## Why
Older executors sandbox reads whenever a policy is supplied, even when the policy allows unrestricted reads. This can interfere with loading skills across executor versions.
## What changed
- Omit sandbox contexts from remote read operations when reads are unrestricted and explicit permission paths are valid for the target path convention. Retain contexts that require executor-side path validation.
- Check remote `windows_mxc` support before sending MXC sandbox contexts for reads or capability discovery, returning an update-executor error when unsupported.
## Testing
Add coverage for permission path validation across read operations, unsupported MXC requests, and skill loading over Noise with mismatched app and executor versions, including selected skill roots with capability discovery disabled.

## Why
Executors older than `0.164.0` can advertise V2 discovery support without implementing the final contract. Enabling V2 also suppressed V1 discovery for incompatible executors, hiding selected capability roots and skills.
## What changed
Require both the V2 capability flag and an executor version of at least `0.164.0` before issuing `capabilities/discoverV2`. Share this compatibility check between session discovery and the exec-server client. Fall back to V1 when incompatible executors provide no V2 snapshots, clearing any previously stored V2 inventory.
## Testing
Add coverage for version boundaries, prereleases, missing and malformed versions, and transport recovery for compatible executors. Verify that an older executor retains selected plugins and skill instructions through V1 with V2 enabled.

## What changed
- Add the disabled-by-default `executor_capability_discovery_v2` feature to acquire `capabilities/discoverV2` inventories from supporting remote executors using each selected environment's working directory and sandbox.
- Share snapshots with MCP and world-state contributors and skill providers without activating discovered plugins or publishing discovered skills to the model.
- Cache inventories by environment instance, executor session, working directory, and sandbox. Coalesce concurrent requests, refresh the cache at turn start, retry transient failures, and refresh MCP projections after recovery.
- Resolve the system skills root in the executor and pass it explicitly to capability location discovery.
## Testing
Add coverage for feature gating, unsupported executors, inert inventories, cache reuse and eviction, turn refreshes, failure recovery, executor replacement, and system skill locations.

## What changed
- Register `capabilities/discoverV2` for initialized sessions and expose it through `ExecServerClient` and remote `Environment` APIs, checking executor support before calling it.
- Reuse startup discovery locations and retry failed prewarming on demand. Replay discovery once after executor recovery while preserving the request parameters.
- Track whether all required filesystem watches registered successfully and reload locations and metadata on each request when registration fails.
## Testing
Add coverage for RPC metadata refresh, omission of skill instruction bodies, sandbox isolation, startup scan recovery, disconnect replay, and successful or failed watch registration.

## What changed
- Accept per-environment `skills.required` lists through `environment/add` and `environments.toml`.
- Before inference, require each named skill to be enabled and supplied by the selected environment. Missing skills fail the turn before a model request, including when environment setup fails.
- Stop applying an environment's requirements after it is deselected. Exempt isolated Guardian reviewers, which have no skill catalog.
## Testing
Add tests for available, missing, disabled, and wrong-environment skills, failed environment setup, recovery after deselection, and propagation of TOML requirements to the correct environment.

## Why
App-server clients need a way to connect to executors that require bearer authentication through `environment/add` and `environments.toml`.
## What changed
- Add optional `authBearerToken` to `environment/add` and `auth_bearer_token` to URL entries in `environments.toml`.
- Send the token as an `Authorization: Bearer` header on initial connections and reconnects, without automatic refresh. Require `wss://` or a loopback destination when a token is supplied.
- Redact tokens in debug output, mark connection headers sensitive, and omit source text from TOML parse errors.
- Preserve unauthenticated behavior when tokens are omitted, including requests with a null `authBearerToken`.
## Testing
Add integration coverage for authenticated and unauthenticated executors through both RPC and TOML configuration, including missing, null, incorrect, and malformed RPC tokens. Extend tests for reconnect authorization headers, token redaction, URL-only token configuration, and timeout preservation.

## What changed
- Add `RemoteEnvironmentOptions` and `upsert_environment_with_options` so embedding hosts can attach trusted HTTP headers to remote exec-server WebSocket handshakes.
- Preserve the headers across session reconnects while redacting them from debug output.
- Reject invalid, duplicate, and WebSocket-controlled headers, and require `wss://` for non-loopback destinations.
- Connect header-bearing loopback WebSockets directly, with DNS results restricted to loopback addresses.
## Testing
- Cover header validation and redaction, delivery on initial connections and reconnects, and loopback destination enforcement.

## What changed
- Add `EnvironmentManager::from_accepted_websocket` so embedding hosts can
construct a remote environment from an already accepted and authenticated
Axum WebSocket.
- Add `replace_accepted_websocket` to retire the current transport and resume
the same exec-server session on a host-supplied replacement connection.
- Serialize replacement handoffs, reject overlapping replacements, and release
the handoff claim when a replacement attempt is cancelled or fails.
## Testing
- Cover initial connection validation and immediate environment readiness.
- Verify replacement retry behavior and recovery of a running process and its
output after reconnecting.
