7dc7096ae1 [codex] Restore thread recency with compatible migration history (#28671)
## Summary

- Revert #28655, restoring the thread `recencyAt` behavior introduced by
#27910.
- Move `threads_recency_at` to migration 0039 so it no longer collides
with `external_agent_config_imports` at version 0038.
- Repair databases that already applied the recency migration as version
38 by moving the matching migration-history row to version 39 before
SQLx validation. The current version-38 migration can then apply
normally.

## Validation

- `just test -p codex-state
migrations::tests::repairs_recency_migration_that_was_applied_as_version_38`
- `just test -p codex-state -p codex-rollout -p codex-thread-store -p
codex-app-server-protocol -p codex-tui`: 3,439 passed; six TUI tests
could not open the machine's existing read-only incident database at
`~/.codex/sqlite/state_5.sqlite`.

- Verified that state migration versions are unique.

cb15c64760 Revert thread recencyAt for sidebar ordering (#28655)
## Why

Revert #27910 to remove the newly introduced thread `recencyAt`
persistence and API behavior from `main`.

## What changed

This reverts commit `fac3158c2a783095768076489815f361fa9b0db4`,
including the state migration, thread-store propagation, app-server API
surface, generated schemas, and related tests.

## Validation

Not run before opening; relying on CI for the initial fast signal.

fac3158c2a Add thread recencyAt for sidebar ordering (#27910)
## Summary

Add a server-owned `recencyAt` timestamp and `recency_at` thread-list
sort key for product recency ordering while preserving the existing
meaning of `updatedAt` as the latest persisted thread mutation.

This is the server-side alternative to #27697. Rather than narrowing
`updatedAt`, clients can sort the sidebar by `recency_at` and continue
treating `updatedAt` as mutation time.

## Contract

- `recencyAt` initializes when a thread is created.
- A turn start advances `recencyAt` monotonically.
- Commentary, agent output, tool results, token/accounting updates, turn
completion, archive, unarchive, resume, and generic metadata writes do
not advance it.
- `updatedAt` retains its existing behavior and continues to advance for
persisted thread mutations.
- Current servers populate `recencyAt`; the response field is optional
in generated TypeScript so clients connected to older servers can fall
back to `updatedAt`.
- Filesystem-only fallback uses existing updated/mtime ordering when
SQLite is unavailable.

## Persistence and compatibility

Migration 0038 adds second- and millisecond-precision recency columns,
backfills them from the existing updated timestamp, creates list
indexes, and includes an insert trigger so older binaries writing to a
migrated database seed recency without causing later mutations to
advance it.

Generic metadata upserts preserve existing recency values. Turn-start
updates use a dedicated monotonic touch, and process-local allocation
keeps millisecond cursor values unique. State DB list, search, read,
filtered-list repair, rollout fallback propagation, and app-server
conversions all carry the new field.

## API

`Thread` responses include:

040dafa32d feat(core): add metadata field to ResponseItem (#28355)
## Description

This PR adds an optional `metadata` field to `ResponseItem` for
Responses API calls. Only mechanical plumbing, no actual values
populated and sent yet. Turns out just adding a new field to
`ResponseItem` has quite a large blast radius already.

This change is backwards compatible because `metadata` is optional and
omitted when absent, so existing response items and rollout history
without it still deserialize and requests that do not set it keep the
same wire shape. For provider compatibility, we strip out `metadata`
before non-OpenAI Responses requests so Azure and AWS Bedrock never see
this field.

My followup PR here will actually make use of it to start storing and
passing along `turn_id`: https://github.com/openai/codex/pull/28360

## What changed

- Added `ResponseItemMetadata` with optional `turn_id`, plus optional
`metadata` on Responses API item variants and inter-agent communication.
- Preserved item metadata through response-item rewrites such as
truncation, missing tool-output synthesis, compaction history
rebuilding, visible-history conversion, rollout/resume, and generated
app-server schemas/types.
- Strip item metadata from non-OpenAI Responses requests while
preserving it for OpenAI-shaped requests.
- Updated the mechanical fixture/test construction churn required by the
new optional field.

608b8b1cc6 [codex-analytics] emit goal lifecycle analytics (#27078)
## Why
- Currently, there is no analytics event for `/goal` behavior
- Existing events cannot identify goal execution or its resulting
outcome
- The original update in
[#26182](https://github.com/openai/codex/pull/26182) was implemented
before `/goal` moved into `codex-goal-extension`.

## What Changed
- Adds `codex_goal_event` serialization and enrichment to
`codex-analytics`
- Emits goal events from the canonical `codex-goal-extension` mutation
and accounting paths:
  - `created` when a new logical goal is persisted
  - `usage_accounted` when cumulative goal usage is persisted
  - `status_changed` when the stored goal status changes
  - `cleared` when the goal is deleted
- Preserves causal `turn_id` for turn driven events and uses null
attribution for external or idle lifecycle events
- Changes goal deletion to return the deleted row so `cleared` retains
the stable goal ID

## Event Details

Includes standard analytics metadata along with goal specific fields:
- `goal_id`: Stable ID stored in the local SQLite goal row and shared
across the goal's events
- `event_kind`: Observed operation (see the 4 lifecycle events cited in
the above bullet)
- `goal_status`: Resulting or last stored status: `active`, `paused`,
`blocked`, `usage_limited`, etc.
  - `has_token_budget`: Indicates whether a token budget is configured
  - `turn_id`: Causal turn ID, or null when no causal turn exists
- `cumulative_tokens_accounted`: Cumulative tokens on `usage_accounted`
events; null otherwise
- `cumulative_time_accounted_seconds`: Cumulative active time on
`usage_accounted` events; null otherwise

76c0a5379c Make runtime workspace roots absolute in app-server API (#26552)
Stacked on #26532.

## Why

#26532 moves cwd normalization to the app-server/core boundary.
`runtimeWorkspaceRoots` still accepted raw paths in v2 requests and in
`ConfigOverrides`, which left core responsible for interpreting those
roots later. This makes runtime workspace roots follow the same
absolute-path boundary as cwd.

## What

- Change v2 `runtimeWorkspaceRoots` request fields for `thread/start`,
`thread/resume`, `thread/fork`, and `turn/start` to `AbsolutePathBuf`.
- Deduplicate already-absolute runtime roots in app-server handlers and
pass them through `ConfigOverrides.workspace_roots` as
`AbsolutePathBuf`.
- Update TUI and exec client request builders to pass absolute runtime
roots directly.
- Update app-server docs, schema fixtures, and focused tests for
absolute runtime roots.

59ca34206b [codex] Preserve logical paths during AGENTS.md discovery (#26465)
## Intent

Follow up on #26205 by avoiding unnecessary filesystem canonicalization
during `AGENTS.md` discovery. The configured working directory is
already absolute, and canonicalization incorrectly switches symlinked
workspaces from their logical parent hierarchy to the target's
hierarchy.

## User-facing behavior

For a symlinked working directory such as:

Before this change, Codex canonicalized `logical-repo/workspace` to
`physical-repo/workspace` before discovery. It therefore loaded
`physical-repo/AGENTS.md` and `physical-repo/workspace/AGENTS.md`,
ignoring the instructions from the repository through which the user
entered the workspace.

After this change, ancestor discovery walks the configured logical path,
so Codex loads `logical-repo/AGENTS.md`. Opening
`logical-repo/workspace/AGENTS.md` still follows the symlink through the
host filesystem, so the workspace document is also loaded.
`physical-repo/AGENTS.md` is not loaded.

## Implementation

Use the logical absolute working directory when discovering project
instructions and reporting instruction sources. Filesystem reads still
follow the working-directory symlink, so an `AGENTS.md` in the target
workspace continues to load while ancestor discovery uses the symlink's
parents.

## Validation

Added integration coverage proving that discovery loads the logical
parent's instructions and the target workspace's instructions, but not
the target parent's instructions.

e64b469bbc Route AGENTS.md loading through environment filesystems (#26205)
## Why

Workspace-specific `AGENTS.md` loading needs to use the selected
environment filesystem so remote workspaces and child agents read
instructions from their actual environment instead of the host
filesystem. The app-server should report the same instruction sources
the initialized thread actually loaded, rather than independently
rescanning configuration and filesystem state.

## What changed

- Introduce `LoadedAgentsMd` to retain ordered user, project, and
internal instructions with their provenance.
- Load and canonicalize workspace `AGENTS.md` paths through the primary
`EnvironmentManager` environment, then render the loaded instructions
when constructing turn context.
- Expose cached loaded instruction sources from initialized threads and
use them for app-server start, resume, and fork responses.
- Preserve global `CODEX_HOME` loading and separator behavior while
excluding empty project files that did not supply model-visible
instructions.
- Add integration coverage for CLI injection, selected-environment
provenance and rendering, empty environment selection, and cached
sources on loaded-thread resume.

## Validation

d45cd26248 Switch runtime to cloud config bundle (#24622)
## Summary

- Adapts the moved `codex-cloud-config` crate from the legacy cloud
requirements endpoint to the new config bundle endpoint.
- Switches runtime consumers from `CloudRequirementsLoader` to
`CloudConfigBundleLoader` so one shared bundle supplies cloud-delivered
config and requirements.
- Removes the legacy cloud requirements domain loader path.

## Details

This intentionally keeps `codex-cloud-config` monolithic for review
lineage: the previous PR establishes the crate move, and this PR shows
the behavior change against that moved implementation. A follow-up PR
splits the module back into focused files.

The new bundle path preserves the important cloud requirements loader
semantics where intended: account-scoped signed cache, 30 minute TTL, 5
minute refresh cadence, retry/backoff, auth recovery, and fail-closed
startup loading. The cached payload changes from a single requirements
TOML string to the backend-delivered bundle, and validation rejects
malformed config or requirements fragments before cache write/use.

ad355d4c96 Fix Windows running thread resume path normalization (#25509)
## Why

Fixes #24944.

On Windows, app-server resume could reject an active running thread when
the requested session path used normal `C:\...` form and the
already-running path used verbatim `\\?\C:\...` form. The paths point at
the same JSONL file, but the resume stale-path guard compared raw
`PathBuf`s, so desktop resume and heartbeat flows could fail with a
mismatched-path error.

## What Changed

- Compare requested and active rollout paths with
`path_utils::paths_match_after_normalization`.
- Extend the existing running-thread mismatched-path test with a
Windows-only same-file resume case before the stale-path rejection.

## Verification

- `just test -p codex-app-server
thread_resume_rejects_mismatched_path_for_running_thread_id`

This PR brought to you via VS Code rather than Codex...

- opened `codex-rs/app-server/tests/common/mcp_process.rs`
- put the cursor on `McpServer`
- hit `F2` and renamed the symbol to `TestAppServer`
- went to the file tree
- hit enter and renamed `mcp_process.rs` to `test_app_server.rs`
- ran **Save All Files** from the Command Palette
- ran `just fmt`

The End

(Admittedly, most of the local variables for `TestAppServer` are still
named `mcp`, though.)

cf0911076f store and expose parent_thread_id on Threads (#25113)
## Why

This PR
https://github.com/openai/codex/pull/24161#discussion_r3325692763
revealed a subagent data modeling issue, where we overloaded
`forked_from_id` to also mean `parent_thread_id`. That's incorrect since
guardian and review subagents can be a subagent and NOT fork the main
thread's history.

The solution here is to explicitly store a new `parent_thread_id` on
`SessionMeta`, alongside `forked_from_id` which already exists. While
we're at it, also expose it in the app-server protocol on the `Thread`
object.

A thread->subagent relationship and a fork of thread history are
orthogonal concepts.

## What Changed

- Added top-level `parent_thread_id` persistence on `SessionMeta` and
runtime/session plumbing through `SessionConfiguredEvent`,
`CodexSpawnArgs`, `SessionConfiguration`, `ThreadConfigSnapshot`,
`TurnContext`, and `ModelClient`.
- Made turn metadata, request headers, analytics, and subagent-start
events read the separate runtime/top-level parent field instead of
deriving general parent lineage from `SessionSource` or
`forked_from_thread_id`.
- Passed parent lineage separately at delegated subagent, review,
guardian, agent-job, and multi-agent spawn construction sites;
copied-history fork lineage remains derived only from `InitialHistory`.
- Persisted and exposed parent lineage through rollout/thread-store
projections and app-server v2 `Thread.parentThreadId`.
- Updated app-server README text and regenerated app-server schema
fixtures for the additive `parentThreadId` response field.

3e7baa00e4 Add thread archive CLI commands (#25021)
## Problem

Saved threads can already be archived through app-server RPCs, but the
command line did not expose direct archive or unarchive commands.

## Solution

Add `codex archive <thread>` and `codex unarchive <thread>`, resolving
UUIDs or exact thread names before calling the existing `thread/archive`
and `thread/unarchive` RPCs. The commands support scoped remote flags so
callers can target remote app-server endpoints when archiving or
unarchiving threads.

This also fixes a long-standing bug in `codex resume <thread id>` and
`codex fork <thread id>` that I found when testing the new commands.
These operations shouldn't be allowed on archived sessions. They now
fail with an error that tells the user to run `codex unarchive <thread
id>` first.

## Verification

Added app-server coverage for rejecting archived thread resume by id and
checking that the error includes the matching `codex unarchive <thread
id>` command.

Adds an optional `clientId` field to app-server v2 `UserInput` and
carries it through the core `UserInput` model so clients can correlate
echoed user input items without relying on payload equality.

## Details

- Adds `client_id: Option<String>` to core `UserInput` variants.
- Exposes the v2 app-server field as `clientId` on the wire and in
generated TypeScript.
- Preserves the id when converting between app-server v2 and core
protocol types.
- Regenerates app-server schema fixtures.

2a1158b8e2 feat(app-server): include turns page on thread resume (#23534)
## Summary

The client currently calls `thread/resume` to establish live updates and
immediately follows it with `thread/turns/list` to hydrate recent turns.
This lets `thread/resume` return that page directly, eliminating a round
trip and the ordering/deduplication gap between the two calls.

Experimental clients opt in with `initialTurnsPage: { limit,
sortDirection, itemsView }`. The response returns `initialTurnsPage` as
a `TurnsPage`, including cursors for paging further back in history.
Keeping the controls in a nested opt-in object provides the useful
`thread/turns/list` knobs without spreading page-specific parameters
across `thread/resume`.

487521733b [codex-analytics] add grouped session id to runtime events (#24655)
## Why
- Runtime analytics events report `thread_id`, which identifies the
individual thread emitting an event
- They don't report `session_id`, which identifies the shared session
for a root thread and its subagent threads
- Emitting both identifiers allows analytics to group related activity

## What Changed
- Adds `session_id` to relevant analytics events (thread_initalized,
turn, turn_steer, compaction, guardian_review)
- Tracks each thread's session ID in the analytics reducer so subsequent
thread scoped events emit the same value
- Carries the shared session ID through subagent initialization

## Verification
- `just test -p codex-analytics` validates event payloads and subagent
session grouping.
- Focused `codex-app-server` tests validate session IDs for thread,
turn, and steer events.
- Focused `codex-core` tests validate root and subagent session ID
propagation.

5cd9b8086a Respect resume cwd overrides for idle cached threads (#24528)
Fixes #24186.

## Why
When the TUI resumes a thread through the local app-server daemon with a
selected workspace, `thread/resume` can hit an already-loaded but idle
cached thread. That path previously rejoined the cached `CodexThread`,
so cwd/config overrides in `ThreadResumeParams` were ignored and the
resumed session kept using the old cwd.

## What changed
App-server now treats a loaded-but-idle thread with no subscribers as a
cache entry when resume overrides differ: it unloads that cached thread
and lets the normal resume path rebuild it with the requested
cwd/config. Threads that still have subscribers, or active runtime work,
continue to rejoin the existing loaded thread so in-flight state remains
observable.

The existing thread teardown helper was generalized from
archive-specific cleanup to shared unload cleanup for this path.

3c83e57bfa Add trace_id to TurnStartedEvent (#23980)
## Why
[Recent PR](https://github.com/openai/codex/pull/22709) removed
`trace_id` from `TurnContextItem`.

## What changed
- Add to `TurnStartedEvent` so rollout consumers can correlate turns
with telemetry traces.
- Note that the branch name is out of date because I originally re-added
to `TurnContextItem`, but we decided to move it to `TurnStartedEvent`.

954a9c8579 Fix empty rollout path app-server handling (#23400)
## Summary
- Coerce `path: ""` to `None` at the v2 protocol params deserialization
boundary for `thread/resume` and `thread/fork`.
- Restore the pre-ThreadStore running-thread resume behavior: if
`threadId` is already running, rejoin it by id and treat a non-empty
`path` only as a consistency check; otherwise cold resume keeps `history
> path > threadId` precedence.
- Add protocol, resume, and fork regression coverage for empty path
payloads; refresh app-server schema fixtures for the clarified params
docs.

ba57aab13a feat: dedicated goal DB (#23300)
## Why

Thread goals are moving toward extension-owned runtime behavior, but
their persisted state was still stored in the shared state database.
This makes the goal store harder to isolate and keeps future storage
splits tied to ad hoc runtime plumbing.

This PR gives goals their own SQLite database while keeping the existing
`StateRuntime` entry point. The goal is to make this the pattern for
adding more dedicated runtime databases later.

This also reduce load on existing DB and reduce contention

## Limitation
Thread preview from goal is not supported anymore. I'm looking into this
[EDIT]: solved

## What changed

- Added a dedicated `goals_1.sqlite` database with its own
`goals_migrations` directory.
- Moved `thread_goals` creation into the goals DB migration set.
- Dropped the old `thread_goals` table from the main state DB with a
normal state migration. There is intentionally no backfill for existing
goal rows.
- Changed `GoalStore` to be backed only by the goals DB pool.
- Removed the old goal-write side effect that filled empty
`threads.preview` values from the goal objective.
- Added shared runtime DB path metadata so startup, telemetry, `codex
doctor`, and repair handling can include future DBs without bespoke path
lists.
- Updated Bazel compile data so the new goals migration directory is
available to `sqlx::migrate!`.

## Verification

0d344aca9b goal: pause continuation loops on usage limits and blockers (#23094)
Addresses #22833, #22245, #23067

## Why
`/goal` can keep synthesizing turns even when the next turn cannot make
meaningful progress. Hard usage exhaustion can replay failing turns, and
repeated permission or external-resource blockers can keep burning
tokens while waiting for user or system intervention.

## What changed
- Add resumable `blocked` and `usageLimited` goal states. As with
`paused`, goal continuation stops with these states.
- Move to `usageLimited` after usage-limit failures.
- Allow the built-in `update_goal` tool to set `blocked` only under
explicit repeated-impasse guidance. Updated goal continuation prompt to
specify that agent should use `blocked` only when it has made at least
three attempts to get past an impasse.

Most of the files touched by this PR are because of the small app server
protocol update.

## Validation

I manually reproduced a number of situations where an agent can run into
a true impasse and verified that it properly enters `blocked` state. I
then resumed and verified that it once again entered `blocked` state
several turns later if the impasse still exists.

I also manually reproduced the usage-limit condition by creating a
simulated responses API endpoint that returns 429 errors with the
appropriate error message. Verified that the goal runtime properly moves
the goal into `usageLimited` state and TUI UI updates appropriately.
Verified that `/goal resume` resumes (and immediately goes back into
`ussageLImited` state if appropriate).


## Follow-up PRs

Small changes will be needed to the GUI clients to properly handle the
two new states.

7ee7fe239f chore: isolate thread goal storage behind GoalStore (#23295)
## Why

Thread goal persistence is being prepared for a dedicated storage
boundary. Before that split, goal-specific reads, writes, accounting,
and cleanup were exposed directly on `StateRuntime`, so core and
app-server callsites stayed coupled to the full runtime instead of a
goal-specific store.

This PR introduces that boundary without changing the goal wire API or
current persistence behavior. Callers now go through
`StateRuntime::thread_goals()` and the new `GoalStore`, while
`GoalStore` still uses the existing state DB pool underneath.

## What changed

- Added `GoalStore` in `state/src/runtime/goals.rs` and exposed it from
`StateRuntime` via `thread_goals()`.
- Moved thread-goal reads, writes, status updates, pause, delete, and
usage accounting onto `GoalStore`.
- Updated core session goal handling, app-server goal RPCs, resume
snapshots, and goal tests to use the store boundary.
- Kept thread deletion responsible for cascading goal cleanup by
deleting the goal through the store only after a thread row is removed.

## Testing

- Existing goal persistence, resume, and accounting tests were updated
to exercise the new `GoalStore` access path.

8a5306ff88 app-server: use permission ids and runtime workspace roots (#22611)
## Why

This PR builds on [#22610](https://github.com/openai/codex/pull/22610)
and is the app-server side of the migration from mutable per-turn
`SandboxPolicy` replacement toward selecting immutable permission
profiles by id plus mutable runtime workspace roots.

Once permission profiles can carry their own immutable
`workspace_roots`, app-server no longer needs to mutate the selected
`PermissionProfile` just to represent thread-specific filesystem
context. The mutable part now lives on the thread as explicit
`runtimeWorkspaceRoots`, while `:workspace_roots` remains symbolic until
the sandbox is realized for a turn.

## What Changed

- Replaced the v2 permission-selection wrapper surface with plain
profile ids for `thread/start`, `thread/resume`, `thread/fork`, and
`turn/start`.
- Removed the API surface for profile modifications
(`PermissionProfileSelectionParams`,
`PermissionProfileModificationParams`,
`ActivePermissionProfileModification`).
- Added experimental `runtimeWorkspaceRoots` fields to the thread
lifecycle and turn-start APIs.
- Threaded runtime workspace roots through core session/thread
snapshots, turn overrides, app-server request handling, and command
execution permission resolution.
- Kept session permission state symbolic so later runtime root updates
and cwd-only implicit-root retargeting rebind `:workspace_roots`
correctly.
- Updated the embedded clients just enough to send and restore the new
thread state.
- Refreshed the generated schema/TypeScript artifacts and the app-server
README to match the new contract.

## Verification

Targeted coverage for this layer lives in:

- `codex-rs/app-server-protocol/src/protocol/v2/tests.rs`
- `codex-rs/app-server/tests/suite/v2/thread_start.rs`

The key regression checks exercise that:

- `runtimeWorkspaceRoots` resolve against the effective cwd on thread
start.
- Profile-declared workspace roots are excluded from the runtime
workspace roots returned by app-server.
- A turn-level runtime workspace-root update persists onto the thread
and is returned by `thread/resume`.
- A named permission profile selected on one turn remains symbolic so a
later runtime-root-only turn update changes the actual sandbox writes.
- A cwd-only turn update retargets the implicit runtime cwd root while
preserving additional runtime roots.
- The protocol fixtures and generated client artifacts stay in sync with
the string-based permission selection contract.

7bddb3083d fix(app-server): thread history redaction for remote clients (#22178)
## Summary

Remote clients can still receive large `thread/resume` histories when
prior turns include MCP tool call payloads or image-generation results.
This adds a temporary response-only redaction path for the known remote
client names.

Longer term we will move towards fully paginated APIs backed by SQLite.

## Changes

- Redact MCP tool call payload-bearing fields in `thread/resume`
responses for `codex_chatgpt_android_remote` and
`codex_chatgpt_ios_remote`.
- Drop `imageGeneration` items from those `thread/resume` responses.
- Keep redaction out of persisted rollout files, `thread/read`,
`thread/turns/list`, live notifications, and token usage replay.
- Cover the behavior with app-server helper tests and a v2 resume
integration test that checks both remote clients plus a non-target
control client.

1e65b3e0af Fix goal update and add /goal edit command in TUI (#21954)
## Why

Users have requested the ability to edit a goal's objective after a goal
has been created. This PR exposes a new `/goal edit` command in the TUI
to address this request.

In the process of implementing this, I also noticed an existing bug in
the goal runtime. When a goal's objective is updated through the
`thread/goal/set` app server API, the goal runtime didn't emit a new
steering prompt to tell the agent about the new objective. This PR also
fixes this hole.

## What Changed

- Adds `/goal edit` in the TUI, opening an edit box prefilled with the
current goal objective.
- Keeps active and paused goals in their current state, resets completed
goals to active, keeps budget-limited goals budget-limited, and
preserves the existing token budget.
- Changes the existing `thread/goal/set` behavior so editing an
objective preserves goal accounting instead of resetting it. The older
reset-on-new-objective behavior was left over from before
`thread/goal/clear`; clients that need to reset accounting can now clear
the existing goal and create a new one.
- Reuses the existing goal set API path; this does not add or change
app-server protocol surface area.
- Adds a dedicated goal runtime steering prompt when an externally
persisted goal mutation changes the objective, so active turns receive
the updated objective.

## Validation

- Make sure `/goal edit` returns an error if no goal currently exists
- Make sure `/goal edit` displays an edit box that can be optionally
canceled with no side effects
- Make sure that an edited goal results in a steer so the agent starts
pursuing the new objective
- Make sure the new objective is reflected in the goal if you use
`/goal` to display the goal summary
- Make sure that `/goal edit` doesn't reset the token budget, time/token
accounting on the updated goal

5ecff05196 feat(app-server): move v2 sessionId onto Thread (#21336)
## Why

`session_id` and `thread_id` are separate identities after #20437, but
app-server only surfaced `sessionId` on the `thread/start`,
`thread/resume`, and `thread/fork` response envelopes. Other
thread-bearing surfaces such as `thread/list`, `thread/read`,
`thread/started`, `thread/rollback`, `thread/metadata/update`, and
`thread/unarchive` either lacked the grouping key or forced clients to
special-case those three responses.

Making `sessionId` part of the reusable `Thread` payload gives every v2
API surface one place to expose session-tree identity.

## Mental model
  1. thread.sessionId lives on `Thread`
2. It is a view/runtime identity for the current live session tree, not
durable stored lineage metadata
3. When app-server has a live loaded thread, it copies the real value
from core’s session_configured.session_id
4. When it only has stored/unloaded data, it falls back to
thread.sessionId = thread.id

## What changed

- Added `sessionId` to the v2
[`Thread`](8fc9e9b4cf/codex-rs/app-server-protocol/src/protocol/v2/thread_data.rs (L105-L109)).
- Removed the duplicate top-level `sessionId` fields from
`thread/start`, `thread/resume`, and `thread/fork`; clients should now
read `response.thread.sessionId`.
- Populated `thread.sessionId` when building live thread responses,
replaying loaded threads, and returning stored-thread summaries so the
field is present across start, resume, fork, list, read, rollback,
metadata-update, unarchive, and `thread/started` paths. See
[`load_thread_from_resume_source_or_send_internal`](8fc9e9b4cf/codex-rs/app-server/src/request_processors/thread_processor.rs (L2824-L2918))
and

- Preserved the stored-thread fallback: if a thread has not been loaded
into a live session tree yet, `thread.sessionId` falls back to
`thread.id`; once the thread is live again, the field reports the active

- Regenerated the JSON/TypeScript schemas and updated the app-server
README examples to show
[`thread.sessionId`](8fc9e9b4cf/codex-rs/app-server/README.md (L306-L310))

We update the meaning of session ids and thread ids:

protocol/client boundary so `session_id` and `thread_id` can diverge
when they need to, while preserving compatibility for older serialized
`session_configured` events.

## Summary
- make `thread_source` an explicit optional thread-level field on
`thread/start`, `thread/fork`, and returned thread payloads
- persist `thread_source` in rollout/session metadata so resumed live
threads retain the original value
- replace the old best-effort `session_source` -> `thread_source`
mapping with an explicit caller-supplied analytics classification

## Why
Before this change, analytics `thread_source` was populated by a
best-effort mapping from `session_source`. `session_source` describes
the runtime/client surface, not the actual thread-level origin, so that
projection was not accurate enough to distinguish cases such as `user`,
`subagent`, `memory_consolidation`, and future thread origins reliably.

Making `thread_source` explicit keeps one thread-level analytics field
while letting callers provide the real classification directly instead
of recovering it indirectly from `session_source`.

## Impact
For new analytics events, `thread_source` now reflects the explicit
thread-level classification supplied by the caller rather than an
inferred value derived from `session_source`. Existing protocol fields
remain optional; callers that omit `threadSource` now produce `null`
instead of a best-effort inferred value.

## Why

`Turn.items` currently overloads an empty array to mean either that no
items exist or that the server intentionally did not load them for this
response. That ambiguity blocks future lazy-loading work where clients
need to distinguish unloaded, summary, and fully hydrated turn payloads.

## What changed

- add a new `TurnItemsView` enum with `notLoaded`, `summary`, and `full`
variants
- add required `itemsView` metadata to app-server `Turn` payloads
- mark reconstructed persisted history as `full` and live shell-style
turn payloads as `notLoaded`

3c2dcbef85 Keep paused goals paused on thread resume (#20790)
## Summary

Early adopters of the `/goal` feature have provided feedback that they
expect a goal they explicitly paused to remain paused when they resume a
thread. Previously, resuming a thread would reactivate a paused goal.

This PR keeps persisted goal status unchanged during thread resume. This
honors the user feedback while also simplifying the core goal logic.

Rather than have the core logic automatically resume a paused goal, that
responsibility is transferred to the client. The TUI now detects a
resumed thread with a paused goal and asks the user whether to `Resume
goal` or `Leave paused`. The prompt appears only for quiet resume flows,
so users who resume with an immediate prompt are not interrupted.

d54493ba1c test: stabilize app-server path assertions on Windows (#19604)
## Why

Windows can represent the same canonical local path with either a normal
drive path or a verbatim device path prefix. The failure pattern that
motivated this PR was an assertion diff like `C:\...` versus
`\\?\C:\...`: different spellings, same file.

That became visible while validating the permissions stack above this
PR. The stack increasingly routes paths through `AbsolutePathBuf`, which
normalizes supported Windows device prefixes, while several existing
tests still built expected values directly with
`std::fs::canonicalize()` or compared `AbsolutePathBuf::as_path()` to a
raw `PathBuf`. On Windows, that can make tests fail because the two
sides choose different textual forms for an otherwise equivalent
canonical path.

This PR is intentionally split out as the bottom PR below #19606. The
runtime permissions migration should not carry unrelated Windows test
stabilization, and reviewers should be able to verify this as a
test-only change before looking at the larger permissions changes.

## Failure Modes Covered

- `conversation_summary` expected rollout paths were built from raw
canonicalized `PathBuf`s, while app-server responses could carry
`AbsolutePathBuf`-normalized paths.
- `thread_resume` compared returned thread paths directly to previously
stored or fixture paths, so a verbatim-prefix spelling could fail an
otherwise correct resume.
- `marketplace_add` compared plugin install roots through `as_path()`
against raw canonicalized paths, reproducing the same `C:\...` versus
`\\?\C:\...` mismatch in both app-server and core-plugin coverage.

## What Changed

- In `app-server/tests/suite/conversation_summary.rs`, normalize both
expected rollout paths and received `ConversationSummary.path` values
through `AbsolutePathBuf` before comparing the full summary object.
- In `app-server/tests/suite/v2/thread_resume.rs`, normalize both sides
of thread path comparisons before asserting equality. This keeps the
tests focused on whether resume returned the same existing path, not
whether Windows used the same string spelling.
- In `app-server/tests/suite/v2/marketplace_add.rs` and
`core-plugins/src/marketplace_add.rs`, compare install roots as
`AbsolutePathBuf` values instead of comparing an absolute-path wrapper
to a raw canonicalized `PathBuf`.

## Behavior

This PR does not change production app-server or marketplace behavior.
It only changes tests to assert semantic path identity across Windows
path spelling variants. It also leaves API response values untouched;
the normalization happens inside assertions only.

## Verification

Targeted local checks run while extracting this fix:

- `cargo test -p codex-app-server
get_conversation_summary_by_thread_id_reads_rollout`
- `cargo test -p codex-app-server
get_conversation_summary_by_relative_rollout_path_resolves_from_codex_home`

4167628622 Add goal core runtime (4 / 5) (#18076)
Adds the core runtime behavior for active goals on top of the model
tools from PR 3.

## Why

A long-running goal should be a core runtime concern, not something
every client has to implement. Core owns the turn lifecycle, tool
completion boundaries, interruptions, resume behavior, and token usage,
so it is the right place to account progress, enforce budgets, and
decide when to continue work.

## What changed

- Centralized goal lifecycle side effects behind
`Session::goal_runtime_apply(GoalRuntimeEvent::...)`.
- Starts goal continuation turns only when the session is idle; pending
user input and mailbox work take priority.
- Accounts token and wall-clock usage at turn, tool, mutation,
interrupt, and resume boundaries; `get_thread_goal` remains read-only.
- Preserves sub-second wall-clock remainder across accounting boundaries
so long-running goals do not drift downward over time.
- Treats token budget exhaustion as a soft stop by marking the goal
`budget_limited` and injecting wrap-up steering instead of aborting the
active turn.
- Suppresses budget steering when `update_goal` marks a goal complete.
- Pauses active goals on interrupt and auto-reactivates paused goals
when a thread resumes outside plan mode.
- Suppresses repeated automatic continuation when a continuation turn
makes no tool calls.
- Added continuation and budget-limit prompt templates.

## Verification

- Added focused core coverage for continuation scheduling, accounting
boundaries, budget-limit steering, completion accounting, interrupt
pause behavior, resume auto-activation, and wall-clock remainder
accounting.

3d3028a5a9 Add excludeTurns parameter to thread/resume and thread/fork (#19014)
For callers who expect to be paginating the results for the UI, they can
now call thread/resume or thread/fork with excludeturns:true so it will
not fetch any pages of turns, and instead only set up the subscription.
That call can be immediately followed by pagination requests to
thread/turns/list to fetch pages of turns according to the UI's current
interactions.

eaf78e43f2 Add sorting/backwardsCursor to thread/list and new thread/turns/list api (#17305)
To improve performance of UI loads from the app, add two main
improvements:
1. The `thread/list` api now gets a `sortDirection` request field and a
`backwardsCursor` to the response, which lets you paginate forwards and
backwards from a window. This lets you fetch the first few items to
display immediately while you paginate to fill in history, then can
paginate "backwards" on future loads to catch up with any changes since
the last UI load without a full reload of the entire data set.
2. Added a new `thread/turns/list` api which also has sortDirection and
backwardsCursor for the same behavior as `thread/list`, allowing you the
same small-fetch for immediate display followed by background fill-in
and resync catchup.

ec8d4bfc77 fix(app-server): replay token usage after resume and fork (#18023)
## Problem

When a user resumed or forked a session, the TUI could render the
restored thread history immediately, but it did not receive token usage
until a later model turn emitted a fresh usage event. That left the
context/status UI blank or stale during the exact window where the user
expects resumed state to look complete. Core already reconstructed token
usage from the rollout; the missing behavior was app-server lifecycle
replay to the client that just attached.

## Mental model

Token usage has two representations. The rollout is the durable source
of historical `TokenCount` events, and the core session cache is the
in-memory snapshot reconstructed from that rollout on resume or fork.
App-server v2 clients do not read core state directly; they learn about
usage through `thread/tokenUsage/updated`. The fix keeps those roles
separate: core exposes the restored `TokenUsageInfo`, and app-server
sends one targeted notification after a successful `thread/resume` or
`thread/fork` response when that restored snapshot exists.

This notification is not a new model event. It is a replay of
already-persisted state for the client that just attached. That
distinction matters because using the normal core event path here would
risk duplicating `TokenCount` entries in the rollout and making future
resumes count historical usage twice.

## Non-goals

This change does not add a new protocol method or payload shape. It
reuses the existing v2 `thread/tokenUsage/updated` notification and the
TUI’s existing handler for that notification.

This change does not alter how token usage is computed, accumulated,
compacted, or written during turns. It only exposes the token usage that
resume and fork reconstruction already restored.

This change does not broadcast historical usage replay to every
subscribed client. The replay is intentionally scoped to the connection
that requested resume or fork so already-attached clients are not
surprised by an old usage update while they may be rendering live
activity.

## Tradeoffs

Sending the usage notification after the JSON-RPC response preserves a
clear lifecycle order: the client first receives the thread object, then
receives restored usage for that thread. The tradeoff is that usage is
still a notification rather than part of the `thread/resume` or
`thread/fork` response. That keeps the protocol shape stable and avoids
duplicating usage fields across response types, but clients must
continue listening for notifications after receiving the response.

The helper selects the latest non-in-progress turn id for the replayed
usage notification. This is conservative because restored usage belongs
to completed persisted accounting, not to newly attached in-flight work.
The fallback to the last turn preserves a stable wire payload for
unusual histories, but histories with no meaningful completed turn still
have a weak attribution story.

## Architecture

Core already seeds `Session` token state from the last persisted rollout
`TokenCount` during `InitialHistory::Resumed` and
`InitialHistory::Forked`. The new core accessor exposes the complete
`TokenUsageInfo` through `CodexThread` without giving app-server direct
session mutation authority.

App-server calls that accessor from three lifecycle paths: cold
`thread/resume`, running-thread resume/rejoin, and `thread/fork`. In
each path, the server sends the normal response first, then calls a
shared helper that converts core usage into
`ThreadTokenUsageUpdatedNotification` and sends it only to the
requesting connection.

The tests build fake rollouts with a user turn plus a persisted token
usage event. They then exercise `thread/resume` and `thread/fork`
without starting another model turn, proving that restored usage arrives
before any next-turn token event could be produced.

## Observability

The primary debug path is the app-server JSON-RPC stream. After
`thread/resume` or `thread/fork`, a client should see the response
followed by `thread/tokenUsage/updated` when the source rollout includes
token usage. If the notification is absent, check whether the rollout
contains an `event_msg` payload of type `token_count`, whether core
reconstruction seeded `Session::token_usage_info`, and whether the
connection stayed attached long enough to receive the targeted
notification.

The notification is sent through the existing
`OutgoingMessageSender::send_server_notification_to_connections` path,
so existing app-server tracing around server notifications still
applies. Because this is a replay, not a model turn event, debugging
should start at the resume/fork handlers rather than the turn event
translation in `bespoke_event_handling`.

## Tests

The focused regression coverage is `cargo test -p codex-app-server
emits_restored_token_usage`, which covers both resume and fork. The core
reconstruction guard is `cargo test -p codex-core
record_initial_history_seeds_token_info_from_rollout`.

Formatting and lint/fix passes were run with `just fmt`, `just fix -p
codex-core`, and `just fix -p codex-app-server`. Full crate test runs
surfaced pre-existing unrelated failures in command execution and plugin
marketplace tests; the new token usage tests passed in focused runs and
within the app-server suite before the unrelated command execution
failure.
