jif 8a40095ea3 Standardize shell execution on unified exec (#39757)
## What changed

- Remove the legacy `shell_command` handler and runtime, leaving `exec_command`
  and `write_stdin` as the shell execution tools.
- Treat legacy `shell_command` model metadata as `unified_exec`, and normalize
  legacy user opt-outs so they do not disable command execution. Managed feature
  requirements and `shell_tool` can still disable it.
- Preserve shell approvals, sandboxing, zsh-fork support, and output truncation
  through the unified execution path.

## Testing

- Cover legacy configuration and model-metadata compatibility.
- Exercise unified shell execution, approvals, truncation, and `apply_patch`
  serialization across the app-server and core test suites.

jif 942af8447b Retire the untrusted approval policy (#39630)
## What changed

- Remove `untrusted` from the CLI, configuration schema, and MCP tool interface. Explicit `approval_policy = "untrusted"` settings now fail with an actionable error.
- Remove the known-safe command allowlist. Projects marked untrusted now request approval for every command unless an explicit exec policy rule allows it.
- Keep command parsing conservative by treating in-place `sed` forms as mutating and ignoring unrecognized commands when recording memory usage.

## Testing

- Cover rejection of the retired configuration value and approval requests for commands in untrusted projects.

pakrym-oai 10cc57c95c Simplify app-server integration test setup (#34786)
## What changed

- Add typed helpers for initialized app-server instances, JSON-RPC requests,
  responses, notifications, and thread startup.
- Add a composable `MockResponsesConfig` builder while preserving the existing
  mock config writers.
- Migrate the app-server integration suite to the shared helpers, removing
  repeated request ID management, deserialization, initialization, and config
  assembly.

## Testing

- Add unit coverage for composing mock provider, feature, and extra config, and
  for compatibility of the existing mock config writer.

Adam Perry @ OpenAI 5c7624a69e test: migrate app-server v2 starts to auto env (#31614)
## Why

We should be running as many integrations tests as possible against the
split cross-OS configuration.

## What

- migrate eligible thread starts and builders to auto env
- keep explicit custom/local-executor cases local with rationale
comments
- keep auto-env coverage where possible and add narrow `TODO(anp)` skips
for fixtures that are not target-native yet

Adam Perry @ OpenAI dbd2df27d1 test: migrate TestAppServer callers to builder (#31451)
## Why

Keep the TestAppServer builder API change reviewable by moving the
repository-wide caller migration into a mechanical follow-up.

## What

- replace TestAppServer constructor callsites with equivalent builder
chains
- preserve each caller's automatic-environment, args, program, env,
managed-config, plugin-startup, and JSON-logging behavior
- make no TestAppServer implementation changes

## Validation

- cargo check -p codex-app-server --tests

## Cleanup stack

Adam Perry @ OpenAI c2b3e3b4f5 test: use automatic environments in app-server integration tests (#29789)
## Why

Topology-neutral app-server integration tests should exercise automatic
environment selection so the same setup covers local and remote
executors.

## What

Migrate eligible tests to `TestAppServer::new_with_auto_env()` and
`send_thread_start_request_with_auto_env()`. Leave explicit-topology
tests unchanged, and skip the request-permissions case on Windows with a
TODO for cross-platform tool routing.

## Validation

- `just test -p codex-app-server`
- `bazel test //codex-rs/app-server:app-server-all-wine-exec-test
--test_output=errors`

Stacked on #29788.

Michael Bolin 6536841d89 fix: rename McpServer to TestAppServer (#25701)
This PR brought to you via VS Code rather than Codex...

- opened `codex-rs/app-server/tests/common/mcp_process.rs`
- put the cursor on `McpServer`
- hit `F2` and renamed the symbol to `TestAppServer`
- went to the file tree
- hit enter and renamed `mcp_process.rs` to `test_app_server.rs`
- ran **Save All Files** from the Command Palette
- ran `just fmt`

The End

(Admittedly, most of the local variables for `TestAppServer` are still
named `mcp`, though.)

Alexi Christakis e92c952b2e [codex] Add user input client ids (#24653)
## Summary

Adds an optional `clientId` field to app-server v2 `UserInput` and
carries it through the core `UserInput` model so clients can correlate
echoed user input items without relying on payload equality.

## Details

- Adds `client_id: Option<String>` to core `UserInput` variants.
- Exposes the v2 app-server field as `clientId` on the wire and in
generated TypeScript.
- Preserves the id when converting between app-server v2 and core
protocol types.
- Regenerates app-server schema fixtures.

danwang-oai 11806faf71 Fix hang on turn/interrupt (#18392)
Fix a bug where the `turn/interrupt` RPC hangs when interrupting a turn
that has already completed.

Before this change, `turn/interrupt` requests were queued in app-server
and only answered when a later TurnAborted event arrived. If the target
turn was already complete, core treated Op::Interrupt as a no-op, so no
abort event was emitted and the RPC could hang indefinitely.

This change fixes that in two places:

* Reject turn/interrupt immediately with `INVALID_REQUEST` when the
requested turn is no longer the active turn.
* Resolve any already-accepted pending interrupt requests when the turn
reaches TurnComplete, covering the case where a turn finishes naturally
after the interrupt request is accepted but before it aborts.

I tested this by adding a failing test in
707487c063. You may view the results here:

Eugene BrevdoandCodex bc969b6516 Dismiss stale app-server requests after remote resolution (#15134)
Dismiss stale TUI app-server approvals after remote resolution

When an approval, user-input prompt, or elicitation request is resolved
by another client, the TUI now dismisses the matching local UI instead
of leaving stale prompts behind and emitting a misleading local
cancellation.

This change teaches pending app-server request tracking to map
`serverRequest/resolved` notifications back to the concrete request type
and stable request key, then propagates that resolved request into TUI
prompt state. Approval, request-user-input, and MCP elicitation overlays
now drop the resolved current or queued request quietly, advance to the
next queued request when present, and avoid emitting abort/cancel events
for stale UI.

The latest update also retires matching prompts while they are still
deferred behind active streaming and suppresses buffered active-thread
requests whose app-server request id has already been resolved before
drain. `ChatWidget` removes a resolved request from both the deferred
interrupt queue and the materialized bottom-pane stack, while
active-thread request handling verifies the app-server request is still
pending before showing a prompt. Lifecycle events such as exec begin/end
remain queued so approved work can still render normally.

Tests cover resolved-request mapping, overlay dismissal behavior,
deferred prompt pruning for same-turn user input, exec approval IDs,
lifecycle-event retention, and the buffered active-thread ordering
regression.

Validation:
- `just fmt`
- `git diff --check`
- `cargo test -p codex-tui
resolved_buffered_approval_does_not_become_actionable_after_drain`
- `cargo test -p codex-tui
enqueue_primary_thread_session_replays_buffered_approval_after_attach`
- `cargo test -p codex-tui chatwidget::interrupts`
- `just fix -p codex-tui`

Eric Traut 9f737c28dd Speed up /mcp inventory listing (#16831)
Addresses #16244

This was a performance regression introduced when we moved the TUI on
top of the app server API.

Problem: `/mcp` rebuilt a full MCP inventory through
`mcpServerStatus/list`, including resources and resource templates that
made the TUI wait on slow inventory probes.

Solution: add a lightweight `detail` mode to `mcpServerStatus/list`,
have `/mcp` request tools-and-auth only, and cover the fast path with
app-server and TUI tests.

Testing: Confirmed slow (multi-second) response prior to change and
immediate response after change.

I considered two options:
1. Change the existing `mcpServerStatus/list` API to accept an optional
"details" parameter so callers can request only a subset of the
information.
2. Add a separate `mcpServer/list` API that returns only the servers,
tools, and auth but omits the resources.

I chose option 1, but option 2 is also a reasonable approach.

Ruslan Nigmatullin 69d7a456bb app-server: Replay pending item requests on thread/resume (#12560)
Replay pending client requests after `thread/resume` and emit resolved
notifications when those requests clear so approval/input UI state stays
in sync after reconnects and across subscribed clients.

Affected RPCs:
- `item/commandExecution/requestApproval`
- `item/fileChange/requestApproval`
- `item/tool/requestUserInput`

Motivation:
- Resumed clients need to see pending approval/input requests that were
already outstanding before the reconnect.
- Clients also need an explicit signal when a pending request resolves
or is cleared so stale UI can be removed on turn start, completion, or
interruption.

Implementation notes:
- Use pending client requests from `OutgoingMessageSender` in order to
replay them after `thread/resume` attaches the connection, using
original request ids.
- Emit `serverRequest/resolved` when pending requests are answered
or cleared by lifecycle cleanup.
- Update the app-server protocol schema, generated TypeScript bindings,
and README docs for the replay/resolution flow.

High-level test plan:
- Added automated coverage for replaying pending command execution and
file change approval requests on `thread/resume`.
- Added automated coverage for resolved notifications in command
approval, file change approval, request_user_input, turn start, and turn
interrupt flows.
- Verified schema/docs updates in the relevant protocol and app-server
tests.

Manual testing:
- Tested reconnect/resume with multiple connections.
- Confirmed state stayed in sync between connections.

gt-oai 7c6d21a414 Fix test_shell_command_interruption flake (#10649)
## Human summary
Sandboxing (specifically `LandlockRestrict`) is means that e.g. `sleep
10` fails immediately. Therefore it cannot be interrupted.

In suite::interrupt::test_shell_command_interruption, sleep 10 is issued
at 17:28:16.554 (ToolCall: shell_command {"command":"sleep 10"...}),
then fails at 17:28:16.589 with duration_ms=34, success=false,
exit_code=101, and
    Sandbox(LandlockRestrict).

## Codex summary
- set `sandbox_mode = "danger-full-access"` in `interrupt` and
`v2/turn_interrupt` integration tests
- set `sandbox: Some(SandboxMode::DangerFullAccess)` in
`test_codex_jsonrpc_conversation_flow`
- set `sandbox_policy: Some(SandboxPolicy::DangerFullAccess)` in
`command_execution_notifications_include_process_id`

## Why
On some Linux CI environments, command execution fails immediately with
`LandlockRestrict` when sandboxed. These tests are intended to validate
JSON-RPC/task lifecycle behavior (interrupt semantics, command
notification shape/process id, request flow), but early sandbox startup
failure changes turn flow and can trigger extra follow-up requests,
causing flakes.

This change removes environment-specific sandbox startup dependency from
these tests while preserving their primary intent.

## Testing
- not run in this environment (per request)

Owen Lin 157a16cefa [app-server] feat: add thread_id and turn_id to item and error notifications (#7124)
Add `thread_id` and `turn_id` to `item/started`, `item/completed`, and
`error` notifications. Otherwise the client will have a hard time
knowing which thread & turn (if multiple threads are running in
parallel) a new item/error is for.

Also add `thread_id` to `turn/started` and `turn/completed`.

Michael Bolin a75321a64c fix: add more fields to ThreadStartResponse and ThreadResumeResponse (#6847)
This adds the following fields to `ThreadStartResponse` and
`ThreadResumeResponse`:

```rust
    pub model: String,
    pub model_provider: String,
    pub cwd: PathBuf,
    pub approval_policy: AskForApproval,
    pub sandbox: SandboxPolicy,
    pub reasoning_effort: Option<ReasoningEffort>,
```

This is important because these fields are optional in
`ThreadStartParams` and `ThreadResumeParams`, so the caller needs to be
able to determine what values were ultimately used to start/resume the
conversation. (Though note that any of these could be changed later
between turns in the conversation.)

Though to get this information reliably, it must be read from the
internal `SessionConfiguredEvent` that is created in response to the
start of a conversation. Because `SessionConfiguredEvent` (as defined in
`codex-rs/protocol/src/protocol.rs`) did not have all of these fields, a
number of them had to be added as part of this PR.

Because `SessionConfiguredEvent` is referenced in many tests, test
instances of `SessionConfiguredEvent` had to be updated, as well, which
is why this PR touches so many files.

Celia Chen b395dc1be6 [app-server] introduce turn/completed v2 event (#6800)
similar to logic in
`codex/codex-rs/exec/src/event_processor_with_jsonl_output.rs`.
translation of v1 -> v2 events:
`codex/event/task_complete` -> `turn/completed`
`codex/event/turn_aborted` -> `turn/completed` with `interrupted` status
`codex/event/error` -> `turn/completed` with `error` status

this PR also makes `items` field in `Turn` optional. For now, we only
populate it when we resume a thread, and leave it as None for all other
places until we properly rewrite core to keep track of items.

tested using the codex app server client. example new event:
```
< {
<   "method": "turn/completed",
<   "params": {
<     "turn": {
<       "id": "0",
<       "items": [],
<       "status": "interrupted"
<     }
<   }
< }
Owen Lin 6582554926 [app-server] feat: v2 Turn APIs (#6216)
Implements:
```
turn/start
turn/interrupt
```
along with their integration tests. These are relatively light wrappers
around the existing core logic, and changes to core logic are minimal.
However, an improvement made for developer ergonomics:
- `turn/start` replaces both `SendUserMessage` (no turn overrides) and
`SendUserTurn` (can override model, approval policy, etc.)
