## Why
API keys with FedRAMP issuance markers previously used commercial endpoint defaults without a FedRAMP request header.
## What changed
- Recognize `sk-proj-fedramp-` and `sk-svcacct-fedramp-` prefixes as routing hints and send `X-OpenAI-Fedramp: true`, preserving the complete key for authentication.
- Select `https://gov.api.openai.com/v1` for default OpenAI API routing and `https://gov.chatgpt.com/backend-api/` for default ChatGPT backend requests, including model discovery and analytics.
- Preserve explicit endpoint overrides, including an explicitly configured commercial default URL.
- Resolve provider routing and authentication from the same credential snapshot, and apply key-dependent routing to realtime WebSocket startup.
## Testing
Add regression coverage for prefix recognition, unchanged key storage and authorization, provider credential precedence, endpoint overrides, credential rotation, model discovery, analytics routing, and realtime request preparation.

## Why
Synchronous auth header construction could silently return no credentials when managed authentication became unavailable. Callers need an error to avoid sending requests with missing or partial authentication.
## What changed
- Make `AuthProvider::add_auth_headers`, `to_auth_headers`, and auth header telemetry fallible, and propagate failures through request and WebSocket setup.
- Return a transient error when managed authentication is unavailable, and preserve network policy denials through auth error conversion.
- Retry transient MCP authentication errors while treating build errors and policy denials as terminal.
- Cancel local MCP event streams before resolving authentication so cleanup still works when credentials become unavailable.
## Testing
Add regression coverage for rejected partial authentication, async header resolution, terminal auth errors without sending requests, managed account changes, MCP retry classification, and local stream cancellation after authentication becomes unavailable.

## Why
Rotating header credentials should not discard the model catalog when a stable account ID is available.
## What changed
- Use the account ID from `CodexAuth::Headers` instead of opaque auth headers in the catalog cache identity. Provider settings remain part of the key.
- Retain credential-sensitive keys when the account ID is missing or the provider uses an explicit API key, bearer token, or command auth.
- Document that hosts using header auth must isolate model managers and cache storage by user and catalog-varying request context, since an account ID can represent a shared workspace.
## Testing
Update the auth-rotation test fixture to omit stable account metadata, preserving coverage for catalog invalidation when opaque credentials change.

## Why
Providers need to serve Codex model metadata independently of their inference endpoint. A custom inference `base_url` alone should not opt API-key sessions into remote catalog discovery.
## What changed
- Add `model_catalog_url` to provider configuration and thread-config serialization. Fetch the full catalog URL with provider authentication, headers, query parameters, and `client_version`, while preserving inference routing.
- Require an explicit catalog URL for API-key discovery with a custom base URL. Keep default OpenAI discovery on the Codex backend, and apply `api_key_model_discovery` gating to provider API keys and cached catalogs.
- Include the catalog URL in cache identity so different catalogs do not share cached metadata.
- Limit explicitly configured catalog responses to 1 MiB, reject redirects, and suppress URL and response diagnostics that could expose credentials.
## Testing
Add coverage for catalog configuration round-trips, query encoding, authentication and header reuse, cache identity and discovery gating, oversized responses, and redirect rejection without credential forwarding. Add an integration test verifying that catalog model metadata and instructions reach conversation requests.

## Why
Providers configured with `gateway_oauth` need gateway credentials alongside primary authentication for inference and model discovery.
## What changed
- Attach gateway tokens through the configured header or cookie while preserving primary authentication, including WebSocket handshake headers.
- Share gateway credential managers across matching provider instances and model discovery so they observe refreshed tokens.
- Reject authentication on gateway setup or token failures, invalid token values, and conflicting auth headers. Mark gateway headers sensitive and avoid exposing issuer error details.
- Include gateway OAuth configuration in model catalog cache identity to prevent reuse across different gateway configurations.
## Testing
Add provider and core integration tests covering combined credentials, header and cookie delivery, token refresh, shared credential state, cache isolation, and request blocking on gateway token or HTTP client initialization failures.

## Why
Realtime connections construct API providers directly, bypassing the residency override applied by higher-level model provider code. Configured provider headers could therefore take precedence over managed residency requirements.
## What changed
Apply the shared process-wide residency requirement in `ModelProviderInfo::to_api_provider`. With `enforce_residency = "us"`, the managed value overrides both static and environment-supplied residency headers, including for realtime WebSocket connections and WebRTC calls and sideband connections. Preserve unrelated headers and configured residency values when no managed requirement is set.
Share the residency policy with default HTTP headers, remove redundant enforcement at callers, and recover the stored policy from poisoned locks.
## Testing
Add a provider unit test for managed and unmanaged header behavior without mutating provider configuration. Add realtime integration coverage across WebSocket and WebRTC transports, managed and unmanaged residency, and static and environment-supplied headers.

## What changed
Add a SHA-256 identity derived from provider routing, headers, and authentication scope. Return it with each model catalog response and persist it in `ModelsCacheEntry`. ChatGPT credentials with stable account and user metadata retain the same identity across token refreshes; opaque API credentials contribute to the digest.
## Testing
Add identity tests covering account, user, email, plan, auth mode, provider routing, headers, and API credential changes, plus stability across ChatGPT token refreshes. Update cache tests to include the persisted identity.
