## What changed

763787d061 Support standalone named function call outputs (#39782)
## Why

External tool events may need to enter thread history without a preceding function call and therefore do not have a `call_id`.

## What changed

- Allow `function_call_output` items to omit `call_id` and carry optional `name` and `namespace` fields.
- Preserve named standalone outputs during history normalization and agent forks while retaining existing pairing behavior for outputs with a `call_id`.
- Accept, persist, and forward these outputs through `thread/inject_items`, and update the app-server schemas and documentation.

## Testing

- Cover paired and standalone JSON round trips, history normalization, agent forks, and injected thread history.

097825f75a Add app-server MCP event streaming (#39761)
## What changed

- Add experimental `mcpServer/event/stream/start` and `mcpServer/event/stream/stop` requests for hosted apps, plus `mcpServer/event/stream/notification` forwarding.
- Scope subscriptions to the owning app-server connection and subscribed thread, enforce unique IDs and a per-connection limit, and clean them up when the thread is unsubscribed or the connection closes.
- Wait for the MCP active notification before completing startup, retry streams that close immediately, and terminate them when authentication or hosted runtime ownership changes.

## Testing

- Add an app-server integration test covering activation, event forwarding, duplicate subscription rejection, and explicit cancellation.

1b450c7912 Persist thread section moves before the first turn (#39523)
## Why

New non-ephemeral threads have no persisted rollout or preview until their first
turn, so moving them into a section could leave them absent from section-filtered
thread lists.

## What changed

- Materialize and flush a non-ephemeral thread before applying an explicit
  section move.
- Include explicitly sectioned threads with empty previews in section-filtered
  queries, and update the partial indexes to cover them.

## Testing

- Add an app-server test that moves a newly started thread into and out of the
  pinned section before its first turn.
- Cover empty-preview threads in recency and section-position query tests.

## Why

App widgets need to read resources using the app and account context of the
tool call that produced them, including after a thread is restored.

## What changed

- Add an optional `originCallId` to `mcpServer/resource/read` requests and
  return it on successful scoped reads.
- Track bounded provenance for successful app tool calls in thread history and
  use the current tool binding, account link, and app policy when reading the
  associated widget resource.
- Reject missing, mismatched, failed, or ambiguous origins, while keeping
  app-only tools available for widget reads without exposing them to the model.

## Testing

Add app-server coverage for scoped widget reads across both history modes,
ephemeral and persistent threads, server restarts, app-only visibility, and
invalid origin cases.

f5e9d66851 Notify clients when Guardian requires strict review (#39157)

## Why

Cold resumes and forks could lose the thread's active permission profile and
fall back to the current configured default.

## What changed

- Restore the latest persisted approval policy, approvals reviewer, and active
  permission-profile ID when resuming or forking a thread.
- Re-resolve persisted profile IDs through current configuration and
  requirements. Missing or invalid profiles fall back to the configured
  default, while threads without a persisted profile continue to use current
  configuration.
- Keep explicit permission and approval overrides ahead of persisted settings.

## Testing

Added coverage for legacy and paginated histories, settings updates, explicit
overrides, removed or changed profiles, requirement fallbacks, and forks.

4343b2bdc4 Add app-server support for reverting paginated threads (#38440)

- Add the experimental `thread/revert` request, which replaces a loaded paginated thread's durable history with the prefix before `beforeTurnId` while preserving the thread ID.
- Interrupt any active turn, reload the replacement history without unloading the thread, preserve mutable thread settings, and emit `thread/reverted`.

5104cb649e Support gRPC code-mode hosts in app server (#38288)

4c89139da9 Add CIMD support to MCP OAuth registration (#38089)
## What changed

- Make automatic MCP OAuth registration prefer Client ID Metadata Documents
  (CIMD) when the authorization server advertises support for public clients and
  Codex is using its native loopback callback. Fall back to advertised Dynamic
  Client Registration (DCR) otherwise.

  OAuth login API. Validate CIMD metadata and callback URLs before starting the
  authorization flow.

Standard MCP forms can require user-entered values even when tool permissions are
otherwise auto-approved in full-access sessions.

## What changed

- Recognize the `openai/standard-form-input` client extension and surface
  non-approval forms in full-access, user-initiated root threads.
- Keep approval forms, automation and subagent threads, headless sessions, and
  clients without the capability on their existing decline or review paths.
- Treat the capability as client-only so it is not advertised to MCP servers,
  and enable it after session startup so required servers cannot block startup
  waiting for form input.

## Testing

Add unit and app-server coverage for accepted form round trips, declined cases,
approval metadata safeguards, resumed threads, and extension filtering.

6622546169 Restore approval policy when resuming threads (#37368)

- On cold `thread/resume`, restore the latest approval policy from persisted
  thread settings instead of falling back to the current configured default.
- Preserve the precedence of an explicit request override over the persisted
  policy.
- Account for settings applied during a turn when the subsequent compacted turn
  context contains a stale policy.

- Add unit coverage for selecting the latest persisted policy.

757c151a0e Add safer TUI defaults for cyber models (#37055)
## What changed

- Propagate the optional `modelSpecialty` field through the model catalog and
  app-server `model/list` response.
- When a newly selected model has the `cyber` specialty, default an active TUI
  thread to workspace-write permissions with on-request approval. Use auto
  review when available, otherwise keep the user as reviewer, while respecting
  configured permission requirements.
- Show a notice when auto review is applied and strengthen the full-access
  warning for cyber models.
- Preserve explicitly selected permissions when only reasoning settings change.

## Testing

- Cover auto-review defaults, requirement-aware fallback behavior, permission

Clients using `thread/read` with `includeTurns: true` need the legacy full-history
view even when a thread stores its history in paginated form.

## What changed

- Reconstruct full projected turns from paginated history for both stored and
  loaded threads.
- Keep `thread/turns/list` and `thread/items/list` as the preferred incremental
  history APIs.

## Testing

- Cover `thread/read(includeTurns=true)` before and after a paginated thread is
  loaded.

3b8d22ec2c Improve paginated TUI history loading (#36949)
## Why

Paginated sessions can have visible messages beyond the first item page, so
bounded hydration could leave resumed history or transcript previews empty and
the transcript viewer incomplete.

## What changed

- Scan paginated history until the configured rendered-row budget is filled,
  while retaining the configured scrollback beyond the visible viewport.
- Fully hydrate transcripts opened from the resume picker and build previews
  from bounded item pages, including when non-visible items precede messages.
- Stop pagination on repeated cursors and let `Esc` cancel an in-flight
  transcript load.
- Replay restored token usage for cold metadata-only resumes when its stored
  turn can be identified.

## Testing

- Add coverage for paginated and legacy transcript loading, bounded resume
  hydration, repeated cursors, transcript cancellation, and resize reflow.

- Match local plugin names, display names, and keywords without case or punctuation sensitivity, while applying global, personal, and workspace scope semantics.
- Deduplicate local and remote copies, preserve local installation metadata on the remote result, and treat the remote global catalog as authoritative over the local curated marketplace.

App-server clients need to advertise structured MCP extension settings, including supported MCP App UI MIME types, rather than only opting into OpenAI form elicitation with a boolean.

## What changed

- Add an `extensions` map to initialize capabilities and preserve `mcpServerOpenaiFormElicitation` as a legacy alias for `openai/form`.
- Capture the declared extension profile when a thread is started, resumed, or forked, propagate it to subagents, and advertise it to downstream MCP servers during initialization.
- Keep the profile stable for the lifetime of the loaded session instead of changing it on later turns or direct tool calls.

## Testing

- Cover extension conversion, downstream MCP initialization, session isolation, legacy form support, and subagent inheritance.

e2c0837923 Make user input blocking behavior explicit (#36410)
## Why

Clients need to know whether a `request_user_input` request must wait for an
explicit response or may auto-resolve. Using `autoResolutionMs` as that signal
conflated the blocking decision with timeout policy.

## What changed

- Add required `isBlocking` fields to user input protocol and app-server
  request payloads. Plan-mode requests are blocking, while requests from other
  enabled modes are non-blocking.
- Drive the TUI's auto-resolution behavior from `isBlocking` and remove
  `autoResolutionMs` from the model-facing tool schema.
- Deprecate `autoResolutionMs` while retaining it for compatibility, and treat
  legacy payloads without `isBlocking` as blocking.

## Testing

- Cover mode-derived blocking behavior, legacy deserialization, app-server
  forwarding, delegated requests, and TUI auto-resolution.
