0318381762 Replace SkillsManager with SkillsService (#28705)
## Why

Host skill discovery was still exposed as a manager even though it is a
process-owned service shared by sessions, the app-server catalog, and
file-watcher invalidation. The skills extension also consumed an ad hoc
loaded-skills wrapper instead of a named immutable snapshot.

## What changed

- replace `SkillsManager` with concrete `SkillsService`
- make the service cache and return immutable `HostSkillsSnapshot`
values
- migrate the skills extension host provider to the snapshot boundary
- migrate app-server catalog, watcher, and invalidation paths to the
service

This keeps the service limited to host discovery, caching, roots, and
invalidation. Catalog rendering and invocation remain extension
responsibilities for the next stacked change.

5b22a8e5b1 feat: render typed envelopes for multi-agent v2 messages (#28368)
## Why

Multi-agent v2 messages need a consistent, model-visible envelope that
identifies what kind of interaction occurred, who sent it, and which
agent it targets. Previously, encrypted deliveries exposed only
`encrypted_content`, while child completion used the legacy
`<subagent_notification>` shape. That meant the client could not
consistently present `NEW_TASK`, `MESSAGE`, and `FINAL_ANSWER` using the
same format.

This change adds the routing envelope as plaintext while keeping task
and message payloads encrypted. No new Responses API field is required:
an encrypted delivery is represented as an `input_text` header
immediately followed by its existing `encrypted_content` item.

Every envelope now follows this shape:

```text
Message Type: <NEW_TASK | MESSAGE | FINAL_ANSWER>
Task name: <recipient agent path>
Sender: <author agent path>
Payload:
<message payload>
```

## Message types

### `NEW_TASK`

`NEW_TASK` is used when the recipient should begin a new turn, including
an initial `spawn_agent` task and a later `followup_task`.

For a root agent spawning `/root/worker`, the request contains a
plaintext envelope followed by the encrypted task:

```json
{
  "type": "agent_message",
  "author": "/root",
  "recipient": "/root/worker",
  "content": [
    {
      "type": "input_text",
      "text": "Message Type: NEW_TASK\nTask name: /root/worker\nSender: /root\nPayload:\n"
    },
    {
      "type": "encrypted_content",
      "encrypted_content": "<encrypted task payload>"
    }
  ]
}
```

Conceptually, the model receives:

```text
Message Type: NEW_TASK
Task name: /root/worker
Sender: /root
Payload:
Review the authentication changes and report any regressions.
```

### `MESSAGE`

`MESSAGE` is used for a queued `send_message` delivery. It communicates
with an existing agent without starting a new turn.

For `/root/worker` reporting progress to the root agent, the request
contains:

```json
{
  "type": "agent_message",
  "author": "/root/worker",
  "recipient": "/root",
  "content": [
    {
      "type": "input_text",
      "text": "Message Type: MESSAGE\nTask name: /root\nSender: /root/worker\nPayload:\n"
    },
    {
      "type": "encrypted_content",
      "encrypted_content": "<encrypted message payload>"
    }
  ]
}
```

Conceptually, the model receives:

```text
Message Type: MESSAGE
Task name: /root
Sender: /root/worker
Payload:
The protocol tests pass; I am checking the resume path now.
```

### `FINAL_ANSWER`

`FINAL_ANSWER` is emitted when a child agent reaches a terminal state
and reports its result to its parent. Completion payloads are already
available locally, so the complete envelope is represented as plaintext
rather than as a plaintext header plus encrypted content.

For `/root/worker` completing work for the root agent, the request
contains:

```json
{
  "type": "agent_message",
  "author": "/root/worker",
  "recipient": "/root",
  "content": [
    {
      "type": "input_text",
      "text": "Message Type: FINAL_ANSWER\nTask name: /root\nSender: /root/worker\nPayload:\nNo regressions found."
    }
  ]
}
```

The model-visible form is:

```text
Message Type: FINAL_ANSWER
Task name: /root
Sender: /root/worker
Payload:
No regressions found.
```

Errored, shut down, and missing agents also use `FINAL_ANSWER`, with a
terminal-status description in the payload.

## What changed

- Render `NEW_TASK` or `MESSAGE` in
`InterAgentCommunication::to_model_input_item`, based on whether the
encrypted delivery starts a turn.
- Replace the multi-agent v2 `<subagent_notification>` completion
payload with a model-visible `FINAL_ANSWER` envelope.
- Document `Task name`, `Sender`, and `Payload` consistently in the
multi-agent developer instructions.
- Prevent local-only history projections from treating an encrypted
message's plaintext header as the complete assistant message.
- Preserve rollout-trace interaction edges when an agent message
contains both plaintext and encrypted content.

Legacy multi-agent behavior remains unchanged.

022f1221e8 [codex] Bind shell snapshots to retained thread environments (#28421)
## Why

Shell snapshots are currently session-scoped even though shell and cwd
are properties of a selected turn environment. That makes snapshot
refresh depend on separate session-cwd plumbing, prevents retained
environments from retaining their snapshot work, and can make snapshot
construction use a different shell than command execution.

This follows #27955 by making the retained thread-environment service
own environment snapshot lifecycles. Session configuration remains the
requested selection state, while `ThreadEnvironments` remains the source
of successfully resolved environments.

## What changed

- Configure the shell-snapshot builder before initial environment
resolution.
- Start each local environment snapshot task when its `TurnEnvironment`
is built and retain that shared task while environment ID and cwd still
match.
- Inherit retained environment snapshots into spawned child threads.
- Carry the selected `TurnEnvironment` through shell runtimes so
snapshot construction and command execution use the same
environment-specific shell and cwd.
- Load project instructions and warm plugins/skills after initial
environment resolution.
- Continue decoding invalid UTF-8 instruction files lossily without
emitting a startup warning.
- Keep requested selections in `SessionConfiguration`; failed or
duplicate resolutions only affect the resolved environment snapshot.

## Validation

- `just test -p codex-home instructions` (6 passed)
- Focused environment, instruction, shell-snapshot, and user-shell tests
(84 passed)
- Focused shell-snapshot, user-shell, and unified-exec tests (126
passed; two event-timing tests passed on retry)

040dafa32d feat(core): add metadata field to ResponseItem (#28355)
## Description

This PR adds an optional `metadata` field to `ResponseItem` for
Responses API calls. Only mechanical plumbing, no actual values
populated and sent yet. Turns out just adding a new field to
`ResponseItem` has quite a large blast radius already.

This change is backwards compatible because `metadata` is optional and
omitted when absent, so existing response items and rollout history
without it still deserialize and requests that do not set it keep the
same wire shape. For provider compatibility, we strip out `metadata`
before non-OpenAI Responses requests so Azure and AWS Bedrock never see
this field.

My followup PR here will actually make use of it to start storing and
passing along `turn_id`: https://github.com/openai/codex/pull/28360

## What changed

- Added `ResponseItemMetadata` with optional `turn_id`, plus optional
`metadata` on Responses API item variants and inter-agent communication.
- Preserved item metadata through response-item rewrites such as
truncation, missing tool-output synthesis, compaction history
rebuilding, visible-history conversion, rollout/resume, and generated
app-server schemas/types.
- Strip item metadata from non-OpenAI Responses requests while
preserving it for OpenAI-shaped requests.
- Updated the mechanical fixture/test construction churn required by the
new optional field.

1aa3e432cc [codex] simplify shell snapshot ownership (#27756)
## Why

Shell snapshot lifecycle state was split between `Shell` and
`SessionServices`: `Shell` carried the receiver while session code
exposed and forwarded the raw sender. That coupled shell identity to
mutable snapshot state and made refresh, inheritance, and file lifetime
harder to reason about.

## What changed

- make each `Arc<ShellSnapshot>` represent one cwd-specific snapshot
generation
- store the active generation in `SessionServices` with `ArcSwapOption`
- have construction start the background build and expose only a
cwd-validated snapshot path
- use `ShellSnapshotFile` ownership to delete snapshot files
automatically
- pass snapshot paths explicitly to shell runtimes instead of storing
snapshot state on `Shell`
- preserve inherited and in-flight generations by pinning their `Arc`
while they are in use

8f2d6416ce Support plaintext agent messages (#27830)
## Why

Multi-agent v2 `send_message` deliveries already reach the receiving
model as typed `agent_message` items with encrypted content.
Child-completion notifications are generated by Codex itself, so their
content is plaintext and previously fell back to a serialized JSON
envelope inside an assistant message.

With plaintext `input_text` supported for `agent_message`, both delivery
paths can use the same model-visible type while preserving explicit
author and recipient metadata.

## What changed

- add plaintext `input_text` support to `AgentMessageInputContent` and
regenerate the affected app-server schemas
- preserve `InterAgentCommunication` as structured mailbox input instead
of converting it to assistant text
- record delivered communications as typed `agent_message` history items
- persist a dedicated rollout item so local delivery metadata such as
`trigger_turn` remains available without leaking into the Responses
request
- reconstruct typed agent messages on resume and preserve fork-turn
truncation behavior
- remove request-time assistant-content parsing
- preserve plaintext and encrypted inter-agent deliveries in stage-one
memory inputs
- normalize and link plaintext and encrypted agent messages in rollout
traces without treating inbound messages as child results
- cover the real MultiAgent V2 child-completion path end to end with
deterministic mailbox synchronization

a19d43a40a Add app-server thread/delete API (#25018)
## Why

Clients can archive and unarchive threads today, but there is no
app-server API for permanently removing a thread. Deletion also needs to
cover the full session tree: deleting a main thread should remove
spawned subagent threads and the related local metadata instead of
leaving orphaned rollout files, goals, or subagent state behind.

## What

- Adds the v2 `thread/delete` request and `thread/deleted` notification,
with the response shape kept consistent with `thread/archive`.
- Implements local hard delete for active and archived rollout files.
- Deletes the requested thread's state DB row as the commit point, then
best-effort cleans associated state including spawned descendants,
goals, spawn edges, logs, dynamic tools, and agent job assignments.
- Updates app-server API docs and generated protocol schema/TypeScript
fixtures.

30ddb3325e [codex] Store compact window id in rollout (#27264)
## Why

Compaction window identity is part of session history, not model-client
transport state. Persisting it with the compacted rollout item lets
resumed threads continue from the reconstructed window without keeping
mutable window state on `ModelClient`.

## What changed

- Added `window_id` to `CompactedItem` and stamp it when
`replace_compacted_history` installs compacted history.
- Moved auto-compact window id ownership into `AutoCompactWindow` /
`SessionState`; `ModelClient` now receives the request window id from
callers instead of storing it.
- Returned `window_id` from rollout reconstruction for resume.
Reconstruction uses the newest surviving compacted item's stored
`window_id` when present, and falls back to the legacy compacted-item
count when it is absent.
- Kept fork startup at the fresh default window id and updated direct
model-client tests to pass explicit test window ids.

f3c1283411 Pair thread environment settings (#26687)
## Why

Thread cwd and environment selections are a single logical setting in
core: updating one without the other can silently desynchronize the
next-turn execution context. This change makes that relationship
explicit in the internal thread settings flow while preserving the
existing app-server public API shape.

## What changed

- Moved the cwd/environment pair through internal
`ThreadSettingsOverrides.environment_settings` instead of a top-level
internal `cwd` field.
- Kept `thread/settings/update` public params unchanged, with app-server
translating top-level `cwd` into the paired internal settings shape.
- Moved `Op::UserInput` environment overrides into thread settings so
user turns and settings updates use the same core path.
- Updated core, app-server, MCP, memories, sample, and test callsites to
construct the paired settings shape.

0526cb56ac Avoid reopening v2 descendants on resume (#26997)
## Why

Multi-agent v2 residency is intended to keep only the threads that need
to be live. The existing rollout resume path still walked persisted open
descendants and reopened the entire descendant tree when resuming a v2
root, which turns resume into an eager reload of work that should stay
unloaded until it is explicitly needed.

The interrupted-agent path has a related residency issue. Interrupted
agents remain open by design, so an idle interrupted resident should be
eligible for eviction just like an idle completed or errored resident.
Otherwise a resident set full of interrupted agents can consume every v2
slot and block later spawns or reloads with `AgentLimitReached`.

## What Changed

- Return early from `resume_agent_from_rollout` after resuming a v2
thread so persisted v2 descendants are not reopened eagerly.
- Treat idle `Interrupted` v2 residents as unloadable in the LRU
residency path.
- Add focused coverage for v2 root resume leaving descendants unloaded
and for eviction of an idle interrupted v2 resident when a new slot is
needed.

## Verification

Added targeted `codex-core` tests covering:

- v2 root resume with persisted descendants, verifying only the root is
loaded after resume.
- residency eviction of an idle interrupted v2 agent when the resident
set is full.

8d415050fc Rename multi-agent v2 close_agent to interrupt_agent (#26994)
## Why

`close_agent` is the wrong model-facing name for the v2 operation after
the residency changes. V2 agents remain reusable by task name, and
residency/unloading owns capacity management; the exposed tool should
describe the action it actually performs: interrupt the target agent's
current turn without making the agent unavailable for future messages or
follow-up tasks.

## What changed

- Rename the multi-agent v2 tool from `close_agent` to
`interrupt_agent`.
- Keep the v1 `close_agent` surface unchanged.
- Update the v2 handler to send `Op::Interrupt`, keep interrupted agents
registered, and reject root/self targets with interrupt-specific errors.
- Route interrupt delivery through the existing dead-thread cleanup path
so stale resident entries do not keep consuming capacity.
- Update tool planning and handler tests for the new v2 surface and
semantics.

743f5aad38 feat: count V2 concurrency by active execution (#26969)
## Why

Multi-Agent V2 concurrency should count active non-root turns, not
resident or durable agent threads. The limit is intentionally best
effort: admission checks are synchronous, but concurrent successful
checks may overshoot slightly.

## What changed

- Keep one root-derived execution limit on the shared `AgentControl`.
- Count active V2 subagent turns with an RAII guard owned by
`RunningTask`.
- Check capacity before spawning or starting an idle agent, including
direct app-server `turn/start` submissions.
- Preserve queued delivery for agents that are already running.
- Exempt automatic idle continuations so `/goal` work is not dropped
when capacity is temporarily full.
- Keep root and V1 turns outside this limiter.

4e803a017c feat: add v2 agent residency lru (#26632)
## Why

Multi-agent v2 treats agents as durable logical agents, not just live
entries in `ThreadManager`. After the reload-on-delivery change, a v2
agent can be addressed even if its thread is not currently loaded.

This PR adds the next layer: loaded v2 subagents can be paged out of
`ThreadManager` when the session has too many resident agents. That
keeps residency separate from logical identity and prepares the stack
for making v2 concurrency count active execution instead of existing
agents.

## What Changed

- Add an `AgentControl`-scoped LRU for resident v2 subagents.
- Reserve residency before spawning or reloading a v2 subagent.
- If resident capacity is full, unload the least-recently-used idle v2
subagent from `ThreadManager`.
- Keep `ThreadManager` as a primitive loaded-thread store; it does not
own the LRU policy.
- Keep unloaded agents registered and durable so they can be reloaded by
the delivery path.
- Preserve the existing v2 cap semantics by using the derived non-root
v2 cap for residency.

Eviction is intentionally conservative. A thread is unloadable only when
it is a v2 subagent, has completed or errored, has no active turn, and
has no pending mailbox work. Before removal, the rollout is materialized
and flushed.

## Assumptions And Non-Goals

- PR #26623 provides the reload-on-delivery path for unloaded v2 agents.
- `ThreadManager` membership means loaded/resident, not logical agent
existence.
- `AgentRegistry` remains the logical identity/metadata source for v2
agents that may be unloaded.
- `list_agents` remains a recent/resident view for now.
- This does not change active execution concurrency; that is the next
PR.
- This does not change `close_agent` semantics.
- This does not change or remove `resume_agent`.
- This does not add a new residency config knob.

## Stack

1. V2 durable lookup and reload on delivery (#26623) - reload unloaded
v2 agents before delivering follow-up/input.
2. V2 residency LRU (this PR) - unload idle resident v2 agents from
`ThreadManager` when resident capacity is full.
3. V2 active-execution concurrency - count running non-root v2 turns
instead of logical agents.
4. V2 close/interrupt semantics - make v2 close interrupt the current
turn without deleting durable identity.
5. V2 resume cleanup - remove the manual resume surface for v2 while
keeping internal reload support.

## Validation

- Added focused coverage for the residency LRU eviction path.
- Local clippy/check/tests were not run; CI will cover them.

d5e4f01af4 feat: reload v2 agents on delivery (#26623)
## Summary

This is the first small step toward making multi-agent v2 agents durable
logical agents whose `ThreadManager` residency is only an implementation
detail.

This PR adds a narrow v2 reload-on-delivery hook:

- If a known v2 agent target is already loaded, delivery is unchanged.
- If the target is still registered but missing from `ThreadManager`,
delivery reloads that exact v2 thread from durable rollout history
before submitting the message.
- If the target is unknown, closed, missing from storage, or not a v2
thread, delivery still fails as not found.

The reload is wired only into existing-agent delivery paths: v2
`send_message` / `followup_task`, and legacy `send_input` when its
target is a known v2 agent.

## Stack

1. **Reload on delivery**: load known unloaded v2 agents before
`followup_task`, `send_message`, or `send_input` delivery. This PR.
2. **Residency LRU**: unload idle resident v2 agents from
`ThreadManager` without making them closed or unreachable.
3. **Execution concurrency**: count active non-root turns, not logical
agents or resident idle threads.
4. **Close semantics**: make v2 close interrupt-only and leave durable
agent identity intact.
5. **Resume cleanup**: remove user-facing v2 resume semantics;
addressing an unloaded durable agent reloads it implicitly.

## Validation

- Ran `just fmt`.
- Left broader tests and clippy to CI.

0b1512c2c8 refactor: split agent control modules (#26610)
## Summary

Mechanically splits `AgentControl` into focused modules so later agent
runtime changes are easier to review. The shared lookup, messaging, and
completion logic remains in `control.rs`, while spawn-specific code and
V1 legacy close/resume behavior move into dedicated files.

## Changes

- Extract spawn-agent code into `agent/control/spawn.rs`.
- Extract V1-only legacy close/resume behavior into
`agent/control/legacy.rs`.
- Keep shared control-plane behavior in `agent/control.rs`.
- Preserve existing behavior; this PR is intended to be mechanical.

## Stack

1. This PR - Mechanical `AgentControl` split: extracts spawn and V1
legacy code without behavior changes.
2. #26614 - Execution slot accounting: separates logical agents from
active execution slots.
3. #26611 - Residency and reload runtime: adds resident-agent LRU,
eviction/reload, durable lookup, and V2 delivery through reload.
4. #26612 - V2 tool semantics: narrows `close_agent` to interrupt-only
and updates V2 tool coverage.

5f4d06ef18 Encrypt multi-agent v2 message payloads (#26210)
## Why

Multi-agent v2 currently routes agent instructions through normal tool
arguments and inter-agent context. That means the parent model can emit
plaintext task text, Codex can persist it in history/rollouts, and the
recipient can receive it as ordinary assistant-message JSON.

This changes the v2 path so agent instructions stay encrypted between
model calls: Responses encrypts the `message` argument returned by the
model, Codex forwards only that ciphertext, and Responses decrypts it
internally for the recipient model.

## What changed

- Mark the v2 `message` parameter as encrypted for `spawn_agent`,
`send_message`, and `followup_task`.
- Treat multi-agent v2 tool `message` values as ciphertext
unconditionally.
- Store v2 inter-agent task text in
`InterAgentCommunication.encrypted_content` with empty plaintext
`content`.
- Convert encrypted inter-agent communications into the Responses
`agent_message` input item before sending the child request.
- Preserve `agent_message` items across history, rollout, compaction,
telemetry, and app-server schema paths.
- Leave multi-agent v1 unchanged.

## Message shape

The model still calls the v2 tools with a `message` argument, but that
value is now ciphertext:

```json
{
  "name": "spawn_agent",
  "arguments": {
    "task_name": "worker",
    "message": "<ciphertext>"
  }
}
```

Codex stores the task as encrypted inter-agent communication:

```json
{
  "author": "/root",
  "recipient": "/root/worker",
  "content": "",
  "encrypted_content": "<ciphertext>",
  "trigger_turn": true
}
```

When Codex builds the recipient request, it forwards the ciphertext
using the new Responses input item:

```json
{
  "type": "agent_message",
  "author": "/root",
  "recipient": "/root/worker",
  "content": [
    {
      "type": "encrypted_content",
      "encrypted_content": "<ciphertext>"
    }
  ]
}
```

Responses decrypts that item internally for the recipient model.

## Context impact

- Parent context no longer carries plaintext v2 agent task instructions
from these tool arguments.
- Codex rollout/history stores ciphertext for v2 agent instructions.
- Recipient requests receive an `agent_message` item instead of
assistant commentary JSON for encrypted task delivery.
- Plaintext completion/status notifications are still plaintext because
they are Codex-generated status messages, not encrypted model tool
arguments.

11bceb8f8b feat: catalog multi-agent v2 config (#26254)
## Why

Model metadata can now select multi-agent v2 even when a user has not
enabled `features.multi_agent_v2` in their config. Some existing configs
still set the legacy `agents.max_threads` knob for v1 multi-agent
behavior, so treating every v2 runtime as incompatible with
`agents.max_threads` would break users whose only v2 signal came from
the model catalog.

The incompatible configuration is specifically enabling
`features.multi_agent_v2` while also setting `agents.max_threads`.
Catalog-forced v2 should use the v2 concurrency setting and ignore the
legacy v1 cap instead of rejecting the config.

## What changed

- Split config validation from runtime concurrency calculation:
`effective_agent_max_threads` now just returns the effective cap for the
resolved multi-agent runtime.
- Added explicit validation for `features.multi_agent_v2` +
`agents.max_threads` at session startup.
- Preserved catalog-selected v2 behavior when `features.multi_agent_v2`
is disabled, so existing configs with `agents.max_threads` keep
starting.
- Updated model-runtime selector coverage so a catalog v2 model still
exposes v2 tools even when `agents.max_threads` is set and the config
flag is disabled.

11e0f3d3ae app-server: remove experimental persist_extended_history bool flag (#25712)
## Summary

Remove the dead experimental `persistExtendedHistory` app-server flag
and collapse rollout persistence to the single policy app-server already
used.

## What Changed

- Removed `persistExtendedHistory` from v2 thread start/resume/fork
params and deleted its deprecation notice path.
- Removed the persistence-mode enums and plumbing through core, rollout,
and thread-store.
- Made rollout filtering mode-free, keeping the existing limited
persisted-history behavior.

## Test Plan

cf0911076f store and expose parent_thread_id on Threads (#25113)
## Why

This PR
https://github.com/openai/codex/pull/24161#discussion_r3325692763
revealed a subagent data modeling issue, where we overloaded
`forked_from_id` to also mean `parent_thread_id`. That's incorrect since
guardian and review subagents can be a subagent and NOT fork the main
thread's history.

The solution here is to explicitly store a new `parent_thread_id` on
`SessionMeta`, alongside `forked_from_id` which already exists. While
we're at it, also expose it in the app-server protocol on the `Thread`
object.

A thread->subagent relationship and a fork of thread history are
orthogonal concepts.

## What Changed

- Added top-level `parent_thread_id` persistence on `SessionMeta` and
runtime/session plumbing through `SessionConfiguredEvent`,
`CodexSpawnArgs`, `SessionConfiguration`, `ThreadConfigSnapshot`,
`TurnContext`, and `ModelClient`.
- Made turn metadata, request headers, analytics, and subagent-start
events read the separate runtime/top-level parent field instead of
deriving general parent lineage from `SessionSource` or
`forked_from_thread_id`.
- Passed parent lineage separately at delegated subagent, review,
guardian, agent-job, and multi-agent spawn construction sites;
copied-history fork lineage remains derived only from `InitialHistory`.
- Persisted and exposed parent lineage through rollout/thread-store
projections and app-server v2 `Thread.parentThreadId`.
- Updated app-server README text and regenerated app-server schema
fixtures for the additive `parentThreadId` response field.

fc9cf62efb Add subagent lineage metadata for responsesapi (#24161)
## Why

We recently added `forked_from_thread_id` which lets us trace where a
thread's _context_ comes from, but we also want to understand subagent
lineage (e.g. which parent thread spawned this subagent? what kind of
subagent is it?) which is orthogonal.

This PR adds `parent_thread_id` and `subagent_kind` to the
`x-codex-turn-metadata` header sent to ResponsesAPI.

## What changed

- Adds `parent_thread_id` and `subagent_kind` to core-owned
`x-codex-turn-metadata`.
- Restores persisted `SessionSource` and `ThreadSource` from resumed
session metadata so cold-resumed subagent threads keep their lineage on
later Responses API requests.
- Centralizes parent-thread extraction on `SessionSource` /
`SubAgentSource` and reuses it in the Responses client, analytics, agent
control, and state parsing paths.
- Extends reserved-key, git-enrichment, thread-spawn, and app-server v2
metadata coverage for the new lineage fields.

## Verification

- Not run locally per request.
- Added focused coverage in `core/src/turn_metadata_tests.rs` and
`app-server/tests/suite/v2/client_metadata.rs`.

e2551a5e36 Reap stale multi-agent slots (#24903)
## Summary

- Let `close_agent` clean up an agent that is still registered in
`AgentRegistry` even when its underlying thread is already missing.
- Preserve the explicit-close boundary: for known stale thread-spawn
agents, mark the persisted spawn edge `Closed`, then treat
`ThreadNotFound` / `InternalAgentDied` as a successful close so the
registry slot can be released.
- Add a regression for MultiAgentV2 task-name targets where
`close_agent("worker")` succeeds after the worker thread has already
disappeared.

## Motivation

A worker can disappear from `ThreadManager` while its metadata still
exists in the root `AgentRegistry`. Before this change, the close tool
failed while trying to subscribe to the missing thread status, so it
never reached the cleanup path that releases the registered agent slot.
With `agents.max_threads = 1`, an explicit close of that stale task-name
agent could fail and leave the session unable to spawn a replacement.

## Scope

This PR intentionally does not add automatic stale-agent reaping to
`spawn_agent`, `resume_agent`, or `list_agents`. A thread being missing
from `ThreadManager` is not the same as an explicit close: persisted
open spawn edges are still the durable source of truth for resume and
task-name ownership until `close_agent` is called.

## Validation

- `just test -p codex-core -E

test(resume_agent_from_rollout_reopens_open_descendants_after_manager_shutdown)'`

487521733b [codex-analytics] add grouped session id to runtime events (#24655)
## Why
- Runtime analytics events report `thread_id`, which identifies the
individual thread emitting an event
- They don't report `session_id`, which identifies the shared session
for a root thread and its subagent threads
- Emitting both identifiers allows analytics to group related activity

## What Changed
- Adds `session_id` to relevant analytics events (thread_initalized,
turn, turn_steer, compaction, guardian_review)
- Tracks each thread's session ID in the analytics reducer so subsequent
thread scoped events emit the same value
- Carries the shared session ID through subagent initialization

## Verification
- `just test -p codex-analytics` validates event payloads and subagent
session grouping.
- Focused `codex-app-server` tests validate session IDs for thread,
turn, and steer events.
- Focused `codex-core` tests validate root and subagent session ID
propagation.

1911021c0e Add forked_from_thread_id turn metadata (#24160)
## Why

When Codex calls responsesapi, we currently send `session_id`,
`thread_id`, and `turn_id` among other things as
`client_metadata["x-codex-turn-metadata"]`. This PR adds
`forked_from_thread_id` which helps explain the "lineage" of a forked
thread.

## What's changed

- Track the immediate history source copied into a forked thread through
thread/session creation, including subagent and review turn metadata
paths.
- Include `forked_from_thread_id` in Codex turn metadata while
preventing turn-scoped Responses API client metadata from overwriting
Codex-owned lineage fields.
- Add coverage for fork lineage in turn metadata and the app-server
Responses API request path.

768848ab6f Add experimental turn additional context (#24154)
## Summary

Adds experimental `additionalContext` support to `turn/start` and
`turn/steer` so clients can provide ephemeral external context, such as
browser or automation state, without turning that plumbing into a
visible user prompt or triggering user-prompt lifecycle behavior.

## API Shape

The parameter shape is:

```ts
additionalContext?: Record<string, {
  value: string
  kind: "untrusted" | "application"
}> | null
```

Example:

```json
{
  "additionalContext": {
    "browser_info": {
      "value": "Active tab is CI failures.",
      "kind": "untrusted"
    },
    "automation_info": {
      "value": "CI rerun is in progress.",
      "kind": "application"
    }
  }
}
```

The keys are opaque and caller-defined.

## Context Injection

When provided, accepted entries are inserted into model context as
hidden contextual message items, not as visible thread user-message
items.

`kind: "untrusted"` entries are inserted with role `user`:

```text
<external_${key}>${value}</external_${key}>
```

`kind: "application"` entries are inserted with role `developer`:

```text
<${key}>${value}</${key}>
```

Values are not escaped. Each value is truncated to 1k approximate tokens
before wrapping.

For `turn/start`, accepted additional context is inserted before normal
user input. For `turn/steer`, additional context is merged only when the
steer includes non-empty user input; context-only steers still reject as
empty input.

## Dedupe Strategy

`AdditionalContextStore` lives on session state and stores the latest
complete additional-context map.

Each `turn/start` or non-empty `turn/steer` treats its
`additionalContext` as the current complete set of values. Entries are
injected only when the key is new or the exact entry for that key
changed, including `value` or `kind`. After merging, the store is
replaced with the provided map, so omitted keys are removed from the
retained set and can be injected again later if reintroduced.

Omitting `additionalContext`, passing `null`, or passing an empty object
resets the store to empty and injects nothing.

## What Changed

- Threads experimental v2 `additionalContext` through app-server into
core turn start and steer handling.
- Adds separate contextual fragment types for untrusted user-role
context and application developer-role context.
- Uses pending response input items so additional context can be
combined with normal user input without treating it as prompt text.
- Adds integration coverage for start/steer flow, role routing,
dedupe/reset behavior, deletion/re-add behavior, hook-blocked input
behavior, empty context-only steer rejection, external-fragment marker
matching, and truncation.

5865ec45e5 Avoid config snapshots in live agent subtree traversal (#24057)
## Why
`/feedback` asks `ThreadManager` for the selected agent subtree before
it uploads logs. The previous live subtree path reconstructed
parent-child links by iterating every loaded thread and awaiting each
thread config snapshot, so unrelated loaded-thread state could stall
feedback subtree enumeration.

The loaded-thread set already belongs to
[`ThreadManagerState`](50e6644c94/codex-rs/core/src/thread_manager.rs).
Reading thread-spawn parents from the captured `CodexThread` session
sources at that boundary keeps unload and resume behavior manager-owned
while avoiding per-session config inspection.

## What Changed
- expose parent-child thread-spawn edges for loaded, non-internal
threads from `ThreadManagerState`

- add regression coverage for live subtree enumeration when no state DB

fd72e99384 config: remove legacy profile v1 resolution (#24051)
## Why

[#23883](https://github.com/openai/codex/pull/23883) moved user-facing
`--profile` selection onto profile v2, and
[#23886](https://github.com/openai/codex/pull/23886) removed the old CLI
`config_profile` override path. Core still had a second legacy path:
`profile = "..."` could select `[profiles.*]` values while runtime
config was built. Keeping that resolver alive preserves the old
precedence model and profile-carrying surfaces even though profile
selection now points at `$CODEX_HOME/<name>.config.toml`.

## What

- Reject legacy top-level `profile = "..."` config while loading runtime
config, with an error that points callers at `--profile <name>` and
`<name>.config.toml` in the [core load
path](3d923366ec/codex-rs/core/src/config/mod.rs (L2524-L2531)).
- Remove the remaining profile-v1 merge points from runtime config
resolution, including features, permissions, model/provider selection,
web search, Windows sandbox settings, TUI settings, role reloads, and
OSS provider lookup.
- Drop the leftover profile override surface from
[`ConfigOverrides`](3d923366ec/codex-rs/core/src/config/mod.rs (L2118-L2148))
and from the MCP server `codex` tool schema.
- Prune profile-precedence tests that only exercised the removed
resolver and replace them with rejection coverage for the legacy
selector.

## Testing

- Not run in this metadata pass.
- Added
[`legacy_profile_selection_is_rejected`](3d923366ec/codex-rs/core/src/config/config_tests.rs (L7942-L7965))
coverage for the new runtime guard.

c53da029bc [codex] Honor role-defined spawn service tiers (#22169)
## Why
Custom agent roles are ordinary config layers, so a role file can
already express `service_tier` just like other config values. The
spawned-agent tier path needs to preserve that effective role config and
follow the same precedence pattern as model/reasoning.

## What changed
- Apply an explicit spawn-time `service_tier` onto the child config
before role application, so a role config layer can override it just
like role-defined model/reasoning settings do.
- Validate the final effective child tier after the final child model is
known, while still falling back to the parent tier when no child tier
survives.
- Add focused integration coverage for both v1 and v2 proving role TOML
loads a service tier, spawned children keep that role-configured tier,
and a role tier wins over a conflicting spawn-time tier.

## Validation
- `just fmt`
- `git diff --check`
- Local Rust tests not run, per repo guidance; CI should exercise the
new coverage.

826b2182ed Preserve context baselines for full-history agent forks (#23352)
## Why

Full-history agent forks should continue from the same prompt prefix as
the parent. Dropping the stored `TurnContext` baseline forced the child
to rebuild startup context on its first turn, which can duplicate
developer instructions and also loses the cache continuity that a
full-history fork is supposed to preserve.

Truncated forks are different: once we keep only the last N turns, the
original prompt prefix is no longer intact, so the child must establish
a fresh context baseline.

## What changed

- Preserve `RolloutItem::TurnContext` when forking with
`SpawnAgentForkMode::FullHistory`, and keep dropping it for truncated
forks:
4090717d94/codex-rs/core/src/agent/control.rs (L98-L126)
and
4090717d94/codex-rs/core/src/agent/control.rs (L399-L401)
- Remove the special-case MultiAgentV2 usage-hint filtering path.
Full-history fork now preserves the cached developer prefix instead of
trying to reconstruct part of it.
- Extend the fork coverage to assert both sides of the contract:
full-history forks keep the parent reference baseline, while last-N
forks rebuild context after truncation:
4090717d94/codex-rs/core/src/agent/control_tests.rs (L603-L759)
and

84d941d07f [1 of 7] Add thread settings to UserInput (#23080)
**Stack position:** [1 of 7]

## Summary

The first three PRs in this stack are a cleanup pass before the actual
thread settings API work.

Today, core has several overlapping "user input" ops: `UserInput`,
`UserInputWithTurnContext`, and `UserTurn`. They differ mostly in how
much next-turn state they carry, which makes the later queued thread
settings update harder to reason about and review.

This PR starts that cleanup by adding the shared
`ThreadSettingsOverrides` payload and allowing `Op::UserInput` to carry
it. Existing variants remain in place here, so this layer is mostly a
behavior-preserving API shape change plus mechanical constructor
updates.

## End State After PR3

By the end of PR3, `Op::UserInput` is the only "user input" core op. It
can carry optional thread settings overrides for callers that need to
update stored defaults with a turn, while callers without updates use
empty settings. `Op::UserInputWithTurnContext` and `Op::UserTurn` are
deleted.

## End State After PR5

By the end of PR5, core will have only two ops for this area:

- `Op::UserInput` for user-input-bearing submissions.
- `Op::ThreadSettings` for settings-only updates.

afa0101ae2 [codex] Move pending input into input queue (#22728)
## Why

Pending model input was split across `Session`, `TurnState`, and the
agent mailbox. That made it easy for new paths to manage queued user
input or mailbox delivery outside the intended ownership boundary.

This PR consolidates the model-facing input lifecycle behind the session
input queue so turn-local pending input, next-turn queued items, and
mailbox delivery coordination are owned in one place.

## What Changed

- Added `session/input_queue.rs` to own pending input queues and mailbox
delivery coordination.
- Removed the standalone `agent/mailbox.rs` channel wrapper and store
mailbox items directly in the input queue.
- Moved pending-input mutations off `TurnState`; `TurnState` now exposes
the queue-owned storage directly for now.
- Routed abort cleanup, mailbox delivery phase changes, next-turn queued
items, and active-turn pending input through `InputQueue`.
- Boxed stack-heavy agent resume/fork startup futures that the refactor
pushed over the default test stack.
- Updated session, task, goal, stream-event, and multi-agent call sites
and tests to use the new queue ownership.

0322ac3df8 [codex] Use compaction_trigger item for remote compaction v2 (#22809)
## Why

Remote compaction v2 was still using `context_compaction` as both the
request trigger and the compacted output shape. The Responses API now
has the landed contract for this flow: Codex sends a dedicated `{
"type": "compaction_trigger" }` input item, and the backend returns the
standard `compaction` output item with encrypted content.

This aligns the v2 path with that wire contract while preserving the
existing local compacted-history post-processing behavior.

## What changed

- Add `ResponseItem::CompactionTrigger` and regenerate the app-server
protocol schema fixtures.
- Send `compaction_trigger` from `remote_compaction_v2` instead of a
payload-less `context_compaction`.
- Collect exactly one backend `compaction` output item, then reuse the
existing compacted-history rebuilding path.

output or persisted rollout/memory content.

9e7cdbd0d2 core: box multi-agent handler futures (#22266)
## Why

This is the base PR in the split stack for the permissions migration. It
isolates stack-safety work that had been mixed into the larger
permissions PR, so reviewers can evaluate the async-future changes
separately from the permissions model changes in #22267.

The main risk this addresses is large or recursive multi-agent futures
overflowing smaller runner stacks. A follow-up review also called out
that `shutdown_live_agent` must remain quiescent: callers should not
remove a live agent from tracking or release its spawn slot until the
worker loop has actually terminated.

## What Changed

- Boxes the large async futures in the multi-agent spawn, resume, and
close tool handlers.
- Boxes the `AgentControl` spawn and recursive close/shutdown paths that
can otherwise build very deep futures.
- Keeps `shutdown_live_agent` waiting for thread termination before
removing/releasing the live agent, preserving the previous shutdown
ordering while still boxing the recursive close path.

## Verification Strategy

The focused local coverage was `cargo test -p codex-core multi_agents`,
which exercises the multi-agent spawn/resume/close handlers, cascade
close/resume behavior, and the shutdown path touched by this PR.

79ad209ce6 [codex] Remove remote thread store implementation (#21596)
Remove the remote thread-store backend and checked-in protobuf
artifacts. We've moved these into another crate that link against this
one.

Also remove the config settings for thread store backend selection,
since we'll instead pass an instantiated thread store into the core-api
crate's main entrypoint.

a8488fec5e Revert state DB injection and agent graph store (#21481)
## Why

Reverts #20689 to restore the previous optional state DB plumbing. The
conflict resolution keeps the newer installation ID and session/thread
identity changes that landed after #20689, while removing the mandatory
state DB and agent graph store dependency from ThreadManager
construction.

## What changed

- Restored `Option<StateDbHandle>` through app-server, MCP server,
prompt debug, and test entry points.
- Removed the `codex-core` dependency on `codex-agent-graph-store` and
reverted descendant lookup back to the existing state DB path when
available.
- Kept newer `installation_id` forwarding by passing it beside the
optional DB handle.
- Kept local thread-name updates working when the optional state DB
handle is absent.

## Validation

- `git diff --check`
- `cargo test -p codex-thread-store`
- `cargo test -p codex-state -p codex-rollout -p
codex-app-server-protocol`
- Attempted `env CARGO_INCREMENTAL=0 cargo test -p codex-core -p
codex-app-server -p codex-app-server-client -p codex-mcp-server -p
codex-thread-manager-sample -p codex-tui`; blocked locally by a rustc
ICE while compiling `v8 v146.4.0` with `rustc 1.93.0 (254b59607
2026-01-19)` on `aarch64-apple-darwin`.

a98623511b feat: add session_id (#20437)
## Summary

Related to
https://openai.slack.com/archives/C095U48JNL9/p1777537279707449
TLDR:
We update the meaning of session ids and thread ids:
* thread_id stays as now
* session_id become a shared id between every thread under a /root
thread (i.e. every sub-agent share the same session id)

This PR introduces an explicit `SessionId` and threads it through the
protocol/client boundary so `session_id` and `thread_id` can diverge
when they need to, while preserving compatibility for older serialized
`session_configured` events.

b3d4f1a9f0 [codex-analytics] rework thread_source for thread analytics (#20949)
## Summary
- make `thread_source` an explicit optional thread-level field on
`thread/start`, `thread/fork`, and returned thread payloads
- persist `thread_source` in rollout/session metadata so resumed live
threads retain the original value
- replace the old best-effort `session_source` -> `thread_source`
mapping with an explicit caller-supplied analytics classification

## Why
Before this change, analytics `thread_source` was populated by a
best-effort mapping from `session_source`. `session_source` describes
the runtime/client surface, not the actual thread-level origin, so that
projection was not accurate enough to distinguish cases such as `user`,
`subagent`, `memory_consolidation`, and future thread origins reliably.

Making `thread_source` explicit keeps one thread-level analytics field
while letting callers provide the real classification directly instead
of recovering it indirectly from `session_source`.

## Impact
For new analytics events, `thread_source` now reflects the explicit
thread-level classification supplied by the caller rather than an
inferred value derived from `session_source`. Existing protocol fields
remain optional; callers that omit `threadSource` now produce `null`
instead of a best-effort inferred value.

7e310bc7f3 Inject state DB, agent graph store (#20689)
## Why

We want the agent graph store to be passed down the stack as a real
dependency, the same way we already treat the thread store.

This will let us inject the agent graph store as a real dependency and
support implementations other than the local SQLite-backed one. Right
now most code instantiates a state DB and an agent graph store
just-in-time. Ideally, we would not depend on the state DB directly but
only read through the higher-level interfaces.

This change makes the dependency boundaries explicit and moves state DB
initialization to process bootstrap instead of hiding it inside local
store implementations.

## What changed

- `ThreadManager` now requires a `StateDbHandle` and an
`AgentGraphStore` at construction time instead of treating them as
optional internals.
- The local store constructors no longer lazily initialize SQLite.
Callers now initialize the state DB once per process and use that shared
handle to build:
  - `LocalThreadStore`
  - `LocalAgentGraphStore`
- App bootstraps (`app-server`, `mcp-server`, `prompt_debug`, and the
thread-manager sample) now initialize the state DB up front and inject
the resulting handle down the stack.
- `app-server` now consistently uses its process-scoped state DB handle
instead of reopening SQLite or trying to recover it from loaded threads.
- Device-key storage now reuses the shared state DB handle instead of
maintaining its own lazy opener.
- The thread archive / descendant traversal paths now use the injected
`AgentGraphStore` instead of reaching through local
thread-store-specific state.

## Verification

4d201e340e state: pass state db handles through consumers (#20561)
## Why

SQLite state was still being opened from consumer paths, including lazy
`OnceCell`-backed thread-store call sites. That let one process
construct multiple state DB connections for the same Codex home, which
makes SQLite lock contention and `database is locked` failures much
easier to hit.

State DB lifetime should be chosen by main-like entrypoints and tests,
then passed through explicitly. Consumers should use the supplied
`Option<StateDbHandle>` or `StateDbHandle` and keep their existing
filesystem fallback or error behavior when no handle is available.

The startup path also needs to keep the rollout crate in charge of
SQLite state initialization. Opening `codex_state::StateRuntime`
directly bypasses rollout metadata backfill, so entrypoints should
initialize through `codex_rollout::state_db` and receive a handle only
after required rollout backfills have completed.

## What Changed

- Initialize the state DB in main-like entrypoints for CLI, TUI,
app-server, exec, MCP server, and the thread-manager sample.
- Pass `Option<StateDbHandle>` through `ThreadManager`,
`LocalThreadStore`, app-server processors, TUI app wiring, rollout
listing/recording, personality migration, shell snapshot cleanup,
session-name lookup, and memory/device-key consumers.
- Remove the lazy local state DB wrapper from the thread store so
non-test consumers use only the supplied handle or their existing
fallback path.
- Make `codex_rollout::state_db::init` the local state startup path: it
opens/migrates SQLite, runs rollout metadata backfill when needed, waits
for concurrent backfill workers up to a bounded timeout, verifies
completion, and then returns the initialized handle.
- Keep optional/non-owning SQLite helpers, such as remote TUI local
reads, as open-only paths that do not run startup backfill.
- Switch app-server startup from direct
`codex_state::StateRuntime::init` to the rollout state initializer so
app-server cannot skip rollout backfill.
- Collapse split rollout lookup/list APIs so callers use the normal
methods with an optional state handle instead of `_with_state_db`
variants.
- Restore `getConversationSummary(ThreadId)` to delegate through
`ThreadStore::read_thread` instead of a LocalThreadStore-specific
rollout path special case.
- Keep DB-backed rollout path lookup keyed on the DB row and file
existence, without imposing the filesystem filename convention on
existing DB rows.
- Verify readable DB-backed rollout paths against `session_meta.id`
before returning them, so a stale SQLite row that points at another
thread's JSONL falls back to filesystem search and read-repairs the DB
row.
- Keep `debug prompt-input` filesystem-only so a one-off debug command
does not initialize or backfill SQLite state just to print prompt input.
- Keep goal-session test Codex homes alive only in the goal-specific
helper, rather than leaking tempdirs from the shared session test
helper.
- Update tests and call sites to pass explicit state handles where DB
behavior is expected and explicit `None` where filesystem-only behavior
is intended.

Focused coverage added in `codex-rollout`:

- `recorder::tests::state_db_init_backfills_before_returning` verifies
the rollout metadata row exists before startup init returns.
- `state_db::tests::try_init_waits_for_concurrent_startup_backfill`
verifies startup waits for another worker to finish backfill instead of
disabling the handle for the process.
-
`state_db::tests::try_init_times_out_waiting_for_stuck_startup_backfill`
verifies startup does not hang indefinitely on a stuck backfill lease.
-
`tests::find_thread_path_accepts_existing_state_db_path_without_canonical_filename`
verifies DB-backed lookup accepts valid existing rollout paths even when
the filename does not include the thread UUID.
-
`tests::find_thread_path_falls_back_when_db_path_points_to_another_thread`
verifies DB-backed lookup ignores a stale row whose existing path
belongs to another thread and read-repairs the row after filesystem
fallback.

Focused coverage updated in `codex-core`:

- `rollout_list_find::find_prefers_sqlite_path_by_id` now uses a
DB-preferred rollout file with matching `session_meta.id`, so it still
verifies that valid SQLite paths win without depending on stale/empty
rollout contents.

`cargo test -p codex-app-server thread_list_respects_search_term_filter
-- --test-threads=1 --nocapture` was attempted locally but timed out
waiting for the app-server test harness `initialize` response before
reaching the changed thread-list code path.

`bazel test //codex-rs/thread-store:thread-store-unit-tests
--test_output=errors` was attempted locally after the thread-store fix,
but this container failed before target analysis while fetching `v8+`
through BuildBuddy/direct GitHub. The equivalent local crate coverage,
including `cargo test -p codex-thread-store`, passes.

A plain local `cargo check -p codex-rollout -p codex-app-server --tests`
also requires system `libcap.pc` for `codex-linux-sandbox`; the
follow-up app-server check above used `CODEX_SKIP_VENDORED_BWRAP=1` in
this container.

d927f61208 feat: add remote compaction v2 Responses client path (#20773)
## Why

This adds the `remote_compaction_v2` client path so remote compaction
can run through the normal Responses stream and install a
`context_compaction` item that trigger a compaction.

The goal is to migrate some of the compaction logic on the client side

We keeps the v2 transport behind a feature flag while letting follow-up
requests reuse the compacted context instead of falling back to the
legacy compaction item shape.

## What changed

- add `ResponseItem::ContextCompaction` and refresh the generated
app-server / schema / TypeScript fixtures that expose response items on
the wire
- add `core/src/compact_remote_v2.rs` to send compaction through the
standard streamed Responses client, require exactly one
`context_compaction` output item, and install that item into compacted
history
- route manual compact and auto-compaction through the v2 path when
`remote_compaction_v2` is enabled, while keeping the existing remote
compaction path as the fallback
- preserve the new item type across history retention, follow-up request
construction, telemetry, rollout persistence, and rollout-trace
normalization
- add targeted coverage for the feature flag, `context_compaction`
serialization, rollout-trace normalization, and remote-compaction
follow-up behavior

## Verification

- added protocol tests for `context_compaction`
serialization/deserialization in `protocol/src/models.rs`
- added rollout-trace coverage for `context_compaction` normalization in
`rollout-trace/src/reducer/conversation_tests.rs`
- added remote compaction integration coverage for v2 follow-up reuse
and mixed compaction output streams in
`core/tests/suite/compact_remote.rs`
