## What changed
- Share a lazily registered runtime across threads, model discovery, and extension clients when `runtime_agent_instances` is enabled, while honoring each thread's authentication mode and credential owner.
- Bind model catalogs to the effective provider and authentication context while preserving cache policy and live API-key discovery settings.
- Resolve credentials, routing, and transport policy together for each request. Preserve proxy and TLS settings, validate runtime account selectors, and prevent pooled connections from crossing runtime identities.
- Accept supported ChatGPT subdomains when selecting the registration environment.
## Testing
Add integration coverage for shared runtime registration, dual-header and assertion-only modes, disabled and third-party providers, credential replacement, subagents, and delegated review. Add coverage for catalog cache policy and runtime-aware connection reuse.

## What changed
- Record connection attempts and durations as `codex.websocket.connect` and `codex.websocket.connect.duration_ms`, labeled with `success`, `reauth`, and the configured `product_sku` when present.
- Detect reauthentication by comparing the current auth revision with the last successful connection, preserving that revision across connection resets. Include `auth.reauth` in connection logs and traces.
- Populate session telemetry with the configured product SKU.
## Testing
Extend tests to check reauthentication labels after account switches, product SKU labels for restored sessions, and failed-handshake counts, durations, and labels.

## What changed
Once Responses requests fall back to HTTP after exhausting WebSocket retries or receiving HTTP 426, keep subsequent requests and turns on HTTP for the remainder of the session. Clear cached WebSocket state and remove background recovery probes, recovery trials, and their acknowledgement timeout.
## Testing
Update fallback tests to verify that a second turn stays on HTTP without additional WebSocket attempts. Adjust retry and `Retry-After` request-count assertions to exclude background probes.
GitOrigin-RevId: a4f6376604527c14db8d60f79bd21c3aaf289f70
## Why
A transient WebSocket failure previously kept the session on HTTP for its remaining lifetime, even if WebSocket connectivity recovered.
## What changed
- Probe WebSocket connectivity in the background during HTTP fallback, allowing a subsequent request to use a ready connection without delaying HTTP requests.
- Verify recovery with a real request and bound its first-response wait to five seconds. Keep HTTP 426 responses and recovery timeouts on HTTP for the rest of the session.
- Keep retries of an HTTP request on HTTP. If a recovered connection fails before output, replay the request over HTTP without consuming its ordinary retry budget; failures after output use normal retry accounting.
- Rate-limit recovery probes and honor `Retry-After` deadlines.
## Testing
Extend integration coverage for successful recovery within a turn, disconnects before and after output, full-context replay followed by incremental WebSocket requests, persistent HTTP 426 fallback, and retry deadlines.
GitOrigin-RevId: 1c139a4bbce1a67442db5622fd70ff0cc8056c9c
## Why
When a response fails with incomplete output, attempted tool calls can appear only in the terminal response payload. Failed inference traces previously omitted those items, losing evidence of the attempted call.
## What changed
Retain output items marked `incomplete` on `ResponseIncompleteError` and include recognized items in failed inference traces, deduplicating by item ID. Keep these items out of streamed output, tool dispatch, and the continuation cache, and omit their payloads from the error's debug representation.
## Testing
Add tests for retaining only incomplete output from terminal responses and recording attempted calls in failed traces without streaming them or creating tool calls. Cover unknown response item types being skipped.
GitOrigin-RevId: 594f6a37a5af543694f19fcffe0c164cb2addb36
## What changed
Move the model client's request-preparation loop into the shared `prepare_provider_request` helper and add `RequestAuthChange` to classify auth state changes. For managed auth, retry preparation when credentials change and reject account changes, including when the provider starts or stops exposing an auth manager during setup.
Capture the auth owner and credential generations together and pass them into the preparation callback so the resulting client setup records the state used for that attempt.
## Testing
Add a test verifying that an explicit provider bearer token survives a workspace switch during setup without retrying preparation.
GitOrigin-RevId: ed8710d1c344b48ebf6182e25e6269214dd2d3fe
## Why
Shared routing cookies can change after a parent connection is established. Guardian reviews need the routing affinity captured with their parent response so unrelated traffic does not replace that hint.
## What changed
- Capture `__oailb` and `__cflb` cookies from successful ChatGPT WebSocket handshakes and pair them with the parent response ID.
- Retain the snapshot throughout a review and its retries, and apply it only to Guardian requests with the matching parent response. Reconnect cached reviewer sockets when the parent affinity changes.
- Preserve cookie expiry and restrict reuse to the original secure origin and endpoint path. Replace routing cookies while retaining unrelated cookies, and keep the captured affinity out of prompts, history, and serialized metadata.
## Testing
Add coverage for cookie scope, expiry, duplicate rejection, and merging; replaying a captured pin after shared cookies change or are deleted; and retaining the matching parent snapshot through compaction without exposing cookies in review prompts or metadata.

## Why
Model catalog refreshes need to use the same registered actor and credential owner as model requests when runtime authentication is enabled.
## What changed
- Add `ProviderAuthContext` to carry credentials and optional runtime authentication into model providers and catalog clients.
- Select runtime authentication independently for inference and catalog destinations. Reject unsupported OpenAI origins while preserving provider authentication for other destinations.
- Include API organization and project selectors in runtime registration. Reuse registered runtimes across projects within an organization and replace them when the organization changes.
- Preserve transport restrictions and account bindings, and strip sensitive headers on cross-origin redirects.
- Apply the catalog timeout to authentication and fetching while allowing credential refresh and registration to finish safely.
## Testing
Add coverage for runtime-authenticated catalogs, provider isolation, resource selection, gateway authentication, cross-origin redirects, and timeout recovery that preserves registration and refreshed tokens.
GitOrigin-RevId: c14f1a02159031ea0af1f9b26ea890f8dca7d6ad
## Why
HTTP response streams retained copies of completed output items even when inference tracing was disabled, keeping their payloads alive until the response completed.
## What changed
Collect completed output items only when inference tracing or WebSocket last-response bookkeeping needs them. Make the last-response sender optional and create its channel only for WebSocket requests.
## Testing
Add a regression test that verifies an untraced HTTP stream releases a completed item's shared payload after delivery, while the response remains open. Update existing tracing and WebSocket bookkeeping tests for the optional sender.
GitOrigin-RevId: 7695534c6d00f3002005324c43c23bf733f01804
## Why
Custom OpenAI base URLs caused Codex backend authentication to lose Responses headers and Guardian metadata when the URL did not match a recognized backend route.
## What changed
Apply model-scoped Responses headers, Guardian reviewer and classifier markers, and eligible ChatGPT parent credit requests based on the OpenAI provider and authentication configuration, regardless of the base URL. Keep API-key requests and other providers excluded from this behavior.
## Testing
Extend coverage for custom `/v1` URLs and direct `/api/codex` endpoints with ChatGPT, header-based, and API-key authentication, including HTTP fallback and WebSocket requests.
GitOrigin-RevId: cc4bb5b4a1a6596904ecf8965237c7a4f09e112b
## What changed
- Add opt-in runtime authentication through `ModelClient::with_runtime_auth`, requiring the provider and runtime manager to share the selected OpenAI credentials.
- Keep runtime identity, credentials, routing, and transport selection consistent for each request and its retries, signing a fresh assertion for each attempt.
- Apply authentication asynchronously to WebSocket handshakes, replace stale authentication headers, and reconnect when the runtime changes.
- Reject account, organization, or project selectors that conflict with the registered runtime, and preserve gateway OAuth composition.
## Testing
Add coverage for dual-header and assertion-only authentication, HTTP retries, runtime replacement and account switches, stale WebSocket headers, selector mismatches, gateway OAuth failures, transport revocation, and Realtime compatibility.
GitOrigin-RevId: 45b9efc2c80481406a0f6f6396a8cdd21d933c12
## Why
Model output alone does not establish that an item entered conversation history. Treating every response item as accepted can include discarded attempts, while relying on a later request misses accepted final output when a turn ends or is interrupted.
## What changed
- Record `HistoryItemAccepted` events and retain Responses item IDs, generating missing IDs consistently across item events.
- Build thread conversation history from request inputs, compaction checkpoints, and explicit acceptance. Keep unaccepted output accessible through its inference call.
- Preserve inference provenance across retries and late response records without overwriting newer request or compaction snapshots.
- Mark turns with errors as failed. Bump trace schemas to version 2 and reject incompatible bundles and event schemas during replay.
## Testing
Add reducer coverage for discarded attempts with identical text, late responses, compaction, inherited context, and legacy bundle rejection. Add CLI integration tests that run `debug trace-reduce` after success, stream failure, and interruption followed by shutdown, including responses without provider item IDs.
GitOrigin-RevId: 8e387a91d9b3f704df82847d630e6f0b08f998b3
## What changed
- Support `model_provider = "gcp"` with a Vertex AI endpoint derived from `GOOGLE_CLOUD_PROJECT` and `GOOGLE_CLOUD_LOCATION`, defaulting to the `global` location.
- Obtain credentials through `gcloud auth application-default print-access-token` by default, and allow `base_url`, `auth`, and `http_headers` overrides under `model_providers.gcp`.
- Use a static model catalog tailored to Vertex AI, preserve function tools, namespaces, and client-side tool search, and support remote compaction V2. Disable unsupported reasoning summaries, Responses Lite, custom tools, web search, and image generation.
- Expose a `gcp` account type through the app-server protocol and generated clients without resolving credentials during account reads or requiring OpenAI authentication.
## Testing
Add coverage for endpoint resolution, configuration overrides, model catalog filtering, authenticated requests, unsupported request fields, missing credentials, account reporting across provider changes, and automatic remote compaction.
GitOrigin-RevId: 97a3158269e6dbc2605e2cf576998e09817ea12a
## Why
Synchronous auth header construction could silently return no credentials when managed authentication became unavailable. Callers need an error to avoid sending requests with missing or partial authentication.
## What changed
- Make `AuthProvider::add_auth_headers`, `to_auth_headers`, and auth header telemetry fallible, and propagate failures through request and WebSocket setup.
- Return a transient error when managed authentication is unavailable, and preserve network policy denials through auth error conversion.
- Retry transient MCP authentication errors while treating build errors and policy denials as terminal.
- Cancel local MCP event streams before resolving authentication so cleanup still works when credentials become unavailable.
## Testing
Add regression coverage for rejected partial authentication, async header resolution, terminal auth errors without sending requests, managed account changes, MCP retry classification, and local stream cancellation after authentication becomes unavailable.
GitOrigin-RevId: 39d981e232cba1e302de502a2fa6239101114ed1
## What changed
Expose `PromptCacheKey` through `StartThreadOptions` with three policies: retain the default session-derived key, send a custom key, or omit `prompt_cache_key` from model requests. Keep session identity headers independent of this policy.
Propagate the policy to child agents, delegated sessions, and guardian review sessions. Hosts must reapply the policy when resuming a thread from storage.
## Testing
Add coverage for all three policies across turns, including unchanged session and thread headers. Extend subagent coverage to verify inheritance of the default and omitted-key policies.
GitOrigin-RevId: f9e8ab31fcf568636f9712726760e4e9a4672428
## Why
Turn recovery and retries after a server overload can leave consecutive reasoning effort updates in history when settings change without an intervening message. Requests should use the latest update in each consecutive run.
## What changed
Coalesce adjacent `configuration_update` items when building Responses requests with reasoning effort overrides enabled, retaining the latest update. Preserve saved history and updates separated by other input items.
## Testing
Extend integration coverage for recovery with unchanged or changed effort, retries after a server overload, and WebSocket continuation and replay after coalescing. Verify that requests retain the latest adjacent update while saved history keeps all updates.
GitOrigin-RevId: bcf97db4c93dac5f466bd276176ac9d238329ecd
## Why
Guardian request counts grouped by input size should reflect admitted sampling inputs without counting synchronous transport retries more than once.
## What changed
- Add a shared `token_bucket` label to admitted total-input observations in `codex.guardian.context.request_tokens` for synchronous and asynchronous reviews. Use inclusive upper bounds in 20k-token steps through 240k, followed by 256k and overflow buckets.
- Record synchronous request tokens after budget admission, once per sampling step, including the complete input. A fresh step counts again even when its input is identical.
- Preserve asynchronous component histograms for oversized classifications, but omit `token_bucket` so rejected inputs do not contribute to counts grouped by that label.
## Testing
Add coverage for bucket boundaries, synchronous retry deduplication and budget rejection, asynchronous retained-history reuse and reset, and admitted versus oversized classification metrics.
GitOrigin-RevId: fac2f102a4c48c5fd21d56bfa701b95a1ff852d0
## Why
Cloning `ResponseItem::AdditionalTools` deep-copies its tool declarations for history and request copies.
## What changed
Store `tools` in an `Arc<[serde_json::Value]>` so clones share immutable declarations. Update constructors and existing test fixtures to use the shared representation.
GitOrigin-RevId: eb8831774afb45fe8431a796f34ae9d1243e884a
## Why
Message roles and content classifications do not identify who supplied each context part. Source attribution needs to survive prompt assembly and history truncation, including messages that combine text from different producers.
## What changed
- Add per-part `ContentItemMetadata` for user input, developer instructions, `AGENTS.md`, skills, harness context, command hooks, and tool context with optional callable namespaces.
- Carry attribution alongside content and classifications through contextual fragments, extension prompts, hook output, and agent messages. Keep tool-provided context-window hints separate from harness text.
- Preserve the source of retained text slices during compaction and attribute generated truncation notices to the harness. Default missing historical attribution to unknown.
- Attribute host MCP calls using cached tool namespaces only when catalog revisions remain stable, without starting servers or refreshing catalogs for attribution.
- Send the new metadata to Responses Lite and strip it from requests to endpoints that do not support it.
## Testing
Extend compaction tests to cover mixed sources, missing or extra metadata entries, and discarded media. Update Responses Lite request assertions to verify endpoint-specific attribution handling.
GitOrigin-RevId: b0ebc929b7c60f3ec49d1affbf897312d63edcc8
## What changed
- Launch background compaction at eligible pre-turn, mid-turn, and post-turn checkpoints when `AsyncCompaction` is enabled, capturing the current world state and client routing state.
- Adopt ready results at compaction checkpoints while preserving newer messages, tool outputs, and world-state updates. Continue normally if an optional result is rejected.
- Run one foreground compaction if an adopted result still exceeds the context limit.
- Prevent cancelled turns from launching new background work while preserving already-running work for later reuse.
## Testing
Add unit and scenario coverage for stale history and account changes, cancellation, pending-result reuse and fallback, retained tool outputs, compaction event pairing, and token usage accounting. Add a context snapshot covering a collaboration-mode change while compaction runs.
GitOrigin-RevId: 04ac028d9fe2e3ea64aacaaeb351c3c776cf3940
## What changed
- Extract an HTTP Responses executor that can run without owning a turn's WebSocket state, with optional authentication-owner checks.
- Add cancellation-aware compaction output collection that records background token usage against the rollout budget.
- Support installing a compacted history prefix while preserving later history items and placing initial context around real user or agent messages.
- Allow callers to supply a context window for history trimming and a captured window for compaction request metadata.
GitOrigin-RevId: 29f3ca0ae8841f295058feb207e3a225af9a1487
## What changed
- Add the disabled-by-default `output_token_replay` feature to request `output.encrypted_content` from OpenAI providers.
- Preserve encrypted message and tool-call output, message and function-call status, and output-text `annotations` and `logprobs` through history and replay. Update the generated protocol schemas and SDK types.
- Strip output ciphertext from requests to other providers while retaining it in stored history, including after replay is disabled. Remove encrypted content from memory extraction inputs.
## Testing
Add integration coverage for the feature/provider combinations, metadata replay across follow-up requests, and history preservation when resuming with replay disabled.
GitOrigin-RevId: 77729a021b6771933402aebe74ece8eb39c057e2
## What changed
- Remove `ContentItemMetadata`, provenance and namespace types, and the `content_item_metadata` protocol field while retaining `content_item_kinds` classifications.
- Simplify context fragments, extension contributions, and hook output to carry content without source attribution. Return MCP tool results directly without an attribution wrapper.
- Render context-window hints together in a single text fragment.
## Testing
Update Responses and Responses Lite request assertions to expect content-kind classifications without per-content attribution, and adapt context, hook, and extension tests to the simplified types.
GitOrigin-RevId: 201894cd4c93c6d4b4c41c845e7a9af1dd3c95ca
## What changed
- Add `ContentItemMetadata` to distinguish user input, developer instructions, `AGENTS.md`, skills, harness context, and tool-supplied context independently of message role and content classification.
- Keep attribution attached to content through prompt assembly, history processing, and hook-output spilling. Distinguish command hooks from MCP hooks and retain tool namespaces when known.
- Treat missing historical attribution as unknown and send per-content metadata only to Responses Lite.
## Testing
Update request and history assertions to cover attribution persistence and Responses Lite metadata handling.
GitOrigin-RevId: 24e56005f5bdb98bf82f2b3943f29c4c81f3d587
## Why
Full WebSocket requests previously used the generic `other` reason when continuation failed because request properties or input changed, obscuring why incremental reuse was unavailable.
## What changed
- Report specific reasons for changed request properties, shortened or mismatched input, and missing response state or response IDs.
- Add `previous_item_type` and `current_item_type` labels to continuation metrics and failure traces. Diagnostics use bounded labels without request contents or identifiers.
- Centralize response item type names in `ResponseItem::item_type()` and reuse them for output logging.
## Testing
Add coverage for unavailable response state and empty response IDs. Expand WebSocket tests to check fallback reasons and item types for rewritten or shortened input, replaced input, changed model, and changed output schema, including precedence when multiple properties change.
GitOrigin-RevId: 3951853dfe87d40b7046af6f6a8efd5d4f5b06d1
## What changed
Move base instructions from the top-level `instructions` field into a prefixed `developer` message in `input` for standard Responses requests, matching Responses Lite. Remove the field from HTTP and WebSocket request types and assign instruction messages stable IDs derived from the thread and instruction text.
## Testing
Extend standard and Responses Lite tests to cover instruction message serialization, stable IDs, and resume consistency. Update WebSocket tests to verify that changed instructions trigger a full request without `previous_response_id`.
GitOrigin-RevId: e2de4b8e30a9339377fac133ceda981abd1a112b
## What changed
Add `tools_change_count` to turn profiles and turn analytics events. Compare the full model-visible tool list before each sampling request, retaining the previous list across turns and connection resets. The first list establishes a baseline; subsequent changes increment the current turn's count.
## Testing
Add coverage for schema changes, tool additions, ordering changes, empty lists, and tracking across sessions and transport fallback. Extend the turn event serialization test to include `tools_change_count`.
GitOrigin-RevId: 44a15c42dc7d0b35c166666faeb9627b718b33dd
## What changed
When `IncrementalTools` is enabled for Responses Lite, track tool definitions in world state and conversation history. Send the initial catalog once, then append only added or changed definitions while preserving earlier request history. Emit developer notices for removed tools and namespace instruction changes.
Use history-supplied definitions for normal requests and remote compaction, and omit empty `additional_tools` prefixes. Keep the existing tool delivery behavior for the Responses API and when the feature is disabled.
## Testing
Add unit and snapshot coverage for catalog and namespace transitions, empty catalogs, and stable hashing across JSON object key order. Add a request-history scenario covering unchanged tools, a model switch that changes one definition, persisted catalog hashes, and Responses API behavior.
GitOrigin-RevId: 76bde175973b81f8ad23873919da3c8ddcdf6065
## Why
Bedrock catalogs cleared all service-tier metadata, and requests omitted every selected tier. This prevented users from selecting `ultrafast` or using tiers defined in custom catalogs.
## What changed
- Advertise `ultrafast` for `openai.gpt-6-astra` on Mantle and the `global.openai.gpt-6-astra` and `us.openai.gpt-6-astra` Runtime variants, while retaining the implicit default tier unless users select otherwise.
- Preserve custom catalog speed tiers, service-tier definitions, and defaults while continuing to normalize Bedrock web search compatibility.
- Send selected service tiers only when the Bedrock model advertises them, including `flex`.
## Testing
Add coverage for `model/list` tier discovery, custom catalog preservation, configured and per-turn HTTP request tiers, and TUI slash-command selection, toggling, and model switching.
GitOrigin-RevId: 4793e7af7b9f604a6b7812611030da6adb8acd35
## What changed
- Remove `namespace_tools` from `ProviderCapabilities` and `namespaceTools` from `ModelProviderCapabilitiesReadResponse`, updating generated schemas and SDK types.
- Preserve tool namespaces and configured collaboration tool namespaces without a provider capability check, and always use Responses Lite tool serialization for Responses Lite requests.
- Gate tool search on `supports_search_tool` and retain the existing image generation and web search checks without requiring a namespace capability.
GitOrigin-RevId: ae1843563736765a40e03a02c41ff5e470018a93
## What changed
Forward explicit `cyberAccessProgram` selections for the built-in OpenAI provider when both `api_key_cyber_access_programs` and `api_key_model_discovery` are enabled. The new feature defaults to disabled and can be toggled through the app server's experimental feature enablement API.
Reject explicit selections in OpenAI API-key sessions when either feature is disabled, instead of silently omitting the program. Leave entitlement and model restrictions to the server, and preserve the requested program when a server rejection triggers transport retries.
## Testing
Add app-server coverage for both feature gates, forwarding all three program values, and server rejection without fallback. Update core coverage for API-key resumes with disabled features and custom-provider omission.
GitOrigin-RevId: aa639afb841aa651b28f6f3f61198299372baa2d
## What changed
- Add `CodexThread::prewarm_with_history()` to prepare a WebSocket response with existing conversation history and executed-tool metadata using `generate: false`. The next turn can reuse the prepared response when its prompt extends that history and request settings match.
- Keep startup prewarming and `prewarm()` on the existing empty-input path, and skip preparation when the cached WebSocket is ready.
- Label prewarm telemetry by input mode and WebSocket continuation metrics with `after_prewarm`.
- Rename `persistent_mode_enabled` to `persistent_execution_enabled` without changing its behavior.
## Testing
Extend WebSocket coverage to check history preservation during reconnect prewarming, reuse of the prepared response on the next turn, and omission of `previous_response_id` for a non-prefix prompt.
GitOrigin-RevId: c7731541b11b2e6668027a47f37f696213268c66
## Why
Steering an active WebSocket response previously dropped the connection and
resent the full history. Draining the response preserves the connection and
allows the follow-up request to continue with `previous_response_id`.
## What changed
- Drain WebSocket responses when steering. For models using
`use_responses_lite`, first send `response.interrupt` with
`mode: "discard_partial_items"` once the response ID is available.
- Treat `response.incomplete` with reason `interrupted` as completion with
`end_turn: false`, preserving token usage and allowing the turn to continue.
Other incomplete reasons remain errors.
## Testing
Update the steering integration test to cover completed and discarded reasoning
items, asserting connection reuse, incremental follow-up input, and token usage
from the interrupted response.
GitOrigin-RevId: bc714b713e797cf1a67e3b26ffbf7664cb92eb33
## Why
Destination-only filtering stripped raw tool result metadata and MCP attribution when the built-in OpenAI provider used a custom endpoint.
## What changed
Add a runtime-only `include_internal_metadata` grant to `ModelProviderInfo` and enable it for the built-in OpenAI provider. Apply the provider grant or the existing first-party HTTPS destination check to both HTTP and WebSocket Responses requests.
Keep the grant out of serialized configuration and schemas, and default it to false for providers received through remote configuration. Providers without the grant retain the existing destination filtering.
## Testing
Update HTTP and WebSocket tests to expect metadata at overridden OpenAI endpoints. Cover provider grants, destination filtering, MCP attribution on requests, and the grant's exclusion from TOML serialization and configuration.
GitOrigin-RevId: 53aad6fb4d52cdfa9c79c6d92fe87383e1d0d1ef
## Why
With `instant_interrupt` enabled, new user input should reach the model without waiting for an unfinished response or stream retry backoff.
## What changed
- Interrupt request setup, response streaming, and retry backoff when user input arrives, then continue the turn with the queued input.
- Reset the WebSocket connection and continuation state after preemption so the next request sends full history.
- Drain pending input after a preempted step even when compaction leaves the estimated context above the token limit.
## Testing
Add regression coverage for WebSocket reconnection with full history, interrupted retry backoff, input queued during compaction, and yielding running code-mode calls. Add a streaming scenario that preserves completed commentary while excluding unfinished assistant text from the replacement request.
GitOrigin-RevId: b7e42b6afffbf87da15af05e0850af8579d1353f
## Why
Removing tool-result metadata alone can leave requests above the 15 MiB message budget when recorded tool calls and arguments occupy the remaining space. If trimming loses calls or arguments from a Code Mode cell, later waits must not report its call inventory as complete.
## What changed
- Bound all optional tool-call observations against the serialized HTTP or WebSocket message size, preserving ordinary tool outputs and continuation history.
- Shed generic result metadata, sources, and recorded arguments before resource-access evidence, retaining tool names and smaller metadata where possible.
- Invalidate completeness across affected cell outputs and later waits when the outgoing copy loses calls or arguments.
- Preserve size-limit omission markers for direct tool-result metadata when they fit, while continuing to exclude that metadata from custom providers.
## Testing
Add regression coverage for HTTP and WebSocket message budgets, unchanged tool results and history, resource-evidence retention, direct-call omission markers, and Code Mode waits after argument truncation.
GitOrigin-RevId: 538dc9d8c11b0830ec70327422f98a1803122e85
## Why
The 32 KiB per-result cap and 128 KiB aggregate budget discard tool metadata even when the outgoing request has room to retain it.
## What changed
- Remove the per-result capture cap and raise the aggregate executed-tool metadata budget to 2 MiB.
- Apply a 15 MiB soft budget to serialized HTTP requests and WebSocket messages, using the actual WebSocket delta. Trim optional result metadata on the wire copy while preserving ordinary content and logical history.
- Prioritize `openai/resource_access` metadata during trimming, and include `overage_bytes` in omission markers.
- Record bytes removed by metadata budgets in `codex.tool_metadata.shed_bytes`, tagged by stage.
## Testing
Add coverage for HTTP and WebSocket message boundaries, resource preservation, residual marker removal, unchanged history, and shedding metrics. Update direct-tool and code-mode tests to verify metadata survives beyond the previous limits.
GitOrigin-RevId: ed00996b77945300f60db1e52eff22b4de2707c1
## Why
Repeated prewarm calls still prepared instructions, tools, and a prompt even when an existing WebSocket connection was already warm.
## What changed
Check for a previous request and a connection that is not known to be closed before startup prewarm preparation. Reuse this readiness check in `prewarm_websocket`, and record `ready` or `needs_prewarm` in the `codex.startup_prewarm.websocket_check` counter before preparation can fail.
## Testing
Extend the WebSocket resume test to verify that repeated prewarm calls skip instruction loading on a healthy connection, while a closed connection triggers preparation again.
GitOrigin-RevId: 6a1b361b36b4e7f16fb283e48a8ce4d4fb996ceb
## What changed
Return completed startup prewarm sessions to the client cache and schedule resume prewarm through the normal session setup path. Keep in-flight warmups shared, remove the separate nonblocking prewarmed checks, and simplify the prewarm handle to own a single task.
## Testing
Update the resume prewarm integration test to assert `User-Agent` and `x-codex-window-id` handshake headers while retaining coverage for connection reuse, response continuation, and reconnection after closure. Remove the assertion that healthy resumes skip instruction preparation.
GitOrigin-RevId: a9b6d4810aca9aba2e6e41df181b7d9d90474230
## Why
A previously used WebSocket can close while a thread is idle. Hosts need a way to prepare the connection before the next turn without repeating warmup on a healthy connection.
## What changed
- Expose `CodexThread::prewarm()` using the existing startup warmup and next-turn handoff.
- Reuse healthy connections and pending warmups, retry failed warmups, and repair closed connections. Treat a finished WebSocket reader task as a closed connection.
- Skip scheduling during an active turn or shutdown, and stop warmup admission before shutdown takes the pending task.
- Expose `active_turn_environment_selections()` so hosts can inspect the environments captured by the active turn, including those still starting or failed, when authorizing steering.
## Testing
Add regression coverage for repeated idle prewarm calls, healthy connection reuse with preserved response continuation, reconnection with `generate: false`, and warmup rejection after shutdown. Update delayed MCP discovery coverage to verify prewarm repairs an eagerly closed socket and the first turn reuses it.
GitOrigin-RevId: aa164b91a399de2375eda3bf28397725439fce93
## What changed
Add `ModelRequestContributor` and `ModelResponseInterceptor` to the extension API. Contributors can add filtered `client_metadata` without overwriting existing or reserved fields and create interceptors for individual requests.
Wire contributors into HTTP and WebSocket requests, distinguishing generation from warmup. Apply response interceptors in registration order before tracing and response bookkeeping so transformed output is recorded consistently. Interceptors can inspect, buffer, or transform events, with stream ownership carrying request state and cancellation.
## Testing
Add tests for metadata protection, incremental event transformation, interceptor ordering, upstream cancellation, and consistency between delivered output, traces, and WebSocket bookkeeping. Integration tests cover output gating and interruption over HTTP and WebSocket, including a contributor that bypasses gating for warmup requests.

## Why
Startup prewarm waited for tool discovery before opening its WebSocket connection. Slow MCP startup therefore delayed connection setup even though the handshake could proceed independently.
## What changed
- Run the WebSocket handshake concurrently with tool capture, then send `generate=false` with the discovered tools and finalized request metadata.
- Include model and effective service-tier routing hints in preconnect, including the inherited tier for spawned subagents. Omit routing hints for guardian reviewer requests.
- Recover authentication failures during preconnect and switch to HTTP fallback on `426 Upgrade Required`.
## Testing
Add and extend regression coverage for handshake overlap with delayed MCP discovery, cancellation before warmup, finalized Responses Lite tool metadata, socket reuse and reconnection, service-tier changes and inheritance, and authentication recovery during preconnect.

## What changed
- Load `application.network` requirements at startup and explicit config/account reloads, applying local policy before authentication and cloud configuration bootstrap.
- Block traffic when policy loading fails, cancel requests that a new policy no longer permits, and reject configuration loads based on superseded policy snapshots. Local requirements edits take effect on the next explicit reload or restart.
- Bind authenticated clients to the current account and revoke them when account ownership changes. Carry policy enforcement through backend requests, realtime connections, workload identity exchanges, and code-mode gRPC streams while preserving gRPC trailers.
- Treat denied credential refreshes as policy errors without retrying them or invalidating stored credentials.
## Testing
Add regression coverage for startup restrictions, reload ordering, cancellation after restrictive or malformed requirements edits, account revocation, gRPC destination denial, workload identity cancellation, and credential preservation after denied OAuth refresh.

## What changed
- Record each MCP source and its first turn, including connector and plugin identifiers when available. Cover direct and Code Mode calls, including returned errors and results that are not printed.
- Persist cumulative attribution checkpoints and restore them across compaction, resume, and forks, including forks limited to recent turns.
- Attach fresh attribution to Responses API `client_metadata` under `mcp_attribution`, outside model-visible content. Restrict transmission to allowed OpenAI HTTPS destinations for both HTTP and WebSocket requests.
- Report `attribution_error` for legacy history without checkpoints, malformed or conflicting attribution, and serialized attribution exceeding 16 KiB.
## Testing
Add coverage for source deduplication, checkpoint persistence and restoration, recent-turn forks, direct and Code Mode calls, destination filtering, and the serialized size limit.

## What changed
Replace `is_retryable()` with `retry_delay(retry_count)`, which returns `None` for terminal errors and a delay for retryable errors. Prefer server advice when present; otherwise use exponential backoff with jitter. Keep `server_retry_delay()` for callers that need only the server-provided delay.
Route sampling and remote compaction errors through the shared response stream handler, which now checks retryability and obtains the delay together. Preserve server-only retry timing when the retry budget is exhausted.
## Testing
Add coverage for attempt-dependent backoff, zero-duration server advice, and terminal errors that remain non-retryable despite server advice. Update API mapping and provider tests to use the combined retry API.

## Why
Memory consolidation and ephemeral title generation need to use their selected request-level reasoning effort even when managed settings enable reasoning-effort overrides.
## What changed
- Exempt current and legacy memory consolidation sessions and ephemeral `thread_title` threads from reasoning-effort overrides.
- Tag temporary title threads with `thread_title` and keep them excluded from agents overview refreshes. Recap threads retain the `system` tag.
- Preserve override behavior for persisted `thread_title` threads and other ephemeral threads.
## Testing
Extend regression coverage to verify request effort, omission of effort updates from exempt workers' requests, preservation of inherited history, title thread tagging, and overview filtering.

## Why
Using Responses Lite does not establish whether a model accepts reasoning-effort `configuration_update` items. Unsupported models need to use the selected request-level effort, including when resuming history containing saved updates.
## What changed
- Add `supports_reasoning_effort_updates` model metadata, defaulting to `false`.
- Require the override feature, an OpenAI provider, and explicit model support for reasoning effort updates, independently of `use_responses_lite`.
- Filter saved configuration updates from unsupported requests without changing persisted history.
- Clear the pinned effort baseline when sampling with an unsupported model so returning to a supported model establishes a fresh baseline. Compaction uses the selected effort without clearing the pin.
## Testing
Add coverage for supported models with either Responses mode, unsupported-model history filtering over HTTP and WebSocket, switching to an unsupported model and back, and compaction versus sampling pin behavior. Verify missing capability metadata defaults to `false`.

## Why
Synchronous Guardian reviewers need to use their selected request-level reasoning effort even when managed requirements enable `reasoning_effort_override` and the parent history contains effort updates.
## What changed
Disable reasoning-effort overrides for synchronous Guardian sessions and use the same decision for request filtering, effort pinning, and update injection. Reviewers omit inherited configuration updates from requests without changing saved history or appending new effort updates.
## Testing
Add regression coverage for both current and legacy Guardian session sources with managed overrides enabled. Verify that the parent retains its pinned effort, reviewer requests follow selected `low` and `high` efforts, and inherited updates remain in saved history.

## Why
Resuming a thread after disabling reasoning effort overrides still sent saved `configuration_update` items until compaction. Disabling the feature should also apply to requests built from existing history.
## What changed
Pass the `ReasoningEffortOverride` feature state into `ModelClient` and filter `configuration_update` items from request input when disabled. Preserve persisted history and other input items while using the request-level reasoning effort.
## Testing
Add regression coverage for resumed threads, compaction, and WebSocket warmup and turn requests. Verify that saved updates remain in history and `agent_message` items remain in request input.

## Why
An explicit `service_tier = "flex"` could be dropped when fast mode was disabled or the model catalog did not advertise Flex, even though Flex is an API request option.
## What changed
- Preserve configured `flex` through core session settings, TUI tier resolution, and request construction regardless of fast-mode or catalog support.
- Omit `service_tier` from Amazon Bedrock requests, including with custom catalogs, because Bedrock only supports the implicit default tier.
## Testing
Add regression coverage for Flex configured at startup or through thread settings, connected thread startup, and TUI turn submission. Update review coverage to expect Flex with fast mode disabled, and verify that both Bedrock providers omit `service_tier`.
