ee46c5ba0e Negotiate MCP extensions per app-server session (#36910)
## Why

App-server clients need to advertise structured MCP extension settings, including supported MCP App UI MIME types, rather than only opting into OpenAI form elicitation with a boolean.

## What changed

- Add an `extensions` map to initialize capabilities and preserve `mcpServerOpenaiFormElicitation` as a legacy alias for `openai/form`.
- Capture the declared extension profile when a thread is started, resumed, or forked, propagate it to subagents, and advertise it to downstream MCP servers during initialization.
- Keep the profile stable for the lifetime of the loaded session instead of changing it on later turns or direct tool calls.

## Testing

- Cover extension conversion, downstream MCP initialization, session isolation, legacy form support, and subagent inheritance.

Client-facing command execution items can include recognizable secrets in the
rendered command or parsed command actions.

## What changed

- Redact secrets in `commandExecution.command` and `commandActions` for live
  items, completed items, and replayed thread history.
- Keep command approval requests backed by the original executable command and
  parsed actions while using the redacted representation for display items.
- Document that execution item commands are redacted display values rather than
  executable commands.

## Testing

- Cover redaction for command conversion, parsed search actions, legacy replay,
  approval flows, completion notifications, and rejected commands.

e2c0837923 Make user input blocking behavior explicit (#36410)
## Why

Clients need to know whether a `request_user_input` request must wait for an
explicit response or may auto-resolve. Using `autoResolutionMs` as that signal
conflated the blocking decision with timeout policy.

## What changed

- Add required `isBlocking` fields to user input protocol and app-server
  request payloads. Plan-mode requests are blocking, while requests from other
  enabled modes are non-blocking.
- Drive the TUI's auto-resolution behavior from `isBlocking` and remove
  `autoResolutionMs` from the model-facing tool schema.
- Deprecate `autoResolutionMs` while retaining it for compatibility, and treat
  legacy payloads without `isBlocking` as blocking.

## Testing

- Cover mode-derived blocking behavior, legacy deserialization, app-server
  forwarding, delegated requests, and TUI auto-resolution.

a01a2d9146 Preserve executor paths in read command actions (#36223)
## Why

Read command actions were omitted when a selected environment used a path
convention foreign to the app-server host. Clients need these actions to refer
to files in the executor's filesystem, not paths interpreted on the host.

## What changed

- Resolve read paths against the executor's `PathUri` and serialize them using
  the executor's native POSIX, Windows drive, or UNC convention.
- Accept same-drive Windows-relative paths while continuing to reject paths
  that require another drive's executor-local current directory.
- Document the path convention used by `commandExecution` notifications.

## Testing

Cover native and cross-platform path serialization, Windows drive-relative
joins, and selected-environment `item/started` and `item/completed`
notifications.

acd540f158 Precompute app-server protocol exports (#36212)
## Why

Normal app-server protocol builds do not need the `ts-rs` and `schemars`
implementations used to generate schema exports.

## What changed

- Embed compressed stable and experimental TypeScript and JSON schema exports,
  and serve the existing export APIs from those artifacts.
- Use no-op schema derives in non-test builds while keeping the real generators
  available for fixture regeneration and tests.
- Move schema fixture regeneration behind a Python helper and update
  `just write-app-server-schema --experimental` to refresh the experimental
  embedded exports.

## Testing

- Verify embedded exports match freshly generated stable and experimental
  schemas.
- Verify export options and on-disk output remain compatible.

OAuth discovery failures do not establish that an MCP server lacks OAuth
support. Reporting those failures as `unsupported` conflates an inconclusive
check with a confirmed result.

## What changed

- Add an `unknown` MCP authentication status across the protocol, app server,
  CLI, and TUI.
- Preserve OAuth discovery errors so callers can report `unknown`, while
  retaining `unsupported` for servers known not to support OAuth.
- Document the distinction in the app server API.

## Testing

- Verify transient HTTP discovery errors are preserved.
- Verify `codex mcp list --json` reports `unknown` when discovery is rate
  limited.

ad6fc66b6d Add persisted manual ordering for thread sections (#36007)
## What changed

- Add `thread/section/move` to atomically move a thread into, within, or out of a section. Threads can be inserted before an existing member or appended, and moves within a section preserve `sectionEnteredAt`.
- Add `section_position` sorting to `thread/list`, with ascending order as its default, and expose `sectionEnteredAt` in thread responses.
- Persist section positions and entry times in SQLite, including migration of existing section members into recency order. Section membership is no longer updated through `thread/metadata/update`.

## Testing

- Cover section moves, reordering, pagination, persistence across restarts and rollout reconciliation, concurrent updates, and rank renumbering.

- Keep `ClientResponsePayload` typed through the app-server outgoing queue and serialize it at the transport boundary, avoiding an intermediate `serde_json::Value` while preserving the JSON-RPC wire shape.
- Return a JSON-RPC internal error when a response cannot be serialized, including over remote control, and skip unserializable analytics responses.
- Scan reverse JSONL input a chunk at a time with a larger buffer instead of processing each byte individually.

101d6b8cb2 Preserve foreign paths in background terminal listings (#35850)
## Why

Background terminal working directories can use a path convention from a
different platform than the app-server host. Converting them to host-native
absolute paths can reject otherwise valid terminal entries and fail the list
request.

## What changed

- Represent `ThreadBackgroundTerminal.cwd` with `LegacyAppPathString` at the
  protocol boundary.
- Convert terminal `PathUri` values directly to their inferred native path
  strings instead of validating them against the host platform.

## Testing

Add protocol round-trip coverage for POSIX, Windows drive, and Windows UNC
working directories.

9ca6df68da Preserve terminal turn errors in replayed history (#35524)
## Why

When rebuilding a thread from rollout events, errors embedded in turn completion
events were ignored. Failed retries could therefore be restored as completed
turns, causing warnings such as model-overload errors to disappear from the TUI
transcript.

## What changed

- Mark turns with terminal completion errors as failed and retain their message
  and structured error information.
- Apply late completion errors to their matching historical turn without
  interrupting the active turn.

## Testing

Added history-builder coverage for current and late completion errors, plus a
TUI replay snapshot covering repeated overload failures.

808d3c2702 Keep session defaults static during config batch writes (#34940)
## What changed

- Treat batch writes containing only `model`, `model_reasoning_effort`,
  `plan_mode_reasoning_effort`, `service_tier`, or `personality` as defaults for
  future sessions, even when `reloadUserConfig` is requested.
- Keep runtime config reloads and skill/plugin cache invalidation for other
  config changes, and document which settings remain session-static.
- Preserve cached skill listings after session-default writes until a forced
  reload, with test coverage for each supported default.

b72079a2cf Use batch metadata for plugin app summaries (#34851)
## What changed

- Load app metadata for plugin read and install responses through the authenticated batch API, splitting requests into batches of 100 and retaining cached metadata if a batch fails.
- Preserve every declared app in plugin responses, using its ID as the fallback name and retaining categories from the plugin declaration when metadata is unavailable.
- Add `isEnabled`, `disabledReason`, and `isReadOnly` to `AppToolSummary`, with defaults that keep legacy responses compatible.

## Testing

- Cover plugin reads with more than 100 apps, partial batch failures, install-time authentication results, tool-state propagation, and legacy tool summaries.

400ee190c3 Add persisted thread pinning to the app server (#34840)

- Add `isPinned` to thread responses and allow `thread/metadata/update` to pin or unpin stored threads.
- Add an `isPinned` filter to `thread/list`, including cursor-based pagination and combinations with relationship filters.
- Persist pin state in SQLite without modifying rollout files, default existing threads to unpinned, and preserve pins through reconciliation and archive transitions.

4ebd976312 Support configurable realtime BEM channel prefixes (#34816)
## What changed

- Route realtime V3 `bemTags` handoffs using `[ANALYSIS]`, `[COMMENTARY]`, and `[FINAL]` prefixes by default.
- Add `codexResponseHandoffChannelPrefixes` to realtime start requests so clients can replace the accepted prefixes for individual channels. Each channel may accept multiple prefixes, and omitted channels retain their defaults.
- Apply the configured prefixes to both complete and streamed handoff messages, without treating empty prefixes as matches.

## Testing

- Cover request deserialization, default and custom routing, partial streamed prefixes, empty-prefix handling, and end-to-end V3 handoffs.

e370d23691 Reduce typed app-server request serialization overhead (#34766)

- Use `ClientRequest::method_name()` directly when preparing typed request
  diagnostics for in-process and remote clients.
- Allocate the owned method name only when constructing an error.
- Remove the serialization-based method lookup and the redundant allocating

f69dc49b15 Reduce app-server JSON serialization overhead (#34761)
## What changed

- Decode `JSONRPCRequest` into `ClientRequest` without serializing the entire request back into an intermediate JSON value.
- Define every client method's wire name explicitly and use it directly for request, response, and tracing method lookups.
- Serialize outgoing transport messages directly to JSON strings instead of first building a `serde_json::Value`.

## Testing

- Add regression coverage that compares direct request conversion with Serde decoding, including omitted and null parameters, legacy methods, and unknown methods.
- Verify that direct outgoing-message serialization preserves the notification wire shape.

## Why

Default read-only protections for project metadata should apply when paths such
as `.git`, `.agents`, and `.codex` exist, without causing sandbox setup to
materialize missing paths as ACL targets.

## What changed

- Add an optional `missing_path_behavior` to filesystem sandbox entries and
  mark default project-metadata protections with `skip`.
- Preserve the behavior through permission transforms and exec/MCP protocol
  serialization while keeping existing path wire variants stable.
- Ignore skip-missing entries when projecting configuration or Windows sandbox
  overrides, while retaining explicit metadata carveouts.

## Testing

- Cover protocol round trips for path and special-path entries.
- Verify default metadata protections and Windows explicit carveout handling.

ee71c4a90f Enforce exact values from managed config requirements (#34597)
## What changed

- Apply managed values for `sqlite_home`, `log_dir`, `model_catalog_json`,
  `check_for_update_on_startup`, `allow_login_shell`, the report-submission
  opt-in, and `windows.sandbox_private_desktop` to the runtime configuration,
  with source-aware warnings when they override configured values.
- Expose these values through `configRequirements/read`, return them from
  `config/read`, include them in session config locks, and show their sources in
  the TUI config debug view.
- Reject app-server writes that overlap an exact managed requirement with
  `configRequirementReadonly` while leaving unrelated sibling keys writable.

## Testing

- Cover runtime overrides, requirement API mapping, config reads and writes,
  session lock serialization, cross-platform path URIs, and debug output.

e52c35b000 Propagate approval rejection reasons (#34400)

- Change `ReviewDecision::Denied` to carry a rejection string and update the generated protocol schemas.
- Preserve specific rejection reasons through command, patch, network, MCP, delegated, and automatic approval flows so tool results can return them to the model.
- Distinguish invalid approval responses from user declines and truncate oversized rejection messages before adding them to model context.

## Testing

- Add coverage for denial serialization, invalid app-server approval responses, dropped automatic reviews, network rejection propagation, and rejection-message truncation.
