runtime fallback from the Codex process.
- Resolve the host executable from the active installation layout and check its availability before selecting tools.
- Fall back to direct tools with a one-time warning when optional code mode is unavailable. Keep `code_mode_only` and `disable_in_process_fallback` configurations fail-closed.
## Testing
- Cover host discovery for standalone and package layouts, including missing hosts and symlinks.
- Verify direct-tool fallback, one-time warnings, and fail-closed code-mode-only behavior.
GitOrigin-RevId: 5aa3c6f1db148b2231fc24089a2ee0e2b00dbddb
## Why
The `<current_date>` in environment context could diverge from the clock used by current-time features, especially when a host provides an external clock.
## What changed
- Read the current time from the session's time provider whenever world state is built, and render its local date in environment context.
- Use each session's own thread ID for clock reads, including guardian sessions.
- Propagate clock-read failures through turn error events and stop before model inference.
## Testing
- Cover date updates across turns and app-server requests.
- Verify guardian clock reads use the corresponding root or guardian thread ID and do not run during prewarm.
- Verify clock failures emit an error without sending an inference request.
GitOrigin-RevId: bb7f32ccc0229b7bc6f14d7356db00bcce78066d
## Why
`thread/fork` rejected threads using paginated history, so they could not use the same fork workflow as legacy threads.
## What changed
- Create paginated forks by referencing a frozen source-history prefix while persisting only child-owned records in the new rollout.
- Support latest, `lastTurnId`, and `beforeTurnId` boundaries, including inherited lineage, model-context reconstruction, interruption markers, and approval-reviewer inheritance.
- Make paginated reads and occurrence search traverse referenced history while excluding source records added after the fork.
- Coordinate fork preparation with archive and delete operations, and materialize compressed rollouts before they become fork references.
## Testing
Added app-server and thread-store coverage for boundary selection, active turns, inherited reads and search, compressed lineage, and concurrent source lifecycle operations.

## Why
Guardian review commands can run without the proxy-port environment used to
configure the parent session. Reconciling persistent Windows sandbox settings
for those commands can discard the parent's proxy configuration.
## What changed
- Add a session-level Windows sandbox proxy-settings mode and use `Preserve`
for guardian review sessions while keeping `Reconcile` as the default.
- Carry the mode through unified exec and the exec-server sandbox context to
Windows process launches.
- In preserve mode, reuse the proxy settings recorded by the existing sandbox
setup marker.
## Testing
Added coverage for guardian session configuration, exec-server transport, and
preserving an existing setup marker when proxy-port environment variables are
absent.

## Why
Startup prewarms and Guardian review sessions do not need workspace Git metadata. Collecting it for those requests performs redundant work, while user turns still need a fresh view of the repository.
## What changed
- Add a per-session Git enrichment policy.
- Skip enrichment for startup prewarm, preview, and Guardian review turns.
- Keep fresh enrichment enabled for regular sessions and one-shot delegated turns.
## Testing
- Cover prewarm and Guardian requests omitting workspace metadata while subsequent user turns observe repository changes.
- Cover concurrent repositories and worktrees retaining distinct metadata.
- Verify concurrent enrichment is coalesced and canceled enrichment can be retried.

## What changed
- Add an `ExtensionData` store to each `StepContext`.
- Pass the step store to context, world-state, turn-input, and tool contributors so extensions can use capabilities bound to the current sampling step.
- Preserve the captured step context when rebuilding initial context during compaction.

## What changed
- Replace the combined `Codex` wrapper with an `Arc<Session>` and a dedicated
`SessionIo` handle for submissions, events, status, and loop termination.
- Make `Session::spawn` return the session and I/O handle separately, and have
`CodexThread` own both.
- Update thread management, delegated sessions, guardian reviews, and agent
control to use the appropriate session or I/O interface.

## What changed
- Add an optional `policy_template` field to auto-review model messages.
- Build Guardian instructions from the catalog template when present, while
preserving the existing precedence for managed and catalog policy content.
- Fall back independently to the bundled template and bundled policy, and
preserve explicit empty catalog values.
## Testing
- Cover missing and empty template deserialization, policy precedence, bundled
fallbacks, and Guardian prewarm instructions.

## Why
Multi-environment turns can run against different filesystems, so a single
thread-wide workspace-root list cannot accurately describe every selected
environment.
## What changed
- Add optional `runtimeWorkspaceRoots` to each app-server environment selection.
Omitted roots default to that environment's `cwd`, while an empty list selects
no roots.
- Carry environment-native roots through turn selection, model-visible context,
permission materialization, and tool sandboxing.
- Keep the top-level `runtimeWorkspaceRoots` field as a compatibility input when
constructing default environment selections; explicit selections use their
own roots.
## Testing
- Cover thread and turn API defaults, overrides, empty roots, and schema
round-tripping.
- Verify foreign roots remain model-visible and remote execution applies the
target environment's roots before sandbox selection.
