Chris Dong 6cfe299849 Preserve image generation request IDs on failure (#47327)
## Why

Failed image requests discarded the `x-codex-imagegen-request-id` header, leaving failure analytics without the request ID.

## What changed

Carry the request ID through `ImageRequestError` and backend errors for image generation and editing. Preserve it on failed image generation items for HTTP errors, response decoding errors, and responses with no image data.

## Testing

Extend the app-server failure test to verify that a request ID returned with an HTTP 500 response reaches `codex_image_generation_event` analytics.

viyatb-oai 2c2a42e65d Restrict MCP OAuth authorization endpoints to HTTP(S) (#47326)
## What changed

Reject authorization endpoints with schemes other than `http` or `https` before client registration or authorization URL return. Document this constraint for `mcpServer/oauth/login`.

## Testing

Make existing asynchronous tests wait for cleanup: wait for `ThreadIdle` before injecting user input in the guardian cached-score test, and poll for queue consumption before asserting that dispatched items are gone.

Owen Lin 286d4ecf44 Persist resume metadata in compaction checkpoints (#47323)
## Why

Compaction companion records do not represent all resume state, including the last started turn and previous turn settings. The selected multi-agent runtime also needs a persisted value when no companion turn context is present.

## What changed

- Add optional `resume_metadata` to `CompactedItem`, recording the multi-agent version, last started turn ID, and previous turn settings. Preserve the distinction between absent metadata and explicitly empty values while retaining compatibility with older records.
- Capture this metadata under the settings persistence lock alongside the compaction checkpoint.
- Rewrite inherited metadata to use the child agent's runtime, and clear previous turn settings when the child must rebuild context.
- Clear pending continuation state before delivering acceptance of standalone settings updates, while holding the persistence lock.

## Testing

Add coverage for metadata round trips with empty values, legacy reader compatibility, checkpoint companion records, inherited fork metadata, and continuation invalidation while settings event delivery is blocked.

Charlie Marsh 4f6c106cdd Use compact display metadata for Guardian review threads (#47302)
## Why

Guardian review threads use synthetic approval prompts as user messages. Deriving thread titles and previews from those prompts exposes large, unhelpful summaries in thread listings.

## What changed

- Default Guardian threads to the title "Guardian review" and preview "Approval review" across metadata creation, rollout reads, and resume.
- Skip synthetic user messages and goal updates when deriving Guardian display metadata, leaving `first_user_message` unset for new threads.
- Preserve custom names, including legacy indexed names, when listing, reading, resuming, and migrating threads to paginated history.

## Testing

Add regression coverage for compact metadata, resume without session metadata, custom names before and after migration, and app-server persistence, listing, and rename/resume behavior.

felixxia-oai 99784bd09b Allow Guardian sync review across compaction hash differences (#47272)
## Why

Thread-owned Guardian review rejected usable parent compaction checkpoints when the reviewer's `comp_hash` differed or was unknown, even though the synchronous reviewer can consume those checkpoints.

## What changed

Remove the synchronous reviewer's compaction hash compatibility check and the associated hash plumbing. Pass usable checkpoints to the backend for payload validation, while continuing to fail closed on unusable checkpoints and review errors.

## Testing

Update compaction and resume tests to expect retained evidence with different, missing, or empty reviewer hashes, while preserving rejection of empty checkpoint content.

victor-openai a1f40f3f13 Expose hosted plugin extensions in app-server summaries (#47263)
## What changed

- Request installed-plugin metadata with `includeExtensions=true` and carry optional `extensions` through the cache into `PluginSummary`.
- Add typed declarations for global, settings, thread, and file entrypoints, quick actions, settings tools, icons, and search mention providers. Update JSON schemas and TypeScript and Python bindings.
- Accept supported snake_case aliases and emit camelCase fields. Discard incompatible extension declarations without invalidating the installed-plugin inventory.

## Testing

Add an integration test covering extension metadata preservation, field normalization, and unknown entrypoint and quick-action types across live and cached `plugin/installed` reads. Update pagination assertions to require `includeExtensions=true`.

jif d3e5840932 Subscribe authors to message-board channels created by posting (#47259)
## Why

Posting to a new channel subscribed its author to replies on that post, but not to new discussion roots in the channel. The creator could therefore miss notifications for subsequent discussions.

## What changed

Subscribe the author to the channel in the same transaction that creates the channel and its first post. Document this behavior in the API and the `new_channel_name` tool parameter.

## Testing

Update coverage to verify that channel creators receive notifications without explicit targeting and that posting to an existing channel preserves an explicit unsubscribe. Adjust thread-deletion test expectations for the additional persisted subscription.

victor-openai 3b50349e1a Add explicit app and account targets to MCP resource reads (#47248)
## What changed

Allow `mcpServer/resource/read` to specify `target: {connectorId, linkId}` for direct hosted app reads without tool discovery, when supported by the backend. Require an explicit `linkId`: a string selects an account, while `null` requests no-auth access subject to backend policy.

Restrict direct targets to `codex_apps` and reject empty connector IDs, empty link IDs, and synthetic link IDs. Preserve originating app/account scope when `originCallId` and `threadId` are supplied, and retain discovery when `target` is omitted.

Update protocol schemas and TypeScript and Python models, preserving required-but-nullable `linkId` semantics in Python code generation.

## Testing

Extend resource-read tests to check target metadata with and without a thread, including explicit no-auth targets and omitted targets. Add Python model tests for string and null `linkId` values and rejection of a missing `linkId`.

alexsong-oai 064e701b0f Add explicit gateway OAuth sign-in to app-server (#47207)
## Why

Clients with a gateway sign-in UI need to control when browser authorization starts and inspect credential readiness before making authenticated requests.

## What changed

- Add `initialize.capabilities.explicitGatewayOauth` to require explicit login while allowing existing credentials to refresh. Preserve automatic authorization for clients that omit the capability, and prevent later connections from undoing explicit opt-in.
- Add `account/gatewayOAuth/read`, `account/gatewayOAuth/login`, and `account/gatewayOAuth/cancel`, plus `account/gatewayOAuth/changed` notifications. Send the authorization URL only to the initiating connection and cancel its login on disconnect.
- Check gateway authentication before returning cached results from `model/list`, with restart guidance when provider settings have changed.
- Update protocol schemas, TypeScript and Python bindings, and document capability probing and the sign-in lifecycle.

## Testing

Add tests for passive readiness reads, credential reuse after login, token exchange failures, cancellation and immediate retry, connection ownership, legacy versus explicit login, and model-list authentication checks.

acrognale-oai 30daed37ad Apply workspace routing to Guardian v2 classifier requests (#47162)
## Why

Classifier requests need to preserve their owning thread's workspace routing and account binding, including when reusing pooled connections or retrying after authentication changes.

## What changed

- Share the thread's `WorkspaceRoutingContext` with Guardian v2 and resolve routing before leasing HTTP or pooled WebSocket connections.
- Reuse WebSockets only when their connection key and authentication state still match. Cache HTTP transports by URL and redirect policy, rejecting redirects when routing requires it.
- Stop classification requests and retries when the account owner changes while allowing token refreshes for the same user and workspace.

## Testing

Add regression coverage for routing failures with prewarmed sockets, shared thread routing state, account switches during HTTP and WebSocket requests, same-account token refreshes, and HTTP redirect-policy cache changes.

Alex Kotliarskyi b23324986d Expose HTTP origins in MCP server status (#47159)
## What changed

Add nullable `httpOrigin` to `McpServerStatus`, derived from the effective configured endpoint for both configured and plugin-provided servers. Return only the HTTP(S) origin, excluding credentials, path, query, and fragment; return `null` for stdio transports, invalid URLs, and non-HTTP(S) schemes. Update the generated protocol schemas and TypeScript and Python types.

## Testing

Extend status integration tests to cover configured and plugin-provided HTTP servers, verify that endpoint paths and query parameters are omitted, and assert `null` for a stdio server. Update protocol tests to check null serialization and compatibility with older inventory responses.

Jesse Du 3fd5160cd6 Honor the configured product SKU in remote plugin requests (#47116)
## Why

Remote plugin requests always sent `OAI-Product-Sku: codex`, ignoring the existing `apps_mcp_product_sku` setting.

## What changed

Pass `apps_mcp_product_sku` through plugin configuration and use it for authenticated remote plugin requests, including discovery, search, installation, and sharing. Keep `codex` as the default and include the SKU when comparing remote service configurations.

## Testing

Extend app-server plugin listing and core agent-turn tests to check default and configured SKU headers. Update the CLI worktree test to wait for the user prompt's model request, skipping background title generation requests.

Jeremy Rose 23ae65bb06 Persist thread creator identity in rollouts and SQLite (#47113)
## What changed

- Capture optional `creator_user_id` and `creator_account_id` from authentication when creating a thread, including forks.
- Carry these fields through thread storage, rollout metadata, and a SQLite migration. Leave them absent when unavailable or when reading older threads.
- Preserve recorded creator values across resume and metadata updates. On revert, recover missing rollout values from SQLite before writing replacement metadata.

## Testing

Add coverage for ChatGPT and API-key sessions, resume under another account, forks using current authentication, metadata extraction and upserts, and repeated reverts after creator fields are removed from a rollout.

Anthony Tafoya f3037cafd3 Refresh host-supplied cloud skills at turn startup (#47074)
## What changed

- Replace the built-in orchestrator skill provider with a host-supplied cloud provider, using `cloud` authority and `c` root aliases in skill tools and prompts.
- Add `cloud.skills.enabled`, defaulting to true but requiring a supplied provider. Keep `orchestrator.skills` accepted as a legacy no-op setting, and remove the app server's automatic orchestrator provider registration.
- Discover cloud skills once per turn during cancellable startup. Serve catalog snapshots to prompts and tools without triggering discovery or retries.
- Scope cached catalogs and resources to authentication state. Preserve them on refresh failures within the same scope, invalidate them when that scope changes, and reject late discovery or read results from replaced cache generations.
- Reuse executor discovery catalogs only when successful discovery inputs and bundled-skill settings match.

## Testing

Add coverage for per-turn refresh, skill removal, same-account failures and reconnects, authentication changes, and stale in-flight results. Update configuration and tool tests for cloud authority, provider gating, and resource reads without an executor.

jif 40eeb6e8a8 Recover corrupt message-board storage during thread deletion (#47042)
## Why

A corrupt agent message-board database can block thread deletion even when the feature is disabled.

## What changed

When corruption prevents opening the database for board deletion, back it up and recreate it before recording deletion tombstones. Recovery resets all boards, retaining their previous storage in the backup. Other database open failures continue to return an error without deleting the thread or replacing storage.

## Testing

Add an app-server regression test with the feature disabled that verifies ordinary open failures preserve storage and the thread, while corruption recovery preserves the backup, deletes the requested thread, retains another thread, and records the board tombstone in the recreated database.

jif 73129716c2 Delete persisted message boards with their root threads (#47029)
## Why

Permanent thread deletion needs to remove persisted agent message board data even when the message board feature has been disabled.

## What changed

- Add a cleanup hook for single and batch thread deletion, invoked after reference and writer checks but before rollout removal so cleanup failures remain retryable.
- Remove the deleted roots' channels, posts, and subscriptions regardless of the current feature setting.
- Record board tombstones and check them within write transactions to prevent delayed writes from recreating deleted data.

## Testing

Add an app-server regression test that creates a board, archives its root thread, restarts with the feature disabled, and verifies that permanent deletion removes the persisted channels, posts, and subscriptions.

jif 8a62cc4dc1 Align message-board tools with the multi-agent namespace (#47028)
## Why

Message-board tools hardcoded the `collaboration` namespace and used a different namespace description from the multi-agent tools. This ignored custom namespace configuration and caused conflicts under strict tool collision checking.

## What changed

- Use `multi_agent_v2.tool_namespace` and the shared multi-agent namespace description for message-board tools, resolving the namespace at runtime startup.
- Support unnamespaced tool specifications when no namespace is supplied.
- Remove hardcoded `collaboration` prefixes from message-board notifications.

## Testing

Add app-server integration coverage for the default `collaboration` namespace and a configured `agents` namespace with strict collision checking enabled. Verify that both tool families are advertised together and that `get_channels` executes through the installed extension.

jif 6149914a0e Wire agent message boards into persistent multi-agent runtimes (#47017)
## What changed

- Register the existing message-board tools with the app server through a lifecycle extension, gated on `agent_message_board`, multi-agent v2, and non-ephemeral sessions. The new feature flag is disabled by default.
- Bind tools to each agent's identity and share the SQLite-backed board across the agent tree and runtime resumes.
- Use the runtime clock for posts and deliver ID-only notifications to running agents, with tools to retrieve post contents. Skip inactive recipients without starting them or queuing notices.
- Emit a warning and leave board tools unavailable if initialization fails.

## Testing

Add runtime scenarios and a context snapshot covering feature gating without creating storage, posting and searching, active-only notifications, sharing with child agents, persistence across resume, and clock failure without channel creation.

teddywyly-oai a6fdb11eda Preserve metadata in native user verification requests (#46952)
## Why

Native `openai/userVerification` requests need to carry optional `_meta` to clients for extension-specific presentation.

## What changed

- Restore metadata from the MCP request context, excluding `progressToken`, and forward it through `mcpServer/elicitation/request`.
- Update protocol types, schemas, and documentation while retaining support for requests without metadata. Challenge bytes and acceptance proofs remain unchanged.

## Testing

Add coverage for metadata validation, transport context restoration, and protocol round trips. Extend MCP integration tests to check that verification metadata reaches the client.

Eric Traut d1e3f9dfe3 Fix realtime V3 transcript reconciliation for handoffs (#46922)
## Why

Interleaved speaker transcripts can fragment handoff context, and delayed final transcripts can overwrite newer speech.

## What changed

- Reconcile V3 transcript deltas and finals with the latest entry for the same speaker.
- Replace accumulated text only when the final transcript extends it, preserving newer speech when a final arrives late.
- Start a new entry after each completed utterance, keeping repeated speech and final-only turns distinct even when empty deltas arrive.
- Preserve live transcript notifications and legacy protocol behavior.

## Testing

alexsong-oai 75db67bbc6 Enforce current provider requirements for the model catalog (#46917)
## Why

The app server retains its startup model provider, but managed provider requirements can change while it is running. Catalog reads and background refreshes must check those requirements even when models are cached.

## What changed

- Gate `model/list` and background refreshes through a shared catalog that validates the startup provider against current requirements.
- Return JSON-RPC error `-32600` with restart guidance when the provider no longer complies, and skip background requests to that endpoint. Requirement load failures also block catalog access.
- Log only the error kind for blocked refreshes to avoid exposing credentials in parser diagnostics.

## Testing

Add coverage for cached catalog access after provider selection or definition changes, invalid requirements, cloud configuration load failures, and recovery when requirements allow the retained provider again. Extend refresh tests to cover startup provider overrides, blocked endpoint requests, and credential-safe logging.

Rennie c45ea25ffb Allow subagents to request MCP elicitation input (#46877)
## Why

MCP requests requiring user input previously failed in subagents with guidance to ask the parent agent, blocking browser sign-in, form input, and interactive tool approval in the child thread.

## What changed

Remove the root-only MCP elicitation guard and `allow_user_interaction` plumbing so subagents can surface prompts and wait for responses under the existing approval policy and automatic review rules. Connector authentication can now return retry guidance after acceptance or preserve the original failure when declined.

Keep legacy tool approvals through `request_user_input` restricted to the root thread.

## Testing

Update unit and integration coverage for subagent prompt resolution, form responses, connector authentication acceptance and decline, automatic approval, strict review, and approval-policy updates on reused connections. Update the browser sign-in scenario snapshot to show the subagent continuing after an accepted response.

Abhinav 3bb0a530d1 Add system proxy fallback for login and startup requests (#46562)
## Why

Login and enterprise configuration bootstrap can fail when their endpoints are reachable only through the system proxy, before cloud configuration can enable `respect_system_proxy`.

## What changed

- Enable `features.system_proxy_fallback` by default, honor managed feature requirements, and prevent project configuration from changing it.
- Retry account discovery and cloud configuration GETs through the system proxy after connection failures or timeouts, bounding the initial request and body read to five seconds.
- Retry OAuth authorization-code exchange only on connection failures before any redirect, with a ten-second connection timeout. Reuse the successful client for the subsequent API-key exchange to avoid changing routes.
- Increase workspace discovery and cloud configuration timeouts to 15 and 20 seconds, respectively, to accommodate fallback.

## Testing

Add coverage for proxy-only browser login through the first completed turn, including cloud configuration enabling `respect_system_proxy` without a restart. Cover stalled GET bodies, preserved request headers, disabled fallback, managed requirements, and project configuration restrictions. Verify that OAuth HTTP errors, redirects followed by connection failures, and failures after sending the POST do not trigger proxy retries, while successful redirects remain supported.

andrewgu-oai 888be42a20 Support explicit provider model catalog URLs (#46561)
## Why

Providers need to serve Codex model metadata independently of their inference endpoint. A custom inference `base_url` alone should not opt API-key sessions into remote catalog discovery.

## What changed

- Add `model_catalog_url` to provider configuration and thread-config serialization. Fetch the full catalog URL with provider authentication, headers, query parameters, and `client_version`, while preserving inference routing.
- Require an explicit catalog URL for API-key discovery with a custom base URL. Keep default OpenAI discovery on the Codex backend, and apply `api_key_model_discovery` gating to provider API keys and cached catalogs.
- Include the catalog URL in cache identity so different catalogs do not share cached metadata.
- Limit explicitly configured catalog responses to 1 MiB, reject redirects, and suppress URL and response diagnostics that could expose credentials.

## Testing

Add coverage for catalog configuration round-trips, query encoding, authentication and header reuse, cache identity and discovery gating, oversized responses, and redirect rejection without credential forwarding. Add an integration test verifying that catalog model metadata and instructions reach conversation requests.

Eric Traut b33199b1fb Add bounded filesystem path diagnostics to codex doctor (#46543)
## Why

Configured filesystem paths can be slow or inaccessible. Doctor needs to identify those paths and their configuration sources without letting a blocked filesystem call delay runtime shutdown.

## What changed

- Add `sandbox.filesystem_paths` to report literal filesystem grants, access modes, resolution outcomes, and configuration sources when available. Exclude deny rules and denied paths without expanding globs or special paths.
- Resolve paths in disposable helper processes, with wait budgets of two seconds per path and eight seconds total, checking at most 32 paths. Warn on slow or unsuccessful probes and incomplete checks; missing paths alone do not trigger warnings. Probes do not test read/write access.
- List paths without probing on Windows or when filesystem read restrictions apply.
- Increase the app-server doctor report timeout from 25 to 35 seconds to accommodate the probes.

## Testing

Add unit and integration coverage for path deduplication and deny filtering, configuration provenance, blocked-helper timeouts, helper execution without loading configuration, and report snapshots for resolved, missing, Windows, and read-restricted paths.

Using Responses Lite does not establish whether a model accepts reasoning-effort `configuration_update` items. Unsupported models need to use the selected request-level effort, including when resuming history containing saved updates.

## What changed

- Add `supports_reasoning_effort_updates` model metadata, defaulting to `false`.
- Require the override feature, an OpenAI provider, and explicit model support for reasoning effort updates, independently of `use_responses_lite`.
- Filter saved configuration updates from unsupported requests without changing persisted history.
- Clear the pinned effort baseline when sampling with an unsupported model so returning to a supported model establishes a fresh baseline. Compaction uses the selected effort without clearing the pin.

## Testing

Add coverage for supported models with either Responses mode, unsupported-model history filtering over HTTP and WebSocket, switching to an unsupported model and back, and compaction versus sampling pin behavior. Verify missing capability metadata defaults to `false`.

iceweasel-oai 74af2496c9 Default local binding to true for MXC managed networking (#46523)
## Why

MXC's native host-loopback access is bidirectional, so it cannot enforce `allow_local_binding = false`. Treating an omitted setting as `false` prevents managed networking from working with the default configuration.

## What changed

- Preserve an omitted `allow_local_binding` until the executor's sandbox policy is known. Default to `true` for Windows MXC and `false` elsewhere, including remote execution.
- Reject an effective `false` for MXC managed networking after applying policy restrictions, without enabling disabled networking.
- Use the executor's resolved value for remote network approval decisions while preserving explicit controller restrictions.
- Document that local binding permits local servers and direct host-loopback connections and skips additional private-network destination checks; proxy domain rules still apply.

## Testing

Add coverage for per-executor defaults, explicit values, MXC rejection of `false`, and remote network review cleanup with the resolved policy.

felixxia-oai 3fd9e7e30e Enable Guardian parent-compaction reuse by default (#46522)
## What changed

Promote `guardian_reuse_parent_compaction` to stable and enable it by default, allowing Guardian to reuse encrypted parent compaction when restarting review sessions.

## Testing

Update Guardian tests to rely on the default setting, including evidence retention after compaction and resume. Adjust cache-key assertions to verify that parent-history changes invalidate cached review sessions by default, while explicitly disabling reuse preserves the previous behavior.

Charlie Marsh 05a93a8a1d Avoid cloning excluded turn items during thread resume (#46511)
## Why

Resuming a running thread with a summary initial page cloned every active-turn item before discarding those outside the summary, including potentially large reasoning payloads.

## What changed

- Snapshot the active turn using the requested `TurnItemsView`, copying only the first user message and final agent message for summaries and no items for `NotLoaded`.
- Preserve full snapshots when the resume response includes full turns, even if its initial page requests a summary.
- Share item selection between snapshots and page responses, and skip selection when a turn already has the requested view.

## Testing

Add unit coverage for summary selection, completed-turn metadata, and metadata-only snapshots. Extend running-thread resume coverage for default summaries, explicit full pages, and full turns returned alongside a summary page.

Charlie Marsh dfb265764d Avoid cloning active turn items for metadata-only thread resumes (#46510)
## Why

Resuming a running thread cloned the active turn's items even when the response did not need them.

## What changed

Add a metadata-only turn snapshot and use it for resume status checks and initial pages with `TurnItemsView::NotLoaded`. Only snapshot items when including turns or requesting an initial page with items, preserving item data for `Summary` and `Full` views.

## Testing

Add lifecycle coverage for metadata snapshots and extend running-thread resume tests to check metadata-only responses and populated `Summary` and `Full` pages.

viyatb-oai 608e4cc9a1 Avoid persisting project trust for projectless directories (#46328)
## Why

Starting a thread in a directory without a project could persist trust and
preapprove project configuration added later.

## What changed

Track whether configuration discovery found no project-root marker, Git checkout,
or project-local `.codex` directory. Skip implicit project trust in `thread/start`
for these directories. Preserve existing trust decisions and permission checks.

## Testing

Add regression coverage for starting a thread with full access, then adding
project configuration and verifying that a read-only thread does not load it or
persist trust. Add loader tests for projectless classification, project markers,
saved trust, managed configuration, and skipped discovery.

marksteinbrick-oai a1efb59c4a Record active plugin inventory in turn analytics (#46323)
## What changed

Add `active_plugin_ids_at_turn_start` to turn analytics, combining active host plugins with selected plugin packages. Prefer remote plugin IDs when present and otherwise use validated package IDs. Sort and deduplicate the inventory.

Report `null` for unknown inventories, invalid IDs, IDs longer than 128 bytes, or inventories exceeding 512 distinct IDs; preserve `[]` for an observed empty inventory. Keep the first received inventory even when later resolved configuration updates arrive.

## Testing

Add coverage for ID selection and validation, deduplication, size limits, serialization, and preservation of the first inventory. Extend app-server tests to check inventories after plugin reconciliation and with selected plugins, including turns without explicit plugin mentions.

vkg-oai 0c9be8a836 Preserve plugin caches across display metadata refreshes (#46309)
## Why

Renewed image URLs and other display metadata changes unnecessarily invalidate loaded plugins and MCP and skill caches, even when installed plugin behavior is unchanged.

## What changed

- Compare installed plugin metadata by identity, version, enablement, policy, and availability before invalidating derived caches. Continue storing the full updated payload so display consumers receive fresh metadata.
- Preserve invalidation when behavioral metadata changes or reconciliation requires an effective plugin refresh.
- Export `remote_catalog_metadata_eq` to compare catalogs independently of display metadata and plugin display order, while retaining marketplace order significance.

## Testing

Add regression tests for display-only updates, behavioral changes, catalog ordering, and preservation of loaded skills and tool suggestions. Add an app-server integration test verifying that image URL renewals and badge updates preserve live MCP sessions and cached skill resources, while an authentication policy change invalidates resource caches.

Charlie Marsh 55db7e8c88 Avoid cloning turn items for app-server active turn lookups (#46305)
## Why

App-server callers that only need the active turn ID or its presence currently create a full turn snapshot, unnecessarily cloning its items.

## What changed

Expose `ThreadState::active_turn_id()` and use it for interrupt validation, elicitation turn ID fallback, and teardown and shutdown logging, preserving the existing turn selection behavior.

## Testing

Update the interrupt integration test to wait for `turn/started` instead of a fixed delay and verify that an incorrect turn ID is rejected before successfully interrupting the active turn.

Charlie Marsh 17baabd01b Separate thread startup metadata from replay history (#46294)
## Why

`CodexThread` retained the full `SessionConfiguredEvent`, including initial replay messages, and cloned it even for callers that only needed thread or session IDs.

## What changed

Store `ThreadStartupMetadata` without replay history and expose it by reference through `startup_metadata()`. Update callers to read IDs from this metadata. When rejoining a running thread, rebuild the startup response using the supplied resume history for `initial_messages`.

## Testing

Extend the resume test to verify that rejoining reuses the loaded thread, preserves startup metadata, and still returns the original replay messages as a prefix.

cassirer-openai 0fd1cd8d99 Add opt-in overhead timing to code-mode responses (#46288)
## Why

Code-mode responses show host duration without exposing time spent outside the host, including app-server waiting.

## What changed

- Add `features.code_mode.experimental_show_cell_overhead`, disabled by default, to show handler duration, host duration, and their difference in `exec` and `wait` response headers.
- Reuse the completed handler measurement from tool-call logging, excluding dispatch waiting, and capture it even when logging is disabled.
- Preserve the existing timing format when the option is disabled or host timing is unavailable, and preserve boolean feature toggles when merging nested code-mode configuration.

## Testing

Add coverage for timing headers, zero and missing host measurements, negative rounding differences, content preservation, and configuration merging. Extend app-server tests across gRPC and stdio to verify timing includes post-host elicitation waiting, and add a response snapshot for successful execution and a failed wait.

acrognale-oai 0a5b999169 Connect app-server workspace discovery to model request routing (#46281)
## Why

Model requests need the selected workspace's routing constraints before sending content. A missing cached route cannot establish that a custom ChatGPT-auth destination is independent of the workspace.

## What changed

- Register the account processor as the workspace routing resolver for Responses HTTP, compaction, and WebSockets, enabling origin selection and routing headers while preserving API paths and rejecting routed HTTP redirects.
- Share concurrent discovery by auth generation, workspace, and backend configuration. Recover from discovery-time `401` responses while allowing token refreshes for the same auth owner.
- Require successful discovery before classifying custom destinations as independent, and require a new thread when a workspace-bound session's bootstrap origin changes.
- Refresh managed requirements using retained session configuration without fetching thread configuration again. Return typed routing errors without account or backend details.

## Testing

Extend coverage for discovery-time token refresh, retained provider definitions under managed requirements, and specific invalid-routing error categories.

jif d7f8e48d7d Preserve Guardian's reusable history prefix across approval requests (#46279)
## Why

Changing previous review decisions or trusted tool and skill evidence should not invalidate the reusable conversation history prefix.

## What changed

Move previous reviews, trusted tool metadata, and trusted skills after the transcript and permission context in Guardian context composition. Keep them before the current action, with history remaining user-role evidence.

## Testing

iceweasel-oai 8b78600dc8 Enable MXC selection through Windows sandbox configuration (#46271)
## What changed

- Accept `windows.sandbox = "mxc"` and preserve the selected backend through environment configuration, command execution, patch writes, and sandbox metadata.
- Treat MXC as enabled in the TUI and report Windows sandbox readiness as `ready`, avoiding legacy setup prompts.
- Keep `allowed_sandbox_implementations` scoped to the legacy elevated and unelevated backends without restricting MXC.
- Default `windows.sandbox_private_desktop` to `false` for MXC while retaining `true` for legacy sandboxes.

## Testing

Add coverage for MXC configuration precedence, legacy requirement handling, sandbox selection, and TUI state. Add a Wine integration test that verifies command and patch routing fails when native MXC is unavailable and reports `windows_mxc` in turn metadata.

Jiwon Kim 108e6a6dbe Replace Sites migration state with a runtime compatibility guard (#46108)
## Why

Older Desktop clients can still provide bundled Sites to an independently updated SSH app-server. A cached remote Sites install must take precedence, even when disabled, while a missing remote bundle must preserve the bundled fallback.

## What changed

- Remove persisted bundled-plugin exclusions, Sites migration checks, and the migration wait when loading local plugin configuration.
- Suppress `sites@openai-bundled` during plugin loading when the remote global catalog is active and a cached remote Sites install is available.
- Remove exclusion-based catalog filtering and read/install guards, and simplify `install_plugin` to accept `ConfigLayerStack`.

## Testing

Expand the agent-turn Sites test to cover enabled remote precedence, disabled remote suppression of bundled Sites, and fallback when the remote bundle is missing.

Rennie 40584fad87 Keep MCP user interaction on the root thread (#46066)
## Why

MCP requests that need human input, including browser sign-in, must be handled by the root thread. Subagents need guidance to hand these blockers to their parent without prompting the user or automatically accepting requests that require input.

## What changed

- Reject interactive MCP elicitations and tool approval prompts in subagents with guidance to ask the parent and wait before retrying.
- Recognize browser authentication and `codex_requires_user_input` metadata even when the form schema is empty, while preserving automatic permission approvals and review decisions.
- Carry user-interaction eligibility through MCP runtime creation and connection reuse, and guard prompt registration.
- Preserve connector authentication diagnostics alongside handoff guidance, subject to normal tool-output limits.

## Testing
